History and Compliance

• PASRR is a federally mandated program that requires all states to screen all individuals (regardless of payor source or age), BEFORE admission to a Medicaid-certified nursing facility.

• PASRR was created in 1987 as part of nursing home reform provisions contained in the Omnibus Budget Reconciliation Act (OBRA).

PASRR has three goals:

1. To identify individuals, including adults and children, with mental illness (MI), intellectual disability (ID) or developmental disability (DD)/related conditions (RC)

2. To ensure appropriate placement, whether in the community or in a nursing facility (NF)

3. To ensure individuals with MI, ID, or DD receive the needed and required services in their service plan

Introducing the New PASRR Comprehensive Service Plan (PCSP) Form

• Providers currently use separate forms to indicate PASRR services the individual will receive. These forms are the Interdisciplinary Team (IDT) and PASRR Specialized Services (PSS) forms.

• A new form called the PASRR Comprehensive Service Plan (PCSP) will replace both the existing IDT and PSS forms.

• TMHP – must be through Internet Explorer for the PCSP form to work.

The Interdisciplinary Team (IDT) tab and PASRR Specialized Services (PSS) forms were designed to function differently and to be submitted by different provider types. Currently, it is difficult to determine if a service has been delivered, completed or discontinued and users need to look at the initial and specialized services (SS) reviews to find all the SS that the individual qualified for. 

The new combined form is called the PASRR Comprehensive Service Plan (PCSP). 

Do not use other browsers to access TMHP except Internet Explorer. The PCSP Form will not open and work properly through other browsers.

The Benefits of the Form Merger

• To clarify the purpose of the interdisciplinary team meeting and service planning team meetings.

• Provides a “one team” approach for nursing facilities and LIDDAs to help individuals succeed.

• Ensures providers and HHSC have a comprehensive picture of the progression of PASRR services over a 12-month period.

Combining the IDT and PSS into one form, PASRR Comprehensive Service Plan (PCSP), will clarify the specialized services identification process. 

HHSC has requested to create one new combined IDT and PSS form to be submitted by the NF for each individual per year. 

The LTC Online Portal (LTCOP) allows NFs and LAs to submit and view one form that captures an integrated service plan for a year as a comprehensive view of the individual's needs.

Most of the information from the old forms such as the submitter information, the date of the meeting and the attendance grid information is very similar.

The list of specialized services are the same with the addition of Habilitation Coordination. 

Habilitation Coordination is a new service that we’ll start talking about soon. This is similar but different from Service Coordination. Habilitation Coordination will be provided to individuals for the entire duration they are residing in the NF. Service Coordination will be added for those individuals who want to transition from the NF to the community. For now, Service Coordinators will continue to function as they normally do.

The Initial IDT

What’s different from the current process?• You now record only one meeting as the initial meeting when the person is admitted to the NF.

• The meeting a year later for the same individual must be labeled as ‘Annual’ when entering the meeting type.

• An Annual meeting cannot be initiated from a PE until the new meeting date being entered is at least 334 calendar days from the last Initial or Annual meeting date submitted on a PCSP DLN from the same PE.

The 334 day limit will be enforced. The form cannot be submitted earlier than this. This will be discussed in detail later in this presentation.

What’s different:• The auto population of many sections of the form will continue (Provider and Individual information).

• Section A2400: :Individual is PASRR positive for…” will now be auto populated from the information confirmed on the PE.

• The new form will not submit with required members missing on A2500 “Participants section.

The NF should review the PE before the NF schedules the initial meeting to ensure the proper team members are present and to review the services recommended.  If NF staff are unsure they should call their LA representative to seek clarification.

We recommend a signature sheet be used for all meetings to help record attendance.

• An RN is one of the required members for initial and annual meetings. Make sure you indicate that an Nursing Facility-RN is noted on the Participant Type and that the Title for RN is selected.

At this time, the required team members are only the members noted on the chart. Please be aware that anyone who works with the individual to provide services, or anyone that the individual asks to attend, should be included.  Sometimes, service providers may not be identified or arranged for at the initial IDT. They should be added at all future meetings with the exception of DME suppliers who should never be included.

The Date of the Meeting will auto‐populate from the meeting date indicated on A0800.

What’s different:• The biggest changes can be found in section A2700.• Note that a column has been added to indicate every service recommended on the PE.

