Multicente, prospective and observational study in newly diagnosed cancer patients during 4 months. Prevalence of anaemia at baseline, in incidence during cancer treatment and prevalencie of absolute and functional iron deficiency prior to chemotehrapy initiation.
N: 295 pacientes y 6 hospitales fueron incluidos en el estudio.Anemia presente al diagnóstico en el 38,6%, pero fue tratado sólo el 32,6%.49,4% gastrointestinal, 35,7% pulmón y 27,9% mama.
106 (60,2%) sin anemia al debut desarrollaron anemia durante el tratamiento
Sólo se analizó el metabolismo del hierro a 151 (51,2%): - 48 (31,8%) presentaban anemia ferropénica: 64,7% acumulada- 41 (37,3%) presentaban déficit funcional de hierro: 73,3% acumulada

2699 pacientes (47% mujeres) programados para procedimientos frecuentes de cirugía mayor no-cardiaca en cuatro centros diferentes:
• Cirugía ortopédica (n=1286) • Cirugía ginecológica (n=207)• Cirugía urológica (n=381)• Cirugía de cáncer de colon (n=678)• Cirugía de resección de metástasis hepáticas (n=147)

La deficiencia de vitamina B12 (10%) y folato (8%) solo fue evaluada en 970 pacientes, encontrándose una tendencia a una mayor prevalencia en el grupo de no anémicos

Conclusiones
La AP afecta a 1 de cada 3 pacientes programados para cirugía mayor no-cardiaca.
Entre los pacientes con AP, 2 de cada 3 presenta un déficit absoluto o funcional de hierro, susceptible ser tratado con hierro (preferiblemente intravenoso), con o sin la administración concomitante de eritropoyetina (según indicaciones).
Más de la mitad de los pacientes sin AP presentan depósitos inadecuados de hierro para recuperarse de la anemia postoperatoria inducida por el sangrado y podrían beneficiarse del tratamiento preoperatorio con hierro (inicialmente oral).

The findings of the 1998 meta-analysis were confirmed, with small variations in some estimates. Identification of 237 references. Thirty-six studies on 12,127 patients were included: 23 showed a detrimental effect of PBT; 22 used also multivariable analyses, and 14 found PBT to be an independent prognostic factor.
Pooled estimates of PBT effect on colorectal cancer recurrence yielded overall OR of 1.42 (95% CI, 1.20 to 1.67) against transfused patients in randomized controlled studies.
Stratified meta-analyses confirmed these findings, also when stratifying patients by site and stage of disease. The PBT effect was observed regardless of timing, type, and in a dose-related fashion, although heterogeneity was detected. Data on surgical techniques was not available for further analysis.

A chain of integrated events that begins with a correct decision that the patient needs blood and ends with an assessment of the clinical outcome of the transfusion. Its goal is to achieve optimal use of blood.

The commitment of the health authorities, health care providers and clinicians are important in prevention, early diagnosis and treatment of diseases/conditions that could lead to the need for blood transfusion.

- Grade of recommendation: . “For patients undergoing orthopaedic surgery expected to develop severe postoperative anaemia we currently suggest IV iron administration during the perioperative period”.
For all other surgeries no evidence-based recommendation can be made. We strongly recommend that large prospective randomised controlled trials are undertaken in patients undergoing surgery expected to develop severe post operative anaemia.

Respect to control, very short-term perioperative IV iron administration, with or without rHuEPO, significantly reduced (*p<0.01):
No clinically relevant AEs were observed.
The scheduled IV iron dose (200-600 mg) may not cover total iron loss, especially in patients with preoperative iron deficiency.
Preoperative rHuEPO was only administered in 351 out of 1059 patients presenting with Hb level <13 g/dL and no contraindication.

Managed with a restrictive transfusion protocol without (control group) or with post-operative intravenous iron (iron group; 600 mg IVI).
Cost analysis model included: acquisition and administration costs of IV iron and allogeneic red cell concentrates, haemoglobin measurements, and prolonged stay in hospital (+0, +1 or +2 days).

Recommendation To avoid causing unnecessary delay to patients, anaemia screening should take place when referral for surgery is first made, in order to allow investigation and correction if appropriate (Grade 1C).
Where surgery is urgent, whatever time is available before operation should still be used for anaemia inves- tigation and treatment initiation (Grade 1C).

208 patients undergoing elective primary or revision hip arthroplasty were randomised to 3 groups. All received daily sc injections ofeither EPO or placebo starting 10 days before surgery. Group 1 (78 patients) received 14 days of placebo, group 2 (77 patients) received 14 days of erythropoietin (300 units/kg to a maximum of 30 000 units), and group 3 (53 patients) received placebo for days 10 to 6 before surgery and EPO for the next 9 days. A primary outcome event (any transfusion or a Hb < 80 g/L) occurred in 46% of patients in group 1, 23% in group 2, and 32% in group 3 (p<0,003). The mean number of transfusions was 1,4 in group 1, 0,52 in group 2 y 0,70 in group 3.

(Hb entre 10 y 13 g/dL) y sangrado moderado-alto Grade 1A)
• 600 UI/Kg por semana durante tres semanas antes (días -21, -14 y -7) y el día de la cirugía.
• 300 UI/Kg diariamente durante 10 días consecutivos antes de la cirugía, el día de la misma y 4 días posteriores.
• Asegurarnos de que los pacientes no presentan deficiencias de hierro al comienzo del tratamiento (FeIV) y que reciben tromboprofilaxis.
¡Se desconoce cual es la dosis mínima eficaz!

The goal of minimizing transfusions to limit patient exposure to adverse reactions is reasonable and practical. To accomplish this, PATIENT BLOOD MANAGEMENT strategies are being widely implemented and have been successful in reducing blood transfusions in tertiary care centers.
The principles of PATIENT BLOOD MANAGEMENT are based on the three pillars concept: optimizing preoperative erythropoiesis, reducing operative blood loss, and harnessing the patient’s physiological tolerance of anemia.
Anesthesiologists should recognize this changing landscape as we are often on the front lines of blood transfusion.
