GAIL(India)Ltd. NCR FOREWORD In National Capital Region Work Permit System is a key element for achieving the high standard of safety, where flammable gases are transported. Departments must evolve a routine whereby personnel (frequently maintenance personnel) can safely complete work in situations which if not controlled would be extremely hazardous.

To ensure safety, the ''human element'' must be eliminated as far as possible by using a system which requires formal action. Such a system is the permit-to- work system which ensures that a formally approved and acknowledged notice is in the possession of the person in charge of the operation, before work begins, to the effect that all necessary action has been taken.

It consists essentially of an organised predetermined drill to ensure that:

1. from a specified time, a designated location or plant is safe to work on 2. nothing is done during the course of the operation to prejudice the safety of the

workers, and 3. everyone is aware of the limitations to their activities which must be strictly

observed if safety is to be assured.

The design of an effective permit-to-work system may be complex and requires considerable forethought For terminal operations, both preventive and online maintenance activities need to be undertaken. The O&M activities in the plant need to be carried out as per well laid out work procedures that take into account not only the precautions necessary for such field engineering activities but also those arising out from the fact of their being carried out in a hazardous environment. Work permit system manual delineates the safety standards that need to be met for carrying out the different activities. It lays down the roles, responsibilities and the required communication between the different people/Departments concerned for proper monitoring, control and smooth & safe execution of the work. The system is developed taking inputs from OISD – STD – 105 and past experience of GAIL.

The work permit system has been in use at GAIL NCR-O&M since inception. The magnitude of activities and the personnel deployed have increased manifold. A need was felt to improve on different aspects of the work permit system like issue procedures, feed back and control procedures for effective monitoring of the activities along with ensuring adequate safety and speed of execution..

With each one of us adhering the safe practices and procedures laid down in this manual, I am sure , we will usher in a new chapter of safe working in our plant and meet the challenges of completing the maintenance or new jobs in a safe manner. Date: 15. 05.2007 ( Anoop Dhar)

GAIL(India)Ltd. NCR 2.0 WORK PERMIT SYSTEM AND TYPES

1.0 WORK PERMIT

The permit is a document giving an authority to the executing department person/ contractor to execute the specific job in specific time limit at a specified location under the specific conditions.

2.0 TYPES OF WORK PERMITS There are Five types of the permits i) Cold Work Permit (Yellow colour). ii) Hot Work Permit (Red colour). iii) Excavation/ Vessel entry/Vessel Box – up (Red colour) iv) Blanket permit (White colour) v) Electrical Isolation and Energisation Clearance.( White Colour) For specimen, please see Annexure – I

2.1 Cold Work Permit Cold work is an activity which does not produce sparks or sufficient heat to ignite a flammable substance or which does not normally cause a serious hazard to the life or health of people.

1. Examples: Checking of vibration of the pump, tightening, grass cutting, Unloading of

Acid tankers.etc.

2.2 Hot Work Permit

Hot work permit is an activity which produce sparks or sufficient heat to ignite flammable substance and the job, which can cause serious hazard to the life or health of personnel, is treated as hot work. Examples: 1. Any job that requires the use of the following:

a) Arc welding equipment, gas cutting and welding torched, soldering equipment, any type of electric heating devices, blowtorches and other open flames.

b) Usage of Diesel generator, Diesel vibrator c) Grinding wheels d) Electric drills and other electric equipment including non – explosion proof lights

connections and extension cords.

e) Shot / sand blasting and similar spark producing surface preparation. f) Stress relieving, preheating and induction heating. g) Electric flash unit / flash bulb for taking photographs and video photography.

Locating and operating welding equipment, internal combustion engines or any other equipment which may create a source of ignition in hazardous areas.

h) Concrete chipping. i) Driving any automotive equipment inside process and utility areas, tank farm,

adjacent to any of the sumps in the oil separator, adjacent to the hydrocarbon pumps and flare stack areas.

2. Radiography, Road closure, Rock blasting, Hot tapping 3. Stopping of steam leaks on live line by using pneumatic drill. 4. Usage of Ultrasonic instruments for thickness measurements. 5. Working at height of 3m from any solid working platform.

