Radiation Safety Surveys

• Radiation Safety Surveys are required for all ionizing radiation producing machines.

Machine Type Survey Due:

Medical or Dental X-ray Within 60 days of acquisition

Therapy, Accelerator, Industrial Radiography, Electron Microscope,

Cabinet System

Prior to first use

Common Deficiencies with Submitted Surveys

• Surveys are illegible • Registration and equipment identifiers don’t

match information on file with Dept.• Diagrams missing key identifiers including

operator position, exposure switch location, lead shielding if present; wall, floor,and ceiling construction.

• Missing information about the survey instrument used (i.e. calibration date, probe information, etc)

Bureau Policy for Deficient Surveys

• If survey deemed incomplete:– returned to the qualified individual who

performed the survey for correction and re-submittal within two weeks

• If no response: – Letter sent to registrant explaining that the

survey does not meet regulatory requirements and must be corrected within 30 days

• Enforcement Action if still deficient
