Last month, several LVHN clinicians and programs were honored at the Lehigh Valley Business Journal’s “Hometown Heroes” awards break-fast. An independent panel judged 47 finalists and chose 10 winners for their impact on the quality of health care in the Greater Lehigh Valley. The AIDS Activities Office at LVHN was the winner of the Education Hero – Organization award. This office practice, located at the 17th Street campus, was recognized for improving the community’s understanding of HIV/AIDS and playing a prominent role in efforts to prevent the disease. Valley Preferred’s BeneFIT Corporate Wellness program that serves employers and not-for-profits was a finalist in this same category.

Continued from Page 1 LVHN patient care coordinator, Kimberly Bartman, RN, won the Nurse of the Year award. She was recognized for her acts of compassion and her leadership with a variety of community-based projects focused on helping patients. Among the finalists for Physician of the Year were Eric Gertner, MD, who was recognized for his work as medical director of the Patient-Centered Medical Home program, and physiatrist Wayne Dubov, MD, with OAA Orthopaedic Specialists. Wayne was honored for serving as a valuable resource to patients and colleagues, contributing to clinical knowledge through his publications and service, and volunteering for charitable community organizations. I also got a mention for our community-based project to improve diabetes care among Latinos. LVHN’s “flu crew” was a finalist for the Innovation Hero award, representing LVHN’s annual, free, drive-through flu shot program, happening since 1999. On April 23, Emeritus Chair of Pediatrics, John Van Brakle, MD, was presented with the Inez and Edward Donley Award for Children’s Advocacy, bestowed by Community Services for Children (CSC). This prestigious award, shared this year with State Senator Patrick Browne, acknowledges “public service at the local, state, and/or national levels which has significantly improved the quality of life for children.” Beyond guiding the growth of the Pediatric Clinic and Pediatric subspecialties during his tenure, Dr. Van Brakle “was also a driving force in establishing the Lehigh County Child Advocacy Center (CAC). He has been a long-time advocate, especially for children who have suffered abuse. Dr. Van Brakle has consulted for the CAC in Northampton County and served as Vice Chair of the Children’s Coalition of the Lehigh Valley. He is a current board member of the Pennsylvania Partnership for Children. He is the consummate child advocate and always dedicated to the health, education and wellbeing of children.” (Excerpts taken from the CSC website on April 23, 2014)

Many of our medical staff clinicians give their best every day – regardless of whether someone is watching. To all of you who serve our patients and our communities – THANK YOU! In addition to the other holidays and special events already scheduled for this month, take some time to celebrate with your teammates. Have a team breakfast or lunch. Send a thank you note to an “unsung hero.” In these dynamic (and frequently challenging) times in health care, a little recognition for what’s going well can help sustain the joy we need to continue. DR. ROBERT BARRACO NAMED INTERIM ASSOCIATE DEAN FOR USF/LVHN SELECT MEDICAL SCHOOL PARTNERSHIP

SELECT, the unique medical education program established by Lehigh Valley Health Network (LVHN) and the USF Health Morsani College of Medicine (USF MCOM), has selected a new interim associate dean. Robert D. Barraco, MD, MPH, currently Associate Professor of Surgery, and Chief, Section of Geriatric Trauma, has been appointed the Interim Associate Dean for Educational Affairs.

“It’s an exciting transition for me,” Dr. Barraco says. “As a lead-ing hospital for nursing and health care, LVHN offers a brand- new type of medical school in partnership with the University of South Florida that focuses not just on teaching our students about medicine, but on providing leadership experiences, developing professionalism and establishing interdisciplinary collaboration skills they will take into their careers.”

Dr. Barraco replaces J. Alan Otsuki, MD, the founding Associate Dean for Educational Affairs, who is taking a senior associate dean position with the Frank H. Netter, MD, School of Medicine at Quinnipiac University in Connecticut.

“Dr. Otsuki was an excellent associate dean for SELECT who brought a depth of experience from his previous position with Emory University School of Medicine,” says Tom Whalen, MD, LVHN’s Chief Medical Officer. “Dr. Barraco will bring his own expertise to the SELECT program, including academic research and medical teaching experiences, and importantly, a deep knowledge of our culture here at LVHN.”

