DearFriends,In2001,IarrivedinHaitiforthefirsttimeasaPeace Corps Volunteer. I knew very littleabout the country that I was about to callhome for next 24 months or the amazingpeople who were about to become myfamily.WhatIdidknowwasthatIwantedtohelpHaitiinanywaythatIcould.Oneofthefirst,andmostimportant,lessonsIlearned in the Peace Corps was theimportance of empowering people to helpthemselves and takeownership of efforts toimprove their communities. I have carriedthat lessonwithmethroughoutthe16yearsI’veworkedinHaiti,andit’swhyI’msoproudof the work that Project Medishare doesevery day to empower Haitians to provideandreceiveaccesstohealthcare.October2016broughttheunexpectedarrivalofHurricaneMatthew,whichcausedmassivedestructionand lossof life inHaiti.Withourpastexperiencerespondingtocrises inHaiti,Project Medishare was able to immediatelyprovidesupportviaafieldclinic inJeremie–one of the areas hardest hit by the storm.Throughpartnerswithsimilarmissionstoourown,wehavestartedtohelpfamiliesrebuildtheircommunitiesandtheirlives.

OnOctober4,2016HurricaneMatthewrippedthroughHaiti,causingmassivedestructionandleadingtothecountry’sworsthumanitariancrisissincetheJanuary2010earthquake.Windsof130–145milesanhour,stormsurgesandheavyrainscausedcatastrophicdamagetohomes,agriculturalcrops,livestock,roadsandbridges.Itisestimatedthatmorethan
1,000peopledied,andhundredsofthousandsmorewerelefthomelesswithlimitedaccesstofood,cleanwaterandmedicalcare.

IntheimmediateaftermathofHurricaneMatthew,ProjectMedishareprovidedover110tonsofsuppliestoaffectedfamilies,includingwaterpurificationtablets,mosquitonets,medications,medicalsupplies,andfoodandhygienekits.Inaddition,ourmedicalstaffandvolunteersdeliveredcaretomorethan4,000patients,treatingavarietyofconditionsfrominfectedwoundstofeverstospinalcordinjuries.
IntheCentralPlateau,thehurricanealsoaffectedProjectMedishare’scommunities,thoughtoalesserextent.Strongwindscauseddamagetoafewhomes,andflashfloodingwipedoutcropsandtriggeredsmalllandslides.Withanticipatedmigrationfromstorm-affectedcommunitiestootherareasofthecountry,alongwithconcernsaboutariseinwaterbornediseases,wetookmeasurestominimizerisktoourcommunities.Initiativestoimprovewater,sanitationandhygieneinfrastructure,andre-educatecommunitiesandhealthworkersaboutthethreatofcholerabeganinNovemberandcontinuedthroughtheendoftheyear.

PartneringtoRebuildLives
ProjectMedisharehaspartneredwithtwolocalorganizations,SmallholdersFarmers Alliance (SFA) and Caris Foundation,to provide support to morethan1,000familiesintheGrand’AnseandSouthdepartments.Through our partnership with SFA, 800 farm families across eightcommunities near Les Cayes have been selected to participate in anagricultural recovery project.Each farmer received the seeds, tools andtrainingneededtomaketheirfarmsproductiveagain.Thefarmerswillalsoestablish and operate tree nurseries, which are expected to generate aminimumof40,000treesin2017.ProjectMedisharewillprovidecommunityhealthservicestoimprovethehealthoffamiliesenrolledintheprogram.With a grant from ProjectMedishare, Caris Foundationwill launch a pilotpoultryfarmingprojectinthehard-hitcommunityofJeremie.Aspartoftheproject,250vulnerable familieswilleach receive10egg-layinghens,alongwithtoolsandtechnicalsupport.Thechickenscanproduceupto70eggsperweek,whichthefamiliescanconsumeorselltogenerateincome.
ReachingtheRavine
Duringthehurricane,apieceofsteelroofingblewinto14-year-oldMaxo,leavingadeep,gapingwoundinhiscalf. A week passed before he could visit the field clinic in Jeremie where Project Medishare helped treatpatientsinjuredbythestorm.Thewoundbecameinfected.Afterhavinghiswoundcleanedandstitched,Maxowalkedhomefromtheclinicthroughstreets litteredwithfallentreesandotherdebris.Hegrimacedinpainalongtheway.Whenhefinallyreachedhishousenearalargeravine,hisgrandmotherwasoutfrontwashingclothes.“TheseclothesI’mwashingaretheonlythingswehaveleft,”shesaid.Thewindtookpartofourroof.Therainruinedthefewthingswehad,andtherockstookhalfourwalls.Wehavenothing.”KnowingthatMaxowasabletoreceivefreemedicalcareliftedoneofhermanyburdens.

