農曆年後雖忙著整理大修繕後

的營地，但還是得顧及有迫切需

要的成癮患者，所以不敢延緩，

隨即在今年的 2/10 日，恢復啟用

因修繕而停止收案 3 個月的花東營地，重新提供

對酒/毒癮患者的服務。 這一次修繕，經曾於蘆葦營接受輔導而持續享

受康復的成癮人士及社會熱心人士的大力資助，

終於湊足了 150 萬工程款，順利完成了此階段的

修繕工程。

這件事在我心裡產生了極大的感動，一群在一

般人刻板印象中似乎無可救藥的人，當他們開始

走向清醒後對社會的貢獻力，竟比起一般人過之

猶無不及，實是令人驚訝，就如在美國許多的研

究發現不謀而合。

這使我想到聖經中有一段耶穌講的比喻:「又有

落在好土裡的，就結實，有一百倍的，有六十倍

的，有三十倍的」(馬可福音 4 章 8 節)。這段的

經文出處中，耶穌對種子落在何方講了好幾種比

喻，我想這些甘願奉獻一己之力的成癮患者，不

是也已經找到了他們生命中的好土了嗎?

那同工們不就是這片土上的耕耘者嗎?

1. 花蓮營地的施工已在今年的春節完成，開始

接收成癮的個案。每期人數可收 6 人，包括

處理復發的個案。

After the Chinese New Year, although much cleaning up remained in our newly renovated Center, the pressing needs of addicts were high on our priority. Therefore, without further delay, after being closed for three months for renovation, our Hualien Center reopened on February 10 of this year.

This renovation cost a total of NT$1.5 million, but with generous contributions from our recovering graduates and enthusiastic supporters, we were able to finance the project. I am deeply touched by how a group of people stereotyped as hopeless by society can become even greater contributors than the average person when they begin their journey in sobriety. Such an observation is consistent with many U.S. studies.

I am also reminded of the parable of the sower in the Bible: “Still other seed fell on good soil. It came up, grew and produced a crop, some multiplying thirty, some sixty, some a hundred times”(Mark 4:8). In this passage, Jesus tells many parables concerning where seeds fall, and I thought about these addiction patients who so willingly contributed—haven’t they too found the good soil in their lives? And aren’t our co-workers, then, cultivators of this soil?

1. Renovation at our Hualien Center was completed during the Chinese New Year, and we have re-opened to addicts seeking recovery. Our capacity is six per term, including relapse intervention.

月做介入毒癮症的演講，針對毒癮個案管理

師授課，也分別做 4 次的個案研討，分析個

案的處遇及提供建議。另外也委托我們從 6

月起帶領成癮者家屬的康復團體共 6 次。

3. 三月歡迎台東家立立社會褔利基金會的心理

師及關懷員共 3 位參訪本營地的戒治，做主

題觀摩。

4. 社團法人新北市臨床心理師公會邀請我們在

4 月做成癮介入的演講，針對各領域的臨床心

理師做繼續教育訓練。

5. 彰化縣觀護室邀請我們在 5 月針對毒癮個案

授課，題目為復發，幫助成癮者了解為何「戒」

了又吸，知道如何預防及處理毒癮疾患的復

發。

2. Targeting their caseworkers, the Changhua County Government Department of Social Affairs Drug

Prevention Center invited us to lecture on addiction intervention in March and May, and also act as consultant on four case presentations. In addition, we were invited to lead 6 recovery sessions for families of addicts, beginning in June.

3. We welcomed a psychologist and 2 caseworkers from the Galilee Family Social Welfare Foundation

to our Center in March for thematic observational learning.

4. In April, the New Taipei City Licensed Clinical Psychologist Association invited us to lecture on addiction intervention for its continuing education for clinical psychologists in various disciplines.

5. In May, the Changhua District Probation Office invited us to introduce to drug parolees the concepts of relapse, relapse prevention and relapse management to help them understand their repeated drug use despite rehabilitation.

