1. WELCOME, INTRODUCTIONS & ROLL CALL Welcome, Introductions and Roll Call

SC-4 Chair Mark Bowman opened the meeting about 8:00 AM and provided opening remarks, including “Message from the Chair” (Mystic Pizza reference to quality).

Officers were introduced: Mark Bowman – Chair Kenn Miller – Vice Chair Mitch Staskiewicz – Secretary Tammy Womack – Alternate Secretary

The Chair then started introductions around the room – name, affiliation, whether SC-4 member or guest.

Special Memorial Presentation – Robert Fletcher

SC-4 Officer Rotation: Kevin Littrell will assume Alternate Secretary positions

IEEE General Data Privacy Requirements (GDPR) Policy – adherence to privacy of personal information; collect only what you need and nothing more; if you obtain unneeded personal information, delete it; e-mail transmittals, etc. to follow new privacy protocols.

ROLL CALL/QUORUM: Of the 34 voting Members (and 21 Corresponding Members) on the SC-4 Roster, 27 Members were present. Therefore, a quorum of 50% or more was present.

MEETING ATTENDEES: There were thirty-eight (38) attendees consisting of twenty-seven (27) members, and eleven (11) guests, including four (4) international attendees.

No new members added, and five (5) removed.

The Chair provided general discussion on several items. Participants, patents, and duty to inform slides were reviewed. Chair presentation is attached to the meeting minutes.

Recognition of corporate sponsors PSStech and ETAP; sponsors were acknowledged for their contribution to the development of quality standards for the nuclear industry.

4. WEBSITE REVIEW – SC-4 & Working Groups

Nader Eldeiry developed contemporary look for SC-4 website, which was previewed during the meeting (Nader to publish the new website). Updated attendance sheet to be provided to Nader by Mitch (Secretary). Working Group Chairs required to submit agendas, meeting minutes and members for publication to the website.

6. Summary of NPEC/ADCOM Activities & NPEC/ADCOM Areas of Interest PARs approved by ADCOM for P308, P741 and P765. Directions from NPEC to SC-4 Chair to “think globally” was reviewed (see Chair presentation,

attached); Tom Koshy provided additional overview and information. “Power Systems Simulation Guidance for NPGS” Task Force was established – Tom Koshy selected as

Chair for this task force. o Task force to determine scope & identify issues to be addressed; Mission of this group is to determine the boundaries for guidance for the power system, emergency diesel generators, and islanding mode. Initial outline for topics to be considered and prioritized include: Modeling Detail; Reporting Detail; Recommended Studies; Operating Experience; Recommended Scenarios; Recommended Alignments; Reporting; Analytical Techniques). The IEEE-SA strongly recommends that at each WG meeting the chair or a designee:

• Show slides #1 through #4 of this presentation • Advise the WG attendees that:

• The IEEE’s patent policy is described in Clause 6 of the IEEE-SA Standards Board Bylaws; • Early identification of patent claims which may be essential for the use of standards under

development is strongly encouraged; • There may be Essential Patent Claims of which the IEEE is not aware. Additionally, neither the

IEEE, the WG, nor the WG chair can ensure the accuracy or completeness of any assurance or whether any such assurance is, in fact, of a Patent Claim that is essential for the use of the standard under development.

• Instruct the WG Secretary to record in the minutes of the relevant WG meeting:

• That the foregoing information was provided and that slides 1 through 4 (and this slide 0, if applicable) were shown;

• That the chair or designee provided an opportunity for participants to identify patent claim(s)/patent application claim(s) and/or the holder of patent claim(s)/patent application claim(s) of which the participant is personally aware and that may be essential for the use of that standard

• Any responses that were given, specifically the patent claim(s)/patent application claim(s) and/or the holder of the patent claim(s)/patent application claim(s) that were identified (if any) and by whom.

NRC Liaison Report - IEEE PES NPEC 18-02 Annie Caputo and David A. Wright, nominated by President Trump and confirmed by the United States Senate, were sworn in as NRC Commissioners in May 2018. The NRC Commission is now fully-staffed with 5 members. 1. Operating Reactors

a. Turkey Point Subsequent License Renewal (SLR) Application was submitted January 2018 and accepted for review on 5/2/18. Letters of intent for SLR Applications have been received from Peach Bottom, Surry, and North Anna. b. TSTF-505 updated traveler and SE template are expected to be published in the next coming months.

i. Battery TS LCO implementation – LCO 3.8.4 c. Open Phase Condition – The staff has issued TI 2515/194 for regional implementation. The initial inspections are covering four of the NEI voluntary industry initiative designs. The initial inspections concluded in June 2018. In addition, staff is currently reviewing LARs from Surry and TVA plants. d. EQ - Inspections are ongoing. A public meeting with industry was held in May as a follow-up to December 2017 meeting. e. All licensees have declared compliance with post-Fukushima Orders EA-12-049 (Mitigating Strategies) and EA-12-051 (Spent Fuel Pool Level Instrumentation).