Collaborative, interdisciplinary training is a hallmark of that culture. “We have trainees in many fields, from pharmacy to pastoral care, nursing to medical students,” Dr. Barraco says. “SELECT provides an opportunity for these trainees to work together and learn together while building a patient-centered team. This is an important aspect of training that will make a difference in the care all patients will receive.”

Dr. Barraco received his Bachelor’s degree from Rutgers College, and earned his medical degree from the Robert Wood Johnson Medical School/UMDNJ in 1989. He completed his residency at Morristown Memorial Hospital in New Jersey, and followed that with a surgical critical care fellowship at the University of Maryland Medical Center in Baltimore. He is board certified in both surgery and surgical critical care, and also holds a Master’s degree in Public Health from the Bloomberg School of Public Health, Johns Hopkins University. Dr. Barraco also serves as President of the Lehigh County Medical Society.

Dr. Barraco has been a member of the LVHN Medical Staff since December, 2003, and has served in several leadership roles including Chief, Section of Geriatric Trauma; Chief, Section of Pediatric Trauma; and Medical Student Clerkship Director for the Department of Surgery.

As the Interim Associate Dean for Educational Affairs, Dr. Barraco will report to Dr. Whalen, as well as to the Vice Dean for Educational Affairs at USF MCOM, Bryan Bognar, MD. “Dr. Barraco will travel frequently to Tampa to meet with Dr. Bognar, as well as the other associate deans,” Dr. Whalen says. “He will also play a primary role when the next class of SELECT is welcomed to LVHN, and when our soon-to-be fourth-year students seek residencies to continue their graduate studies in medicine.”

“SELECT is a very unique medical school model,” Dr. Barraco says. “I’m excited to have a hands-on role helping create a group of learners who are equipped to embrace change, as well as serve as agents of change in the evolving landscape of health care.”

EPIC UPDATE

In the November, 2013 issue of Progress Notes, you were introduced to Epic, the new integrated electronic medical record (EMR) system coming to Lehigh Valley Health Network (LVHN). Epic will link all of LVHN together as one – one patient, one record, one system. When complete, a single and complete medical record for a patient will be available regardless of where the patient presents for care – in the Emergency Department, in the hospital, or at an LVPG ambulatory practice. Epic provides the opportunity to standardize care across the continuum. Epic EMR will replace dozens of other systems used by departments and offices across LVHN. A few of the new Epic applications which you may be interested in learning about include:

EpicCare Ambulatory EMR will replace Centricity Physician Office (CPO). EpicCare Ambulatory EMR is a clinical documentation and orders management system for ambulatory practices. Resolute Professional Billing (PB) and Cadence will replace Centricity Business (CB or IDX). PB is where the provider/service fees are billed in IDX. Cadence is the enterprise scheduling system.

Prelude will replace Centricity Enterprise (CE) for only LVPG. Prelude is the Epic registration system. This will focus on the demographics and insurance for Wave 1 areas only. An interface from Epic to CE is being implemented prior to Wave 2 Go-Live (August 1, 2015).

The project is well on its way and is moving on track. In just nine short months, training for Wave 1 end users will begin. At that point, you and your colleagues will take part in hands-on, scenario-based training classes to learn new workflows and processes for some (or many) of the new Epic applications. Training will include opportunities to practice with the Epic applications, both in the training classroom as well as at your own work site. All of these learn-ing opportunities will help you and your colleagues prepare for a smooth go-live on February 18, 2015. JOHN HART NAMED SENIOR VICE PRESIDENT OF MEDICAL SERVICES

John W. Hart, who has served as Vice President of Medical Staff Services for the past 30 years, was recently named Senior Vice President of Medical Services, a new position at Lehigh Valley Health Network (LVHN). In this role, John will lead five departments that focus on providing

supportive services to LVHN’s clinicians. These departments include Medical Staff Services, Physician and Executive Recruiting, Health Information Management (commonly known as Medical Records), the Division of Education and the Network Office of Research and Innovation (NORI). As LVHN creates and supports new partnerships that require senior leaders to conduct business outside our health network, this new position will ensure an LVHN leader is present to address the needs of our clinicians. This position is a natural extension of what John has accomplished throughout his career at LVHN. During his tenure, John success-fully merged medical staffs on three occasions, managed LVHN’s first physician practices, authored a medical staff development plan that assures LVHN attracts and retains the highest-quality physicians, and established an organization that allows advanced practice clinicians to collaborate with members of the medical staff. In his new role, John’s experience will allow him to lead and support colleagues who are doing work integral to our health network’s success. For example, in Health Information Management, John will support the implementation of the Epic electronic medical record transformation, as well as LVHN’s transition to ICD-10.

