    Introduction:

    Nursing today is a difficult profession to be in from the Hospital to the Frontline. Nurses are no

    longer viewed as only working in a hospital in an acute setting, Dr. Elaine Duffy says. Nursing roles are

    much broader than 'illness' and 'hospital' oriented and the changes in nursing have evolved in response to

    changing societal needs and changing health care.

    Above statement clearly says that the evolution of nursing education has been influenced by the

    conditions and changes in society over past few years. The development of science, medicine

    humanitarian reform, the hospital industry, the feminist, and the war, all bought about conditions that

    helped to crave for the nursing education in order to better serve society, nursing progressed from a very

    simple apprehend training to a highly organized theoretical and technical body of knowledge. As nurse

    has to meet the change that is made on her society there should be the changes in line of general system ofeducation.

    Therefore we see a trend to develop nursing education as part of the education system, which

    develop from grass-root level to university level.

    Genesis of B.Sc nursing training programme in India :

    1854: the school of nursing started in Madras.

    1886:the school of nursing started in Mumbai.

    1946:Bhore committee recommended recommended opening of more schools, so the nursing training

    centres with better facilities and supervision were established.

    1946:CMC Velloor College of nursing.

    1946:RAK College of nursing at Delhi.

    1948:Indian nursing council act came in to existence to constitue a council of nurses who would safe

    guard the quality of nursing education in the country. Mandate was to maintain and establish uniform

    standards of nursing education. Today nursing council is a statutory body that regulates nursing education

    in country by through prescription, inspection, examination, certification and maintaining a uniform

    syllabus at each level of nursing education.

    1949:university education commission inder the chairmanship ofProfessor S.Radhakrishnan gave a

    strong plea for raising level of nursing education in the country and integration in to the system of highereducation. Since than the progress has been made to some extent.

    In Karnataka:

    In 1971 the government of Karnataka started Govt College of nursing, Bangalore started 3 courses

    namely Basic BSc, BN (for general graduates winded in two years) and P.C.BSc.

    Baccalaureate nursing education in Karnataka

    Graduate nursing education started in Karnataka in the year 1971 in Government of Nursing fort,

    Bangalore later several private institutions started this course.

    Till 1996 - The colleges were affiliated to universities with other general courses and later in

    the RGUHS came in to existence. From then all the colleges in Karnataka except for

    deemed universities are affiliated to RGUHS.

    Manipal University started in the year 1994 before manipal college of nursing was affiliated to Mangalore

    University.

    Baccalaureate Nursing Program and Requirement:

    Guidelines For Establishment Of New Nursing Colleges In India Approved By Indian Nursing

    Council.

    1. Any organization under the Central Government, State Government, Local body or a Private orPublic Trust, Mission, Voluntary registered under Society Registration Act or a Company

    registered under companys act wishes to open a School of Nursing, should obtain the No

    Objection/Essentiality certificate from the State government.

    2. The Indian Nursing council on receipt of the proposal from the Institution to start nursingprogramme, will undertake the first inspection to assess suitability with regard to physical

    infrastructure, clinical facility and teaching faculty in order to give permission to start the

    programme.

    3. After the receipt of the permission to start the nursing programme from Indian Nursing Council,the institution shall obtain the approval from the State Nursing Council and Examination Board.

    4. Institution will admit the students only after taking approval of State Nursing Council andExamination Board.

    5. The Indian Nursing Council will conduct inspection every year till the first batch completes theprogramme. Permission will be given year by year till the first batch completes.

    Regular Inspection Committees are: Government INC, KNC, LIC from RGUHS are the regular

    inspection committees. Task Force Committee was the inspection committee when Gurumurthy

    commission was established by governor to find out the standards in relation to infrastructure, faculty etc.

    Budget:

    Government financially supports government colleges .

    Financially private colleges are self-financed or through charitable trust, co operative way depents upon

    the nature of management.

    Philosophy for BSc (Basic) and PC BSc according to INC:

    INC believes that

    1. Health is a state of well-being that enables a person to lead a psychologically, socially andeconomically productive life. Health is a right of all the people. Individuals, families and

    communities have responsibilities towards maintaining their health.

