PROGRESS REPORT ATTACHMENTS (GAS ) PHMSA Form No. PHMSA F 999-92 INSTRUCTIONS: These attachments request information either for the entire calendar year (CY 2017: January 1 through December 31, 2017) or as of (or on) December 31, 2017. Please report actual as opposed to estimated numbers on the attachments. Be careful to provide complete and accurate information since the PHMSA State Programs will be validating the attachments during the state's next annual evaluation.

Attachment 1: State Jurisdiction and Agent Status Over Facilities. Requires the state to indicate those pipeline operator types over which the state agency has jurisdiction under existing law. If the state does not have jurisdiction over an operator type, indicate why not in the column designated No, using the one alpha code (A or B) which best describes the reason. If the state agency has jurisdiction over an operator type, place an X in the column designated Yes and provide information on the number of operators, the number and percent of operators inspected, the number of inspection units, and the number and percent of inspection units inspected. If the jurisdiction over a type of operator is under a Section 60106 Agreement, indicate X/60106 in the column designated Yes. [If the same operator/inspection unit is visited more than once during the year, count only once under number of operators inspected/number of inspection units inspected on Attachment 1. The multiple visits would, however, be reflected under total inspection person-days in Attachment 2.].Attachment 2: Total State Field Inspection Activity. Requires the state to indicate by operator type the number of inspection person-days spent during CY 2017 on inspections; standard comprehensive; design, testing, and construction; on-site operator training; integrity management; operator qualification; investigating incidents or accidents; damage prevention activities; and compliance follow-up. Attachment 2 should include drug and alcohol inspections. Counting "In Office" Inspection Time - An inspector may choose to review pipeline company procedure manuals or records away from the company facility in order to effectively use on-site inspection time. The amount of time spent reviewing procedures and records may be counted as part of the inspection process. It is important that an inspector only record time for activities that normally would be completed as part of an on-site inspection. For example, an inspector may attribute the three hours he or she spent reviewing a pipeline operator's procedure manual and records prior to an on-site inspection towards the total inspection time. Each supervisor must carefully review the reported time to ensure the time attributed is consistent with the activity completed and is carefully delineated from normal office duties..Attachment 3: Facility Subject to State Safety Jurisdiction. States should only list the facilities that are jurisdictional under Parts 192 and 193 (Gas) and Part 195 (Hazardous Liquid) of which the state has safety authority over. This attachment requires the business name and address of each person subject to the pipeline safety jurisdiction of the state agency as of December 31, 2017. Also indicate the operator type (e.g., intrastate transmission) consistent with the listing in Attachment 1 and include the number of inspection units in each operator's system. The operator identification number (OPID) assigned by PHMSA must also be included on this attachment. If an operator has multiple types of system (i.e. gas distribution and intrastate transmission), each type should be counted in corresponding category. Total operator count listed in Attachment 3 may not match Attachment 1 totals due to multiple types of systems per operator..Attachment 4: Pipeline Incidents. Requires a list of incidents investigated by or reported to the state agency that involved personal injury requiring hospitalization, a fatality, property damage exceeding $50,000, and others deemed significant by the operator. Clearly identify the operator’s reported cause AND the state's determination of the cause of the incident using the one most appropriate alpha code footnoted in the attachment. We summarize this information for Congress by classifying the cause into one of eight categories: (A) corrosion failure; (B) natural force damage; (C) excavation damage; (D) other outside force damage; (E) material failure of pipe or weld; (F) equipment failure; (G) incorrect operation; (H) other accident cause. You can also choose (IP) Investigation Pending for those incidents remaining under investigation as of December 31. Then provide a summary of incident investigations.

.Attachment 6: State Record Maintenance and Reporting. Requires a list of records and reports maintained and required by the state agency..Attachment 7: State Employees Directly Involved in the Pipeline Safety Program. This attachment requires a list by name and title of each employee directly involved in the pipeline safety program. Be sure to include the percentage of time each employee has been involved in the pipeline safety program during 2017. If an employee has not been in the pipeline safety program the full year of 2017, please note the number of months working on the program. Indicate a Qualification Category for each of the state's inspectors (see Attachment 7a). The categories are shown in descending order of education and experience. Please enter the number of the highest description applicable to each inspector. For each inspector and supervisor, indicate the month and year he/she successfully completed the training courses at the Pipeline Safety Office of Training and Qualifications in Oklahoma City, OK. Finally, provide in summary form the number of all staff (supervisors, inspectors/investigator, damage prevention/technical and clerical/administrative) working on the pipeline safety program and the person-years devoted to pipeline safety. Person-years should be reported in hundreds (e.g., 3.25).

