Dear Supporter,

As we move into our next year at Primary Health Care, Inc. (PHC), we reflect on the numerous accomplishments that we have made over the past year. A newly-renovated clinic in Story County, a promising program to help individuals and families experiencing homelessness in Des Moines, and a dramatic 20% increase in our patient volume throughout Central Iowa are just a few of the 2014 accomplishments we are excited about. The work that we have accomplished in 2014 demonstrates our continued commitment to imporove access through the delivery of high quality care and services to those in need in Central Iowa.

While we are enhancing existing partnerships (Mercy Medical Center, Central Iowa Shelter & Services) and working on new initiatives (Centralized Intake), our core mission is to be a team of caring professionals that provide health care and supportive services for people in Central Iowa to improve their quality of life, which we did for nearly 34,000 central Iowans in 2014.

Despite the well-documented changes in our health care system, we are striving to maintain a consistent and stable level of care. At the time of this publication, we are working on ways to engage patients online through a patient portal—giving them better access, increasing their ability to reach out to their care team and empowering them to be more proactive in their own care plans.

Of course, we cannot do this alone. In Marshalltown, in Ames, and in Des Moines, we work with dozens of community partners to refer patients, coordinate care, provide links to services and ensure that our patients receive all of the healthcare and supportive services that they need.

Your support and advocacy for PHC and for the work we do in our communities means everything to us. You know our hearts are in the right place, and we thank you so much for partnering with us and taking a look at our Community Report. The foundation of a healthy community begins with healthy people. Our team of dedicated professionals at Primary Health Care work to provide access to quality health care for each member of the community in order to improve quality of life for all.

As a nonprofit community health center, we strive to find and successfully treat medical conditions before they become serious enough to require hospitalization or emergency treatment.

We provide medical, dental and behavioral health care, pharmacy assistance and other supportive services to help patients in need. Through case management and social work advocacy, we work to assure that our clients have greater access to resources in the community.

In 2008 and again in 2010, we applied for a federal grant, but unfortunately, we were not awarded. Finally in September 2013, the third time was a charm, and we began the hard work of building a new clinic in Ames.

After months of meeting with community partners, with medical providers and with donors, our vision paid off. By early January, 2014, we had already opened a temporary clinic in Ames at 1202 Duff Ave. It was tiny and it was challenging, but we were able to slowly begin building a base of patients for years to come.

The Primary Health Care dental clinics and staff provide quality, comprehensive dental services to our communities. We have incorporated the new Affordable Care Act program policies into our practice, resulting in improved access to patients that previously faced barriers in receiving dental care.

Our clinics have extended hours to provide better access for patient appointments. Our patients appreciate the convenience of not having to miss work to come to appointments.

We also focus on children’s oral health in our clinics. When we offer our “Treasure Your Teeth” children only theme days in the summer months, we are addressing their dental needs prior to the school year, reducing missed time from

school and also making their dental visit a positive experience.

doors at the newly-renovated and spacious clinic at 3510 Lincoln Way. It boasts 12 exam rooms, a procedural room and a triage suite. We also have a certified nurse midwife, which is a first for us!

The support that we have seen in the Ames community has been incredible. We have built relationships with partners from the private sector, from nonprofits and from the local government, which means that we already have the trust and connections that we will need to be helpful and integral in Story County.

The competitive federal grant that we

received mandates that we see 4,000 patients in our first two years, and we are well on our way.

Thanks to state and federal funds, we provide more vaccines and physicals for refugee families than any other medical provider in central Iowa. Shortly after arriving in the United States, each refugee is required to receive a physical. Due to the traumatic environments refugees may come from, they may have significant health issues that remain unresolved.

Through these essential services, Primary Health Care is able to diagnose lingering health issues that continue to impact their lives. By providing refugee physicals, we play an important role in ensuring that refugees are able to work and attend school in central Iowa.

Primary Health Care is proud to partner with local refugee resettlement agencies to ensure that each refugee starts life in the United States on the right foot.

Often times our patients and clients have challenges that go beyond a flu shot or chest cold. We are proud to have a Behavioral Health Consultant (BHC) to help us to provide holistic health care that addresses physical, behavioral and emotional needs through Integrated Care.

The BHC is available the same day/time a need is identified. A key part of Integrated Care is the “Warm Handoff,” during which the medical provider introduces the BHC. The BHC seamlessly enters the patient’s visit and can help the patient and provider to address the need. This allows us to meet the patient where they access care and avoid stigma and other barriers that could exist.

We use screening tools to identify risk for depression, as well as using the SBIRT tool: Screening, Brief Intervention and Referral to Treatment. SBIRT helps patients understand the potential health risks for risky substance use patterns.

PHC leads the way in Integrated Behavioral Health Care in central Iowa, and we are proud to partner with numerous organizations on changing the way clinics, medical professionals and patients understand and access behavioral health care.

CENTRALIZED INTAKE

Centralized intake is a cutting edge program designed to streamline healthcare and supportive services for individuals and families experiencing homelessness in central Iowa.

There are many great organizations that work with the homeless population in Des Moines, but for years, there was a lack of coordination among providers and agencies. We intend to fix that.

Primary Health Care refers people to relevant agencies with the goal of reducing the number of steps an individual or family must take to receive services. The more steps it

takes to be housed -- and the less coordinated the system -- the more likely it is that an opportunity to provide services will be lost. Through the Centralized Intake system, the number of steps required is reduced and there will be an increase in stable and permanent housing in our community.

Early data suggests that our efforts through Centralized Intake are working! We are excited that 2015 could be the first year that Polk County sees data for the homeless making big movement in a different direction.

Primary Health Care’s Homeless Outreach program serves individuals and families from every walk of life. Whether that’s a veteran sleeping in his or her car or an individual who has lived in homeless camps for 20 years, Primary Health Care works to provide each client with case management and a solution to end their homelessness.

Through the Housing First approach, Primary Health Care’s Homeless Outreach team houses an average of 35 cases each month--slightly more than one each day.

We are well-above the national average in linking and retaining patients with HIV medical care. In fact, Iowa is #1 in the nation in the retention of patients in HIV care, #4 in rates of viral suppression, and #5 in linking patients to HIV medical care according to the 2014 Centers for Disease Control’s State HIV Prevention Progress Report.

Of course, prevention is also a big part of our work with HIV, and we are on the forefront of offering a promising medication breakthrough. PrEP, or PreExposure Prophylaxis, is a once a day medication taken to greatly reduce the risk of becoming HIV positive. It must be taken before coming in contact with HIV to reduce the risk of infection.

When taken properly, PrEP reduces the risk of infection by as much as 92%.

We refer an average of 10-15 referrals for PrEP per month through our testing services, and we are the only location where people can get free HIV and sexually-transmitted infection (STI) tests and a PrEP consultation prior to a provider visit. Along with our benefits specialists (who enroll folks in social safety net programs) and our pharmacy, all in one spot, we truly are a one-stop shop for sexual health!

The Project linked a record number of patients to care in 2014. This does not mean that a record number of people were diagnosed with HIV- it means that those who were diagnosed were provided access to health care and supportive services.

Our work has expansive reach in our community, with these results:

• 600 HIV tests conducted, • 85% of those tested received comprehensive STD screens during their visit, free of charge• 77 new patients received clinical services (from Ryan White Part C), which is a record

number, • A total 455 patients receiving medical case management• 96% of patients are on HIV treatment• 81% are virally suppressed
