E mergency Medical Services, Report 2001, is a continuation of the Division of Emergency Medical Services’ ongoing assessment of Mississippi’s emergency medical system. The Emergency Medical Services Annual Report captures EMS activities through a year in review chapter, an overview of the Mississippi EMS Operating Fund, and a yearly run report and provider information. An addition to the report is a section pertaining to trauma, a rapidly expanding program within DEMS. T he Division of EMS dedicates performance to ensuring high quality EMS care for all Mississippians. Proper training and testing, inspections and surveillance for proper equipment and certified staff, and monitoring skills performed through the run reporting system contribute to that assurance. The data is used by many entities in Mississippi, as well as nationally. Mississippi’s prehospital data collection system is one of the best in the country. The Mississippi Emergency Medical ServicesOperating Fund (EMSOF) was established in 1982

through legislation that added $5 to fines assessedstatewide on hazardous moving traffic violations. Thefunds are then distributed per capita to eligiblegovernmental units (cities, counties, EMS districts) forlocal level EMS support. DEMS has administered theEMSOF since its establishment.

DEMS makes disbursements annually upon requestfrom each governmental unit. According to law,

these distributions must be used in addition to existingannual emergency medical services budgets of thegovernmental units.

Collections in fiscal year 2001 totaled $2,014,725 anddisbursements of $1,001,663 were distributed to one

EMS district, 7 cities, and 62 counties ruled eligible.

EMSOF Disbursements

The criteria used for determining priority for localexpenditures follow:

• Basic Life Support – To increase the availabilityand/or improve the quality of basic ambulanceservice as described in Sections 41-59-1 through41-59-49 of the Mississippi Code of 1972 asamended.

• Advanced Life Support – To increase theavailability and/or improve the quality ofadvanced life support services as defined inSections 41-60-11 through 49-60-13 of theMississippi Code of 1972 as amended.

• Regionalization – To support or increase supportfor the establishment, administration, and/orexpansion of EMS districts as defined in Sections41-59-51 through 41-59-59 of the MississippiCode of 1972 as amended.

The total collections since 1983 are $23,385,697;disbursements since 1984 are $20,814,793. Use of

the cumulative EMSOF disbursements are categorizedbelow.

Chapter 2Providers Of Mississippi EMS

Personnel certified by the Credentialling Branch ofDEMS provide emergency medical care in

Mississippi. DEMS is responsible for ensuring thatquality emergency medical care is provided throughcoordination of training for all levels of emergencymedical technician (EMT) and EMS-Driver (EMS-D)certification, which is required for drivers of ambulancevehicles. Training for the EMS-D includes classroomand vehicle operation.

Branch staff also test all EMT candidates and certifythose who meet the required educational standards.

DEMS uses the National Registry of EmergencyMedical Technicians’ (NREMT) examination system.All levels of EMTs in Mississippi must performNational Registry practical skills and pass a writtenexamination to be certified. Mississippi adheres to thethree national standard levels of training for the EMT:

• EMT-Basic (EMT-B)• EMT-Intermediate (EMT-I)• EMT-Paramedic (EMT-P).

Numbers of certified personnel at each level in eachcounty are listed in the county summaries.

Emergency Medical Technician – Basic

EMT-Basic (EMT-B) is the first level of training inthe Emergency Medical Technician career structure.

The course covers all techniques of basic life supportand provides a solid foundation for care of injured or illpatients in the prehospital setting. The minimumclassroom time for the EMT-Basic course is 110 hours.Each student must also document five emergencyambulance runs and 10 hours clinical training tocomplete the course.

With the cooperation of the Gove rn o r ’s HighwaySafety Program, the Mississippi State

D e p a rtment of Health, the American College ofS u rgeons-Mississippi Committee on Trauma, and theMississippi Chapter of the American College ofE m e rg e n cy Physicians, the Mississippi Vo c a t i o n a l -Technical Education Division of the Department ofEducation offers the EMT-Basic training coursethrough the Mississippi Community College system.Also, approved A d vanced Life Support trainingp r ograms are authorized to provide EMT-B training. Inaddition to national training standards, state guidelinesassure standardization among all in-state EMT- Bc o u r s e s .