2. New Reactors

a. The Commission issued the Combined Licenses for Turkey Point, Units 6 and 7, in April 2018. b. APR1400 Design Certification Review is currently in Phase 5 (Response to ACRS regarding SER with no Open Items issued) and 6 (Final SER issued); no electrical design open items. c. NuScale Design Certification Review is ongoing. ACRS SubCommittee Meeting on Chapter 8 (Electric Systems) took place in June 2018.

i. NuScale Power’s “Safety Classification of Passive Nuclear Power Plant Electrical Systems” Licensing Topical Report was approved by the NRC on January 2018.

d. With respect to the Advanced Reactor General Design Criteria, the staff issued, RG 1.232, “Developing Principal Design Criteria for Non-Light Water Reactors.”

3. Rulemaking

a. The final rule on Mitigation of Beyond-Design-Basis Events is under Commission’s consideration for approval.

IEEE std 946-2004 includes non-conservative design recommendation ;

When the battery charger is connected in parallel with the battery, the battery capacitance will prevent the battery charger contribution from rising instantaneously. Therefore, the maximum current that a charger will deliver on short circuit will not typically exceed 150 [percent] of the charger full load ampere rating. Instantaneous battery charger current rise should only become a concern during periods when the battery is disconnected.

(1) IN2017-06 (2/2) IN shows results of several experimental tests by BNL. This technical concern is related to type of Battery Charger. SCR type : Silicon Control Rectifier

Test results showed SCR charger provides 7 -10 times of the rated current during initial 100msec at DC short circuit fault.

This value is beyond 150% which is stated in IEEE.

According to manufacturer remark, fault current’s

There are two type of SCR Charger ; - SCR type without Instantaneous Overcurrent protection

(note) In past, there was no available design on instantaneous overcurrent protection.

Fault current will be cut off by Fuse. Because Fuse will be blown-out, the Charger will be

physically isolated. - SCR type without Instantaneous Overcurrent protection

Overcurrent Protection will be activated by “200% of rated current”.

Once the fault point is cleared, the Charger will be started automatically.

(3) Main Discussion in Japan Japanese regulation and Industry discussed how to resolve the issue:

1st Step: Contribution from Charger Magnitude of Contribution from charger and impact on the system should be checked by analysis and/or test. 2nd Step: Short Circuit Protection Evaluation If result of 1st step shows possibility of any impact on protection device by contribution from charger (i.e. unnecessary trip), protection against short circuit current should be evaluated.

Design Challenges

• Siemens was responsible for design and implementation of the upgrade, but took exception for all physical electrical modification work.

• Siemens legacy design records did not always match that of the existing plant documentation.

• Siemens sized and routed all piping based on system flow rates, but Enercon was responsible for analyzing for pipe stress.

• Technical specification was prepared by an individual with an electrical background, so level of detail was inconsistent for the other disciplines.

Design Challenges (Cont.)

• Design reviews were not very timely, design deliverable dates constantly moved out.

• Project calculation started per the schedule, but receipt of final design inputs resulted in significant delays.

• Siemens drawings are the responsibility of the PG&E to review and approve, normally Enercon can perform the review function, but in this specific contract, the Siemens is our client.

• Siemens was not aware of the details of the Design Change process.

The auto transformer consists of 3 single phase step-up transformers connected between the Unit 1 230kV yard and the Unit 2 525kV yard, with an installed single phase spare transformer. Prior to the failure, the single phase spare auto transformer was already in use. The Y phase was available as an installed spare. There were no signs of imminent failure from Oil Sampling testing of the auto transformer.

The in-service spare phase (serving as the Y-phase) failed catastrophically. The failure of the in-service spare phase transformer caused extensive collateral damage to the adjacent X-phase transformer. Neither the failed transformer or the damaged transformer could be repaired on site. The 500 kV H1 Bushing faulted internally and failed catastrophically, initiating the event.

Updated voltage schedules were issued Switchyard Voltages went up.

Transmission relocated an auto transformer from another substation to replace the failed transformer. Transmission will be replacing the transformer at the substation. The replacement transformer has been installed and the Autobank has been returned to service. The Generator Voltage Schedule has been revised again for the new transformer.
