** During the third calendar quarter (July/August/September) of 2018, perform Reduced/Triennial monitoring for TTHMs and HAA5s under Stage 2 of the Disinfectants and Disinfection Byproducts Rule by collecting 1 dual sample set. A dual set of samples must be collected at the location (and during the specific month) identified in your DEP approved Stage 2 monitoring plan.

*** Collect one full set of SOCs (all 29 contaminants) during the second calendar quarter (April/May/June) of 2018. This will then also count as your Annual sample for the SOC of Dalapon which must be collected during the second calendar quarter of 2018.

**** Collect Start-up Bactis at two locations in the distribution system for each of two consecutive days and perform a 10 sample well survey prior to placing the Plant or the Well in service.

***** Collect a sample for the Radiological contaminant of Uranium only, sometime during the 2018 calendar year.

****** Collect samples for Lead and Copper (in accordance with your approved sampling plan) sometime between June 1 and September 30, 2018. Submit the analyses results to the DEP by no later than October 10, 2018.

* During the third calendar quarter (July/August/September) of 2018, perform Annual reduced monitoring for TTHMs and HAA5s under Stage 2 of the Disinfectants and Disinfection Byproducts Rule by collecting 1 dual set of samples at the 517 Beachcraft Lot 171 location.

** You are required to collect one full set of samples for all of the SOCs (except Dioxin) during the 2018 calendar year. However, if you submit a Reduced Monitoring Application Questionnaire for Synthetic Organic Contaminants to the Department, available at: http://www.dep.state.fl.us/water/drinkingwater/forms.htm, you may receive Department approval to either fully or partially waive monitoring for SOCs in 2018. If you do not receive approval to waive any of the required monitoring for SOCs for 2018, or do not submit the Questionnaire, you will have to perform one full set of monitoring for SOCs in 2018.

*** Please note that each water system is required to develop and adhere to a written RTCR sample siting plan that is subject to State review and revision.

**** Collect samples for the Radiological contaminants of Gross Alpha, Uranium (conditional), Radium 226, and Radium 228, sometime during the 2018 calendar year.

***** Collect samples for Lead and Copper (in accordance with your approved sampling plan) sometime between June 1 and September 30, 2018. Submit the analyses results to the DEP by no later than October 10, 2018.