Most Mississippi community colleges offer EMT-Basic courses within their college

district boundaries via satellite campuses.

In FY’01, 54 separate courses were provided throughthe community college system in Mississippi,

resulting in 506 individuals completing the course. Uponcompletion of an approved EMT course and verificationof competence in each of the required practical skillareas, students may apply to take the National Registryexamination, which is given four times a year. In FY’01,462 exams were administered with 240 passing scores of70 percent or greater — a pass rate of 52 percent.

E m e r g e n cy Medical Technician —Basic

EMT-Intermediates (EMT-I) provide skills of EMT-Basics plus limited Advanced Life Support (ALS)

skills (defibrillation, initiate IV fluids, draw bloodsamples, and limited advanced airway management).The EMT-I training program includes a minimum of 150didactic hours, 40 hours of clinical, and 40 hours fieldexperience.

EMT-Paramedics (EMT-P) provide the highest level ofprehospital care. In addition to EMT-I skills, EMT-

Ps are trained to perform advanced cardiac resuscitation,medication, and advanced IV therapy for medical andtrauma emergencies along with advanced airwaymanagement. In addition to EMT-I, minimum hours forEMT-Paramedic training are 1,200 didactic, 250 clinical,and 250 field experience.

Mi s s i s s i p p i ’s eight A d vanced Life Support (ALS)educational programs teach at both the EMT-

I n t e rmediate and EMT- Paramedic levels. The U. S .D e p a rtment of Tr a n s p o rtation curriculum serves as thestandard for advanced training. Mississippi requiresALS training programs to be accredited by the JointR ev i ew Committee on Educational Programs for theE M T- Paramedic (JRCEMT-P), a branch of theCommission on Accreditation of Allied HealthEducational Programs (CAAHEP). All eight schools aref u l ly accredited.

Concurrently, 89 exams were administered for theEMT-Intermediate with 67 passing scores resulting

in a 75 percent pass rate. For the EMT-Paramedic, 106exams were given with 58 students passing with a 55percent pass rate.

Emergency Medical Services – Driver

Mississippi requires drivers of ambulance vehicles tobe EMS-Driver certified. Driver programs must

adhere to the U.S. Department of TransportationTraining Program in Operation of Emergency Vehicles.A number of other driver programs meeting thisstandard have been approved by DEMS. Approvedprograms in Mississippi include:

• Allsafe• Emergency Vehicles Operators Course• National Academy of Professional Driving• National Institute of Fire & EMS • National Safety Council (Coaching the

Emergency Vehicle Operator)• U. S. Fire Administration (Emergency Vehicle

Driving Training)• Volunteer Fire Insurance Service.

Compliance with MississippiEMS Law, Rules, and Regulations

The compliance branch assures compliance with thestate EMS Law, Rules, and Regulations. This

includes the licensing of ambulance services andinspection of vehicles. The compliance branch licensesambulance services by their level of care. The levelsinclude:

The Invalid Transfer level offers:• At minimum an EMS Driver • Transportation of non-emergency patients.

patients in prehospital and/or interfacilitysituations.

The Advanced Life Support-Intermediate (ALS-Intermediate) offers:

Level of Ambulance ServiceLicensure in Mississippi

The compliance branch of the Division, understatutory authority of the Mississippi Annotated

Code, licenses ambulance services by location andissues permits for each vehicle operated. Licenses areissued for ground or air ambulance services. The graphillustrates the number of licensed ambulance services forFY’99, FY ‘00 and FY’01. These numbers arerepresented as fiscal years (July 1 – June 30).

In Mississippi, corporate ambulance service ownershipexceeds the number of hospital and government

owned ambulance services. This trend is similar to thenational trend towards privatization. However, 51 ofMississippi’s EMS providers are still hospital based,which include public, private, non-profit, church, andpublic-lease hospitals.

Every ambulance vehicle permitted in the State ofMississippi is inspected an average of four times

annually to ensure compliance with EMS Law, Rules,and Regulations. The total number of ambulancespermitted for the fiscal years 1999, 2000 and 2001follow:

Air service is provided through helicopter and fixedwing aircraft. Seven licensed helicopter services

provide emergency scene flights to designated areas ofthe state. Complete state coverage of emergency airambulance service has not yet been accomplished. Non-emergency coverage is available statewide through 11helicopter and two fixed-wing aircraft.

