Paying it ForwardSpeaker: John Siciliano

Friday, September 20 8:00 – 9:00 a.m.

The John Siciliano story will take you on an emotional and physical journey about the car accident that resulted in an amputation of his leg and changed his life forever. He was a young college student playing soccer and studying theater and journalism in Pittsburgh, PA. He will talk about his rehab process where he took his first steps to later running in the Paralympic Games. John moved to NYC and Los Angeles where he attended graduate school at the University of Southern California and wrote a play about his life. He currently lives in Los Angeles and spends his time acting in TV and Film. He also travels the country doing motivational speaking for Physical Therapists. He pays it forward by educating amputees and teaching soldiers how to walk and run. He works with numerous charities such as the Challenged Athlete Foundation, Camp No Limits and the Amputee Empowerment Partner’s Program.. Instruction level is basic.

Keynote Address Astronaut Low Back Pain in Space, HNPs Post-flight and Proposed Countermeasures

Astronauts during microgravity frequently report moderate to severe low back pain and with an apparent correlation to a high incidence of HNPs post-flight. Pathomechanism of back pain in microgravity is possibly discogenic (or mechanical) associated with reduction of gravitational compressive loads. The pain mechanism appears somatic referred via sinuvertebral nerve impulse propagation due to excessive expansion and annular deformation of the lumbar intervertebral discs (IVDs). The fetal tuck position may increase IVD hydrostatic pressure by flexion and transfer spinal compressive forces towards the anterior IVD, subsequently reducing IVD volume and providing pain relief. The IVDs adapt by hypotrophy in the absence of 1G cyclic loading to the spine. Quantitative results on morphological, biochemical, metabolic, and kinematic spinal changes in the lumbar spine may provide a more definitive understanding of spine physiology and a basis for countermeasures to prevent low back pain in microgravity and reduce the incidence of HNPs post-flight. Synergizing Rehabilitation & Regenerative Medicine for the Treatment of Skeletal Muscle Injuries

Speaker: Fabrisia Ambrosio, PT, PhD

The purpose of this talk is to highlight research demonstrating the synergy between mechanical stimulation and regenerative medicine approaches to enhance skeletal muscle regeneration in

models of injury or disease.

Research investigating the application of rehabilitation modalities to enhance the efficacy of cellular therapies and tissue engineering approaches will be discussed. Topics covered will include how the addition of a muscle overloading protocol enhances the functional incorporation of stem cells and bioinductive scaffolds transplanted into injured or diseased skeletal muscle and how such synergistic efforts may affect future physical therapy clinical practice. Concussion and Brain Injury: Acute, Sub-Acute and Chronic Mechanisms and Considerations

Acute Care Wound Care: Understanding Wound Healing in the Acute Care Setting

Caring for patients with wounds in the acute care setting requires a multi-discipline effort that includes the physical therapist, even the one who is not interested in being a wound care

therapist! This presentation discusses basic principles of wound healing as it applies to the acute care patient, differential diagnosis of the most common wound types seen in the acute care setting, and the role of physical therapy in caring for these patients. Recent advances in robotics technology have brought new possibilities with respect to the development of assistive devices for purposes of enhancing the mobility and/or functionality of persons with physical disabilities. This talk will focus on the development of three such assistive devices: a powered transfemoral prosthesis for lower extremity amputees, the development of a multigrasp hand for upper extremity amputees, and the development of a lower limb exoskeleton for legged mobility assistance in individuals with paraplegia and stroke. Stem Cells and Regenerative Medicine: Harnessing the Body’s Natural Ability to Heal

Speaker: Gretchen Meyer, PhD

This presentation will be an introduction to stem cells and regenerative medicine that will highlight recent advances and applications to physical therapy. It will cover basic definitions of terms used in

the field, how stem cells behave in the body and what

As members of the physical therapy profession, each of us provide one voice, one face, and one experience to a profession defined collectively by us all. As we embark upon efforts to achieve

mass adoption of direct access physical therapy, it is imperative that we place a collective focus on the one aspect which defines us — our brand.

In this session, I will bring attention and guidance to the components of our brand that must be expressed consistently in order to achieve recognition as direct access providers. Inflammation, Atherosclerosis, Neurodegneration, and Cancer

Speaker: Bradley Stockert, PT, PhD

Chronic inflammation has been linked to an increased risk of developing cardiovascular disease, multiple sclerosis, Parkinson disease and Alzheimer disease. Adipose tissue secretes hormones that

produce chronic inflammation while physical activity causes muscles to secrete hormones that have an anti-inflammatory impact and decrease the level of chronic inflammation present. The purpose of this course is to provide evidence of how anti-inflammatory medications and physical activity can be used as interventions against atherosclerosis, some forms of cancer and neurodegenerative disease. This presentation will review basic cellular and molecular biology through the phases of healing and tissue-specific cellular responses to loading of injured and healing tissue. The audience will be

encouraged to embrace and contribute to the evolving science of mechanobiology to determine the genetic and biological effects of our interventions and participate in innovation to combine mechanical loading with other biologic interventions (gene therapy, biologic tissue engineering, etc.) in an effort to optimize the associated clinical outcomes. Filling the Gaps with Today’s ProstheticsSpeaker: Leif Nelson, PT, DPT, ATP, CSCS

