2 PREPARATION AND DATAUPLOAD
2.1 About SRS Registry
Background Information
The Stereotactic Radiosurgery Registry (SRS Registry) is a joint project of the AmericanAssociation of Neurological Surgeons (AANS), NeuroPoint Alliance (NPA) and Brainlab. It hasbeen developed to collect data on the SRS treatment of:• Brain metastases• Benign brain tumors• Arteriovenous malformations (AVMs)
The registry objectives are to:• Define national patterns of care in radiosurgery• Improve healthcare outcomes• Support informed decision making
The AANS created the NPA as a not-for-profit corporation to coordinate projects involving theacquisition, analysis and reporting of clinical data.
Overview of Data Collection
Data fields for the SRS Registry are defined by a scientific committee organized by the NPA. Theregistry collects data on various patient events, such as SRS/SRT treatments, WBRT, surgicalresections and follow-up examinations. The collected data includes:
SRS/SRT Treatments Patient Baselines and Follow-Up
• Lesion volumes and locations• Dose distribution for treated lesions and or-
gans at risk (OARs)• Dose fractionation• Treatment device configuration details
• Lesion volume changes and recurrence• Neurological examination findings• Patient quality of life measures• Adverse events• Survival rates
For further information on data collected, refer to the NPA Stereotactic Radiosurgery RegistryData Dictionary.
How Data is Collected
Quentry and Elements technologies facilitate collection and automated structuring of this complexdata, reducing data entry errors. The process of collecting clinical and imaging data is outlined below.
PREPARATION AND DATA UPLOAD
Software User Guide Rev. 1.1 Quentry for SRS Registry Ver. 3.1 15
Step
1. Depending on the event type, data is uploaded to Quentry for SRS Registry:a) SRS/SRT and WBRT treatment data is uploaded via your treatment planning systemconforming to DICOM standards.b) Tumor outlining for surgical resection and follow-up events are uploaded via the CranialContouring workflow or a third-party planning/contouring system. Surgical resection andfollow-up events are uploaded either via the export function of Patient Data Manager ordirectly from the third-party planning/contouring system.
2. Quentry for SRS Registry is used for entry of clinical data using web-based case reportforms, image data enrichment and processing, and storage.
3. The registry data is aggregated, de-identified and reported to the NPA for analysis andquality control. It is available for export or analysis using Analytics or Patient Dash-board.
About SRS Registry
16 Software User Guide Rev. 1.1 Quentry for SRS Registry Ver. 3.1
2.2 Data Requirements
Background Information
Data must be prepared in a specific manner and meet certain criteria for upload to the SRSPatient Registry. You must enter certain parameters into Quentry for SRS Registry, such asNumber of Fractions and Date of Treatment. Therefore, it is recommended to have a printoutfrom the treatment planning system available.NOTE: Certain fields may be automatically populated and will not require user data entry, depending on the source treatment planning system.
Treatment Plan Preparation
Prepare treatment plans as follows:• Include an MR dataset (recommended).
NOTE: If no MR data set is available, automatic OAR segmentation and dose calculation arenot available.
• Outline both the CTV and PTV (recommended).• Name each CTV and PTV meaningfully.• Export DICOM message types: RT Dose, RT Structure Set and RT Plan.
NOTE: It is not possible to import multiple RT Dose, RT Structure Set or RT Plan instancessimultaneously.
• Ensure that all patient data contains the same patient name, ID, date of birth and gender.• Export the complete dose and not just the dose around certain structures.
NOTE: The dose matrix must be defined for the entire tissue.
Preparation of MR Datasets
Quentry for SRS Registry requires MR data sets for the automatic segmentation of OARs.For optimal automatic segmentation, prepare the MR data set according to the following MRscanning parameters:
General MR ImagingRequirements
• High tissue contrast resolution• High signal-to-noise ratio• Always use 3D distortion correction if available
MR Scan Instruc-tions
To achieve the best segmentation results in the cranial region, it is recom-mended to provide at least a T1 and a T2 or FLAIR sequence-based scanwith the following parameters:MR T1 complete head (e.g., MPRAGE):• ≤ 1 mm slice resolution• ≤ 1 mm slice thickness• > 100 slices of the complete head
MR T2 and/or FLAIR:• ≤ 2 mm slice resolution• Full region of interest included

Data Requirements by Event Type
Event Type Data Requirements
SRS/SRT
• Patient's approved SRS/SRT treatment plan• Associated MRI and CT (if used in planning) scans• Image fusion/registration if available
NOTE: If no fusion is available and treatment planning is based on a CTdata set, dose calculation on automatically segmented OARs will not beavailable.
WBRT
• Patient's approved RT treatment plan• Associated MRI and CT (if used in planning) scans• Image fusion/registration if available
NOTE: If no fusion is available and treatment planning is based on a CTdata set, dose calculation on automatically segmented OARs will not beavailable.
Surgical Resection
If surgery took place before SRS/SRT treatment:• Patient's pre-operative MRI scan• Outlined tumor object(s)
If surgery took place after SRS/SRT treatment:• Patient's post-operative MRI scan• Outlined residual tumor object(s)
Follow-Up• MRI scan taken at time of follow-up examination• Outlined tumor object(s) (e.g., new, residual or recurring)

2.3 Import Criteria
DICOM Data Types Supported by Quentry for SRS Registry
Data type Description
RT Plan Contains treatment plan parameters and a reference toan RT Structure Set.
RT Structure Set Contains objects created upon treatment plan exportfrom treatment planning software.
RT Dose Contains the dose information.
REG Contains registrations.
MR, CT, PET Image data
Segmentation Contains created objects (e.g, with SmartBrush).
NOTE: It is only possible to import orthogonal image series of RT Structure Sets.
NOTE: DICOM RT export data is supported, however, the Brainlab proprietary file format, xbrain isnot supported.
NOTE: After import, verify that the contours are properly aligned to the images.
Importing Treatment Plans
To upload dose volumes to Quentry for SRS Registry, the following criteria must be met inDICOM:
Criteria Technical Details in DICOM Terminology
The dose volume to be loaded must representthe overall dose of a single radiotherapy plan.
Dose Summation Type must be PLAN.Doses calculated for two or more RT Plans(MULTI_PLAN) and doses that do not repre-sent the entire plan (FRACTION, BEAM, CON-TROL_POINT) will be rejected.Type BRACHY will be rejected.
The dose must be given in gray units. Dose Units must be GY, not RELATIVE.
Only physical (not effective) and prescriptiondose can be loaded.
Dose Type must be PHYSICAL, and not EF-FECTIVE or ERROR.Dose Reference Structure Type POINT is notsupported.
Image data associated with the dose must beavailable.
An image series must be available that can betraced back via RT Structure Set/RT Plan to in-stances that were exported alongside the doseand resides in the same coordinate system(Frame of Reference).
Associated images must be of modality MR,CT or PET.
Image modalities currently not supported in-clude NM (Nuclear Medicine), XA (X-ray An-giography) and RTIMAGE (Radiotherapy Im-age).
No single pointsImport of single points like MARKER, ISOCEN-TER and REGISTRATION through the DICOMRT Structure Set is not supported.
Further information can be found in the DICOM conformance statement http://www.brainlab.com/dicom.