Leaders in each of the five departments will report directly to John. They include:

John’s record of success and proven commit-ment to LVHN and its clinicians make him uniquely qualified for this position.

CMIO UPDATE SalarTeamNotes: Impact of the ICD-10 Delay TeamNotes, the inpatient provider documentation tool, has been live at LVH-M since November, 2013. Hospital Medicine at Muhlenberg and the Medicine subspecialists have done an amazing job of acclimating to the new system. One of the primary reasons for implementing TeamNotes was to provide improved documentation and coding to support the transition to ICD-10, which was expected to begin October 1, 2014. However, embedded in the “SGR Fix” that was passed at the end of March was a change to the ICD-10 date, pushing it back to October, 2015 (at the earliest). The full ramifications of this change are still being investigated, both nationally and at LVHN. What it does mean is that the impetus to continue to implement Salar TeamNotes has been removed, as LVHN will be live on Epic Ambulatory (February, 2015) and In-Patient (August, 2015) before ICD-10 goes into effect. Consequently, Salar TeamNotes will not be implemented at LVH-Cedar Crest, and it will be limited to specific divisions at LVH-Muhlenberg. For the specialties that will continue to use TeamNotes, those notes will continue to be available in Centricity Enterprise under the DocDisplay tab.

Thoracentesis – You Want a Tube with That? The Diagnostic/Therapeutic Thoracentesis order set has been live for more than a year. There has been some confusion regarding how to order the procedure if a tube is to be left in (Pigtail vs. PleurX). The Thoracentesis order set contains orders for a procedure with no tube (done through Ultrasound), and orders for procedures in which a tube is left in place (done in Interventional Radiology). The order with tube now contains a required field to designate which type of tube is to be used (see screenshot). Using these orders appropriately should decrease confusion and potential errors.

PHYSICIAN DOCUMENTATION ICD-10 Intranet Site Now Available While lawmakers in Washington, D.C., have delayed compliance with ICD-10 until October, 2015, preparations continue here at Lehigh Valley Health Network. This article serves to introduce a valuable tool – a central-ized website for providers’ ICD-10 education divided into specialty-specific Powerpoint presentations, documenta-tion tip sheets, ICD-10 articles, and ICD-10 links.

THE RETAIL PHARMACY VIEW Continual Bad News for Acetaminophen (APAP) According to a report from UCLA based on mothers in a Danish National Cohort questionnaire, maternal use of Acetaminophen during pregnancy is associated with a higher risk for HKD and ADHD-like behaviors in children. The authors write: Because exposure and outcomes are frequent, these results are of public health relevance but further investigations are needed. This should not change the practice of APAP usage but raises questions and cautions. Some prescribers are still writing for APAP combination products containing 500mg or 750mg of the drug. While this is not illegal, the FDA has prohibited manufacturers from making these medications, so the availability will be diminishing quickly. In order to avoid phone calls from pharmacies or patients returning with the prescriptions, combo products should be written for 300mg or 325mg of APAP with Hydroco-done or 325mg with Oxycodone.

HEALTH NETWORK TO TERMINATE UPTODATE SUBSCRIPTION Following careful scrutiny and extensive discussion, Lehigh Valley Health Network has decided not to renew its subscription to the UpToDate Clinical Decision-Support System when it expires on June 30. Instead, providers are encouraged to utilize the DynaMed product, which is available throughout the organization. “It is our practice at the health network to approach everything we do with the goals of improving quality, access and cost to ensure that we are making the best decisions on behalf of our community and colleagues,” said Thomas V. Whalen, MD, Chief Medical Officer. “We feel DynaMed is superior in many aspects and will better serve us now and far into the future.” In a side-by-side comparison of key features and benefits of the two products, network officials concluded that it will be replaced by a better system:

Quality: DynaMed is fully evidence-based, provides direct links to original supporting evidentiary literature and includes both ICD-9 and ICD-10 codes. In contrast, UpToDate provides only partial levels of evidence. Access: DynaMed allows widespread network access on all campuses and office practices, including remote and mobile access. Epic integration, which is imminent and critical to our network’s future, will be available at no additional cost. However, UpToDate access is restricted to LVHN hospitals and select health centers, and the cost for mobile access is exorbitant.

