Housing First - Social Services Model
• The primary goal for each PSH tenant is housing stability• Social service provider does a thorough intake assessment
before or immediately after a tenant moves into the building. • Based on the assessment, a support services plan is put in
place for each tenant, with an overarching goal of housing retention
• Once in residence, tenants are strongly encouraged to remain engaged with the case workers, but ongoing social services are not required of the tenants
• Services provided on-site are complemented with access to additional services in the community provided by other agencies