• Note that we now have a drop-down menu to select an appropriate action for each service.

• Every service must have an option selected.

We must discuss each service the individual is eligible for at every meeting that is documented on this form.

Every service must have a selection from the key code recorded or the form will not submit. 

For cases where the individual does not have Medicaid at the time of the meeting or is under 21 at the time of the meeting, use “Not Needed” as the option. Record in the required comments where services can be obtained outside of PASRR provided services and who will provide them: Example: “Individual opted to pay out of pocket for services.” or “The individual will receive OT and PT from their school.”

What’s different:• Individuals may refuse one or all services for each area of service (NF, ID, MI) they are eligible for.

• Services indicated as “Not Needed” or “Discontinued” will require a comment as to why the service was not needed or discontinued.

• The initiation of the form is from the PE and not the IDT

Individuals are PASRR positive for MI only will not be eligible for NF or IDD services.  Use the menu/drop‐down choice of “Not Needed” to enter a choice for each service they are not eligible to receive. A second enhancement to the form will correct the need to do this. We’ll notify you when the correction to this issue has been resolved.

For all individuals, every service on the form including the DME section, must be reviewed and a determination of status be made. The system will not allow the PCSP form to be submitted without it. If the individual is not eligible for any service, the menu/drop‐down choice of “Not Needed” will be selected and a comment will be required in the comment section indicating they are not eligible to receive that service. A second enhancement to the form will correct the need to do this. We’ll notify you when the correction to this issue has been resolved.

The new form can be submitted from the tool bar on the PE.

What stays the same?• When a positive PE is submitted the NF will still get an alert to certify their ability to serve the individual and a reminder to conduct the IDT.

• The NF must schedule the IDT meeting and invite the required attendees as soon as possible after receiving the alert.

• The PCSP must be in the portal for every active positive PE* for the NFs Long Term Care Medicaid (LTCMI) to process.

As we mentioned earlier, there are many areas of the form that are the same as the current IDT form.

It is important to note that IDT meetings are the beginning of getting the individuals the services they need so schedule them promptly.

*At this time, individuals enrolled in Hospice Care are exempted from needing an initial, quarterly and annual meeting.

Do not wait for your first MDS assessment denial before you conduct the initial or annual meetings. Keep up with your IDT due dates.

Quarterlies and Updates-SPT/PCSP

What’s different?• The LIDDAs first SPT meeting will be the same meeting as the IDT. LIDDAs no longer have to conduct a separate meeting.

• The LA (LIDDA, LMHA and LBHA) staff completing the evaluation can be paid for attending the initial IDT meeting as part of the units allowed for the PE.

• Service Coordinator, can bill for their attendance in addition to the PE evaluator attending the IDT meeting.

One of the purposes of the form merger is to establish the “one team, one meeting” process that will benefit the individual better than the current system.

The initial meeting may take longer than meetings currently held, but it needs to include all aspects of both the IDT and the SPT meeting.

At the initial and annual meetings the RN must be there. You will also have providers of specialized services attend but more likely they will attend meetings after the initial once providers are located . Another staff person from the NF who knows the individual best can also attend the initial and annual. This NF staff must continue to attend the quarterly and update meetings. See chart for more information.

The first year of deployment issues:

After deployment of the new PCSP form there may be quarterlies that need to be entered but a IDT on the new form has not been entered. This would occur if the IDT is not due yet.

LIDDAs may initiate a quarterly meeting from the PE and this meeting may be the first meeting recorded for that Document Locator Number (DLN).

Submitter Information (A0100 ‐ A0600)A0100‐A0600. Submitter information ‐ Most of the information is prepopulated after you use the FSI (Form Status Inquiry) search to submit a form. The only field that is not prepopulated is A0600 County when the LA is submitter. This is because some LAs have more than one County that they serve.

A0700‐A1200. Meeting Information: 

A0700. Type of Meeting ‐ Field A0700 is auto‐populated and disabled when the PCSP is initiated from a PE and the meeting type is then entered on the Initiate Form page.  Field A0700 is enabled and required when an LA adds a meeting to an existing PCSP DLN. Using the key code on the form under "Meeting Information", select the option that represents the type of meeting held: "1. Initial IDT/SPT", "2. Annual IDT/SPT", "3. Quarterly" or "4. LA Update". 

