As per Punjab Govt. notification no. 5/26/2016-5HB-III/557 dated 25.01.2018, Physical counselling of NEET-PG-2018 qualified candidates for admission to PG Medical/Dental courses of state quota seats and all seats of Private Medical/Dental colleges including Private/Deemed Universities and minorities institutions of the state of Punjab will be held at GGS Medical College, Sadiq Road, Faridkot. The detailed schedule will be made available on the University website www.bfuhs.ac.in.

Note : 1. Admission for Post Graduate Diploma in Sports Medicine at Netaji Subhas

National Institute of Sports, Patiala is also covered under NEET-PG-2018.

2. Candidates can download the prospectus from University website or obtain from University after making payment of Rs. 300/- in cash.

INTRODUCTION

BRIEF HISTORY:

The University was established at Faridkot under an Act of Punjab State Legislature

(Punjab Act No. 18 of 1998) in the name of great Sufi Saint Sheikh Farid Ganj-E-Shakar,

(1173-1265 A.D.) in 1998 for purposes of affiliating, teaching and ensuring proper and

systematic instruction, training and research in Modern Systems of Medicine and Indian

Systems of Medicine. It is not only affiliating and examining body but also a teaching and

research centre in health sciences. It is expected to play a vital role in improving the

standards of medical and health education in the state and in conducting relevant research

in all aspects of health sciences, basic as well as applied, which ultimately will ameliorate

the health standards of the people of Punjab. II is believed that the university will be a trend

setter in developing appropriate modes and models of health care to ensure quality health

care to the people.

The University has been recognized by MCI vide letter no. V/11015/3/2000-ME (UG)

dated 9th June, 2000. The UGC has also included the University in the list of recognized

universities maintained by the University Grants Commission, under section 2(1) of the UGC

Act. 1956 vide letter no. F.9-3/97 (CPP-I) dated 4 July. 2002. The Regional Director. World

Health Organization, Regional Office for South-East Asia, World Health House, New Delhi

has also forwarded the name of the University for inclusion in the World Directory of Medical

Schools vide letter No. M12/6212 dated 6 May 2002. The University is also included in the

list of Commonwealth Universities and their yearbook 2001 at page No. 692 (Vol. l).

Established with a mission to create an intellectual, academic and physical

environment conducive to free flow of ideas and exchange of information between various

faculties of the university, between this university and other universities of health sciences in

the country and abroad, thereby opening a window to the world for the health professionals,

health planners, health managers, biomedical and social scientists and educationalists in

health sciences of the country. Efforts are on to establish contact with various institutions

towards achieving this goal.

UNIVERSITY'S PRESENT STATUS:

All the new buildings of the University and its constituent college, GGS Medical

College & Hospital were completed in the year 2011. The university was shifted to its new

building situated at Sadiq Road, Faridkot in July 2011. Hon'ble Chief Minister, Punjab.

inaugurated two new University buildings (Senate Block and Academic Block) on 22-09-

2011. Hon'ble Deputy Chief Minister inaugurated the Radiotherapy Unit, and other buildings

of GGS Medical College & Hospital, on 23-09-2011. The GGS Medical College & Hospital, a

constituent college of the University is also shifted to its new building.

The University Campus comprises of GGS Medical College along with Auditorium of

800 capacity, Senate Block, Academic Block, University College of Physiotherapy,

University College of Nursing, Drug De-addiction Centre, Telemedicine, Edusat and a well

equipped state-of-the-art 500 bedded hospital. Coveted projects to set up Trauma Centre,

Regional Cancer Centre all in process. The University has added to its academic convoy

University Library & Informatics Division, Department of Health Sciences Library &

Information System, University Centre of Excellence in Research, Rose Garden and Herbal

Garden.

UNIVERSITY'S NEW PROJECTS: University is on the way to establish:

1) Advanced Cancer Diagnostic, Treatment & Research Centre at Bathinda.

2) University Regional Centre for Public Health & Paramedical Sciences at Goindwal

Sahib. University has acquired 20 Acres of land for its Regional Center at Goindwal

Sahib. Hon'ble Chief Minister, Punjab has already inaugurated both the projects in

October 2011. The University has planned to run a number of employment-oriented

diploma courses and certificate programmes in health sciences.

The University is also in process of developing the following five study centres:

1) Centre for Distance Learning (Medical) and Educational Technology

miscellaneous.

2) Centre for Health Systems and Health Services Management.

3) Centre for Bio-information Technology.

4) Centre for Human Resources Development and Health Management.

5) Centre for Population Sciences.