2.3 Excavation / Vessel entry / Vessel Box – up

Excavation of the ground to a depth of more than 0.75 m, road cutting at any place in the plant requires a Excavation permit. Entry into tanks, boilers, furnaces, Vessels, drums, sections of the pipelines etc. requires a Vessel entry/ Vessel Box – up permit. Excavation/Vessel entry/Vessel Box- up permits are combined in a single permit for simplicity.

2.4 Blanket Permit A Blanket permit is a permit issued for carrying out multiple jobs, both cold and hot, under the specific conditions. Examples: Any fabrication yard.

2.5 Electrical Isolation and Energisation Clearance/Permit:

Any job related to the power supply/cut- off requires Electrical Isolation and Energisation Clearance/Permit. Example: Power supply cut off to motor

3.0 BASIC POLICIES AND GUIDELINES

3.1 It is the policy of the company to ensure the safety of people, equipment and materials

when performing any work within the plant premises. 3.2 Jobs shall be performed only after obtaining the stipulated permit. 3.3 Permit shall be issued/ renewed and received only by personnel designated for the same

and are to be signed by both the issuing/ renewing and receiving personnel. 3.4 All necessary checks and tests shall be conducted to ensure safety of the jobs prior to issue

of permits. All precautionary measures required to be taken shall be clearly indicated on the permit and their compliance ensured before handing over the permit to receiver.

3.5 Permits shall be worded so as to limit the work to the designated areas / equipment /

facilities, agencies and time. 3.6 If any of the conditions vary during the work, which may affect safety of human beings and

equipment, the job is to be stopped, the permit to be withdrawn and given back to the issuer. Reason for withdrawing the permit to be mentioned on the permit.

3.7 If a permit is withdrawn, the work should be restarted only after safe conditions are

established back and clearance obtained from the issuing/renewing authority and Fire Safety

3.8 Work permits shall be made readily available at the work site for verification by concerned

personnel. Failure to present the permit on demand for verification shall result in stoppage of work.

3.9 Work permits are valid only for the time period specified in the permit. Normally the permits

will be issued for the General Shift timings i.e. 9.15am to 6.00pm or General shift hours. 3.10 If the job is to be continued beyond the time limit specified in the permit, it has to be

renewed at the beginning of the next shift. In case the job is to be carried beyond day light hours or on holidays the permission letter from OIC of the plant is to be taken in writing for execution of the job.

3.11 Blanket permits can be issued for major project, fabrication yards, work shops etc.,

jobs for a maximum period of 3 months. However this permit must be presented everyday prior to starting of the work in each shift by the concerned executing person to issuing authority for renewal.

3.12 While renewing a work permit that has already been issued by another issuing authority of

the concerned department, the renewing authority is no way relieved of the duties and responsibilities specified by the original issuing authority.

GAIL(India)Ltd. NCR 3.13 At the time of renewal of a permit or when the job is in progress, if the issuing or renewing

authority as the case may be feel that some additional conditions or precautions are to be stipulated or any of the conditions or precautions already stipulated are not necessary based on the prevailing conditions, a new permit is to be issued incorporating the changes and the previous permit should be withdrawn.

3.14 All work permits must be closed on completion of the job by the Supervisor of the executing

department /section and the permit duly signed for closure is to be returned to the issuer. Work permits once closed cannot be renewed.

3.15 The work permit should be issued for one job only. Separate permits to be issued for

performing separate work on same equipment. 3.16 No hot work shall be allowed to be included in the hot work permit already issued for other

jobs. 3.17 Whenever there is a job in pipe track which is in close vicinity (5meters) of process unit

then prior to issuing of the permit the clearance from the concerned unit – in – charge should also be taken.

3.18 Always “Receiver” has to sign the permit last, after taking all the necessary approvals and

understanding the necessary safety measures. 3.19 The job demarcation of each supervisor shall be clearly defined on the permit before hand,

in case more than one supervisor working in the same area. 3.20 All the permits stands cancelled on declaration of emergency. Fresh renewals are

to be taken after All clear signal for emergency is given.