Ground Ambulance Services

Five types of ground ambulance vehicles transportpatients in Mississippi: • Type 1 ambulances are cab and chassis

ambulances with separation between the caband patient care box

• Type 2 ambulances are van type ambulances• Type 3 ambulances are cab and chassis

ambulances without partition between the caband chassis

• Invalid vehicles are stretcher vans that may beType 1, 2, or 3

• Special-use vehicles are supervisory or sprint carspermitted for emergency operation in connectionwith emergency medical service calls.

Included in the above types of ambulances areintensive care units which provide specialized services

such as neonatal and cardiac transfers. Most ambulancesare the van Type 2 ambulances.

Mississippi established the first Mississippi EMSInformation System (MEMSIS) in 1992. Every

licensed ambulance service was required by law toreport every EMS response to the Division of EMS(DEMS), either by scannable form or electronically.This data collection system was modeled after theNational Highway Traffic Safety Administration’s(NHTSA) minimum data set for prehospital providers.In 1993, DEMS added the second component toMEMSIS. This second component maintainedinformation on testing, certification, and ambulancelicensing.

In FY’01, DEMS began the implementation of the newMEMSIS. The new MEMSIS is a paperless patient

encounter form system. Software is provided by DEMSto all licensed ambulance services. EMS encounterinformation is entered into the computer locally, and isthen transmitted to the state via a modem. The newsystem minimizes errors and shortens time framesof when information is available to fill report requests. Italso allows DEMS to collect a larger amount of data,which gives a better picture of EMS care in Mississippi.

The implementation of MEMSIS began with betatesting that was conducted by American Medical

Response-South and Kings Daughters Medical CenterAmbulance Service. Concurrently, DEMS upgraded thecertification portion of MEMSIS internally. In May,2000, deployment of the new software began. As ofJuly 1, 2001, all providers are required to submit datautilizing the new system.

The new MEMSIS has several significant changes.Previously, there were two types of calls, medical

and trauma, with interfacility transfers documented asmedical-calls. Now MEMSIS recognizes four types ofcalls, medical, trauma, transfers, (which are onlybetween acute care facilities), and transports.

In FY’01, data was collected in both the old scannableform system and the new electronic system. For the

purposes of this report, the data from both systems was

integrated as much as possible. However, discrepanciesmay be apparent, but will be resolved by FY’02, whenall providers will be online with the new system.

In FY’01, Mississippi providers responded to 313,402EMS calls. This number includes all emergency, non-emergency, medical transfer, and cancelled calls. Theresponses were made by 125 ground and eight airambulance services.

Fifty-four percent of all EMS patients encounteredwere white, 40 percent were black, six percent were

Asian, American Indian, Hispanic, or Other.

As in previous years, over 50 percent occurr e db e t ween the hours of 8 a.m. and 4 p.m.

The highest call volume occurred on Friday, with 16percent of the total calls. A high percentage of EMS

calls result from medical transfers between facilities andto-and-from home. Most of these transfers occur duringthe week, resulting in higher percentages on these days.

The implementation of the 911 system has decreasedresponse times between calls received, dispatched,

and enroute. These all occur within seconds of oneanother. The longest times were spent on the scene andenroute to the final destination.

In FY’01, 37 percent of all incidents occurr e d in thepatient’s residence. Because of transfers made by EMS

providers either between facilities or from the hospitalafter being discharged, the second largest groupoccurred in the hospital.

Most of these are reported as obvious injuries toeach body region. Internal injuries are diffi c u l t

to determine in the prehospital setting; the patient,when complaining of pain to the affected area, isdocumented by EMS personnel with “pain” as the typeof injury, making this categ o ry signifi c a n t ly higherthan others. Lacerations to all of these areas arecommon in MVC’s, making this injury numerous.After pain, soft tissue wounds - along with lacerations- are the most common injuries sustained to theseareas.