6) University Institute of Nursing, Jalalabad, Distt. Fazilka attached with upcoming

100 bedded Hospital.

UNIVERSITY'S CONSTITUENT COLLEGES: The Government of Punjab incorporated the three institutions as constituent colleges

of the Baba Farid University of Health Sciences, Faridkot viz. School of Nursing (upgraded

to University College of Nursing) vide notification no. 11/299/01-5SS3/4770 dated

14.10.2002,

Guru Gobind Singh Medical College & 500 bedded Hospital. Faridkot vide notification no.

8/02/2005-1HB-I11/6308 dated 31.10.2006 and State Institute of Nursing & Paramedical

Sciences, Badal vide notification no. 1/46/07-1HBIV/1163 dated 28.11.2007. In addition to

above, the Board of Management of University also accepted the proposal for starting of

Physiotherapy College in the University campus in its meeting held on dated 03.04.2007

and fourth constituent college was established viz. University College of Physiotherapy.

University has also established its own Institute of Pharmacy. Recently, Govt. of Punjab has

transferred 100 beded Civil Hospital, Badal to University for strengthening and expanding

the connection between rural medical services and the education and research carried on

by the Baba Farid University of Health Sciences. The GGS Medical College & Hospital, a

constituent college of the University has 25 departments and 21 post graduate departments

and all are recognized by MCI.

COLLEGES AFFILIATED WITH UNIVERSITY:

At present the University is having 146 affiliated colleges comprising 136 affiliated

and 10 constituent Colleges (with an approximately 35,000 students).

University will soon endeavor to achieve excellence in health care delivery and medical

education and research in the State. Admissions to various undergraduate and post

graduate courses in the affiliated institutions of this University in the State of Punjab are

made on merit basis through the conduct of Entrance Tests.

The university is now primarily focusing on strengthening postgraduate medical education

and research and evolving as a centre of excellence in a rural backdrop.

College Inspections: The University has started conducting the surprise inspections of all the colleges and

due to this, the colleges are compelled to maintain their standard related to academic staff,

discipline and availability of amenities etc. Moreover, the University has started highlighting

the shortcomings which the colleges have failed to rectify. This information is made

available to the prospective students at the time of admission, before they select the college

of their choice.

University Examination System:

The University is going to introduce Onscreen Marking Solution and e-transfer

system of question papers of various courses to the examination centres w.e.f session 2016

to maintain the sanctity of examinations. This process and implementation strategies will

help us to deliver, efficiency, reliability in high stakes examination and their certifications.

A. In case candidate want to make any change in his/her online registration

form, he/she can do the same by logging on again in his/her online

application form by using the password provided by the University on or

before the last date for submission of form. Online Admission Application

Form will be locked automatically after the cut-off date for submission of

online admission application form. No request for any correction in admission

application form will be considered thereafter.

B. GENERAL INSTRUCTIONS (PLEASE READ IN DETAIL)

1. Registration for admission to PG Medical/Dental, PGDSM and six months training in Ultrasonography course is to be undertaken online at the University website www.bfuhs.ac.in.

2. Application form for admission to PG courses can't be submitted offline/printed copy by post.

3. This prospectus contains two parts :

Part - A Pertains to the general procedure for applying to admission, submission of admission application/counselling form, deposition of fee, important instructions, joining, etc.

Part -B Pertains to Contains Punjab Govt. Notifications for admission to post graduate Medical and Dental Courses and six months training in Ultrasonography, proformas for different categories, self undertaking, etc.

4. Candidates must follow the instructions strictly as given in the Prospectus and

University website. Candidates not complying with the instructions shall be summarily disqualified.

5. Incomplete application not in accordance with instructions shall not be considered and are liable to be rejected. The fee shall not be refunded under any circumstances.

6. This prospectus is subject to alteration(s) / modification(s) at any time without notice. For updates please visit University website from time to time.

7. Instructions in this prospectus are liable to changes based on decision/s taken by the Punjab Government/University from time to time. There is no equity or any rights that are/or deemed to be arising in favour of candidate.

8. Admissions including Eligibility to various PG Courses shall be governed and regulated as per the rules and regulations etc, as per Punjab Govt. notifications.

9. The candidate should read the eligibility condition carefully and just applying for admission/counselling does not entitle him/her for admission. Eligibility conditions for admissions are given in the Notification. The eligibility shall be determined by the Admission Committee/Principal of concerned college at the time of counseling/admission. Those who are not eligible as per Punjab Govt. notification will not be considered for admission. The candidates will participate in counselling process at their own risk & responsibility. Eligibility is also subject to medical fitness and such criteria as may be prescribed by the Punjab Government/ University/Medical institutions/ Medical Council of India/Dental Council of India.