Today’s prosthetics devices containing microprocessors, gyroscopes and accelerometers. Field divisions between robotics and prosthetics are being blurred resulting in technological progress in the devices for people living with limb loss. This presentation will provide insight into the Helix Hip, X2, Genium, Power Knee 2, iWalk BiOM, iLimb and more, some of which incorporate robotic technologies. In addition to these commercially available technologies, we will also explore what is on the bionic horizon for prosthetic robotics. Instruction level is intermediate.

PTSD and the Physiotherapy PatientSpeaker: Kim Dunleavy, PT, PhD, OCS

The psychological impact of trauma including physical violence, sexual abuse, or war injuries is extensive, and can influence short-term and long-term physical and psychological health of survivors. The psychological sequelae of trauma necessitate adaptation of physical therapy interventions and responsive management

strategies. There are often occasions when psychological effects of stressors or symptoms of acute, chronic or delayed onset post-traumatic stress symptoms and post-traumatic stress disorder (PTSD) either from recent trauma or previous history may emerge during hospitalization. This presentation will discuss the identification of PTSD symptoms and trauma-cognizant management strategies during Physical Therapy treatment. The influence of the co-morbidity on outcomes and referral mechanisms will be discussed. Instruction level is basic.

Affordable Care Act: Healthcare is Changing. Are you?

The Medicare Act (1965) and Americans with Disability Act (1990) are examples where Congress responded to the health needs of the elderly and people with disability. The Affordable Care Act (2010)

is upon us now. This presentation will emphasize how the ACA is an opportunity to shift our focus from a hierarchical, medical-model physical therapy that focuses on response to disease and injury to a biopsychosocial model of health and wellness. The physical therapist of the 21st Century will provide individual and family-centered social model of health care that emphasizes health promotion so that all children and adults, with or without permanent disability, have a greater chance of living a healthy, physically active lives. Unfortunately, many physical therapists are often unaware of the legal system until they are faced with a lawsuit. Even with a verdict in favor of the physical therapist it can take a toll in personal

stress, discouragement and time away from the practice to fight the allegations. Lawyers cannot always provide a precise answer to a legal question or predict the outcome of a legal disagreement. Having an understanding of liability claims and knowing the risks that confront physical therapists is important in protecting patients and reducing risk. Instruction level is basic.

The Body Electric: Your Guide to the Functional Dissection of the Nervous System

Speakers: Roger Nelson, PT, PhD, FAPTA; Michael Lescallette, PT, DPT, ECS and Scott McCauley, PT, DPT, OSC, COMT

This presentation provides an overview of the use of Electrodiagnosis in the functional dissection of the nervous

system. The use of a differential system review of the nervous system is highlighted within the backdrop of the electrodiagnostic model. The presentation includes video clips/demonstration laboratories to illustrate concepts of nerve conduction studies and electromyography. The clinical practice value of this specialty area is presented along with the background of the APTA’s first recognized residency in EMG/NCV. Additionally the “Requirements for Electroneuromyography Certification by the PT Board of California, the recent 2010 positive ruling in California related physical therapists performance of EMG/NCV studies and the recent approval of the 11-month EMG/NCV course as meeting the requirements of study will be discussed in detail. Space Fitness and Newton’s Fourth Law Speaker: Vincent Caiozzo, PhD

The goal of this presentation will be to provide an over-view of the impact of microgravity to the regulation of muscle mass and muscle fiber composition. This will be linked to ground-based models such as bedrest, which is used to simulate microgravity. Using these two forms of mechanical inactivity, important unresolved issues related to skeletal muscle will be highlighted with a special emphasis on clinically relevant issues related to rehabilitation. Clinical Decision Making for Low Back Pain: It’s Much More Than Your Hands

Speaker: John Childs, PT, PhD, MBA

The focus of this course will be on the differential diagnosis of LBP and reviewing the latest sub-grouping methodology to match the right treatment to the right patient at the right time. We will describe

screening strategies for identifying patients with potentially serious medical conditions; how to screen for patients at risk for chronicity using the LBP STarT criteria. We will examine the importance of process of care delivery, how patients with LBP access certain health care practitioners and health care services, how much care costs, and what happens to patients as a result of this care under real-world circumstances. Join us for a discussion on how physical therapists can position themselves as leaders in the cost effective prevention and management of patients with LBP. Instruction level is intermediate.

Comprehensive Cervical Spine Management: Are Sensorimotor Deficits Important?