4.0 DUTIES AND RESPONSIBILITIES UNDER WORK PERMIT SYSTEM

Permit issuing authority, executing authority and F&S representative to discuss all critical jobs before issuing the permit.

4.1 Issuing/Renewing Authority

The issuing/renewing authority is primarily responsible for the safe execution of the job covered under the work permit and as such is to perform the following duties:

4.1.1 Analyse and determine the condition of the facility and the area involved and Stipulate necessary measures to be taken to ensure safe execution of the work.

4.1.2 Stipulate precautionary measures such as area preparation, blinding/isolation /tagging,

provision of safety and fire protection equipment, standby personnel etc., in the permits and ensure the compliance.

4.1.3 Stipulate preparatory, preventive and precautionary measures to be carried out and ensure

their compliance. 4.1.4 Co ordinate with other departments/sections wherever precautions are necessary to be

taken from their end also and counter signature to be obtained prior to issuing of the permit.

4.1.5 Verify and certify installation and removal of blinds/isolation and their tagging. 4.1.6 Perform necessary gas and other tests and the person performing the gas test to sign on

the permit. 4.1.7 Ensure that the facilities and the area at and around the place where permit is issued /

renewed are made safe prior to commencement and also during the progress of the job. 4.1.8 Checks periodically the progresses of the job permitted and stop the job if conditions

become unsafe. 4.1.9 Ensure that while hot work is in progress, no other work conducive to hazards is permitted

in the vicinity eg. Repairs to Oil or Gas lines, bleeding or sampling of flammable materials, draining of liquid hydrocarbon, venting of gas in the area etc.

4.1.10 Ensure that all personal protective equipment/ clothing stipulated in the work permit are

available at site before commencement of work and the same used by the personnel while performing the work.

GAIL(India)Ltd. NCR 4.1.11 If a hot work permit is issued or if hot work permit continues beyond the normal office

working hours (after 6.00pm) then the information about the same is to be given to Fire Safety Control Room and Control Rooms (shift) by telephone.

4.1.12 If the job is being done through contractor and in case more than one contractor is working

in the same area then the issuer shall clearly define before hand the demarcation of each job per contractor to avoid over lapping of different jobs.

4.1.13 The issuer shall give clearance to the Job on Gas / Oil pipeline to Engineer (GAIL) for

carrying out the hot jobs and not directly to the contractor personnel. 4.1.14 In case of Hot work permit for cutting, welding etc., the issuer or his representative and the

receiver must be present for the first cut or first weld. 4.1.15 In case of tank truck (Ex. Acid , Alkali etc) unloading the concerned operations supervisor

will give cold work permit to concerned job supervisor and not to the contractor or the tank truck driver.

4.1.16 For unit housekeeping, mopping (regular contract) operations supervisor will give cold work

permit to concerned GAIL engineer. 4.1.17 Ensure that “Receiver” returns back permit duly signed on completion of job and “House

keeping” is maintained. Scrap is shifted to designated area. Returned closed permit are preserved for minimum 90 days.

4.2 Receiving Authority/Executing Personnel

Although a work permit has been issued for performing a certain job, no way it relieves the supervisor of the executing department or the person executing the work of their responsibility for safe execution and completion of the job. The receiving authority/executing personnel is/are responsible for the compliance of the precautions / conditions prescribed in the permit. Their duties and responsibilities include the following:

4.2.1 Secure necessary work permit from the concerned department before commencement of the job.

4.2.2 Provide for and comply with all necessary precautionary measures stipulated in the permit. 4.2.3 Ensure basic safety precautions like usage of Safety belts, provision of ELCB (of

30mA),Flash back arrestor to cutting torch, DCP extinguisher / charged fire hose are provided as per the need of the job. Ensure that DCP and fire hoses are in good operable condition and available at site prior to start of the permit.

4.2.4 Take extra care when commencing the work, especially while breaking flange joints, cutting

through lines, striking an arc for the first time etc. Permit receiver to be present at job site for first cut or first weld.

