law is eligible to become and is located in the city of Chicago can become a Chicago VFC provider.
• You do not have to be a Medicaid provider to participate in Chicago VFC.

recommended vaccines) – FQHCs only.

Chicago VFC – Why?
• Chicago VFC
• The measles epidemic of 1989-1991 in the U.S. resulted in tens of thousands of cases of measles and hundreds of deaths. CDC found that over half of the children who had measles had not been immunized, even though many of them had seen a health care provider.
• Providers
• Keep your patients in their medical home.
• Reduce your up-front costs.
• Help provide quality care to vulnerable children and adolescents.
• Children 18 & younger
• There is no charge for vaccines* given by a VFC provider to eligible children.
• VFC-eligible children and adolescents receive free vaccines that help prevent up to 16 vaccine-preventable diseases.
*Other costs associated with the vaccination may apply (office visit, administration fee, etc.)