10. Candidates' eligibility is purely provisional & subject to the fulfillment of eligibility criteria as per Punjab Government Notification incorporated in this prospectus.

11. Applications of candidates producing false or fabricated information shall not be considered and candidates shall be further debarred from participating in any Counseling's conducted by BFUHS, Faridkot.

12. Any attempt by a candidate to influence directly or indirectly any person concerned with the admission will disqualify him/her.

13. Final seat matrix shall be displayed at the time/venue of physical counselling. Tentative seat matrix will be made available on the website of university.

14. The admission for PG courses shall be made only to those institutions and specialties which have permission from all of the concerned authorities and affiliated with the University.

15. The fee structure for the Government/Privately managed colleges is as per Punjab Government notification(s) for Post Graduate Degree/Diploma courses. If Punjab Government issues any subsequent amendment/corrigendum for change in fee structure, the same will be applicable in letter and spirit.

16. In case of any discrepancy or contradiction between the Government notification/s and the University instructions contained in this Prospectus, the letter and spirit of the notification/s shall be prevailing, however in academic matters the University shall be the final Authority.

17. Deposition of fee does not confirm the admission of the candidate, if it is challenged in the Hon'ble court.

18. Changes, if any, in the Schedule / Counselling etc. shall be notified only through website of the University i.e. www.bfuhs.ac.in

19. Candidates must retain the copy of prospectus with them till admissions. They should read carefully the instructions and the rules & regulations of the Punjab Govt. Notification/s and comply with all instructions therein strictly.

20. The allotment of specialty and college to the applicants for admission, shall be made as per merit determined on the basis of NEET-PG-2018 and the choice of the speciality and college opted by the candidate at the time of physical counselling.

21. Baba Farid University of Health Sciences, Faridkot has provided access to e-resources [HSLIBNET] to the students of all the affiliated colleges of the University for which Rs. 2,000/- per annum will be charged for HSLIBNET from the student.

22. Undertaking (ANNEXURE-B) is compulsory for all the candidates applying for admission, except in-service candidates.

23. Undertaking (ANNEXURE-C) is compulsory for all in-service candidates applying for admission.

24. This is for the information of all candidates that as per Hon’ble Supreme Court of India, if any incident of ragging comes to the notice of the authority, the concerned student shall be given liberty to explain and if his explanation is not found satisfactory, the authority would expel him/her from the institution.

25. First of all candidate will have to log on university website i.e. www.bfuhs.ac.in and read the Punjab Govt. notification/instructions as contained in the Prospectus carefully before filling online admission application form.

26. Candidates will fill only online admission application form through BFUHS website i.e. www.bfuhs.ac.in. Candidates are advised to fill online admission application form before the due date. Online Admission application form will not be available on university website after the due date/time. Candidates are advised to fill the online admission application form well in advance to avoid the network congestion in the last days. BFUHS will not be responsible for network congestion/problems. No request for accepting the forms after cut-off date will be entertained.

27. For the detail of rounds of counseling, please keep yourself updated with the website of university.

28. Candidates desirous of seeking admission will have to apply online separatrely for MD/MS/Dip/MDS, six months training in Ultrasonography and PGDSM courses and will have to deposit fee separately.

29. Application Fee

(a) Admission Application Fee of Rs. 5000 (Rs.2500 for SC candidates) will only be deposited in any OBC Branch through the Challan Form generated after successful filling of online admission application form on university website.

(b) Only those Admission Application Forms will be considered for counselling which are filled online by due date/time and the fee of which has been deposited. Candidate will submit the proof of deposition of fee (copy of bank challan form) at the time of physical counselling.

(c) No request for accepting the Admission Application Forms after due date/time will be entertained.

(d) Merely deposition of fee will not entitle the candidate for claim of admission. (e) Fee once deposited in University Account will not be refunded/adjusted in any

case. (f) Fee shall not be accepted in any other mode i.e. Cash, Cheque, Demand Draft

etc.

30. Notwithstanding the participation of candidates in counselling, only eligible candidates, as per Government notification/s, shall be considered for admission.

31. Candidates applying under any of the reserve category are directed to fill the right category code mentioned in this prospectus for claiming reservation under a category and also bring/attach all relevant documents/proof of respective category with admission application form and also directed to bring along photocopies and originals at the time of counseling/admission.

