Carers● Early Recognition where possible

● HART will be in a good position to identify

● Allocated Worker

● Include most/all of carer’s needs in Support Plan

● Extra Needs – carer vs service user

SSAF 3My RAS and Overview Assessment

● Who completes it?● Self Supported Assessment Form records service

user needs and any informal support they are receiving

● Responses are entered onto the RAS validation spreadsheet

● Produces an indicative budget ● To be completed before the support plan

Page 8: Personalisation – Process Pathway Together we will enable individual and Council success by supporting continuous improvement through learning

SSAF 4Support Planning

● Support Plan is about the SU so encourage, where possible, to complete their own plan

● After the RAS when the indicative budget is known

● It is a record of decisions made by the service user

● Identifies outcomes the service user wants now and in the future

● Important to add hopes, dreams and aspirations for the future (not used to doing this)

Options for managing the personal budgets● The service user – as a cash payment

● A representative – someone who has been asked by the service user to manage the cash payment for them.

● A suitable person – someone to manage the cash payment because the service user is unable to make that decision.

● The council – as a managed budget

● As a mixed budget – a combination of the above

● ISF/Provider Managed Budget – when a person wants a provider to manage their money on their behalf

Having accessibility to the garden and being able to create a sensory garden, with areas that would attract wildlife (part) – growing own fruit and veg – is important as it gives me shared interest with my wife. It also keeps my mind active and helps with my short term memory loss.Being able to be outside gives me a sense of freedom. I can chat to my neighbour over the fence and have a banter about football. I can hear the activities going on around me and find a sense of well-being when surrounded by the smell of different flowers and trees and also hearing the wildlife that comes into the garden. It is important for me to be able to bring the outside into the house and vice versa. The garden and bungalow are my haven and being able to have family and friends around and “showing off” my garden will give me a sense of achievement.

Days when the weather is warm and I am not feeling well, I can sit outside in some shelter, listening to my wife being busy in the garden and feeling part of it as well. Plus knowing that she can keep an eye on me, without having to constantly be running inside to see how I am.

I want quality time with my wife instead of feeling that the carer side has to come first. I cannot undertake too many social groups outside of the house as I get tired very easily and I want to enjoy the groups I go to rather than feeling I must go. Sometimes I cannot make the groups as I am not feeling well or have had a ‘bad night’ and going out will put extra pressure on my medication conditions which can and have put me into hospital.Managing my conditions are difficult and staying out of hospital is one of my goals and having the right equipment and environment is essential to my wellbeing.
