Situations lead to Project Arrive Alive

Two separate Incidents involving Hauls Trucks occurred in 2016

Micro Sleeps in Haul Trucks

What Project Arrive Alive Incident One:

Truck 414 reached top of the main ramp and turned left heading to the ROM. Operator appears to have had a micro-sleep and truck has contacted the windrow.

What is Project Arrive Alive Incident 2:

While being operated and travelling down the main North Ramp into E42 Stage G pit, truck 431 has run off the haul road and has

climbed the base of batter face before returning unto the haul road. Operator has reported afterwards as having had a micro

sleep.

Incident 2: From the investigation it became

evident that the operator had potentially driven for

450m down the ramp while asleep.

Worker Fatalities Statistics As at 23 September 2016 – 125 Australian workers have

been killed at work in 2016 From the 125 – 4 are from within the Mining Sector In 2015 – the Australian Mining Sector had 12 Work-

related Fatalities Over the 2003 to 2014 period, 65% of worker fatalities

involved incidents with vehicles. Of the vehicle-related incidents, almost half (49%) occurred on a public road

Effects of fatigue Fatigue affects the ability to think clearly. As a result, people

who are fatigued are unable to gauge their own level of impairment.

Health effects, when the circadian rhythm is disrupted, the treatment of some medical conditions can be affected eg Asthma, Depression, Diabetes

Circadian disruptions affect eating and sleeping habits and have been linked to Heart Attacks, High Blood Pressure, Blocked Arteries

Mental Health - Anxiety and depression can be triggered or made worse

Micro Sleeps - People who have micro sleeps are not always aware when they occur � this can have a significant affect on safety

Reducing the risks associated with fatigue-related

incidents Ensuring – Maximum 14 hours Travel / Work / Travel Education sessions for all employees and their families

to understand the dangers of fatigue Introduction to fatigue management equipment and

improved methods to identify fatigue

1 What does it mean for the

Employees All Employees were required to complete a Usual Place of Residence Form

by choosing one of the options as below.

□ Local Employee: is a Shift Employee whose Usual Place of Residence whilst on shift is within a 45 minute (Drive) radius of Cowal Gold Operations.

□ Local Employee: is a Day Employee (10 hour Day Worker) whose Usual Place of Residence whilst on roster is within 1:15 hour radius (Drive) of Cowal Gold Operations.

□ Non Local Employee: is a Shift Employee whose Usual Place of Residence whilst on shift is outside the 45 minute radius (Drive) of Cowal Gold Operations.

□ Non Local Employee: is a Day Employee (10 hour Day Worker) whose Usual Place of Residence whilst on roster is outside the 1:15 hour radius (Drive) of Cowal Gold Operations

Declaration of Usual Place of Residence determined employees your right to entitlements either as a Local or Non Local Employee for the purposes of residing during the Shift / Roster at Cowal Gold Operations.

1 Initiatives implemented: Private Vehicle Travel to and From Site Policy

Private vehicles are not permitted to travel to and from

Cowal Gold Operations (CGO) unless an Essential Driver Authority or similar exemption is in place.

Company provided transport to and from site is to be utilised

where possible. Contractors are also expected to provide transport for their employees.

A short term health and wellness program rolled out using Matecheck

A holistic psychological and behavioural approach to wellbeing.

The program covers:

1 Initiatives Implemented: Health and Wellness Program to be rolled out Supply of Fitbits to all employees to enable participation

in Sleep Monitoring Program

Fitbits have the ability to sync to a computer and mobile device, which allows for the user to view their progress towards a set goal

Through MateCheck we provided psychological

counselling for any employee and family members Through an agreement with a local medical clinic the

psychologist utilised a spare consultation room Counselling was also available on site

This service was for 2 ½ days per week 74 people utilised the service over an 18 week period

Initiatives yet to be rolled out: Healthy Business Group – 12 month contract

Health Business physiologist on site 5/2- 4/3 roster They identified key areas that the program will focus on: • Sleep & Fatigue • Musculoskeletal disorders • Stress / mental health • Obesity • Cardiovascular disease • Diabetes • Poor nutritican • Lack of physical exercise • Smoking cessation • Alcohol consumption