C. ELIGIBILITY CONDITIONS

1. The admission to MD/MS/PG Diploma/MDS and six months training in Ultrasonography shall be open to candidates who have qualified NEET-PG-2018 and eligible as per Punjab Govt. notifications and subsequent amendment(s)/corrigendum(s) and possessing basic qualifications i.e. MBBS for various Medical PG Courses and BDS for various Dental PG Courses.

2. Candidates cannot claim a seat/admission on the basis of the NEET-PG-2018 rank alone. Eligibility and Merit of a candidate for admission against any PG seat will be determined by the Admission Committee, strictly as per rules and regulations laid down in the Punjab Govt. Notification(s) as published in this prospectus and all other such documents as are required with the ADMISSION APPLICATION FORM.

3. Counseling / Admissions shall be conducted by the Admission / Selection Committee constituted by the Punjab Government for this purpose.

D: INSTRUCTIONS FOR FILLING UP ONLINE ADMISSION APPLICATION FORM

1. Candidates will have to log on University website i.e. www.bfuhs.ac.in. and read

the Punjab Govt. notification and instructions as contained in the Prospectus carefully.

2. A candidate who qualifies in NEET-PG-2018 is eligible to apply ONLINE for admission to Post Graduate Courses including Post Graduate Diploma Courses and six month training in Ultrasonography.

3. Candidates are required to register themselves for admission to PG Medical/Dental and for six months training in Ultrasonography through online registration system at BFUHS website i.e. www.bfuhs.ac.in. Online registration must be completed by due date and time.

4. Candidates are advised to fill the online admission application form well in advance to avoid the network congestion in the last days. BFUHS will not be responsible for network congestion/problems. No request for accepting the forms after cut-off date will be entertained.

5. Following documents are required to upload on the online registration form :

I. Soft copy of Photograph of the candidate. (Maximum size 30 KB)

II. Soft copy of the signatures of the candidate. (Maximum Size 30 KB) 6. Specifications for Photograph:–

I. A colour photograph of minimum 35x45 mm with at least 75% coverage of face & head of the candidate.

II. A caption indicating name of candidate and date of taking photograph should be there at the bottom of photo.

III. Photograph should be taken in a white /very light colored background. IV. Photograph should not have cap or goggles. Spectacles are allowed if being

used regularly. POLAROID and COMPUTER generated photos are not acceptable.

V. The photograph needs to display full front view of the face. Please look directly into the camera with a neutral expression.

VI. Please avoid photograph with reflection or shadow on the face with red eyes.

VII. The photograph needs to be printed on a high quality paper with at least 600 dpi resolutions.

VIII. The colours must possess the natural appearance and skin tone.

IX. The photograph must not have kinks, scratches and stains. Note: Applications not complying with these instructions or with unclear photographs are liable to be rejected.

7. After completion of filling online admission application form, University will issue a unique Registration ID (i.e Login ID) and password will be provided to the candidate at his/her registered Mobile number in admission application form through SMS/ email. Registration Number and password are mandatory for further process, so keep it secret and don’t disclose to any unknown person.

8. Merely submitting of online application form & fee does not confer any automatic rights to secure a Postgraduate MD/MS/Diploma/MDS/PGDSM seat.

9. Candidates should ensure before applying for the admission that their MBBS/BDS degree is recognized as per provisions of Indian Medical/Dental Council Act. If it is found at any time that MBBS/BDS degree is not recognized, the candidature/result shall be cancelled/ deemed to be cancelled.

10. Fill category(ies) carefully in the online admission application form. Category(ies) once claimed will not be changed in any case after submitting the form. Those candidates, who will not fill/claim any category code as per prospectus or wrongly filled category code other than mentioned in prospectus, will be treated as Open/General Category candidate. The candidate may select a category code from the codes given below and fill the category accordingly. More than one category may be filled by a candidate if it is considered applicable in the candidate's particular case. Category codes are exclusively for the purpose of facilitating the candidates to know and understand his/her respective merit placement at the time of counselling in various reserved categories as might have been filled by him/her. Reserve category selection on the part of the candidate shall not in any case be construed to give him/her any legitimacy or right or claim on the benefits for reserved seats during admission. E. INSTRUCTIONS FOR PHYSICAL COUNSELLING.

1. Physical centralized counselling will be held as per schedule published on the website of the university for admission to 50% State Quota seats in Govt. colleges and all seats in Private Medical / Dental Colleges, including Private/ Deemed Universities and Minority Institutions of the State of Punjab, on the basis of Merit of NEET-PG 2018. In case of physical counselling, personal appearance of candidates is essential at the time of interview/counseling and exemptions from personal appearance may not be sought as it shall not be granted.