Only the LAs can submit quarterly and update meetings on the PCSP forms. 

Update meetings can be held at any time by the LA after the initial meeting to add, change, remove services or to discuss transfers, transitions or discharges.

Note for the LIDDA’s only: The LIDDA is no longer required to conduct a separate initial meeting right after or on the same day as the NFs initial meeting. The first meeting will be the “1. Initial IDT/SPT” meeting submitted on the PCSP form by the NF. A separate meeting is no longer needed.  The LIDDA’s first quarterly will be scheduled three months after the initial team meeting initiated by the NF.

A0800. Date of Meeting ‐ Field A0800 is enabled and required for certain meeting types and reason codes. Field A0800 will be disabled when field A1200 is enabled and required for certain meeting types and reason codes, and it auto‐populates the disabled field A0800. Enter t he actual date that the meeting was held with the participants. Enter the date using the "mm/dd/yyyy" format, or via the date picker icon next to this field. This date must be on or after the assessment date of the last Positive PE, and it also cannot be a future date. 

A0900. Reason Code ‐ The field will only be required when "4. LA Update" has been selected in field A0700. 

A1000. Transition To ‐ This field will only be required when "6. Transfer" or "7. Transition" has been selected in field A0900. 

A1100. Other: This field will only be required when "9. Other" has been selected in field A1000. 

A1200 Date of Event: This field will only be required when "3. Deceased", "4. Discharged", "5. Refusal of Habilitation Coordination", "6. Transfer", "7. Transition" or "8. Refusal of MI Specialized Services" has been selected in field A0900. 

Events codes are used to indicate that there would not be future quarterly/update PCSP meetings entered into the portal for this DLN.

If this is an actual discharge/deceased the NF is still required to complete section B0650 and B0655 which will inactivate the PL1 and PE.

This entire section is disabled when the NF initiates a PCSP form from the PE or when the LA adds a meeting to an existing PCSP DLN This section is enabled for fields A1300 and A1400 when the LA initiates a PCSP form from the PE.  The LA enters the data in A1300 and clicks on the lookup tool next for that field.  Fields A1500 and A1600 will be auto‐populated.  The LA also needs to enter data in field A1400 to complete this section.

Complete and submit quarterly or update meetings

• Only the LIDDA, LMHA and LBHA can submit quarterly and update meetings on the PCSP forms.

• The LIDDA's first meeting will be the "1. Initial IDT/SPT" meeting submitted on the PCSP form by the NF. A separate meeting is no longer needed.

• The LIDDA's first quarterly will be scheduled three months after the initial team meeting initiated by the NF. This means that the LIDDA will only have three (3) meetings indicated as "quarterly meetings".

Additional Guidelines

• The initial, quarterly, update and annual meeting dates to be submitted on a PCSP DLN in date order.

• The required participants for a specific meeting type must be entered on the PCSP form, even if they weren't in attendance.

• The meeting date must be on or after the PE assessment date(s).

• LAs: The Update/Quarterly submitted added to a PCSP DLN must be within 365 calendar days from last Initial or Annual meeting date.

All meeting must be submitted to the portal in the date order which they are held. Meeting cannot be submitted out of date order.

The LA cannot add a meeting to a PCSP DLN if the meeting date is more than 365 calendar days from the previous Initial or Annual meeting date. The LA must wait for the NF to submit the new Initial or Annual meeting first. Document the reasons for late submissions. Communicate with NFs to ensure meetings stay on schedule.

NFs can enter a initial or annual meeting past the 365 calendar days if uncontrolled circumstances prevents the meeting from being entered on time.

Print Function

• A new Print function added to the PCSP form allows you to print any meeting that is submitted on the PCSP DLN. The Print function does not print history notes or the "Summary" tab.

• When you click on the "Print" button, the Print function expands completely over the yellow "Form Actions" bar, and there is a printer icon located to the far right of every meeting listed. Simply click on the printer icon to print the meeting you desire. Any meeting on the list can be printed, but you can only print one meeting at a time.

Save a PCSP Form Before Submission

• The PCSP form can be saved as a draft by clicking on the "Save as Draft" button.

• You will be immediately redirected to the Drafts page where your saved draft form will be displayed on the list. A draft form will only be displayed on the Drafts page for 60 days. After 60 days, they are deleted and cannot be retrieved.
