THRESHOLD DECISION MEMO AND SUMMARY OF FINDINGS

PURPOSE AND SCOPE OF THE REQUEST FOR CATEGORICAL EXCLUSIONThis is a Categorical Exclusion Request for an activity entitled “Supporting the Electronic Health Information Systems (eHIS) in Ethiopia”. USAID supports the Ethiopian HIS in order to contribute to the countries information revolution which supports both our countries goals of improving the quality of health service, improving people's quality of life and building a sustainable health care system through use of data for decision making.

PROJECT/ACTIVITY SUMMARY The objectives of the activity are summarized into four main objectives listed below:

Objective 1: Support Information Technology Systems and Data Repository: Under this objective the contractor will support the digitalization, integration and scale-up of priority health information systems at all levels (eHIS). Work under this objective will be

o Based on Ethiopia’s eHealth Architecture (Appendix 1.0) support the Government of Ethiopia (GOE) to prioritize, digitize and scale-up eHIS components.

Objective 2: Build a Culture of Data Use: The contractor will work with GoE to improve use of data to inform health planning, performance management, and delivery decisions across the health systems.

Objective 3: Capacity Building and Governance: The contractor will work on effective transfer of knowledge and skills and to GoE and building the MoH capacity to facilitate adoption by government agencies and implementing organizations of identified best practices in data use, data system architecture, and HIS governance.

Objective 4: Stakeholders and Donors Collaboration: The contractor shall collaborate with other donors and various stakeholders within GoE to increase commitment by global stakeholders and MoH to align and invest in integrated health information systems and tools, strengthen data use, and health workforce capacity at the country level as key strategies necessary to achieve improved health outcomes.

ENVIRONMENTAL DETERMINATIONS Categorical Exclusions are recommended for the following classes of activities, except to the extent that the activities directly affect the environment (such as construction of facilities). Specifically, this is for activities covered by the following citations in Reg. 216, subparagraph 22 CFR 216.2(c)(2):

(a) §216.2(c)(2)(i) Education, technical assistance, or training programs except to the extent such programs include activities directly affecting the environment (such as construction of facilities, etc.)

meetings;

(c) §216.2(c)(2)(v) Document and information transfers;

(d) §216.2(c)(2)(xiv) Studies, projects or programs intended to develop the capability of recipient countries to engage in development planning, except to the extent designed to result in activities directly affecting the environment (such as construction of facilities, etc.)

Upon approval of this document, the determinations become affirmed, per Agency regulations (22CFR216).

CLIMATE RISK MANAGEMENT This is a capacity building activities with LOW Climate Risk (Annex 1). Activities with LOW climate risk are exempted from developing climate risk management.

BEO SPECIFIED CONDITIONS OF APPROVALN/A

IMPLEMENTATIONIn accordance with 22CFR216 and Agency policy, the conditions and requirements of this document become mandatory upon approval. This includes the relevant limitations, conditions and requirements in this document as stated in Section 3 of this RCE and any BEO Specified Conditions of Approval.

1.0 PROJECT AND ACTIVITY DESCRIPTION1.1 PURPOSE AND SCOPE OF RCE The purpose of this document is to establish that all proposed activities belong to classes of actions eligible for Categorical Exclusions as set out in Agency regulations (22CFR 216.2(c)) and that there are no foreseeable significant direct or indirect impacts that would preclude them from receiving a Categorical Exclusion. Upon approval of this document, the Categorical Exclusions are affirmed for the project/activity. This analysis also documents the results of the project/activity level Climate Risk Management process in accordance with USAID policy (specifically, ADS 201 mandatory reference 201mal). This RCE is a critical element of a mandatory environmental review and compliance process meant to achieve environmentally sound activity design and implementation.

1.2 PROJECT OVERVIEW USAID supports the Ethiopian HIS in order to contribute to the countries information revolution which supports both our countries goals of improving the quality of health service, improving people's quality of life and building a sustainable health care system through use of data for decision making. Despite extensive investment in the national HIS, Ethiopia faces challenges to realizing the Information Revolution roadmap (IRR) objectives linked to achieving the Health Sector Transformation Plan (HSTP)goals.

1.3 PROJECT DESCRIPTIONThis activity is to support GOE’s electronic information systems and will achieve the objectives above by focusing on the following:

a. An environment of innovation and collaboration to develop and implement solutions for electronic health and health-related data capturing, processing, accessing, sharing, and utilizing; electronic health data systems which are standardized, harmonized, and authenticated..

b. Create strong standards, protocols, and management that enhance institutional capacity and collaboration mechanisms to oversee the functionality and improvement of the national HIS. This includes supporting development and implementation of organizational and career structure for Health Information Technology Directorate (HITD) and the Policy and Planning Directorate (PPD).

c. Develop tailored electronic platforms (e.g., DHIS2, electronic logistics MIS (eLMIS)/HCMIS, eHRIS, electronic laboratory IS (eLIS), electronic community health information system (eCHIS), electronic integrated financial management IS (eIFMIS), mHealth solutions, Geographic Information System (GIS) enabled Decision Support Systems) integrated as standards in the public sector, with increasing private sector coordination to integrate with national HIS.

d. Establish health data repository for the Federal Ministry of Health (FMOH) and stakeholders to access, as appropriate established.

e. Develop a functional mechanism for ensuring interoperability among electronic HIS sub-systems in the country through relevant technical interoperability services functioning.

f. Enable the GOE, working closely with the Vital Event Registration Agency (VERA) realize its priority of creation of a functional birth and death registration/notification system. The system will be part of eCHIS and integrated or interoperable with the DHIS2 system.

g. Develop and implement sustainable in-country capacity building mechanisms with resources identified for their support beyond the life of the project. These could include:

h. Enhance the Ethiopian ICT education capacity through local universities and Technical & Vocational Education & Training Centers (TVET). This could link to the iHRIS system to track trained personnel.

i. Strengthen Bachelor and Masters-level training using standardized national curriculum in competencies to innovate, develop, apply, and maintain electronic solutions.

j. iii. Create and provide grant to a community of users, developers, academic institute, and youth groups for the open-source solutions to maintain source code, further develop the tools, provide technical support and trainings on the different platforms.

2.0 ENVIRONMENTAL ANALYSIS2.1 JUSTIFICATION FOR CATEGORICAL EXCLUSION The activities under the Supporting the Electronic Health Information Systems (eHIS) in Ethiopia are among the classes of actions listed in 22CFR216.2(c)(2) and have no foreseeable significant direct or indirect adverse effect on the environment. Therefore, under 22CFR216.2(c)(1), neither an IEE nor an EA will be required for these activities. Instead, a Categorical Exclusion is recommended for the projects/activities described above in Section 1.3 as follows:

3.0 LIMITATIONS OF THE CATEGORICAL EXCLUSION

The categorical exclusions recommended in this document apply only to projects/activities and sub-activities described herein.

Other projects/activities that may arise must be subject to an environmental analysis and the appropriate documentation prepared and approved, whether it be a new Request for Categorical Exclusion, an amendment, or other type of 22CFR216 document.

It is confirmed that the projects/activities described herein do not involve actions normally having a significant effect on the environment, including those described in 22CFR216.2(d).

3.1 MANDATORY INCLUSION OF ENVIRONMENTAL COMPLIANCE REQUIREMENTS IN SOLICITATIONS, AWARDS, BUDGETS, AND WORK PLANSUSAID will ensure the environmental compliance requirements are incorporated into solicitations, awards, budgets, and work plans, including relevant limitations of Section 3 above. In addition, climate risk management requirements will also be incorporated.
