Thank you for allowing us to continue to provide the highest quality care to patients,

families, and caregivers on the Central Coast. There is a direct relationship between

our ability to build the capacity we need to address growing healthcare changes

and needs in our community while maintaining your trust in our ability to do so.

In the past year we have enhanced efforts for community engagement, and you will

notice an update to our brand, which as a legacy home health and hospice agency,

is critical to our future. At VNA we want you to clearly understand how to gain

access to us and the services we provide.

Our dynamic board and senior leadership in the last quarter has undertaken an

engagement with Simione Healthcare Consultants, spending time with their team

on-site and developing an ongoing agency work plan. This has allowed VNA to

bridge the gap to further drive quality care, take a closer look at our financial

capacity and really understand what is needed operationally.

We have a strategic plan, improved teamwork, and our VNA Hospice program

continued its growth and was awarded the largest foundation grant ever received.

Going into 2019; we welcomed Dwight Wilson, RN, Chief Executive Officer and other

key members to our senior leadership and have begun product story messaging.

We have achieved gaining a better insight into our core service lines:

VNA Community Services, VNA Home Health and VNA Hospice.

Again, thank you to your dedication and support VNA has the right resources

and opportunities to serve patients, families, and caregivers.

The purpose of CCCHC is to provide administration, management and other non-direct services and counsel that would create efficiency and depth of management to the local non-profit VNA home health, hospice and community service agencies engaged in health care or health-related services.

WE HONOR VETERANS - CELEBRATING 2018 VETERANS DAYWe honored over 120 Veterans who reside in assisted and skilled nursing facilities in our community. At two of the sites we were honored to have Congressman Jimmy Panetta and at all the sites Officers from Naval Postgraduate School, which allowed us to have a special link between the past and the present service members.

VNA SPOTLIGHT

ERIC PETERSON, DPT, DIRECTOR OF COMPLIANCE Eric is married to Melissa Peterson and together have two sons, Cooper (8) and Chase (3). Eric and Melissa moved to the Central Coast from North Carolina in 2008. They both are practicing Physical Therapists in home health care. Both Eric and Melissa began their career with VNA in 2015. “We fell in love with the Central Coast and have plans to stay rooted in the best region in California.” Both enjoy spending their time traveling, whether in the state, to Chicago to visit family, or other parts of the world. They also enjoy hiking, running, watching their kids play sports and relaxing at the beach. “We love our jobs and are fully committed to helping others maintain a happy, healthy lifestyle, so we all can enjoy this beautiful area we call home.”

NANCY JO CLARK – VNA HOSPICE PATIENT AND VETERAN “Hi my name is Nancy Jo Clark. I am a VNA hospice patient. I’ve been a VNA Hospice patient now for two years. They take care of me in so many ways. The doctors come to my home, the nurses come to my home. I have spa days where I get baths. I just love them. I have masseuses that come and massage me. My medication all comes to my front door. It gives me so much more free time, in my life, than sitting in doctors’ offices waiting. I am also a Vietnam Veteran and VNA helped me to get my paperwork all done and get it all submitted. I now have a burial space over in the new cemetery over at Fort Ord where I used to serve. I’m real proud to be PART of the VNA TEAM.”

We are a non-profit agency serving Monterey, San Benito, Santa Cruz, and South Santa Clara Counties.

A hospice nurse was recently asked, “How can you work in hospice it, must be so sad!”

She answered, “I love providing highly skilled,

compassionate care, because I know I make a huge

difference in the quality of a person’s life. Hospice isn’t

about dying; rather it’s about living as fully as possible

despite a life-limiting illness. I love that I can use my

training as a nurse to bring comfort and dignity to my

patients and seeing the relief on their faces and on the

faces of those who care for them. I love being a part

of a professional hospice team that works together

to offer individualized, holistic care to families when

it is critically needed. I love that I can offer practical

solutions to patients and families, at one of life’s most

challenging moments.”

The care involves a clinical team-oriented approach to medical care, pain management, emotional and spiritual support expressly tailored to the patient’s and family’s needs and wishes.