    2.Nursing contributes to the health services in a vital and significant way in the health care deliverysystem. It recognises national health goals and is committed to participate in the implementation

    of national health policies and programmes. It aims at identifying health needs of the people,

    planning and providing quality care in collaboration with other health professionals and

    community groups.

    3. Scope of nursing practice encompasses provision of promotive , preventive, curative andrehabilitative aspects of care to people across their life span in wide variety of health care settings.

    Practice of nursing is based upon application of basic concepts and principles derived from the

    physical, biological and behavioural sciences, medicine and nursing.

    4. Nursing is based on values of caring, and aims to help individuals to attain independence in self-care. It necessitates development of compassion and understanding of human behaviour among its

    practitioners to provide care with respect and dignity and protect the rights of individuals and

    groups.

    5. Undergraduate nursing program is broad based education with in an academic frameworkspecially directed to the development of critical thinking skills, competences and standards

    required for practice of professional nursing and midwifery as envisaged in national health policy

    2002.

    6. The teacher have the responsibility to be role models and create learning environment that enablesstudents to acquire enquiry driven, self directs learning and foster an attitude of life long learning.

    7. Under graduate nursing education program prepares its graduates to become exemplary citizen byadhering to code of ethics and professional conduct at all times in fulfilling personal, social and

    professional obligations so as to respond to national aspirations.

    The College/School of Nursing should have a separate building. The college/school of Nursing

    should be near to its parent hospital having space for expansion in an institutional area. For a

    College/School with an annual admission capacity of 60 students, the constructed area of the

    school/college should be 23720 square feet.

    School and College of nursing can share laboratories, if they are in same campus under same name and

    under same trust, that is the institution is one but offering different nursing programmes. However they

    should have equipments and articles proportionate to the strength of admission. And the classrooms

    should be available as per the requirement stipulated by Indian Nursing Council of each programme.

    State Nursing Registration Council should register additional qualification like P.B. B.Sc. (N), M.Sc.

    (N), One year speciality course prescribed by Indian Nursing Council etc. The original Registration

    Number will remain the same. Only the additional qualification will be added.

    Aim (according to INC)

    The aim of under graduate nursing program is to

    1. Prepare graduates to assume responsibilities as professional, competent nurses and midwives inproviding promotive, preventive curative and rehabilitative services.

    2. prepare nurses who can make independent decision in nursing situations, protect the rights of andfacilitate individuals and groups in pursuit of health, function in the hospital, community health

    services and conduct research studies in the areas of nursing practice. They are also expected to

    assume the role of teacher, supervisor and manager in a clinical/public health setting.

    Objectives

    On completion of 4 yearBSc(N) and 2 years ofPC BSc(N) program the graduate will be able to:

    1. Apply knowledge from physical, biological and behavioural sciences and medicine includingalternative systems and nursing in providing nursing care to individuals, families and

    communities.

    2. Demonstrate understanding of life style and other factors, which affect the health of individualsand groups.

    3. Provide nursing care based on steps of nursing process in collaboration with individuals andgroups.

    4. Demonstrate critical thinking skill in making decision in all situations in order to provide qualitycare.

    5. Utilize the latest trend and technology in providing health care.6. Provide promotive, preventive and restorative health services in line with national health policies

    and programmes.

    7. Practice with in the frame work of code of ethics and professional conduct, acceptable standardsof practice with in the legal boundaries.

    Admission requirements (Acc INC)

    Basic B.Sc. (Nursing)

    1. The minimum age for admission shall be 17 years on or before 31 st Dec. of the year of admission.2. The minimum educational requirements shall be the passing of: Higher secondary school certificate

    examination (12 years course) or Senior School Certificate Examination (10+2), Pre Degree

    Examination (10+2) or An equivalent with 12 years schooling from a recognised board or university

    with science(PCB) and English with minimum of 50% aggregate marks (PCME).

    Entrance/Selection testSelection of the candidates should be based on the merit of the entrance examination held by

    University or competent authority.

