REGISTERED NURSE OR REGISTERED PRACTICAL NURSE SENIORS DAY PROGRAM FOR INDIVIDUALS WITH DEMENTIA & THEIR CAREGIVERS Position Description: The Nurse will provide person-centered support to older adults with dementia while engaging them in interactive life enrichment activities. Under the direction of Jamaican Canadian Association Nurses (JCAN) and in collaboration with TAIBU Community Health Centre (CHC), this position will directly report to the President of JCAN. The program will operate at TAIBU CHC. Transportation support will be made available to program participants through the Ubuntu Village bus. Duties and Responsibilities: • Coordinate programs and services • Provide initial & ongoing assessment to determine participant needs • Develop nursing component of Individual Plan of Care and supervise provision of the services • Document all programming provided daily including quarterly signed/dated progress reports • Provide education and counseling to participants and families as indicated in Individual Plan of Care • Assist participants with ADL’s and/or IADL’s as needed • Maintain current and accurate written case management records, prepare variety of written reports and correspondence • Participate in daily activities of the center including assisting as needed with meal service, clean-up, etc. • Perform related tasks as assigned • Maintain confidentiality of client and organization’s information Qualifications: • Nursing Diploma or degree from a recognized college or university • Current registration with the CNO and First Aid/CPR Asset: • At least one year of experience working with people with dementia or developmental disabilities • Behavior management techniques • Day program facilitation and instruction • If interested please submit resume to [email protected] by September 27, 5:00pm or call 905-839-1717