. Attachment 8: State Compliance with Federal Requirements. This requires the state to indicate whether it is in compliance with applicable federal requirements. If a particular requirement is not applicable to the state (e.g. offshore inspections), indicate NA in the column designated Y/N/NA and indicate in the notes section why the regulation is not applicable. If a regulation has been adopted, indicate the date adopted (e.g., 05/01/04) in the appropriate column. If the regulation is applicable but has not been adopted indicate N in the Y/N/NA column and explain why not in the appropriate column (e.g., requires legislative action). If the state has not adopted the maximum penalty amounts for a related series of violations please indicate civil penalty levels in effect in the state as of December 31. For State Adoption of Part 198 State One Call Damage Prevention Program if a state has any penalty amount for its damage prevention law please mark item 7.h as “Adopted but Different Dollar Amounts” and list the penalty amount in the Note section. Note at the end of Attachment 8 we are requesting each state to indicate the frequency its legislature meets in general session. This information will be taken into account when determining if applicable federal regulations have been adopted within 24 months of the effective date or two general sessions of the state legislature.

.Attachment 10: Performance and Damage Prevention Questions. This attachment requires a narrative of each states goals and accomplishments. In addition it requires a narrative on each states progress toward meeting the nine elements of an effective damage prevention program as described in the PIPES Act of 2006.

Attachment 5: State Compliance Actions. This requires a summary of state pipeline inspection and compliance actions. [In the Number of Compliance Actions Taken column, keep in mind one compliance action can cover multiple probable violations.]

DEFINITIONS

. Inspection Unit. An inspection unit is all or part of an operator's pipeline facilities that are under the control of an administrative unit that provides sufficient communication and controls to ensure uniform design, construction, operation, and maintenance procedures for the facilities. (See Glossary of Terms in Guidelines for States Participating in the Pipeline Safety Program for application of the inspection unit concept to transmission and hazardous liquid pipeline systems, distribution systems, liquefied gas systems, municipality, master meter system, regulated gathering pipeline systems, and propane-air systems/petroleum gas systems.)

Inspection Person-Day. An inspection person-day is all or part of a day spent by a state agency representative including travel in an on-site examination or evaluation of an operator or his system to determine if the operator is in compliance with federal or state pipeline safety regulations, in an on-site investigation of a pipeline incident, or in job-site training of an operator. Time expended on such activities should be reported as one inspection person-day for each day devoted to safety issues, regardless of the number of operators visited during that day.

.Probable Violation. A probable violation is a non-compliance with any section or, where a section is divided into subsections (a), (b), (c), etc., any subsection of federal or state pipeline regulations. Each numbered section should be counted separately. Multiple non-compliances of a numbered section discovered on the same inspection should be counted as one probable violation with multiple pieces of evidence.

.Compliance Action. A compliance action is an action or series of sequential actions taken to enforce federal or state pipeline regulations. One compliance action can cover multiple probable violations. A compliance action may take the form of a letter warning of future penalties for continued violation, an administratively imposed monetary sanction or order directing compliance with the regulations, an order directing corrective action under hazardous conditions, a show-cause order, a criminal sanction, a court injunction, or a similar formal action.

3112/7434 Agency Tracking 01/23/2017 - (1450) Incident # 3112 - Chris Grissom of CNG reported that the calculations of lost gas exceeded the 3 million cubic foot threshold for Federal reporting. Federal Number is 1169311 Pipe was damaged by Frost Heave due to extremely low temperatures for the area. This was originally called in on 1/19/2017 as a state reportable leak due to gas blowing for over 2 hours. After it was determined that the gas loss exceeded 3mmcf the info was called into Lex Vinsel on 1/23/2017 as a Federally reportable incident. A UTC Form H will be filled out detailing the investigation of this incident. UTC staff requested that CNG conduct a lab analysis of the failed pipe. There was approximately a 4-inch separation between the 2 ends of the 2-inch steel main. There also appeared to be corrosion on the steel at the X-Tru coat wrap to steel interface where the wrap was removed for the welded joint. this would have been in an area of hand wrapping during the installation. CNG decided to not have a lab analysis completed but instead conducted an in-house exam that determined the failure was caused by poor weld penetration and excessive force applied on the pipeline due to extreme weather. The report was submitted to UTC Staff on 4/27/2017.

3132/7459 Agency Tracking On 3/7/2017 PSE reported an evacuation (state rule WAC 480-93-200(1)(c) due to a gas leak. At this time it was not considered to be Federally reportable as it did not meet the definition of an incident. PSE could not pinpoint the leak and continued to monitor it and ensure that it was safe while plans were made to replace the pipeline. On 5/2/2017 staff asked for an update and the date the leak had been repaired.

On 5/30/2017 staff again asked for an update and the date the leak had been repaired. On 6/2/2017 PSE provided an update stating that approximately 600' of 1.25" Dupont Aldyl-A PE pipe had been replaced because the leak could not be pinpointed. The date of the replacement and shut down of the leak was reported as 04/24/2017 at 03:30 pm. At this time staff asked PSE what the total cost of the incident was and were told that they need to file an NRC report if over $50,000. An NRC report was filed on 6/12/2017. The cost of the leak response, monitoring and replacement of the main was reported at $211,000. The leak cause was actually never identified but due to the volume of gas present was assumed to be a butt fusion that failed. The pipe was Aldyl-A vintage pipe with a history of both brittle like cracking and fusion failure.