2. The candidates should read the eligibility condition carefully and just appearing in physical counselling does not entitle him/her for admission. Eligibility conditions for admissions are as per Punjab Govt. Notification(s). The eligibility shall be determined by the Admission Committee at the time of counseling. Those who are not eligible as per Punjab Govt. notification will not be considered for admission. The candidates will appear in counselling at their own risk & responsibility.

3. The allotment of specialty and college shall be made as per the choice of specialty and station by the candidate at the time of the counselling by the Admission Committee on the basis of the merit determined as per marks obtained by the candidate in NEET-PG-2018 and other terms and conditions contained in the Punjab Government Notification for admission to Post Graduate Courses.

4. The candidates shall have to produce all the certificates/degrees/diplomas/ documents in original at the time of joining/admission in respective colleges or at the time of physical counseling.

5. The choice(s) of courses/ institutions shall be exercised by the candidate at the time of Counseling/ interview. No waiting list shall be prepared.

6. After provisional allocation of seat by the admission committee, candidate shall have to report to the allocated college for verification of documents, checking of eligibility and joining thereafter. Allocation of seat is provisional subject to verification of original documents/checking of eligibility by the Principal of concerned Medical/Dental College. It is sole responsibility of the Principal to authenticate the eligibility of the allocated candidate before joining/admission. Those who are not eligible as per Punjab Government Notification will not be considered for admission. In case of any discrepancy, matter should be brought to the notice of BFUHS immediately. In case candidate is found not eligible as per Punjab Govt. notifications/University norms, his/her admission will be cancelled at any stage by the University.

7. The respective colleges will send the Registration Return of candidates as per due date and proforma alongwith original documents of the candidate for registering the candidate in particular course/speciality/quota. If any candidate is found not eligible as per Punjab Govt. notification by the University, then his/her selection will be cancelled by the University immediately.

8. Deposition of Fee after provisional allocation : Candidate will have to deposit tuition fee as specified in the Punjab Govt. notification on the spot at the venue of counseling in the shape of Demand Draft, favouring Registrar, Baba Farid University of Health Sciences payable at Faridkot failing which his/her provisional selection will be treated as cancelled without any intimation to the candidate.

9. All the candidates selected for various PG courses will have to submit their bond (complete in all respect) as per Punjab Govt. notification at the time of reporting with the institution concerned at the time of joining, failing which their joining will not be accepted. The specimen copy of bond to be submitted is attached as Annexure F. The candidates

will attach the attested copy of proofs of the ownership of property which is being pledged by the sureties. The bond should be attested by the Executive Magistrate. The incomplete bond will not be accepted at all.

10. Fee from selected candidates will be collected at the venue of counselling in the shape of Demand Draft favouring Registrar, Baba Farid University of Health Sciences payable at Faridkot.

11. Minority Quota seats: The final eligibility for getting seat under Minority Quota will be determined by the Admission Committee after the verification of the minority status by the representative of concerned institutions (in accordance with Punjab Govt. notifications and as per rules and regulations followed by their institutions) at the time/venue of counselling. Candidates of Minority quota will bring all relevant documents at the venue of counselling to determine his/her eligibility under Minority Quota.

*** 1. The Government of Punjab is pleased to notify policy for admissions to Post

Graduate Degree/Diploma courses and six month training in Ultrasonography, “The Fundamentals in Abdomino-Pelvic Ultrasonography: Level one for MBBS Doctors”, in the Health Sciences Educational Institutions (Medical/Dental) in the State of Punjab for the year 2018.

2. All admissions of Govt. and Private Institutions, including Private/ Deemed Universities and Minority Institutions, shall be based on merit of National Eligibility cum Entrance Test (Post Graduate) (NEET-PG-2018).

3. The eligibility criteria and other conditions shall be as prescribed in Information Bulletin for National Eligibility cum Entrance Test (Post Graduate) for admission to MD/MS/Post Graduate Courses 2018 issued by National Board of Examinations, New Delhi.

