If you are currently using a meter and test strips, you will need to change the type of meter and test strips you are using. HealthPartners covers meters and tests strips that are just as safe and effective as the current blood glucose testing products you are currently using. HealthPartners preferred blood glucose testing products: Accu-Chek Guide Or TRUE Metrix What do you need to do? Review the attached flyers. Select which meter you would like to use. Next, decide if you want to work with your pharmacist or your doctor to help you get the meter. Once you’ve decided who you are going to work with, follow the steps below under “Get meter from your Pharmacy” or “Get meter from your Doctor”. Get meter from your pharmacy: Print the flyer for the meter you select IMPORTANT: Bring the flyer to your pharmacy so you won’t be charged for the meter. A pharmacist will need to call your doctor to get a prescription for the new meter and test strips. Once your doctor sends the prescription to your pharmacy, the pharmacy will give you the meter at no charge. Your diabetic supplies are covered at 100% at contracted pharmacies. Process for Switching to a HealthPartners Preferred Blood Glucose Testing Meter and Strips

We know your blood glucose testing meter and strips are important to your health. If you are currently using a meter and test strips, you will need to change the type of meter and test strips you are using.
