PACS networks: the reality today

When sending a 1000-slice CT exam (512 MB) over a T3 metropolitan network connection, the theoretical transfer time is 1m 36 seconds. In practice, it easily takes up to 15 – 30 minutes. And when cases get stuck in the PACSʼs sending queue, it could take up to hours!

Why are many centers suffering from slow image transfer?

If you think of a WAN link as a water hose with a diameter (bandwidth) and length (latency), it is clear, that you can only achieve a maximum flow when the hose is kept full.

A DICOM Sender (PACS or modality) only sends a limited amount of data in the network at a time, then waits for an acknowledgement by the receiver before sending the next piece of data. In other words, a typical, single-threaded study transfer only uses a fraction the bandwidth.