4. The Government of Punjab is further pleased to authorize Baba Farid University of Health Sciences (BFUHS), Faridkot to conduct centralized counselling for admission to 50% State Quota seats in Govt. Colleges and all seats in Private Medical / Dental Colleges, including Private/ Deemed Universities and Minority Institutions of the State of Punjab, on the basis of Merit of NEET-PG 2018 as per procedure laid down by MCI/DCI. The process and schedule of counselling shall be decided by Baba Farid University of Health Sciences, Faridkot. The details shall be published by the BFUHS, Faridkot. (www.bfuhs.ac.in)

5. One year compulsory rotatory internship is an essential prequalification for acquiring eligibility for admission to Post Graduate courses and the candidate should be registered with the State Medical Council/Dental Council. The cut off date for completion of Internship shall be 31st March 2018. However, the condition of one year Compulsory Rotatory Internship is not applicable to those candidates who have passed their final BDS Examination on or before 1988 and hold a BDS Degree from a Dental College recognized by the Dental Council of India and have obtained permanent registration from the State Dental Council. No relaxation regarding condition of internship shall be accepted except for the students of the BDS batch admitted during the academic session 2007-2008 and have completed 4½ year BDS Course along with 06 months compulsory rotatory Internship.

6. A candidate who, after passing the final qualifying examination (MBBS/BDS or the foreign Medical/Dental Degree recognized by GOI/MCI/DCI), is undergoing 12-month compulsory rotatory Internship/practical training and is likely to complete the same by 31st March 2018 shall be eligible for admission provided he/she completes the compulsory rotatory Internship and obtains provisional or permanent registration. Such a candidate shall produce a certificate from the Head of Institution that he/she will be completing internship by 31st March 2018.

7. The tentative distribution of seats shall be displayed by Baba Farid University of Health Sciences, Faridkot on its official website i.e. www.bfuhs.ac.in, once being provided by the office of DRME, Punjab.

8. Any candidate who has taken admission in previous session in MD / MS / PG courses shall be eligible for fresh admission in new MD / MS / PG courses as a fresh candidate based on basis of result of NEET / PG. However, in case a candidate was admitted previously and drops out or leaves the Post Graduate Degree/Diploma course in between and seeks a fresh admission, the candidate shall have to deposit fee for full course (3 years for degree & 2 years for diploma course).

9. A Candidate having completed P.G. diploma, shall be eligible to apply for Postgraduate Degree course.

10. The schedule of admission process i.e. commencement of classes, cut off date, etc. shall be as declared by the Medical Council of India / Dental Council of India.

11. The application for admissions, complete in all respect shall be submitted to the Baba Farid University of Health Sciences, Faridkot, Punjab. BFUHS may fix and charge fee for Prospectus, Application Fee etc. as approved the Government of Punjab.

12. A candidate from reserve category shall have the right to compete for admission in open merit or general category seats as per his/her merit on the day of counselling and candidate so selected shall not be counted towards admission in the reserve category. A combined merit list and separate merit lists of each category shall also be prepared and displayed on the website of the University.

13. In case of any dispute in distribution of seats for all the institutions situated in the State of Punjab, Director Research & Medical Education, Punjab shall be the appellate authority.

14. Distribution of Seats in Govt. Institutions (Govt. Medical/Dental College, Amritsar & Patiala, GGS Medical College, Faridkot.) I. In the Govt. Institutions, 50% of the total seats shall be filled by the

Government of India at All India level, and remaining 50% seats shall be filled at the State level as State Quota seats.

II. 50% seats of the State Quota seats shall be reserved by way of Institutional Preference, for general category candidates who have passed their qualifying examination from Baba Farid University of Health Sciences, Faridkot.

IV. Eligibility criteria:-

A. For in-service Regular PCMS/ PCMS (Dental) doctors:- a) The Eligibility requirements for grant of incentive shall be as

under :

i. Regular PCMS/ PCMS (Dental) employee ii. Has completed 4 full years (48 months) service in very

difficult (Category- D) area or 6 full years (72 months) service in difficult (Category C) area or an appropriate combination of both. In case of candidates who have completed 5 full years (60 months) of service (as on 01-01-2012), they should have completed 2 full years (24 months) of service in most difficult areas or 3 full years (36 months) of service in difficult areas. Very difficult (Category D)/ Difficult (Category C)/ Most Difficult/ Difficult area, as the case may be, shall be as defined by Department of Health & Family Welfare, Government of Punjab.

iii. RMOs once they are selected in PCMS/ PCMS (Dental), will be given benefits of rural service rendered by them as RMOs under Zila Parishad.

iv. Has cleared the probation period v. Has Good service record vi. Has no vigilance/departmental/disciplinary inquiry pending

against him/her. vii. Will have 10 years of service left after completion of the

course.

b) The period of rural service shall be computed as on 31st March, 2018.

c) Adhoc service rendered in respective category will be counted for the purpose of computing the stipulated period.