EncouragingSelfCareforBusyMoms
Forty-one yearold Luciani andher9-month-old sonwerepatientsatamobileclinicintheremotevillageofPalmary.Themotherofsixregularlyvisitsourmobile clinics– theirprimary sourceofhealthcare– tomakesureherchildrenarehealthyandup-to-dateontheirvaccinations.Asarecentwidowwhosehusbanddiedduringwhileshewaspregnant,lifehasbecomedifficultforLucianiandherchildren.Withherfocusonthehealthandwell-beingofherchildren,sheneglectedherownhealthin the process. Luciani came to theclinic suffering fromneckand backpain;theeffectsofthehardphysicallaborsheenduresasafarmer.Shealsocomplainedofheadaches.The doctor determined that Luciani was dehydrated – a commonconditioninHaitiforbreastfeedingmoms.Totreatherdehydrationandpain, she was provided with oral rehydration salts, nutritionalsupplements and pain relievers. She also received an importantremindertocareforherselfthesamewayshecaresforherchildren.
PreventingtheSpreadofCholera
After several days of heavy rains following Hurricane Matthew, the community of Lahoye experienced acholeraoutbreak.Fourteencaseswerereported.Twopeopledied:a10-year-oldgirlandan80-year-oldman.ProjectMedisharestafftransportedtheother12peopletoacholeratreatmentcenterinThomonde.Our community health team immediately implemented a series of activities to contain the potential for awidespreadcholeraoutbreak.Thisincludededucationalactivities, theinstallationofhandwashingstations,andthedistributionofwaterpurificationtablets,oralrehydrationsaltsandchlorineproducedin-house.ProjectMedisharehasbeenproducingchlorinesince2010. It isdistributed throughoutourcommunities toprovideschools,familiesandhealthfacilitieswithaccesstoclean,safewater.

AMother’sDream
It’s3o’clockonaFridayafternooninMarmont,Haiti,aruralcommunity60milesandathree-hourdrivefromPort-au-Prince.ThemidwivesatProjectMedishare’smaternal health clinic are eagerly awaiting the arrival of thenextbaby.Ononeofthebedslies29year-oldClautilde–amotheroftwochildrenages11and9.She’sinlaborwithherthirdchild.Clautilde lives in the Dominican Republic with her boyfriend and twochildren.TheywerevisitinghermotherinHinchewhenClautildestartedtofeel her first contractions. At hermother’s suggestion, she took an hour-longmotorcycleridefromHinchetothematernitycenterinMarmont.It didn’t take long for Clautilde’s contractions to become stronger, longerand closer together. Project Medishare’s midwives were at her bedside,holdingherhandandkeepinghercoolwithacoldcompress.Ataround8o’clock,Clautildedeliveredahealthybabyboy.Thenextmorning,Clautildewasupandabout,fullofenergyandexcitedtoshow off her newborn. When her boyfriend arrived at the clinic, a hugesmilespreadacrosshisfacewhenhesawhissonforthefirsttime.BeforereturningtoHinche,Clautildeproudlyexpressedherdreamsforherchildren,includingherthoughtsonwhatmakesagoodmother.“ThewayIeducatemychildren,sothattheyareabletoknowhowtoactandbeapartofsociety,makesmeagoodmother. Iwilldoeverythingforthemtobeabletogotoschoolandhopefullylater,touniversity,”shesaid.
ExpandingRuralMaternalHealthServices
In2012,ProjectMedishareopenedamaternitycenterinMarmont,providingthousandsofwomeninthesurrounding communities with convenient access to prenatal and postnatal care, delivery services withtrainedpersonnelandfamilyplanningcounseling.Priortothis,manywomenhadtotravelmore thananhourawaytoreceivetheseservices–iftheycouldaffordthem.Inthefiveyearssincethematernitycenter’sopening,demandfor itsserviceshasgrownsignificantly.Onaverage,46birthspermonthtookplaceatcenterthroughout2016.Severalofthewomenwhochoosetodeliver theirbabiesat thecenter live invillages inMarmontandnearbyThomonde.Others travel longerdistancesfromcommunitiesincludingLahoye.Tomeet the growing demand for ourmaternaland reproductive health services, and improveaccess totheseservicesforthousandsmorewomen,ProjectMedisharestartedconstructiononasecondmaternitycenter inLahoye.Theopeningofthenewmaternalhealthcenterwill reducethenumberofhomebirthsthatstilloccurintheLahoyecommunityduetoitsremotelocation.TheLahoyematernitycenterisscheduledtoopenSpring2017.

