REQUEST FOR PROPOSAL Lawn Care 2013 - 1 - Proposal For: Property Management of Iowa LLC PO Box 42293 Urbandale, Iowa 50323 This proposal is requested by Multiple Associations, herein after referred to as Association. Prepare and Address individual Proposals for each Property. 1. Scope of Work: Work will be completed according to the following specifications: Lawn Care Per Mow Pricing: Based on 26 Mowing events and Line trimming Per Application Pricing: Fertilization Applications o Early Spring Application o Spring Application o Fall Application o Late Fall Application Tree and Bush Trimming 2. RFP Deadline: All bids must be submitted by _April 12, 2013. 3. Working Hours: Contractor agrees that work will be conducted between the hours of 7:30 a.m. and 6:00 p.m. Monday – Friday. No work will be conducted on Saturday or Sunday without express written consent of the Association. 4. Requirements: Contractor must conform to all applicable codes, industry standards, or manufacturer’s specifications. Contractor must clean up after the work is performed and restore the property to the condition prior to the work begins. 5. Materials: Contractor will provide specifications for all required materials. 6. Storage of Equipment: Job trailer may be left in visitor parking located at the entrance of the property while work is in progress. Job trailer must be removed immediately upon completion of the entire project. 7. Damage/Restoration: If the contractor’s employees, agents, or subcontractors damage any common or individual property in the community while performing contracted work, the contractor agrees to fully restore the property to its original condition.

8. Insurance: The Contractor will provide the association with a current copy of the Contractor’s Certificate of Insurance, including liability, worker’s compensation, and vehicle insurance, prior to any work beginning.

9. Licenses and Permits: The Contractor must obtain all necessary licenses and permits for the work prior to beginning the work.

10. Warranty: Contractor agrees to warranty his work (according to the above defined scope) 1 year New Plant materials and 1 year labor warranty after the completion of the job.

11. Clean-Up: Contractor will be required to clean up, dispose of trash, and police the work area on a daily basis including the use of a magnet to collect nails and metal. All tools and equipment should be kept in a neat, orderly, and safe manner. Contractor will be responsible for hauling of any waste off site. No trash receptacles will be provided by the Association.

12. Indemnification: The Contractor will indemnify or reimburse the Association for any amount the Association is required to pay because a claim was made against the Association as result of the contractor’s work, as well as for any legal costs associated with defending the Association against any claims. Both parties to this contract agree to indemnify and hold harmless Property Management of Iowa, LLC and any of its employees or agents.

13. Compliance: The Contractor is required to comply with all Federal, State, and local laws, regulations, or codes that are applicable.

14. Termination: The Association may terminate this agreement with 24 hours written notice with or without cause at any time. The Contractor may terminate this agreement with 24 hours written notice at any time PRIOR to the beginning of the work.
