OVER ANY PART OF THE ORIGINAL REQUEST FOR QUALIFICATIONS AND

INFORMATION IN CONFLICT THEREWITH.

This schedule is subject to change, and all respondents will be provided the earliest possible

notice of changes.

V. PROPOSAL SUBMISSION DEADLINE

Respondents must submit their responses to CSDC directed to the attention of:

Bureau of Purchasing, Chief Procurement Officer, 1300 Perdido, Suite 4W07, New Orleans, LA

70112, not later than December 15, 2015, at 4:00 pm (Central Time). Respondents shall provide

seven (7) signed hard copies of the response marked "Development of 542 Tchoupitoulas and

545 South Peters Streets" and one (1) digitally signed proposal on a CD or Flash Drive, in

Microsoft Word, Excel and/or PDF format only, together with an electronic transmittal of project

design plans, marked "Development of 542 Tchoupitoulas and 545 South Peters Streets", and

shall provide primary contact information which shall include name, company, address,

telephone, and email address.
