A 26.00 Society Examining Committee
Composition and Operation
• Comprised of five Fellows lead by an appointed Chair
• Recommends changes to the Policies and Procedures
governing the process and qualifications for advancement
to Fellow
• Recommends changes to the Fellow matrix and Fellow
nomination form
• Fairly and ethically evaluates each nomination for
advancement to Fellow
• Compiles the list of nominees to be recommended for
advancement to Fellow

A 26.01 Evaluation of Fellow Nominations
• Evaluate the eligible nominations and ensure that at least
four of the five members of the Examining Committee
approve each nomination
✓ Meets minimum policy requirements
✓ Achieves the minimum rating in each proficiency area.
✓ Achieves a total of at least 28 points on the criteria
rating
• Drafts letters to the sponsors of unsuccessful nominations
• Recommends those qualified for advancement to Fellow to
the president of the Society

G 01.02 Fellow Membership
Duties and the responsibilities
➢ Fellow members have the duty to follow the Code of Ethics,
Bylaws, and Policies and Procedures of the Society and of
their member units
➢ Fellow members have the duty to report misrepresentation
or unethical behavior
➢ Fellow members have the duty to work in good faith with
member units, officers of the Society, staff, and each other
and (not in the policy)

G 02.02 Advancement to Fellow Membership
• Member must be endorsed by two persons whose membership
type is Senior, Fellow, or Honorary member
• Senior member in good standing for at least 60 full consecutive
calendar months prior to May 1 of the year in which the
nomination is submitted
• Attained distinction in one or more of the following ways:
➢ Planning or operation of quality-related work activities
➢ Teaching a major course in a quality-related academic
discipline at an accredited school or institution
➢ Expounding useful theories, principles, or methods of
quality-related work
➢ Directing work of significant scope in the application of
quality related work

G 02.02 Advancement to Fellow Membership
• Nominees require a minimum of 28 points for
recommendation for advancement to Fellow.
• Points are scored in six proficiency areas, with a minimum
required in each area, as follows:
➢ Technical competence (4 points).
➢ Occupational responsibility (3 points)
➢ Publications (2 points)
➢ ASQ activities (4 points)
➢ Professional affiliations other than ASQ (2 points)
➢ Teaching (for nonprofessional teachers) or consulting
(for professionally employed teachers) (2 points)

• The Matrix is a guide that provides examples of acceptable
evidence
• It includes examples for each point total even those that
are below the minimum for a particular category
• The examples of evidence provided are not all inclusive

Appendix B: Fellows nomination
• Ensure you are using the latest version of the nomination form
• Please use the MS Word document for the nomination
• DO NOT DELETE anything from the nomination form
• Type or clearly print all information
• Use a minimum of 10pt font
• The Nomination is limited to 100 pages including the required
nomination form – so chose your documentation wisely
• Clearly label all evidence
• Ensure the evidence tells the story by addressing how the
criteria is met
• Only completed action/items are considered – no promises
accepted
• Do not assume the Committee knows what you are thinking
• Ask for and Use a Fellows Process Coach

I. Technical competence
• Citation or memo from company on letterhead describing
the application of the concepts and the actual results
• Copy of current certificate or wallet card or provide a
printout of My Certifications from the ASQ website
• Copy of medical or other state or federally recognized
license
• Copies of awards, citations, and background or description
of each organization granting the recognition
II. Occupational responsibility
• Résumé with job descriptions.
➢ Show numbers of personnel supervised and scope of job
functions
➢ How level of professional or technical expertise in use of
quality and business improvement tools used or needed to
perform job acceptably
➢ Show extent of development and application of processes
or theories developed
➢ Show that the position is corporate or multi-unit in scope of
coverage
• Copies of citations, awards, publicity showing the recognition
given, including when, where, and who gave the recognition.
III. Publications
• Show the date, location, organization, and subject matter
presented
• Section, Division, or organization newsletters or
announcements, certificate, thank you letter, along with
copies of the cover or index of the transactions with the
nominee’s name listed
• Copies of citations of the published work in publications
written by others who are recognized as experts
• Show covers and typical publisher information, including
dates and editions
IV. ASQ activities
• Copies of newsletters or Section or Division organization
charts
• Letter from certification department
• Letter from the chair of the sponsoring organization
• Minutes of meetings, showing attendance at division or
interest group committee meetings, showing level of activity
V. Professional affiliations other than ASQ
• Minutes of or publications from the organization showing
activity of the nominee, not just membership
• Possibly include charter of organization to clarify explicit
focus on quality, if it is not clearly stated
• Copy of recognition along with the description of the
organization’s mission and scope of activities
• Copies of certificates of position or listings of members and
positions
• Provide evidence of how quality methods were successfully
applied
VI. Teaching (for nonprofessional teachers)
• Summary of classes and listing of dates they occurred
• Internal memos from company listing content of class or
session presented and dates of presentations
• Copies of ASQ minutes/publications/flyers or brochures
showing delivery of the sessions
• Letter from the Department Chair or Dean indicating the
class taught, number of hours for the class, and semester
including the year
• Provide evidence that the listed class was actually held and
that the nominee taught the class
VI. Consulting
(for professionally employed teachers)
• Correspondence or memos from clients listing the
particulars of the sessions, including content and delivery
information
• Correspondence or company memos showing the nature
and extent of the consulting along with duration of the
engagement
• Citations, internal company memos, or correspondence on
company letterhead to the nominee, with details of the
recognition
G 02.04 Resubmission of Nominations for
Advancement to Fellow • A resubmission is allowed one time
• Strongly recommended the use of a Fellows Process
Coach
• The person preparing a nomination for resubmission
may resubmit only those proficiency areas requiring
improvement, at their discretion
• Each proficiency area resubmitted will be evaluated
without regard to the previously submitted nomination
• Signature of endorsers, with the words “PARTIAL
RESUBMISSION” clearly printed in the section
normally used for the citation
Teaching Example
Criteria: Prepared and instructed, on a part-time basis,
quality-related credit bearing classes at a university or
college, over multiple academic years; paid by the
university or college.
A class is defined as a credit bearing class of 16 hours or
more from an accredited college or university. A course,
session, or seminar can include virtual instruction

Teaching ExampleDear ASQ Fellow Committee,
This letter is to confirm that John Smith has served as an adjunct faculty
member since 1999. During this time, he taught and was paid for the
following quality related credit courses within our Technology
Department to undergraduate students:
QUAL 101 – Introduction to Quality (Fall 1999 – current) – 2 credits (32
hours of instruction)
QUAL 250 – Design of Experiments – (Spring 2008 – present) - (48 hours
of instruction)
I would also like to share that Quality University is a regionally accredited
institution.
