Physician Results Forms enhance any biometric wellness screening program by giving employees an alternate screening method. This offering helps employers capitalize on primary care physician encounters where screening results are already available, thereby avoiding redundancy in healthcare costs. Physician Results Forms also engage employees in remote areas, where onsite screenings may not be available and screening at a Patient Service Center may not be an option.

Employees who use the Physician Results Form option receive full access to the benefits of screening in their employer’s biometric screening program, including earning incentives and receiving a personalized results report.

When Physician Results Forms are available for an employer’s screening program, employees simply log into the participant platform and ask their healthcare provider to complete the form with their most recent lab values. The completed form can then be uploaded through the same Online platform, or faxed to Quest Diagnostics for processing.
