Lesson Eight

The “4R’s” Part C – “Reinoculation”

The Microbiome

• Please review Lesson Five (D) and accompanying resources/materials covering the microbiome.

100 Trillion Guests

• Types of microbes comprising the microbiomeinclude:

– Bacteria

– Yeast

– Fungi

– Mold

– Viruses

– Parasites

100 Trillion Guests

Good Bugs - Vulnerability

Predominant bacteria are vulnerable to the following:

Good Bugs – Why Reinoculate?

– Colonisation resistance

– Help normalize the gut immune response

– Manufacture vitamins and other nutritional factors

– Stimulate gut motility (bowel movements)

– Reduce allergies

– Produce chemicals that feed the colon cells

– Reduce inflammation

Good Bugs - Why Reinoculate?

Good Bugs – Benefits of Probiotics

• Increase nutritional value (better digestibility, increased absorption of vitamins and minerals).

• Promotion of intestinal lactose digestion.

• Positive influence on intestinal and urogenital flora (antibiotics and radiation induced colitis, yeast infections and vaginitis in women).

• Prevention and reduction of intestinal tract infections (bacteria or virus induced, Candidaenteritis, H. pylori).

Good Bugs – Benefits of Probiotics

• Regulate gut motility (constipation, IBS).

• Decrease incidence and duration of diarrhea (antibiotic associated, C. difficile, travel, and rotavirus).

• Maintain mucosal integrity.

• Improve and support immune function.

• Possibly prevent colon cancer.

Good Bugs – Benefits of Probiotics

• Reduce catabolic products eliminated by kidney and liver.

• Prevent of osteoporosis.

• Improve development (growth).

• Anti-carcinogenic, anti-mutagenic and anti-allergic activities.

• Enhance general feeling of well-being.

• Have anti-Candida properties.

• Improve hepatic encephalopathy.

Good Bugs – Benefits of Probiotics

• Regulate inflammatory conditions such as IBD (Crohn’s disease and Ulcerative Colitis).

• Reduce and eliminate of small bowel bacterial overgrowth.

• Relief from urinary tract infections.

• Positive influence on autistic children.

• Provide antagonistic environment for pathogens.

• Block adhesion sites from pathogens.

• Inactivate enterotoxins / endotixin.

Good Bugs – Benefits of Probiotics

• If you go to Pubmed and type “probiotics” into the search facility, you’ll find many thousands of supporting studies. But it’s important to realise and understand that not all probiotic supplements are the same, and not all people are the same!

Good Bugs – What is a “Probiotic”?

• The word “probiotic” is derived from the the Greek language and means “for life”. Probiotics are currently defined as, “Microorganisms, which when consumed in adequate amounts, confer health benefits to the host.”

Good Bugs – Replenishment

• It’s important to note that several factors can influence the ability of an individual to properly restore optimal microbial (“good bug” levels):

– Mother’s microbial composition

– Birth modality

– Breastfeeding (and alternatives)

– Early childhood infection

– Stress

– Antibiotic use early in life

– Diet

Good Bugs – Replenishment.

“The survival of ingested probiotics in different parts of the gastrointestinal tract varies. As a result of their

concentration in the lumen, they contribute to transient modulation of the microflora ecology, at least during

the period of intake.”

Good Bugs – Replenishment

“Once consumption stops, the number of probiotic microorganisms quickly falls. This applies to all probiotic supplements available in the market

today.”

Good Bugs – Replenishment

“Many studies have demonstrated significant shifts in bacterial counts in human faeces following

consumption of specific probiotic strains, generally resulting in increased numbers of health-promoting

genera (Lactobacillus and Bifidobacteria) & decreased numbers of potentially harmful ones (such as several

strains of Clostridum, Enterococcus and Candida).”

Good Bugs – Replenishment

“These studies, however, reflect the bacteriological situation in faecal matter only and do not provide an

accurate picture of the situation in different parts of the GI tract or in the mucosal layer of the gut. Furthermore, many species of intestinal bacteria from faecal samples

cannot be cultured on specific plates.”