4.2.5 Ensure safe working practices throughout by continuous supervision (Either by executing

personnel or contractor personnel) 4.2.6 Ensure the availability of all the Personal Protective Equipment/Fire fighting equipment at

site before commencement of work and also ensure that the same are used during the work.

4.2.7 Stop the job if conditions become unsafe and report to the issuing/renewing authority.

GAIL(India)Ltd. NCR 4.2.8 Ensure that the work permit is duly put in a plastic folder and displayed at the work site for

verification. 4.2.9 Ensure that no blinds or other isolations specified for the work are removed or disturbed

during the work. 4.2.10 Ensure that 24 Volt (DC) hand lamps are used in the confined space. 4.2.11 Ensure that grade area is barricaded by red & white colour tape to cover excavated area or

in case of material handling at overhead. 4.2.12 On completion of the job, measures and maintain normal conditions at the area of work.

Clear all scrap material and maintain good house keeping. 4.2.13 On completion of job the receiver will sign for closure of the permit after specifying the job

status at space provided at the back of the permit and return the permit to the issuer. 4.2.14 Ensure that the Contractor has received a booklet on “Safety Regulations for Contractors”. 4.3 Fire and Safety Personnel / Representative 4.3.1 Fire and safety personnel will advise and ensure that the work is being carried out safely

and should counter sign. Terminal In-Charges are the respective Fire & Safety Representatives of their respective region.

4.3.2 Fire and Safety representatives are to provide necessary additional/ special fire fighting

facilities at the request of the issuing /renewing authority depending on the criticality of the job being executed.

4.3.3 The Fire & Safety representative to make surprise checks at the various places to prevent

occurring of unsafe practices. 4.3.4 To continuously monitor the LEL in the area, if required.

GAIL(India)Ltd. NCR 5.0 PROCEDURE FOR COLD WORK PERMIT 5.1.1 The job card with safety precautions may be deemed as Cold Work Permit, wherever

management has authorised such use; but, no columns of the permit/job card should be left blank and it should be duly signed by the authorised personnel.

5.1.2 Issuing/Renewing Authority

Cold work permits will be issued and /or renewed by authorised personnel in the the rank of Executives. Cold work permit can be renewed for next shift only.

5.1.3 Receiving Authority 5.1.4 For jobs performed departmentally, the new as well as renewed cold work permits will be

received by the concerned employee of the executing department not below the rank of Executives.

5.1.5 For jobs carried out through contractors the cold work permits are to be received directly by

the employee of the executing department. The work request shall be closed by the receiver after job completion in the CMMS package.

6.0 PROCEDURE FOR HOT WORK PERMIT

6.1 Duties of Issuing/Renewing Authority 6.1.1 Hot work permits will be issued and /or renewed by duly authorised Supervisor not below

the rank of Executives from Operations dept. A list of such personnel should be displayed at Control Rooms.

6.1.2 Identify the hazards and note on the permit. Suggest additional safety measures to carry

out the job safely. 6.1.3 Permit valid only for one shift and may be extended for one subsequent shift if condition

permits. Issuer to note the extension of the permit on CMMS. 6.1.4 Issuer and receiver should be GAIL employees. The permits are issued between 9.15am to

10.00am from Control rooms generally. In case of any exigencies the permits are issued on requirement. The Permit is valid for the Morning shift up to 2.00pm . The permit if required is extended in the evening shift up to 6.00pm. The permits are issued only for the day.

6.1.5 After office hours the hot work (i.e. Welding, Gas cutting, Grinding etc.) to be carried out

with a fire watch from Operations department /Executing dept./F&S, as decided by the permit issuing authority.

6.1.6 If the permit is renewed, issuer will inform RGMC and Terminal IC on telephone. 6.2 Duties of receiving Authority

The permits are to be received by the company employee after understanding the scope of the job and ensure that the contractor personnel also understand /follow the safety regulations mentioned in the permit. For jobs carried out through contractors, the permits, fresh or renewal are to be received directly by the employee of the executing department. The work request shall be closed in the CMMS package by the receiver after the job completion. The receiver should duly fill all the required columns before returning the permit and he must ensure the site conditions are normal before returning the permit to Control Room.