Over 36 percent of all incidents originated in priva t eresidences. The age group receiving the most care

from EMS providers are those from 70 to 89. T h i sgroup uses the gr owing area in EMS — medicaltransfers conducted between medical facilities and/orp a t i e n t s ’ homes. Medical transfers decreased seve npercent, with total medical calls decreasing ten percent.Trauma calls have also decreased over FY’01. Motorvehicle crash (MVC) related calls totaled 39,647, the

Trauma is a significant public health problem inMississippi. Trauma is the leading cause of death

for all age groups from birth to age 44. It is one of themost serious social, economic and medical problems ofour time. Serious injury and death resulting from traumaevents such as vehicle crashes, falls and firearms, claim2,000 lives and disable 6,000 Mississippians each year.Trauma victims require immediate, expert attention.

Passage of legislation during the 1991 Mississippilegislative session designated the Division of

Emergency Medical Services (DEMS), Mississippi StateDepartment of Health (MSDH), as the lead agency fortrauma system development in Mississippi. Since thepassage of this law, the Trauma Care Plan for the Stateof Mississippi has been written and adopted.

In 1992, DEMS took the first steps in developing astatewide trauma system by implementing a statewide

trauma registry. The trauma registry was originallyinstalled in five regional hospitals strategically locatedthroughout the state. To date, each hospital participatingin the Mississippi Trauma System is actively collectingtrauma data and submitting it to DEMS. This dataprovides a stable foundation for the development of theMississippi Trauma System.

In 1997, the Mississippi Legislature established theTrauma Care Task Force (TCTF) to review the status

of trauma and its impact on the public’s health. The taskforce focused on three developmental areas for theimplementation of a statewide trauma system:prevention and public awareness, financial support andlegislative authority, and prehospital and hospitalstandards of trauma care. The recommendations of theTCTF were formalized into a report that was presentedto the Governor and Legislature on December 15, 1997.The report was used as a guideline for drafting andsubsequent passage of House Bill 966, “an act relatingto a statewide Trauma Care System”.

The 1998 Legislature took to heart the report given tothem by the Trauma Care Task Force and passed

legislation (HB 966) giving the Division of EmergencyMedical Services, Mississippi State Department ofHealth the authority to develop a statewide trauma caresystem. This legislation also established a permanentfunding source through a $5 assessment on all moving

traffic violations, creating the Trauma Care Trust Fund.This money is available for administrative functions atboth the state and regional levels. The legislation alsoexpanded the Mississippi Emergency Medical ServicesAdvisory Council to include trauma professionals whomake up the Mississippi Trauma Advisory Committee(MTAC).

The passage of this legislation means many things todifferent entities. Participation in the statewide

system is voluntary. Hospitals and medical staff makethe decision on whether or not to participate. If an acutecare facility decides to participate, it works inconjunction with other facilities in its region to developregional trauma plans and protocols. Prehospitalproviders receive new trauma specific training, new fieldtriage protocols, and become more involved in theevaluation of patient outcomes.

The Mississippi Trauma Advisory Committee(MTAC) was developed as a subcommittee of the

EMS Advisory Council. In 1998, it developed theMississippi Trauma Care Regulations that weresubsequently adopted by the State Board of Health inOctober, 1998. These regulations describe therequirements for regional plan development and thetrauma center designation process. They also state thehospital requirements for trauma program development,which include the entire continuum of care from injuryto rehabilitation.

The Mississippi legislature added $6 million to theTrauma Care Trust Fund during the 1999 Legislature

Session. These additional monies brought the totalamount in the Trauma Care Trust Fund to about $8million per year. Legislators authorized annual fundingfor regional support and uncompensated trauma care asdefined by the trauma registry through regionalcontracts with the Department of Health, payable fromthe fund.

The funds became available on July 1, 1999, fordesignated trauma care regions through annual

contracts with the State Department of Health, Divisionof Emergency Medical Services. The first checks weredistributed in FY’00 after hospital designations wereannounced for Level I and II trauma centers.

In FY’2001, a total of $6,538,545 was distributed fromthe Trauma Care Trust Fund for reimbursement of

uncompensated care. The fund was divided betweendesignated trauma centers and eligible physicians basedon allocation of 70% to hospitals and 30% to eligiblephysicians.

