J RC POSITION STATEMENT

Site Visit and Accreditation Programs in Cooperation with State Licensing Agencies

The Position of the J oint Review Committee on Education in Radiologic Technology

The Joint Review Committee on Education in Radiologic Technology recognizes the right of each state to act on behalf of tne welfare of its citizens by requiring minimum standards for education, and certification prerequisite to the practice of radiologic teChnology in any and all of its disciplines. ThiS Joint Review Committee shares these same goals as evidenced by its activities in the accreditation of educational programs in radiography and radiation therapy technology.

Continue Next Column

J RC STATEMENT continued

The J RC has cooperated with various states in joint site visit and accreditation activities since 1976. Experience has shown that educational programs can benefit from joint activities in several ways. Foremost among the benefits to the prosrams are reduction in the number of site visits, prerequisite paperwork and the costs of accreditation. Experience has also shown that programs can be effectively evaluated for compliance with specific state standards and the Essentials of an Accredited Edu~a.tion~1 r~ogram through a single Jomt sIte VISIt.

The details of a cooperative site visit and accreditation program can vary depending on the workm~ relationships most desirable, and SUIted, to the individual state., In all cases, however, the J RC emphasizes that the state agency or board retains its autonomy. I n each case of cooperation, an agreement is made between the state agency and the Joint Review Committee which may be supplemented by guidelines containing the working details of the cooperative relationship.

Some of the options for a cooperative site visit and accreditation program are:

1) The J oint Review Committee conducts evaluations routinely and according to published procedure and notifies the state of the results.

2) The J RC notifies the state of the schedule of visits within that, state but oth'erwise conducts the evaluations according to normal procedures.

3) The, Joint Review Committee and the state cooperate in establishing the schedule of site visits and members of the state agency may accompany the site visit team.

4) The J RC and the state cooperate in establishing the schedule of sIte visits and jointly appoint the visiting team, which may include site visitor development programs. This option also includes the joint filing of reports between the agencies and to the edu.;ational program.

5) The J RC conducts the evaluations , for the state according to a schedule

determined by the state agency with reports developed and processed in a mutually agreeable manner.

Th~ fees normally char~ed to educational programs underWrite the costs. Programs experience savings throuJdl some cooperative activities because die state funds the participation of its site visitors. The options above are only examples and can serve as a framework for designing any number of working relationships. If a joint venture could serve the needs of your state and the educational programs, please contact the Joint Review 'Commi'ttee's Exec: utive Office for additional information.

Looks as if a few thin,p are changing in our profession, doesn t it? When costs are cut, administrators start to economiz~ and there is a shift jn personnal duties and even iobs. The diaP.osis related groups (DRG's). are under a fixed prospective payment and in doubt hanging an effect on some institUtions. As f~ as the, r,adiotherapy free standing cancer center IS concerned, little effect, if any, will be noticed. However, hospital Dased therapy centers may have some concern.

Ra~i~tion therapY' teams were in competition at the 7th Annual Midwest Radiation Therapy Bowl, s~nsored by the Chicago Area Radiation Therapy Technologists and the .llIinois State Society of Radiolo&i-, Technologists on May 5 - 6, 1984. Teams came from Illinois, Ohio, Minneso~ Michipn Indiana, Tennessee, CaIiTornia, New York, Iowa, and Wisconsin. First place went to St. Joseph Hospital in MiIw~iJkee, WI; second place to the Michael j. Owens Technology Institute of Toledo, Ohi~i third plac'e to the ~ayo School of nealth Related Science In Rochester MN; and fourth place to ~ontefiore Hospital and Medical Center In Bronx, NY.

A student review session was conducted during the two day session, as well as a program for educators. A total of 153 students and educators were in at-· tendance.

NEW TEST SPECIF'ICATIONS TO BE PUBLISHED AND IMPLEMENTED

At its annual meeting in July 1983. the Registry Board of Trustees reviewed the progress of the ARRT job analysis project and revised the schedul~ for complet.ion of the fourth and final phase of the proJect. The project was begun in July 1980 with the purposes of identi~ying the tasks typically performed by the staff technologIst at entry level and using this information to assure that the coverage of ARRT certification examinations corresponded to all the knowledge areas underlying performance of those tasks.

Phase I of the project was carried out during July 1980 - June 1981 and resulted in the first draft of a task inventory. The tasks had been identified by on-site observations and generalized by a mail survey to samples of entry-level technologists. .

Phase II of the project was completed dUring July 1981 - June 1982 when job analysis advisory committees met to identify those knowledges necessary to perform e~ch of the tasks identified during Phase I.

Phase III of the project was conducted from July 1982 - June 1983. A mail survey was conducted during the fall of 1982 with larger. more diverse samples to verify the selection of tasks to be covered on the examination. The examination committees and Board of Trustees reviewed this material as weH as feedback from technologist seminars held to critique documents developed in the project.

The final phase of the job anaylsis project involves translating the materials developed in the fi~st ~hree phases into content specifications for the e~amm~tlons. The content specifications will serve as a blueprmt for development of examination forms by indicating the areas to be covered and number of items to be included from each area.