    Post Basic B.Sc. (Nursing)

    To be eligible for admission to this course, a candidate shall have:

    1. Passed the Higher Secondary or Senior Secondary or Intermediate or 10+2 or an equivalentexamination recognised by the university for this purpose. Those who have done 10+1

    in or before 1986, will be eligible for admission.

    2. Obtained a certificate in General Nursing and Midwifery and registered as R.N.RM with the State Nurse Registration Council. A male nurse, trained before the implementation of the new

    integrated course besides being registered as nurse with State Nurses Registration Council, shall

    produce evidence of training approved by Indian Nursing Council for a similar duration in lieu of

    midwifery in any one of the following areas.

    Revised post basic syllabus implemented from 2005-2006

    Undergraduate nursing program at post basic level is broad based education with the academic frame

    work, which builds upon the skills and competencies acquired the diploma level. It is specifically

    directed to the upgrading of critical thinking skills, competencies and standards of in service nurses for

    practice of professional nursing and midwifery. This syllabus is in tune with national health policy 2002

    for training of graduate nurses vis a vis diploma nurses.

    Challenges in University Nursing education in India.

    Today nursing profession is better than what it was in the 80s. The services of nurses is more recognised

    in hi-tech super speciality hospitals. In Karnataka we have about 103 colleges of nursing under private

    management. The nursing council has registered 151 postgraduates of nursing and among them almost

    half of them have left the country. (2004 KNC Report) With this back ground lets discuss challenges we

    face in university education: basically challenges can be classified in two areas Academic Challenges and

    Threat from non professional people.

    Academic challenges: As advancement in technology nurses need to be trained in these areas to make

    Indian nurses globally competent, mainly computer application in the field of nursing. Holistic care is a

    need of modern society so alternative therapies such as a palliative care, yoga, sidda, yoga to be taught

    during course itself. So with this scenario nursing faculties need to well knowledgeable with updating

    new skills and knowledge. Scarcity of nursing faculties is another major threat to profession, which

    resulted in shortage of nursing faculties as well as dearth of experienced staffs in colleges. There is agreat

    damand for graduate, post graduate and doctorate holders in nursing to work as teaching faculty.

    In view of the need for Nurses participation in the highest decision-making bodies at the national

    level, it is recognised that Nursing Education programme have a major role in the preparation of Nurses

    for leadership and decision making functions. Nurses, and the Nursing discipline as a whole, need to

    respond to the changing environment of health care delivery. They have to establish and maintain control

    of well-defined and cost-effective Nursing services.

    The Trained Nurses Association of India believes that Nursing Education and Training should be

    organized with reference to the health care needs of the country.

    y Basic Nursing Education should include both theory and practice, and conform to the regulationsand syllabi prescribed by the Indian Nursing Council.

    y Career guidance and information should be provided to persons wishing to take up Nursing as acareer. This type of information should be made available to all eligible young persons, especially

    women, at the district level.

    y The TNAI supports two levels ofBasic Nursing Education and Training:1. B.Sc. Nursing degree for preparation of professional Nurses to provide General Nursing

    and Midwifery services in the community, hospitals, and through other health care

    agencies.

    2. Auxiliary Nurse Midwife (ANM) programme forPreparation of Nursing Auxiliaries/Health Workers for the institutions and the Community Health care system respectively.

    3. Schools of Nursing conducting GNM programmes, especially those Schools which areattached to Medical College Hospitals, should be upgraded to Colleges of Nursing.

    Non professional threat:

    Due to lack of vision by the government mushrooming of schools and colleges of nursing hascommercialised nursing profession. Nursing profession became a victim of privatisation of

    education rather than glow.

    Most of private nursing colleges are owned by business people and became a business venturerather than rather than as a centres for life saving personnel.

    Non-professional commercial people own most of the colleges.As a result most of the private colleges are running with lack of in fracture, insufficient faculties.

    Does these colleges serve the purpose of nursing? Guess only future will give answer to this. But hard

    truth is ultimate sufferers are students studying in those bad colleges. These colleges nip the budsrather than let it to bloom.
