Petition for Consent Final Settlement of Minor Conservatorship

Comes your petitioner, ______________________________________________, as Conservator of the Estate of __________________________________________________________________, a minor, and respectfully represents and shows unto the Court as follows:

1. That petitioner has been heretofore appointed as Conservator of the estate of the said _________________________________________________________________________, a minor, by decree of the Probate Court of Montgomery County, Alabama, rendered on heretofore the ___________ day of __________________________________, ________, and qualified and has been acting as Conservator of said estate from the date of appointment, up to the present time.

property due me by _____________________________________________________________________, as Conservator, and

do hereby accept service of notice of filing, by the said ______________________________________________________

as Conservator of a petition for final settlement of said conservatorship estate and do hereby waive

formal notice of the same and consent that the said ________________________________________________________

may be discharged as such Conservator, without further notice to the undersigned; and do expressly

relieve said Conservator from filing the vouchers in said estate and any other or further account of said
