Mining challenges • Mining boundary /perimeter fencing not being monitored.
• Employer not consulting with the Health and Safety.
• Safety beams not established and not well maintained.
• High walls not according to the angle of response.
• Walkways not properly demarcated.
• Poor demarcation of the crest on active benches were people are working.
• Examine and make safe not properly conducted in the quarry.
• Health and Safety representatives not trained but appointed responsible for health and safety in the work places.
Mining Challenges
• Quarry viewing points not properly demarcated with the necessary signs.
• Working plan/quarry plan not updated to cater current mining.
• No provision of life buoys at the pit sump.
Trackless Mobile Machinery• Operators of TMM’s performing repairs on TMM’s
without being trained.
• No procedure of supporting the bin of the TMM’s when
operators are cleaning the TMM’s or when repairs are
done.
• TMM haulages are less than 0,5 m between side wall and
the machines.
• TMM COP and related procedures not shaft specific.
General: Machinery & Equipment
• Poor traffic control of Tmm’s;
• Poor risk assessments conducted;
• Over inspections and PTO’s not done regularly and effectively;
• Effective guarding still a problem;
• No proper control on outside trucks and contractors coming into the mine;

• Compliance to mines TMM COP by trucks from the clients that entire the mine area to collect aggregates or sand;
• Supervision by the authorised electrician in the sub station when assistants are cleaning the sub.
• Inadequate or lack of understanding of H&S reps and Full time H&S Reps in terms of their delegated duties and responsibilities in the operations.
Occupational Health Challenges
• Submission of AMR due to non payment of the OMP
service provider.
• Failure of the employer to do recommendation
referrals as per service provider instruction.
• No follow up by the service provider instruction.
• Employees are only examined after a year or two
during periodic medicals.
Occupational Hygiene & Ventilation Challenges
• Turn over of Occupational Hygiene in small operations due to lack of payment.
• Lack of maintenance at crusher point noise and dust controls.
• Reluctance to apply dust suppression agents.
• Failure to implement contractor hygienist ‘recommendations.
• Failure to enforce PPE by supervisors.
• Only 12.1 assistants visit mine.
• Samples turn around.