EmpoweringMomstoRaiseHealthyChildren
“Health is important to life,” said 26-year-old Guerline while standingoutsideofourhealthclinic inLahoye. Shebroughther3-year-olddaughterGuerlandie Camille to the clinic for an examination. Guerlandie wascomplainingofastomachacheandhadafever lastingseveraldays. It tookthe mother and daughter 90 minutes by motorcycle taxi to arrive at theclinic–thenearesthealthfacilitytotheirremotevillageofIkak.Guerline is amotherof four. Shegave birth to her first child, a boy,at12yearsold.Shehadhersecondchildat14,thethirdat17,andtheyoungest,Guerlandie,at23.Withthemeagerearningsshereceivessellingpeanuts,ithas become increasingly difficult for her to care for her children and sendthemtoschool.Despite,orperhapsbecauseof, thosechallenges,Guerlinefeels a sense of pride in being able to take her children to see a doctorwheneverneeded.
NourishingHaiti’sFuture
During the2015-16academic school year,ProjectMedisharepiloteda school lunch programbenefitting496students.Theprogramprovidedawonderful resource forparentswhodidnothave themeans to sendtheirchildrentoschoolwitha lunch.Moreover, itservedasan incentiveforchildrentoattendschoolonaregularbasis,improvingeducationaloutcomes.Duetothesuccessofthepilot,theprogramwasexpandedtoanadditionaltwoschoolsforthe2016-17schoolyear,nearlydoublingthenumberofstudentswhoreceiveadaily,nutritiouslunch.Formanyofthesestudents,it’stheonlymealtheyeatallday.Wealsoemploywomenfromthecommunityascooks,andsourcefruitsandvegetablesfromlocal“MadameSara’s”,extendingthebenefitsoftheprogram.

DevelopingtheNextGenerationofPediatriciansWithfewtrainedpediatriciansinHaiti,manychildrendonotreceivethecaretheyneed,resultingindeathratescausedbyeasilytreatableandpreventableillnesses.In2013,ProjectMedishareinpartnershipwithSaintDamienPediatricHospitalandNotreDameUniversityMedicalSchoolinHaiti,launchedapediatricresidencyprogramatHospitalBernardMevs.Thethree-yearprogramtrainsresidentstobecomespecialistsinthecareofchildrenages0to17.It’soneoffewpediatricresidencyprogramsapprovedbytheHaitianMinistryofHealth.
Inthefallof2016,afterpresentingtheirfinalresearchprojectsatasymposiumhostedbytheHaitianSocietyofPediatrics,thefirstgroupofpediatricresidentsgraduated.Allsixwereabletofindemployment,includingDr.FrantzyDieudonnewhowashiredbyProjectMedishare.Dr.Dieudonnehasalreadymadeabigimpactinthecommunitiesweserve.HehasfilledacriticalneedinruralHaitiandhelpedProjectMedishareachieveourlong-timegoalofprovidingspecializedcaretochildren–allthankstothesuccessofpediatricresidencyprogram.

Dr.SamAbelsonisanEmergencyRoomdoctorfromMinnesotawhoispassionateaboutHaiti.Hehasbeenalong-timevolunteerwithProjectMedishare.Since2010hehasvolunteeredatHospitalBernardMevsinPort-au-Princeabout 20 times. He has also volunteered with us in the Central Plateau and in Jeremie following HurricaneMatthew.During one of his2016 trips, Dr. Abelson sharedhis firsthandperspectiveon the transformation he’switnessedatHBMoverthelastseveralyears.“Overall, I have seen dramatic improvement in the care and progress at the hospital. When I first arrived [inNovember2010],wewerestillindisastermode.TherewereveryfewHaitianDoctorsornursesavailabletocarefor patients. Since that time, things have improved dramatically,” he said. “The EMTsatHBMare secondary tonone anywhere I have worked. The nurses are outstanding, all over the hospital, and many have developedexcellentcriticalthinkingskillsthroughtheirexperiencesandthroughoutstandingleadership.”Hecontinued:“WhenIbegan,therewasnotasingleERdoctoratHBMcapableofstaffingtheERalone.Onmytrip[in August 2016] the ER is now staffed 24/7 with excellent, kind caring and compassionate Haitian physicians,capableofrunningtheER24/7withnovolunteerbackup.Thatistremendousprogress!”
PartnershipsthatBuildSkillsandKnowledge
It wasmore than 20 years agowhen the first group ofmedical students and faculty traveled to Haiti’s CentralPlateauwithProjectMedishare.Sincethen,thousandsofmedical students have helped us deliver healthcare tovulnerablefamilies.Through our partnerships with universities, medicalstudent groups, under the supervision of licensedmedical professionals and faculty, spend at least oneweekworkingalongsideourteamatmobileclinics.Asanextensionofourteam,medicalstudentvolunteershelp increase the number of patients we treat, andprovideinvaluablelearningopportunitiesforourstaff.
