Customer Service Award Official Entry Form Program The Customer Service Award recognizes and rewards County programs that demonstrate innovaon and/or resourcefulness in providing outstanding customer service over a period of me (at least one year). Selected programs receive a cash award and Countywide recognion. Rating Criteria All programs meeng the first two (2) elements will be evaluated for the following three (3) areas by a panel of senior managers. Qualifying Criteria 1. Program - The submission meets the following program definion: An established and ongoing set of related measures and acvies aimed at accomplishing a clear business objecve, with dedicated staff, metrics/an evaluave process, and strategic oversight. 2. Time Requirement - The program had been established for a minimum of one year and has served customers in FY218-2019. Rang Criteria 3. Customer Sasfacon & Innovaon – The program addresses a customer service delivery challenge in an innovave and data driven manner. 4. Impact & Outcomes – The program aligns with the County’s Shared Vision 2025 and posively impacts program, department, and/or County level outcomes. 5. Connuous Improvement – There are plans in place sustain exceponal customer service. Submission Process & Instructions Program submissions must be complete and no longer than three pages. Entries must be emailed to [email protected] . The deadline for submissions is September 30, 2019. STARS Award FY 18-19

The Customer Service Award recognizes and rewards County programs that demonstrate innovation and/or resourcefulness in providing outstanding customer service over a period of time (at least one year).

Selected programs receive a cash award and Countywide recognition.

Rating Criteria

Selected programs will be notified. STARS recipients will do a 2-3 minute for Executive Council and the Board of Supervisors. Awards will be presented in January/February.
