Process for Switching to a HealthPartners Preferred Blood Glucose Testing Meter and Strips

We know your blood glucose testing meter and strips are important to your health. If you are currently using a meter and test strips, you will need to change the type of meter and test strips you are using. HealthPartners covers meters and tests strips that are just as safe and effective as the current blood glucose testing products you are currently using.

What do you need to do? Review the attached flyers. Select which meter you would like to use. Next, decide if you want to work with your pharmacist or your doctor to help you get the meter. Once you’ve decided who you are going to work with, follow the steps below under “Get meter from your Pharmacy” or “Get meter from your Doctor”.

Get meter from your pharmacy:

Print the flyer for the meter you select

IMPORTANT: Bring the flyer to your pharmacy so you won’t be charged for the meter.

A pharmacist will need to call your doctor to get a prescription for the new meter and test strips.

Once your doctor sends the prescription to your pharmacy, the pharmacy will give you the meter at no charge.Your diabetic supplies are covered at 100% at contracted pharmacies.

Get meter from your doctor:

Print the flyer for the meter you select and bring to your doctor.

Talk with your doctor to see if Accu-Chek Guide or TRUE Metrix will work best for you.

Ask your doctor to write a new prescription for the meter you selected and the test strips.

Check if your doctor can give you a free meter at your visit.

If you can’t get a free meter from your doctor, bring the attached flyer for the meter you selected to yourpharmacy, along with your new prescription from your doctor.

Your pharmacy will provide the new meter you selected at no charge as well as the test strips at your normalcopay or coinsurance.

Please note: You can receive your free meter prior to your HealthPartners coverage effective date of January 1, 2018. You will need to wait until January 1, 2018 to receive coverage under your new HealthPartners plan for the test strips.

If you’re using an insulin pump and can’t switch to a new product, your doctor can submit a request for prior authorization from HealthPartners, and if approved, you may continue using your current test strips.
