Survivorship Care exists in an effort to supply the patient with some guidelines and goals post treatment which include:

1) Detecting recurrent disease at the earliest opportunity2) Preventing and detecting secondary and new primary cancers

3) Monitor, prevent, treat long-term and late effects related to disease or treatment4) Providing support to patients and families

5) Encourage adherence and optimal health behaviors6) Improving modifiable health behaviors

Page 7: Pacesetter Exclusive Event - November 6, 2014

During a Survivorship appointment, the clinician and patient:

1. Review treatment2. Discuss side effects3. Manage symptoms4. Receive psychological support5. Give referrals to specialists

From here, the clinician works to complete a Care Plan for the patient, which includes, but is not limited to:

1. Prevention, treatment, and monitoring of long-term/lateside effects

2. Follow up and preventative screenings3. Counseling, genetic testing, and fertility evaluation4. Providing support to patients and families5. Encouraging adherence and optimal health behaviors
