PARENTS/GUARDIANS
Nebraska law requires that your child must have a physical
exam, vision exam and updated immunizations BEFORE they can
start kindergarten. A complete immunization record with dates of
all immunizations and a physical and vision form filled out by
your healthcare provider MUST be turned in by the first day of
school. If the school does not have all of the needed information
your child will not be able to attend until the requirements are
met. You will be notified by letter in April or May as to what your
child needs to meet the state requirements.

many truly dedicated people. After being involved in
education for over 50 years, I will continue my life long
