STEP 1: SET UP YOUR ONLINE PORTAL ACCESS

Easy access to your account: Complete and SIGN* this form, even if you are currently

faxing your orders

Assign providers and designees for your health organization ‐ eachemail address will receive an invite

Set up a username/password to access your account

Health organization designees may administer orders and results based on selected permission(s) for portal access

Designees may use their permissions to administer for EACH provider listed for the health organization

View your complete order history on the first login

*The signature acknowledges responsibility that access is limited to providers and designees for use permitted by HIPAA guidelines.
