This is designed to walk a student or parent through the processof completing forms for a new school or school year. Most oftenseen for students transitioning from 8th to 9th grade. (MiddleSchool/Junior High to High School)

We recommend you have both an activeemail and personal cell phone numberon record. If you don't please updatethese fields and click next.  Phone numbers and emails should bespecific to you.  It is not required but is helpful for staff in theevent of an emergency.

Changing Schools: Delete theCurrent School & begin typing thename of the School you will attend

in the Fall.  

When the Name displays below thebox, select it and click Next

Staying at Current School: If you are

staying at the same school don'tchange anything and click NEXT

Update your address and contactinformation. 

These fields can help speed up the forms processas most of this information is set to auto fill into

your forms.

If you use a Student ID # at your schoolEnter it here.  If you do NOT have or use a student ID #then click next.

Click on 'Save'If you do not save, your school selection and

other data entered will not be stored and you willnot be able to complete forms for your new

school.

Click on 'Athletic Forms'

Make sure the year '2019 - 2020' isdisplayed. If not Click on '2018 - 2019'

and change it to '2019 - 2020'.

Designate your sports of interest.

Only check sports you know you play orintend to try out for.

Click on the first form name hyperlink tobegin completing your forms for next

school year.

Complete the open form, sign and submit then follow the

prompts through the remaining forms.