Cost: DynaMed’s annual cost increase is expected to rise less than 10 percent over the next two years. The UpToDate subscription will increase by nearly 20 percent in 2015; over the coming three years, this price rise will approach 50 percent. Epic integration will further raise UpToDate’s cost significantly.

“I am aware this change will be challenging for some of our colleagues who have used UpToDate for many years and have come to depend on it,” Dr. Whalen continues. “I’m asking them to approach DynaMed with an open mind. If necessary, we will arrange free tutorials and other opportunities through the vendor to assist colleagues in learning how to use DynaMed.” Colleagues unable or unwilling to adapt to using DynaMed also will have the option of purchasing an individual subscription to UpToDate on their own. CONGRATULATIONS Stefano Camici, MD, Division of Neurological Surgery/Spine Surgery, Section of Neuro Trauma, was recently recertified in Neuro-logical Surgery by the American Board of Neurological Surgery. Dr. Camici has been a member of the Medical Staff since September, 2003. He is in practice with Neurosurgical Associates of LVPG. Daniel M. Kraus, MD, Division of Maternal-Fetal Medicine/Obstetrics, was recently certified in Obstetrics and Gynecology by the American Board of Obstetrics and Gynecology. Dr. Kraus has been a member of the Medical Staff since September, 2012. He is in practice with LVPG-Maternal Fetal Medicine. Nicholas E. Lamparella, DO, Division of Hematology-Medical Oncology, was recently certified in both Hematology and Medical Oncology by the American Board of Internal Medicine (ABIM). He is also board certified in Internal Medicine by the ABIM. Dr. Lamparella has been a member of the Medical Staff since July, 2013. He is in practice with Hematology-Oncology Associates.

P. Mark Li, MD, PhD, Chief, Division of Neurological Surgery, was recently recertified in Neuro-logical Surgery by the American Board of Neurological Surgery. Dr. Li has been a member of the Medical Staff since November, 2001. He is in practice with Neurosurgical Associates of LVPG. Niral N. Parikh, DDS, Division of Oral and Maxillofacial Surgery, was recently certified in Oral and Maxillofacial Surgery by the American Board of Oral and Maxillofacial Surgery. Dr. Parikh has been a member of the Medical Staff since September, 2012. He is in practice with Valley Oral Surgery, PC. Kevin A. Vrablik, MD, Section of Occupational Medicine, was recently recertified as a certified Medical Review Officer by the Medical Review Officer Certifica-tion Council (MROCC). MROCC certification is intended to ensure the public of quality services and the maintenance of ethical conduct by qualified physicians involved in drug and alcohol testing review. Dr. Vrablik has been a member of the Medical Staff since July, 2008. He is in practice with HealthWorks.

Another Ethics Week and symposium have come and gone. Expanding beyond the confines of a seven-day period, Ethics “Week” actually began in March with the Emerging Health Professionals Program in Ethics for local high school seniors. The topic was about hazing and resulted in a great interactive case discussion among the over 50 attendees. The week prior to Ethics Week, Dr. Robert Barraco gave Grand Rounds to the Department of Medicine. On Tuesday of Ethics Week, a joint Family Medicine-Surgery Grand Rounds began the day. The topic was “Conflicts of Interest,” delivered and moderated by Dr. Steve Lammers and Dr. Robert Barraco. The session stimulated lively discussion. Pediatrics Grand Rounds followed on “Professionalism: Lost…and Found.” On Wednesday, April 16, the 7th Annual Ethics Symposium was held at DeSales University. Their support for the program and the venue were again outstanding. The focus of the day was ethical decision-making. Over 150 attendees were present, including nursing students from area nursing schools, physician assistant students from DeSales and, for the first time, our own SELECT medical students. What a tremendous learning environment! The keynote speaker was Dr. Karen Brasel from the Medical College of Wisconsin, well known as a pioneer in palliative care and ethics education for surgical students and residents. Her lecture concerned challenging cases in ethical decision-making. Interactive breakouts were held on brain death, never competent patients and patient centered decision-making in the context of family. This was followed by a session on advance directives. Special thanks to all of our speakers who enlightened those in attendance. Also, a big thank you to the planning committee who helped to make the day a great success. The day-long offering received excellent on-site reviews and positive comments for future topics.

