Home ownership document (e.g. bill of sale, property tax bill, mortgage statement, master statement

from closing documents, etc.)

Properly signed forms for those who are “Living with an Oak Park Resident," "In Lieu of Lease" or

enrolling a student who is not their biological child. Please visit www.op97.org and click on

Registration, then Forms to access the required documents

TWO recent utility bills (If you just moved in, we can accept vehicle/voter registration card, transfer of

services email from gas or light company that lists your name and new address, moving company

receipts, credit card or bank statements with new address, etc.)

Illinois transfer form (Required) If your student is coming from another Illinois public school, please

contact your old school for this form prior to coming in to register.