Speaker: Robert Landel, PT, DPT, OCS, CSCS, FAPTA

Patients with cervical injury, pain and dysfunction can present with a wide variety of complaints, some of which they vaguely describe even when specifically

questioned. Despite their elusive nature, these symptoms are associated with objectively measurable impairments of cervical sensorimotor function.

The purpose of this session is to discuss the sensorimotor impairments associated with neck pain, to present the evidence supporting a rational examination for these impairments, and to offer a management approach for these patients. The participant will gain an understanding of the components of a truly comprehensive management program for patients with cervical pain. Instruction level is intermediate.

Ultrasound Imaging in the Management of Common Musculoskeletal Conditions

Ultrasound imaging (USI) is a major advancement of physical therapist practice and is fast becoming a must have tool for physical therapists in the management of

many conditions. USI provides added value to physical therapist management by: facilitating desired motor control and function, improving clinical decision making, engaging the patient in recommended management, as well as documenting and monitoring progress. This presentation will discuss common applications of USI in musculoskeletal practice including the most common presenting conditions of the spine and extremities. Instruction level is intermediate.

The COMPTSIG is pleased to offer the well-received series showcasing how experienced clinicians manage patient cases involving pain and neuromusculoskeletal dysfunction of the spine and extremities. Mr. Asavasopon will present a case on low back pain, revisiting the “core” of the problem with a “hip” solution; Dr. Kirages will present a case regarding tailbone pain after a fall – the male pelvic floor connection; and Dr. Leal, PT, MPT, DPT, OCS, FAAOMPT will present a case elucidating a combined manual therapy and pain sciences treatment approach of a patient with elevated psychosocial variables and persistent neck pain. They will share their clinical reasoning in prioritizing the elements of the examination process, developing a clinical physical therapy diagnosis and prognosis, choosing inter-ventions, and assessing outcome. The program may include demonstration and practice of techniques. Instruction level is intermediate.

Clinical Decision Making for Orthopedics: Evidence Based Examination, Clinical Prediction Rules and Pragmatic TreatmentSpeaker: Chad Cook, PT, PhD, MBA, FAAOMPT

The course is designed to review all components of decision making involved in critical reasoning and endeavours to evaluate the “effect” of each element of the clinical decision making model. The diagnostic utility of patient history, screening, clinical examination and imaging elements are discussed as well as “newer” tools such as clinical predic-tion rules. The course also weighs the value of clinical experience and expertise toward decision making on final results. Instruction level is intermediate.

8:00 a.m. – 5:30 p.m.Clinical Decision Making for Low Back Pain: It’s Much More Than your HandsSpeaker: John Childs, PT, PhD

Total annual direct healthcare costs in the United States incurred by patients with low back pain (LBP) exceed $85 billion. Wide practice variation in how health care practitioners manage these conditions and the few incentives for providing high quality and efficient health care contribute to the problem. Therefore, evidence-based clinical decision-making is essential for managing these patients. The focus of this course will be on the differential diagnosis of LBP and reviewing the latest sub-grouping methodology to match the right treatment to the right patient at the right time. We will describe screening strategies for identifying patients with potentially serious medical conditions. We will also discuss how to screen for patients at risk for chronicity using the LBP STarT criteria. Based on the latest health services research, we will examine the importance of process of care delivery, how patients with LBP access certain health care practitioners and health care services, how much care costs, and what happens to patients as a result of this care under real-world circumstances. Come join us for a discussion on how physical therapists can position themselves as leaders in the cost effective prevention and management of patients with LBP! Instruction level is intermediate.

Ultrasound Imaging in the Management of Lumbar, Shoulder and Knee Conditions Hands-On Speakers: Doug White, PT, DPT, OCS; Scott Epsley, PT, SCS; Wayne Smith, PT, DPT, ATCR, SCS, RMSK

Ultrasound imaging (USI) is a major advancement of physical therapist practice and is fast becoming a must have tool for physical therapists in the management of many conditions. USI provides added value to physical therapist management by; facilitating desired motor control and function, improving clinical decision making, engaging the patient in recommended management, as well as documenting and monitoring progress. This session will introduce the application of USI to the lumbopelvis, shoulder and knee. The participants will gain an introduction of USI and be able to apply this knowledge to the identification of sonoanatomy of the lumbopelvis, shoulder and knee. Common conditions of these regions will be discussed and the value of USI to physical therapist management will be presented. Advanced US equipment optimized for musculoskeletal conditions will be used. Instruction level is intermediate.

This course will challenge the practitioner to make a paradigm shift; acknowledge the importance of the cardiopulmonary system as an integral component of postural control. The speaker will present a model of postural control (Soda Pop Can Model) that demonstrates how breathing mechanics are linked to motor and physiologic behaviors. The speaker will present how to use this information clinically to develop positioning and ventilatory strategies that establish the pulmonary system as an asset rather than a liability for patients. This presentation will focus on the pediatric patient with multiple co-morbidities. Instruction level is intermediate.