d) PCMS/ PCMS (Dental) in service candidates will submit along with the application a certificate regarding length of service, length of rural service, number of years of service left after completion of PG course & that no department/vigilance inquiry is pending against the candidate.

e) All PCMS/ PCMS (Dental) doctors who are selected for admission to post graduate courses shall have to produce a No Objection Certificate from Department of Health & Family Welfare, Govt. of Punjab.

f) All in service doctors shall have to submit a bond of Rs 50 lakhs to serve the Punjab Government for a period of 10 years after completion of Post Graduate degree course or bond of Rs 25 lakhs to serve the Punjab Government for a period of 6 years after completion of Post Graduate diploma course. candidate fails to do so he/she shall have to deposit the bond money with the Government.

B. For State Quota candidates [except in-service PCMS/PCMS (Dental) candidates]:-

a) Any candidate in State Govt. employment (other than PCMS/PCMS Dental) shall produce No Objection Certificate from his/her employer along with the application for the counselling and shall not be given any incentive of marks for service rendered.

b) Candidates selected in State Quota shall get fixed emoluments / stipends as determined by the Punjab Government from time to time for the complete course, subject to the following conditions:-

i. The candidate is to submit a bond of Rs15 Lakhs to serve the Government of Punjab for a period of two years after completion of PG. This clause will not be applicable in case the offer is not given by the Government of Punjab within a period of one year of passing of the postgraduate examination.

ii. The candidate will inform the Government of Punjab that he/she has passed the postgraduate examination.

iii. Failure to serve the Government of Punjab for a period of two years will lead to deposition/ recovery of bond money by the Government of Punjab.

• For All India quota candidates

All India quota candidates shall submit a bond of Rs. 10 Lakhs to serve the State of Punjab for one year after completion of Post Graduate course. All other conditions shall remain the same, as for State Quota candidates.

V Reservation i. Scheduled Caste 25% ii. Backward class 10% iii. Physically handicapped 3%

The reservation of physically challenged persons shall be as per the provision of Right of Persons with Disability Act-2016.

(iv) All the certificates shall be as per latest instructions issued by the Government of Punjab.

III. NRI Seats

In Private Institutions, NRI Seats shall be 15% of the total seats. The seats shall be filled on the basis of merit in NEET-PG-2018, in order of preference as under:- Category I: NRIs/Children of NRI who originally belonged to the State of

Punjab. Category II: NRIs/ Children of NRI who originally belonged to an Indian

State other than Punjab. (a) Under this policy Non Resident Indian means a person of

Indian origin, whether holding an Indian Passport or Foreign Passport, who is either permanently or temporarily settled outside India and in either case:

(i) For or on taking up employment outside India ; or

(ii) For carrying on a business or vocation outside India; or

(iii) For any other purpose, in such circumstances, as would indicate his intention to stay outside India for an uncertain period;

For ascertaining the eligibility relevant documents including Foreign Country Passport or Green Card or Permanent Resident Card or Proof of Residency of Foreign country issued by competent authority or OCI/PIO Card issued by the Government of India shall be taken into consideration.

(b) If sufficient number of candidates under first category is eligible and available then they will be admitted first even if students under subsequent category are higher in merit.

(c) The left over seats in the NRI quota in private institutions will go to management quota and shall be filled on merit based upon the NEET-PG-2018.

(d) Candidates applying under NRI category must get eligibility certificate from BFUHS, Faridkot.

(e) The NRI students will have to give a Bank Guarantee / Surety Bond for the balance of fee, if they opt to pay the fee in instalments on annual basis proportionately.

(f) Any misrepresentation, false information, fake documentation and impersonation for securing admission shall be punishable under the relevant provisions of Indian Penal Code and other applicable laws along-with cancellation of admission.

(g) Availability of NRI seats shall be as per roster maintained subject-wise by each Private College.

IV. Fee Structure in Private Colleges:

Fee structure for the Private Institutions for the year 2018 is as under (as per notification No. 5/14/2015-5HB III/844 dated 07.05.2015):-

17. Schedule of Fee

i) Tuition fee for the six months shall be paid to Registrar, Baba Farid University of Health Sciences, Faridkot.

ii) If candidate is shifted from one College to another in the subsequent Counselling, the fee shall be refunded / adjusted after completion of the admission process. The University shall refund / adjust the fee within 30 days.

iii) The fee shall be payable on six monthly basis and the institutes shall not insist the candidate to deposit fee of the entire course in advance.

iv) Hostel fee and security advance (Annual and provisional) for private institutes.

