Any licensee employe! by a massage therapy school $ho #no$s orhas reason to #no$ that the school is not abi!ing by this proision $ill be sub9ect to !iscipline

    1 /he applicant is un!er inestigation or has eer been !enie! professional licensure or!iscipline! by any professional licensing authority of the ommon$ealth or any other9uris!iction of the =nite! &tates or a foreign country.

    2 /he applicant has surren!ere! a massage therapy license or other professional license inthis ommon$ealth or any other 9uris!iction of the =nite! &tates or a foreign country.

    3 /he applicant has been charge! $ith or conicte! of a mis!emeanor or felony in thisommon$ealth or any other 9uris!iction of the =nite! &tates or a foreign country.

    % /he applicant is unable to practice massage therapy $ith a reasonable s#ill an! safety byreason of use of alcohol' !rugs' narcotics' chemicals or any other type of material.

    + /he applicant is unable to practice massage therapy $ith a reasonable s#ill an! safety byreason of illness or as a result of any mental or physical con!ition.

    e An applicant shall erify that the applicant has rea!' un!erstoo! an! $ill comply $ith theact an! this chapter.

    f An applicant is responsible for ensuring that the 5oar! receies all require! !ocumentation.

    If the application is incomplete' the 5oar! $ill notify the applicant by means of first class mail'$ithin < $ee#s of the receipt of the application' that the application is incomplete.

    g Applicants shall supply the missing !ocumentation $ithin , months from the !ate theapplication is e*ecute! by the applicant. After that time' if the !ocumentation has not beensubmitte!' the application $ill be !enie! an! the application fee forfeite!. An applicant $ho$ishes to reapply shall submit a ne$ application an! application fee.