這幾年來我到各單位演講及授課，感

受到我們當中許多專業人員的無

奈，因被委任毒癮及酒癮的康復工

作，但事實上對這個領域缺乏工具，

結局往往與家屬所做並沒有很大的

差別，只是比家屬多了一些資源或公權力，對成

癮者的疾病仍然不知該如何介入。即使聽過一、

兩次，甚至三次的演講，但這樣的訓練只能稱為

簡易概論，足於協助轉介工作，無法直接及實地

的協助成癮者的康復。因此除了上次所提的

Certified Peer Specialist (CPS)訓練課程，另也設計

了針對專業人員的訓練套餐，包括簡易概論、成

癮家屬康復的訓練、戒治工作的訓練、及支持成

癮康復的訓練 (Recovery Support Specialist,

RSS)，希望能提供給前線專業幫助者在成癮問題

上更能備有介入的工具。在職訓練是專業人員的

專業誠信，雖然要付出時間、精神等代價，但對

我們所服務的對象及對自己，都是倍數的收獲。

今天的話題是「快講重點」。是上衛教課或來電

諮詢，很多家屬無法聽到我們所要表達的資訊，

因為心中只有一個重點，無論如何一定要「讓」

成癮者去戒治，以為只要能把成癮者弄到戒治中

心，問題就解決了。家屬沒有心先了解什麼是酒

癮或毒癮，什麼是戒治，更沒有心去思考自己與

成癮者如何陷入這疾病裡的病態互動而延誤治

療。然而對疾病、治療及病態互動的病識正是家

屬想幫助成癮者「戒酒」或「戒毒」的三大要點，

也是他們唯一能做的事情。有一個成癮者的姐姐

說得真好，她對不耐煩的父親說: 「爸!這些就是

重點!」的確，對疾病沒有病識，家屬要以什麼為

介入的根基呢?對戒治不了解，又如何正確的支持

成癮者的康復呢?自己處在疾病的方程式中，又何

能促使成癮者走向康復呢? 成癮致使今天的生活

失控並不是一朝一夕所形成的，家屬與成癮者的

毁滅性互動也如此。成癮者的家屬已多年的慌忙

與奔波，暫停一天，甚至一個月，重新調整方向

再介入，不會太晚，對成癮者、對自己更是有幫

助。

In the last few years of lecturing and teaching at

various organizations, I have felt the frustration of many professionals. While tasked with addiction recovery work, they lack the necessary tools. As a result, they are often at a loss of how to intervene, and simply mirror what families have been doing, except with more availability of resources and vested authority. One or even a few lectures can only count as conceptual overview, adequate for referral but not enough for direct practice in addiction recovery work. Therefore, in addition to the Certified Peer Specialist (CPS) mentioned in our previous newsletter, I’ve designed other training programs for professionals, including basic conceptual courses, family recovery training, addiction recovery training, and Recovery Support Specialist (RSS) training. I hope that these courses can better equip frontline professionals with tools for addiction intervention. Continuing education is a part of professional integrity. Although demanding of time, energy and other resources, the rewards are manifold for both our clients and ourselves. Please visit our website for course descriptions.

Today’s topic is “Get to the Point”. Whether in family education classes or during phone inquiries, families are often deaf to what we have to say. They are impatient in their resolve to get the addict into a rehabilitation center, believing this to be the ultimate solution to their problems. Hence they have little interest in the nature of addiction, the nature of recovery, and even less interest in understanding how their addictive interaction with the addict hinders treatment. Yet understanding the disease, its recovery and dysfunctional dynamics is key to helping the addict, and the only thing a family can do. The sister of an addict said it well to her anxious father, “Dad! These are the key issues!” Indeed, from what premise is a family to intervene without understanding the disease? Without concept of recovery, how is a family to support recovery? While trapped in a dysfunctional dynamic, how is a family to guide the addict toward recovery? Addiction does not bring about a loss of control overnight; likewise, the destructive dynamic between a family and the addict. After years of revolving around the addict, a day or even a month of moratorium on busyness in order to re-gather will not hinder intervention, but will in fact facilitate recovery for both the addict and the family.

這幾年在蘆葦營進進出出的人已上百了，但每

次看到個案自己提著包包，走進營地，說是要來

戒酒或戒毒的，那所謂的戒酒或戒毒的精義何在?

關鍵何在?