a. Non Air Conditioned Independent Room : Rs.16,500/- per year for the First year (Maximum) for single bedroom. This fee may be hiked by maximum upto 5% for each subsequent year.

b. Special Room (Attached bathroom and AC) : Rs.26,400/- per year (Maximum). This fee may be hiked by maximum upto 10% for each subsequent year.

c. Security Advance Rs.27,500/- or its equivalent (refundable in the shape of demand draft.)

d. Mess charges- Actual and to be managed by the students.

e. These charges are exclusive of electricity and water charges but inclusive of every other charge. All charges shall be levied from students on six monthly basis.

v) The fee for six months will be collected on the spot by the Baba Farid University of Health Sciences, Faridkot on behalf of the respective institutions at the time of counselling and the same

will be transferred to the respective institutions after completion of admission process.

vi) The fee will be payable on six monthly basis for all the categories and the institutes shall not insist upon the candidate to deposit fee of the entire course in advance. At the most, candidates can be asked to furnish a Bond/Bank Guarantee that they will be liable to pay the balance fee for the whole course even if they leave the institute/course midway.

vii) The fee shall be refunded / adjusted if candidate is shifted from one college to another in the subsequent counselling.

17. Conversion of seats:

All India Quota - The vacant seats transferred from All India quota, shall be filled as per MCI/DCI and GOI guidelines. In case there are no guidelines, these seats may be filled as per the guidelines issued by Government of Punjab.

18. Procedure for all seats remaining vacant after 2nd counselling including NRI seats.

i) The left over NRI seats shall be shifted into General pool.

ii) For all left over seats in Private or Government Medical/ Dental Colleges the counseling will be held in GGS Medical College Faridkot with the same merit as that of NEET-PG-2018. No fresh applications shall be called by individual institutions.

19. Admission criteria for training in Ultrasonography.

a) Admission shall be based on merit in the NEET-PG-2018.

b) Counselling for Six month training in Ultrasonography shall be held at once, however, classes for 2nd batch will commence after completion of 1st batch.

c) Only PCMS candidates shall be eligible for admission in Govt. Medical Colleges. However, remaining vacant seats shall be filled from eligible Medical Graduates from the State of Punjab.

d) From amongst PCMS doctors, only those doctors are eligible who have rendered minimum 3 year service and have successfully cleared their probation period.

e) The PCMS candidate shall have to serve Government of Punjab for 5 years after training and shall have to furnish bond of Rs 30 Lakhs for serving 5 years in PCMS.

f) Medical Graduates from the State of Punjab are eligible for admission in Private Institutions for training in Ultrasonography.

g) There shall be reservation of seats as prescribed for PG Degree courses in Government and Private Institutions.

h) Fee in Government institutions for whole course shall be Rs. 20,000/- only.

i) Fee in private institutions shall be Rs. 1,00,000/- only.

j) No stipend shall be granted.

20. In case of any doubt, the Administrative Secretary, Department of Medical Education & Research, Government of Punjab may issue clarification which will have force of the policy.

21. For any dispute arising out of this notification or of the admissions under this notification, the Administrative Secretary, Department of Medical Education & Research, Government of Punjab shall be competent to resolve whose decisions shall be final.

22. This notification supersedes all the previous notifications for admission to the postgraduate degree/ diploma courses & Six month course in ultrasonography, in the State of Punjab.

from such service by the government for any reason and no such resignation accepted or

purported to have been accepted by any authority shall for the purpose of this bond only,

become effective unless the pervious permission in writing of the competent authority has

been obtained by the Principal.

The decision of the competent authority as to the commission of breach or as such

indiscipline, misconduct on the part of the Principal and as to whether the Principal has or

has not performed and observed the conditions and obligations under these presents shall

be final and binding on the principal and the sureties.

And upon making such payment the above written bond shall be void and of no

effect but otherwise it shall be and remain in full force and virtue.

It is further agreed that in the event of the Principal being unsuccessful in any of the

qualifying examination conducted in the said institute/college/hospital, the government may

at their discretion, with hold the payment of scholarship for the prosecution of further studies

and the decision of the competent authority in this behalf shall be final and binding.

It is agreed that the liability of the sureties hereunder shall not be impaired or

discharged by reason of time being granted or any fore-bearance, act or omission of the

government (whether with or without the knowledge or consent of the sureties) in respect or

in relation to the several obligations and conditions to be performed or discharged by a

Principal or by any other matter or thing what so ever, the law relating to sureties shall but

for this provision have the effect or releasing the sureties from liability nor shall it be