Summary2

3139/7509 Agency Tracking Northwest Pipeline West LLC (Williams) reported that a relief opened at the North Tacoma Gate Station at 24th St E and East Valley Hwy. An 8-inch regulator failed which caused the pressure to rise to the relief set point at which time the twin stack 6-inch relief opened and vented gas to the atmosphere. This line supplies gas to PSE's 16-inch supply pipeline that serves Tacoma and the tideflats. The open relief valves caused PSE's pressure to drop slightly and PSE dispatched a pressure controlman to determine the cause at which time he saw that Williams relief were relieving pressure. Williams was notified and responded also. Emergency responders shut down East Valley Hwy and routed traffic away from the area. The relief valve was fully open for approximately 75 minutes and relieved approximately 21 mmcf of gas. Staff are working with Williams to determine if an open relief that passes over 3 mmcf is reportable under CFR 191.3. In the meantime, PSE received approximately 60-70 odor calls. Even though interstate gas is non odorized there are some natural odors and it is assumed that the odor calls were due to the natural odors in the gas being vented through the relief stacks. PSE investigated all of the calls and found no leaks. UPDATE: Williams has agreed to file a NRC report on 3/31/2017

3156/7447 Agency Tracking 5/11-CNG called to report an over pressure event at regulator station 19. Crews responded to a blowing relief last night and found a bad regulator. CNG then checked their pressure monitor downstream of the regulator and found it had reached 273.25 psi. MAOP is 249. Did not reach the threshold for federal reporting. They are investigating and will submit 30-d report.

5/11-Update on MAOP exceedance. CNG called back to report the address was incorrect on first report and it was regulator station 18 not 19. CNG also reported that they performed a calculation on the amount of gas released. If they assume worse case scenario (regulator opens full) for each time period it exceeds the relief set point of 270 psi (the regulator relieves until it falls below the set point) they calculate 6.5 million cubic feet of gas could have been released. They reported to NRC #1178094.

CALENDAR YEAR (CY) 2017

Planned Performance: What are your Planned Annual and Long-term goals for your Pipeline Safety Program?1. Continue to meet state requirements & obligations for participation in federal DOT Pipeline Safety Program and CY18 Interstate agent agreement. Refine use of intrastate IA in CY18. 2. Continue to enhance and refine our internal database during CY2018 and beyond. Refine operator fact sheet (historical information) to improve our ability to make data-driven decisions about inspection frequency and focus. 3. Improve tracking capabilities for compliance items, ensuring we accomplish things in a timely and accurate manner. 4. Expand/enhance GIS use by the entire program. Long term focus is on a functional, sustainable system that allows us to increase and catalog our knowledge of all of our inter- and intra-state systems. 5. Continue to complete inspection reports in a timely manner with a target completion date of 45 days from exit interview. This target will be met at a minimum of 95% frequency. 6. Past Performance: What did the Pipeline Safety Program accomplish during the subject year (to this document) to contribute toward the program's annual and long-term goals?

1. The 2017 certification program went well; the progress report, office inspection, and field weeks scored well. Michael Thompson did an exceptionally thorough job evaluating our program and providing feedback that we will use to modify our procedures and approach with an aim toward making our program better. 2. Progress in the state-specific IA pilot went well. We had several interactions with Dave Lykken and CYCLA, who were very helpful in expanding our knowledge of the system. 3. We continue to enhance our data analysis capabilities. We hired a Transportation Specialist 3 to help us collect, analyze, and use data from inspections and our damage prevention program to help inform future decisions. 4. Small gas systems/master meter elimination program is ongoing. We continue to identify these systems and have begun initial implementation of a program to encourage LDCs to eliminate/take over these systems. 5. We implemented a compliance tracking system for both docketed and non-docketed items in our database. This system provides a dashboard for individuals assigned to the program, as well as a holistic, program-wide view of how well we are meeting deadlines. 6. We implemented several improvement projects, including: a. monthly operator summaries for all unannounced crew/construction inspections b. 30-day report project with embedded data collection c. DIRT report with embedded data collection d. Electronic Return Receipt mail for official communication e. Operator Newsletter; began the design phase of a newsletter intended to accomplish quarterly information sharing of technical and program information. The actual delivery of this newsletter will occur in 2018. 7. Initiated an independent, third-party review of our program conducted by the Pipeline Safety Trust. We will review the findings and implement changes to improve our program once the final document is released (Spring 2018).

If yes to question 2, where does the state or agency stand on implementation of the nine elements contained in the PIPES Act of 2006? Washington's Pipeline Safety program is in compliance with the nine elements outlined in the PIPES act. In 2014, PHMSA's State Damage Prevention Program Characterization determined that Washington was meeting all nine elements, after the revised dig law went into effect on Jan. 1, 2013. Additionally, Washington is using an enhanced penalty matrix for repeat violations; completed allocation of the 2016 PHMSA damage prevention grant that carried over into 2017 and was used for outreach in the form of radio, billboards, and advertising at high school championship events; and for the third year in a row will be using the one-call grant funds for enforcement and investigative activities. The program used penalty funds collected through damage prevention enforcement actions to contract with trainers for dig safe and contractor training. 2017 resulted in the legislative amendments which removed the expiration date for the agency's authority to enforce violations of the dig law. Work continued in 2017 with dig law stakeholders to identify additional language to strengthen and enhance our state dig law, resulting in bill submission in 2018.