Good Bugs – Replenishment - Example 1

• Female, early 40s.

• Used expensive probiotic protocol in addition to traditional diet with fermented foods.

• No significant change in predominant bacteria levels as shown by GI Effects stool test after 90-days.

• BUT, Aeromonas opportunistic overgrowth had cleared.

Good Bugs – Replenishment - Example 1

Good Bugs – Replenishment - Example 1

Good Bugs – Replenishment – Example 2

• Male, early 20s.

• Used cheaper (mid-range price) product bought from high street.

• Exceptional improvements in predominant bacteria and Achromobacter opportunistic overgrowth disappeared.

• Note – antimicrobial protocol had been used prior to probiotics.

Considerations for Probiotic Supplements

• Factors to consider when choosing specific probiotic products:

– Potency (number of CFU’s per serving)

– Capsule or powder form?

– Are Lactobacilli contraindicated by SIBO / high D-lactate in organic acids test?

– Is the product heat stable?

– Is the product manufactured for the specific age of the person (i.e. infants and children vs. adults)?

Good Bugs - Prebiotics

• Bacteria multiply very quickly once they reach their destination, but they need protection and food to survive the trip through the stomach and intestinal tract.

Good Bugs - Prebiotics

• Bacteria multiply very quickly once they reach their destination, but they need protection and food to survive the trip through the stomach and intestinal tract. A prebiotic is defined as, “Nondigestiblesubstances that provide a beneficial physiological effect for the host by selectively stimulating the favorable growth or activity of a limited number of indigenous bacteria.”

Good Bugs - Prebiotics

• Some manufacturers include prebiotics such as fructo-oligosaccharides (FOS) or inulin in their formulations. You’ll find some clients/patients bloat and develop GI discomfort from the prebiotics. Foods and separate supplements can be used to provide prebiotics. Probiotics that don’t contain prebiotics may be needed for some people.

Good Bugs – Prebiotics

Prebiotic Compound Food Sources Targeted Microorganism

Beta-glucooligomers Oats Lactobacillus spp.

Xylo-oligosaccharides Corn on the cob Bifidobacter spp.

Probiotic Foods

• Certain foods are rich in probiotics:

– Fermented yoghurt

– Kefir

– Sauerkraut

– Miso

– Kimchee

– These foods need to be made properly.

Specific Probiotic Supplements

• I always recommend slowly titrating the dose when commencing a probiotic protocol because probiotics, like any other supplement, can cause significant reactions. I have seen some incredible side effects when clients introduced probiotics in larger doses.

Specific Probiotic Supplements

• It’s generally believed that probiotics are best taken away from food. However, some people tolerate them better when they take them with meals. Some of bacteria will be “lost in digestion” when taken with food. Tolerance levels can vary according to circadian rhythm (it’s believed the microbiome may be controlled by autonomics).

Specific Probiotic Supplements

Specific Probiotic Supplements

This table shows Klaire Labs’ probiotic range (great company).

Specific Probiotic Supplements

• Saccharomyces boulardii is a yeast organism that doesn’t colonize the GI tract. It can be very helpful in the management of loose stools and diarrhea (it can constipate).

Probiotics - Summary

• Probiotic supplementation seems to cause a transient increase in stool bacteria levels that diminishes when supplementation is stopped. But this does not necessarily reflect what is happening in the intestinal lining/mucosa, where significant colonisation and population regeneration may be occurring.

Probiotics - Summary

• Research indicates many potential benefits in supplementing with probiotics. Individual reactions and responses should be taken into consideration from product-to-product and person-to-person. Furthermore, taking probiotics with or without food and at different times of day may elicit different responses.

Thank You!

• Thanks for consuming this information. I hope it’s been helpful. In the next lesson we’ll cover the final phase of the 4R’s process – Repair/Reintroduce.
