ELIGIBILITY

• Employees who regularly work at least 30 hours per week

• Your spouse and dependent children

• Your domestic partner and his/her dependent children

• All benefits except life insurance

Note: If you regularly work fewer than 30 hours per week, you may be still eligible for the Transamerica medical plans and some Aflac options.

WHAT’S NEW

• Annual Enrollment: 10/13 – 10/27

• Plan designs are similar to last year, with some new offerings

• This year is an active benefits enrollment for the 2017 plan year for:• Medical• Dental • Vision• Spending Accounts

• There is a new $4,500 deductible medical plan.

• In order to enroll or re-enroll, check premiums, or make changes, you’ll need to take the steps as noted in this presentation and in the enrollment materials.

• Telemedicine is available for all BCBS and UHC medical plans. This is new for Blue Cross Blue Shield Plans for 2017.

• Low on time and money for a doctor’s visit? • Talk to a live doctor or nurse by phone with

the new Telemedicine feature offered in our 2017 BCBS and UHC medical plans.

• Get medical advice, prescriptions, and more while saving an average of $60 perdoctor’s visit*.

You must enroll either during the Annual Enrollment period (Oct. 13 – Oct. 27), or if you are a new hire, during your benefits eligibility waiting period.

Coverage takes effect: January 1st for Annual Enrollment New hires become effective the first of the

month after 60 days from the end of your 30-day Orientation Period, but in no case longer than 120 days from your hire date.

SINGLE SIGN-ON INSTRUCTIONS

With single sign-on, you are now able to log on to the Mercer Marketplace using your SYKES NT/network username and password at: sykesbenefits.sykes.com

What if I lock out my SYKES NT Network password?

If you attempt to log in too many times with the wrong password and get locked out, you can:

Option 1: Wait 30 minutes for the password to unlock.

Option 2: Contact the SYKES SDC helpdesk at 1-888-556-0450.

HOW TO ENROLL ONLINE

What should I do if I still can’t get logged in?Call Mercer Marketplace at 1-855-543-4322. A licensed Benefits Counselor will log your issue

and take your enrollment via phone.

You can make changes to your benefit electionsuntil the enrollment period ends.

SUPPLEMENTAL MEDICAL INSURANCE• Supplemental medical insurance (such as Accident, Critical

Illness, or Hospital Indemnity) protects you from expenses not covered by your medical plan.

• In fact, you may be able to save money by adding a supplemental plan to a lower cost medical plan.

• Supplemental plans are available in most, but not all states.

• Coverage is available for you and your dependents.

• On their own, supplemental plans do not provide the minimum level of coverage needed to meet ACA requirements.

Here are two easy ways you can save moneyon healthcare expenses:

In-network vs. out-of-network: Before you visit a doctor, dentist, provider, or facility, make sure that the person or facility is in-network, meaning he or she (or it) is part of your health plan. If you see an out-of-network provider, your costs will be higher. You can check on this by calling the provider’s office, or by looking online. Usually, it’s safest to call.

Generic medications: Make sure that when you visit the pharmacy or call mail-order for your (or your family’s prescriptions), you ask for the generic equivalent of the medication. Sometimes, however, generic will not be available.