來蘆葦營尋求康復的成癮者，都要先經過一個

諮詢的階段，大多數是以電話來進行這個過程，

也有極少部分是親自前來進行諮詢的。無論是哪

一個諮詢管道，個案清楚了他自己將要來此做甚

麼以後，一般就會開始問些雜事，例如要帶甚麼

東西過來、甚麼東西能不能帶、需要多少錢等事

宜，最後…到了報到日，個案從家裡自行搭乘火

車，提著包包，走進營地。

以上似乎不重要的小細節，卻是戒治效力的關

鍵，我們把這個過程稱為「預前」。基本上預前的

工作就是戒治的開始，如果由個案親自處理，其

康復的比例就相當高，因為成癮者已開始承擔自

己的疾病與康復；如果是由家屬或其他的幫助者

處理的，成癮者康復的比例竟然是極低的，因為

他的疾病與康復仍然是別人的事。預前正確能促

使成癮者易走向康復，已是我們多年在這文化裡

的經驗所證實的。反言之，預前不正確的結果也

已經幾乎可以預料得到了。

因此，每當家屬或其他幫助者能放棄自己「陪

伴」或「護送」成癮者的需要，把康復的責任歸

回給成癮者，他康復的比例就更提升。

回答回答回答回答::::首先，沒有喝酒或吸毒不代表清醒，僅能稱

為戒斷緩解狀態。相對的，復發並不代表再度使

用，而是走向再度使用的過程，是始於生理及心

理的後遺症，就算成癮者還沒開始想要喝酒或吸

毒，他也可能已在復發了。戒斷只是買到一張可

以走向康復的門票，此時眼前是一道斜坡，上往

清醒，下往復發。成癮者一生要面臨保持清醒與

隨時復發之反覆中，但要記住: 復發的結局並非

絕對是再度使用，而再度使用是因為沒有妥當處

理復發的最後收場。再者，復發不是突然的，它

具有相當長的程序及預兆。成癮者必須認識他獨

特的復發警告訊號，以中斷他的復發，才能防止

發展到最後的失控。三者，復發是康復過程不可

或缺的現象，如同飽足感與飢餓感是血糖的兩種

Over the years, hundreds of addicts have been through our Center. When a person walks in with bag in hands seeking treatment, what is he looking for? In other words, what is the essence, the key to recovery?

All addicts seeking treatment at our Center must first go through an interview, usually by phone and occasionally in person. Once a person grasps his purpose and tasks at the Center, he starts asking logistical questions such as what should or should not be brought, how much money is needed, and other routine details. Finally comes registration day, and toting his luggage, he boards a train and shows up at the Center. Seemingly trivial, these details are vital to recovery. We call this process the “Preparation,” and it is in fact the beginning of recovery. By personally managing these details, the addict’s chance of recovery is high because he has taken responsibility for his own disease and recovery. If his family or other helpers assume these details for the addict, his chance of recovery is low because his disease and recovery become the responsibility of others. Our years of experience in this culture have shown that appropriate preparation facilitates recovery, while inappropriate preparation predictably hinders.

Therefore, when a family or other helpers give up their personal need to “accompany” or “escort” the addict, and return the responsibility of recovery to him, his chance of recovery is enhanced. “My husband has been in an out of rehab countless

times, and each time he gets out, he quickly relapses.

I guess he’s hopeless.” Answer: First, not drinking or using drugs does not mean being sober or clean; it is merely a remissive state of abstinence. On the other hand, neither does relapse mean a return to use, but refers to moving toward use. At the root of relapse are physical and psychological factors. Even before an addict might have any thought of using, he could be in relapse. Abstinence is only a ticket to recovery; in its face are a steep uphill towards recovery and a slippery downhill towards relapse. The ongoing challenge of sobriety and relapse is a reality for the addict, but remember! Relapse does not have to end in use; rather, return to alcohol or drug use is the final outcome of a poorly managed relapse. Second, relapse is not sudden—it has a rather long progression of warning signs. Each addict must learn to recognize his unique pattern to interrupt

表徵。換言之，有清醒就會有復發，它們是康復

的正反面。所以問題不在於成癮者是否會復發，

就算他是保養的模範生，生活一帆風順，他還是

會復發，因為他的身體還在承受酒癮或毒癮這個

慢性疾病的破壞，在後遺症的攻擊下，他必然會

有復發。所以重點是，在戒斷的起點，成癮者如

果努力去建立病識、處理後遺症、認識自己的復

發模式、練習沒有繞在酒或毒的思考，他就是進

入了所謂的清醒，即使往復發下滑，也能察覺到

並盡早中斷他的復發，就算他中斷失敗而再度使

用了，也知道該如何處理及求助。這就如同我們

平日的保健能讓我們有足夠的免疫力去應付環境

裡的細菌和病毒，就算生病了，也有足夠的健康

讓我們康復。

Then when he relapses, he can quickly identify his symptoms and curb its development. Even if he fails to manage his relapse to the point of using again, he knows what to do and how to seek help. This is similar to maintaining a healthy immune system so that it is capable of defending us against diseases, and should we fall ill, we are still capable of recovering.
