PROCEDURE 10Issuance of Certificate of registration for the establishment of branch,

representative office (for enterprise with foreign investment capital is headquarters in the provincial city for opening branch, representative office are not associated with the investment project in the province's industrial zones)

-The sequence of execution: Step 1: Full file prepared in accordance with the law. Step 2: Filing at Phu Yen Economic Zone Authority. The civil servants receiving the files will check the validity and content of

holidays.- How to: Directly at Phu Yen Economic Zone Authority.- The composition and number of files:a)File components include:

a1) Notice of the branch or representative office. Notice shall include:

+ Name and headquarters address of the enterprise

+ Line of business of the enterprise;

+ Name of the branch, representative office planned to be established. Name of the branch, representative office to bring together enterprise name and the word "branch" for registering the establishment of the branch, the word "representative office" for registering the establishment of representative office;

+ Headquarters address of the branch, representative office

+ The content and scope of activities of the branch and representative office

+ Full name, residence, number of identity card or passport or other lawful personal identification of the head of the branch or representative office;

+ Full name and signature of the legal representative of the enterprise.

a2) A copy of the Certificate of business registration of enterprise;

a3) A copy of the company's charter for limited liability companies, joint stock companies, partnerships;

a4) Decision in writing and copies of minutes of meetings of the Council members for limited liability companies with two members or more, the company owner or Council member or the company president for the company limited a member of the Council of joint-stock company, general partner of the partnership for the establishment of branch and representative office;

a5) A valid copy of the decision to appoint the head of a branch, representative office;

a6) Land lease agreement or workshop in the province's industrial zones.(For business branch lines must have a practicing certificate must also include copies of valid practicing certificate of the head or deputy head of the branch).

b) Number of files: 03 sets (of which at least 1 original set)- Time resolved: 10 working days since the date of receiving enough valid

files.- Object execution administrative procedure: Organizations and individuals.