B y the end of FY 2001, the Mississippi StateDepartment of Health had provisionally designated

a total of 61 trauma centers which include two level I,five level II, twelve level III, and 42 level IV traumacenters. Additional hospitals are participating and haveapplied to be inspected in FY’02

The Emerg e n cy Medical Services for Children (EMSC)P r ogram is a federally-funded initiative designed to

reduce child and youth disability and death due to seve r eillness or injury. The first EMSC legislation was passed in1984. At present, all 50 states have received funding throughthe EMSC Program.The EMSC Program is jointlyadministered by the U.S. Department of Health and HumanS e rvices through the Health Resources and Serv i c e sA d m i n i s t r a t i o n ’s Maternal and Child Health Bureau and theU.S. Department of Tr a n s p o rt a t i o n ’s National HighwayTr a ffic Safety A d m i n i s t r a t i o n .

EMSC is the only Federal program that focuses oni m p r oving the quality of children’s emerg e n cy care. It

builds upon existing Emerg e n cy Medical Services (EMS)systems.Its goals are to ensure that state-of-the-arte m e rg e n cy medical care is ava i l a ble for ill or injuredchildren and adolescents, to ensure that pediatric service iswell integrated into an emerg e n cy medical services system,and to ensure that the entire spectrum of emerg e n cy serv i c e s- including primary prevention of illness and injury, acutecare, and rehabilitation - are provided to children andadolescents. To reach these goals, the EMSC Progr a mp r ovides grants to states to improve and enhance pediatrice m e rg e n cy care.

The Division of Emerg e n cy Medical Serv i c e s ,Mississippi State Department of Health was awarded the

EMSC Program Planning grant in August of 1998. T h egoals of the planning phase were to evaluate the current stateof pediatric care in Mississippi, to develop programs toi m p r ove the care of pediatric patients in the state, and tod evelop programs geared toward injury prevention andcommunity invo l vement in pediatric issues. This wa saccomplished by conducting a pediatric needs assessments u rvey.

As a result of this survey, two areas of need in pediatriccare were found: advanced pediatric education and

i n j u ry prevention programs. Programs were developed toaddress these needs.

DEMS was awarded the EMSC Program Implementationgrant in FY’00. This grant provided funding to

implement programs developed during the planning gr a n t .The programs implemented included advanced pediatriceducation courses and a comprehensive school-based injury

p r evention curriculum. The courses are Pediatric Educationfor Prehospital Professionals (PEPP) and Pediatric BasicTrauma Life Support (PBTLS). The injury preve n t i o nc u rriculum that was implemented was a program deve l o p e dby the National Fire Protection Association called RiskWa t c h .

PEPP represents a comprehensive source of prehospitalmedical information for the emergent care of infants and

children. It teaches prehospital professionals how to betterassess and manage ill or injured children. This course isadministered at eight sites across the State. This course hasbeen offered by the Mississippi EMSC Program toMississippi physicians, nurses, and all Mississippi cert i fi e de m e rg e n cy medical technicians at no cost to the participants.

Risk Watch is a comprehensive school-based injuryp r evention program designed for children in preschool

through eighth grade. The Mississippi EMSC Progr a mp r ovides the curriculum to any school in Mississippi thatwishes to participate in the program.

The PBTLS course is a one-day training program devo t e dto pediatric trauma care. This program prov i d e s

i n f o rmation about pediatric trauma and pediatric assessmentskills for critically injured children. The PBTLS course iso ffered by the Mississippi EMSC Program to Mississippip hysicians, nurses, and Mississippi cert i fied emerg e n cymedical technicians at no cost to the part i c i p a n t .

For FY’01, a total of 2,700 professionals were trained inboth the PEPP and PBTLS courses. Risk Watch has

been implemented in 20 schools across the State. It is thegoal of the Mississippi EMSC Program to expand thesep r ograms throughout the duration of the three-ye a rimplementation gr a n t .

In the coming years, the Mississippi EMSC Program plansto address several other areas of need as they pertain to

c h i l d r e n ’s issues. There remains much to be done to ensurechildren receive optimal medical care. Health carep r oviders, parents, careg ivers, teachers, and localo rganizations invo l ved with or interested in child health carecan play a significant role in the eff o rt to improve the carethat children receive in Mississippi.