The interdisciplinary attendance was apparent and fed stimulating discussion, including participants from social work, pastoral care, respiratory therapy, nursing, legal, advanced practice clinicians and physicians. This year, more physicians attended than in year’s past, which was quite encouraging. So make sure to look for a “Save the Date” for next April and the 8th Annual Ethics Symposium. ADVANCED PRACTICE CLINICIANS UPDATE Meet the APC Staff A member of the LVHN Allied Health Professional Staff since November, 2006, Justin F. Cooper, PA-C, is this month’s featured Advanced Practice Clinician. As a physician assistant with Diseases Specialists for almost eight years, Justin

cares for a wide variety of patient populations and diseases. The ability to care for patients in all areas of the hospital is an aspect that has always attracted Justin to the field. He rotates through several areas of LVHN including LVH-Cedar Crest and LVH-Muhlenberg. When Justin rotates through LVH-Muhlenberg, he also completes rounding at the Good Shepherd Long Term Acute Care Hospital. As he rounds, Justin completes follow-up evaluations for patients who have consultations from the infectious disease service. Justin enjoys the team aspect of infectious disease. He works closely not only with his attendings, but also with other specialists, case managers, and nurses to provide patients with the best care experience possible. He takes pride in ensuring that the hospital staff and, most importantly, his patients find him approach-able. Communication is key, and Justin places great focus on explaining complicated conditions to patients in a way they can understand. The most common conditions the infectious diseases specialists care for include bacteremia, septicemia, urinary tract infections, pneumonia, and wound infections. In many of these situations, they must address the challenges of antibiotic selection, especially in patients with multi-drug resistant bacteria. In addition, infectious diseases specialists also address the challenges of evaluating the patient with fevers after returning from foreign travel. Sometimes these evaluations reveal the diagnosis of malaria or other tropical illnesses. To prevent such illnesses associated with foreign travel, Justin along with several physicians in his group, provide outpatient pre-travel consultation appointments to discuss vaccinations that are required or recommended for their patient’s trip. Additionally, at these appointments, they will counsel patients regarding malaria prophylaxis, if necessary, or other prophylactic treatment and education. Justin is a graduate of DeSales University where he earned a Bachelor of Science in Medical Studies and a Master of Science in Physician Assistant Studies. To give back to the program, Justin introduces first year students to the clinical world, and he periodically presents infectious disease and travel medicine lectures to DeSales University physician assistant students as part of their didactic training. In a leadership role, Justin serves as the medicine subspecialty representative on the LVPG Advanced Practice Clinician Advisory Committee.

Justin recently married the love of his life, Annie, who is a nurse at LVH-Cedar Crest. The couple recently purchased a house in Lower Macungie, and Justin is thrilled with his quick five minute commute to the LVH-Cedar Crest site. He enjoys bike riding and hanging out with his friends, and he also enjoys shooting sporting clays at Lehigh Valley Sporting Clays.

APC Elections At the quarterly meeting of the Advanced Practice Clinicians (APC) held on Thursday, March 27, members of the APC staff elected Wendy J. Prutsman, CRNP, as President-elect of the Advanced Practice Clinicians. In addition, Matthew J. Ottinger, PA-C, was appointed by the Chair of the Department of Surgery to serve as a departmental representative on the APC Executive Council.

Wendy J. Prutsman, CRNP, a nurse practitioner with LVPG-Maternal Fetal Medicine, has served as Secretary/Treasurer of the APC Executive Council since May, 2010. Wendy is a graduate of Pennsylvania State University where she earned her Bachelor of

Science degree in Nursing. She also received her Master of Science degree in Nursing from the University of Pennsylvania. Wendy has been a member of the APC staff since November, 1997.

Matthew J. Ottinger, PA-C, is a physician assistant with VSAS Orthopaedics. He earned a Bachelor of Science degree in Medical Studies and a Master of Science in Physician Assistant Studies from DeSales University. Matt has been a member of the APC staff since November, 2008.

UPCOMING SEMINARS, CONFERENCES AND MEETINGS General Medical Staff Meeting Due to the overwhelming success of last year’s presentation ceremony of the educational teaching awards at the June General Medical Staff meeting, this year’s teaching awards will again be presented at the General Medical Staff meeting.