Although it was originally intended to base the 1984 examinations upon the job anaylsis results. it has been necessary to revise the schedule so that the first examination forms based upon these results will be those administered in 1985. The documents are currently undergoing editing and reformatting in preparation for the weighting of categories. It is anticipated that the new test specifications will be published by July 1984.

SILVER THEFTS.DO NOT GO UNNOTICED

As a result of state investigations into Medicare fraud. several registered technologists have been convicted of crimes involving the sale of medical x-ray film or silver recovered from used film. Some of these technologists were identified in newspaper articles. Others were reported by individuals in the community. A few have contacted the Registry voluntarily and admitted their convictions. Legal counsel for the Registry has advised that a felony conviction for this type of offense constitutes grounds for revocation of a technologist's certificate under provisions of the current ARRT BYLAWS and RULES AND REGULATIONS even in cases where the conviction follows a plea of nolo contendere. At its July 1983 meeting. the Registry Board of Trustees reviewed the cases of three registered technologists who are currently under sentence for felonies involving the illegal sale of x-ray film and/ or silver. The Board voted to commence due process to revoke the certificates of all three.

"ACTIVE" DEFINED Previous editions of the ARRT NEWSLETTER re

quested suggestions and comments to assist the ARRT Board of Trustees in a future revision of ARRT reregistration processes and requirements. The Registry Board is extremely grateful to those who took the time and trouble to respond with constructive comments and suggestions. As a result of this input. the Board has been able to develop a new definition of the active registered technologist which defines the active registered tech-nologist as: .

"Presently employed in the field of radiologic technology as a technologist or in a management. commercial. research. or educational capacity."

LIMITED PERMIT EXAM TO BE DEVELOPED

The Registry Board of Trustees has authorized development of a limited permit examination for use by states whose laws provide for a limited permit in radiography. A committee is being selected to work on the examinations which will be limited to chest and extremities.

COMPUTER GENERATED R.T. LISTS NOW AVAILABLE

For several years the Registry has provided computer addressed mailing labels with the names and addresses of registered technologists at cost to societies afliliated with the American Society of Radiologic Technologists for use in connection with the educational and professional activities of those societies.

In response to requests from many technologist soci~ties, the Registry is now also making available comput~r printouts of names and addresses of registered technologists by state, category of registration, or any combination thereof. These lists are available to societies affiliated with the American Society of Radiologic Technologists at the Registry's approximate cost which includes a set-up charge of $35 per order plus $25 per 1000 names (or portions thereoO and priority rate U.S. postage. Orders for printouts must be signed by the Affiliate Society Treasurer and will be billed to the Affiliate Society. Payment in advance is not accepted because the actual count and shipping weight are not known until the order has been completed.

REINSTATEMENT IS A COSTLY ALTERNATIVE TO RENEWAL

From its earliest beginnings, the ARRT has required all registrants to renew their certificates annually and has dropped from good standing those who failed to do so. Prior to 1983, renewal was on a calendar year basis with all certificates renewable in January. New registrants were issued credentials which were in force only for the calendar year in which issued but were renewable in January of the following year. Registrants in good standing at the end of one year were sent applications for renewal for the following year. The applications were mailed about Christmas time, payable the following January. A second statement was usually sent to those who failed to respond to the first mailing. All registrants who failed to renew by June 30. the end pf the Registry's fiscal year. were dropped from good standing. After being dropped, a technologist could not be returned to good standing without applying for reinstatment and remitting all accrued arrears renewal fees plus the current year's fee. Under that system, reinstatement was simple and inexpensive for the registrant. Unfortunately, the lack of any real deterent to late re-registration caused some technologists to develop a rather lackadaisical attitude toward the renewal process. It made little difference to them whether they paid $6 to be renewed in January or $6 to be reinstated in July. Either way, they ended up in good standing at the end of the year. However. the growing number of reinstatements to be processed by Registry staff each year was becoming a matter of some concern since reinstatments. unlike renewals. are handled individually and therefore require proportionately more time to process than do renewals. To illustrate the proportions of the problem, 2.975 technologists were dropped from good standing in the fiscal year ending June 30. 1981. Subsequently 1.175 of these same individuals were reinstated to good standing in the fiscal year ending June 30. 1982.

In 1983 the Registry implemented a system for processing reregistrations on a year-around basis. Each technologist's 1983 reregistration was scheduled for the month in which hel she was born. To put all registrants into the proper reregistration cycle. each was initially billed an amount prorated to cover the period between January I. 1983 and the month in which he/ she was scheduled to enter the reregistration cycle under the new system.

All technologists were given until June 30, 1983 to respond to the initial statement. Those who failed to do so were dropped from good standing as in previous years. However, unlike previous years. their return to good standing is not such a simple and inexpensive process. Processing of applications for reinstatement and the insertion of reinstated certificates into the new renewal cycle are individual. labor inten'sive processes. Therefore, a $25 reinstatement service fee was introduced to encourage prompt turnaround of renewal applications. Those who responded to the initial statement in time were sent credentials of registration covering the period from January I. 1983 until the month of their next birth anniversary. About one month prior to the expiration date of those credentials. tRey were, or will be, sent a renewal application and fee statement for the next 12 month renewal period. All registrants are cautioned not to let their current credentials expire because expiration will result in their being dropped from good standing and therefore subject to payment of a reinstatement service fee to be returned to good standing.