GAIL(India)Ltd. NCR 7.0 PROCEDURE FOR EXCAVATION PERMIT Duties of Issuing/Renewing Authority 7.1 Excavation permits will be issued and /or renewed by duly authorised Supervisor not below the

rank of Executives from Operations dept. A list of such personnel should be displayed at Control Rooms.( including Control Room)

7.2 Identify the hazards and note on the permit. Suggest additional safety measures to carry out

the job safely. 7.3 Check for clearance from the respective departments namely civil, electrical, instrumentation,

fire and safety etc shall be ensured before issue of permit. 7.4 The area shall be barricaded, No Entry sign boards to be installed and alternate means for

movement of ERV / fire tender shall be directed. Normally, permission for full road cutting is not given.

7.5 Permit valid only for one shift and may be extended for one subsequent shift if condition

permits. Issuer to note the extension of the permit on CMMS. 7.6 Issuer and receiver should be GAIL employees. The permits are issued between 9.15am to

Duties of receiving Authority The permits are to be received by the company employee after understanding the scope of the job and ensure that the contractor personnel also understand /follow the safety regulations mentioned in the permit. For jobs carried out through contractors, the permits, fresh or renewal are to be received directly by the employee of the executing department.

The executing department shall ensure for the clearance from all other departments namely electrical, Instrumentation, Civil, Fire and Safety etc for any cables or piping in the excavation area.

The work request raised in the CMMS package shall be closed by the receiver after the job completion.

The receiver should duly fill all the required columns before returning the permit and he must ensure the site conditions are normal before returning the permit to Control Room.

8.0 PROCEDURE FOR BLANKET PERMIT A blanket permit is issued on the basis of location where the multiple jobs are to be carried out at a safe location. This permit is issued to have a limited control over the activity. HOD F&S / Terminal In-Charges are authorised to issue a Blanket permit, to carryout the multiple jobs both Hot and Cold under single permit. The authorisation from the operations department is as follows:

8.1 Blanket Permit requirements

i. A safe area away from the terminal area. ii. Cordoning off the areas. iii. Provision of the fire fighting facilities. iv. Built in Electrical safety devices like ELCB of 30mA. v. Power supply source and emergency Telephone No. for isolation of the power to be

written on the panel. vi. First aid box. vii. Display of safety posters.

8.2 Specific duties of Issuing /renewing authority 8.2.1 The Blanket permit for the specific location will be decided by the OIC of the NCR on

recommendation of HOD( F&S) and HOD(Ops) on satisfying with the site conditions. The authority for issuing the Blanket permit cannot be down delegated for the first issue however subsequent renewals can be done by Unit managers.

8.2.2 Will carryout gas tests and record the reading of the permit and initial. 8.2.3 Will inform RGMC and note the permit No. of the Blanket permit. 8.3 Specific duties of Executing Authority

Executing department will maintain the record of the following: No. of the agencies working at the site. No. of personnel working – list of jobs proposed to be taken under a Blanket permit

is to be attached duly signed by the issuer and receiver.

9.0 PROCEDURE FOR ELECTRICAL ISOLATION AND ENERGISATION CLEARANCE

Any job related to power supply/ cut off requires Electrical isolation and energisation clearance. Ex: Power supply cut off to motor. At NCR : The Electrical Work Permit is communicated between Issuer and Electrical dept. through software package installed in their respective depts. The software package comprises of all the electrical equipments within in the Terminal. Moreover the process is confirmed further by telephonic talk. This software system is made foolproof by incorporation passwords for respective depts. This system has reduced the man hours for the issue of electrical work permit. The history of the electrical work permit of any equipment can be known through this software any time. The detailed procedures are: a) Terminal Control Room authorised persons shall raise request for de-energisation of

process equipment in the software ‘ELP’ by mentioning tag number of equipment, Work permit number, Department which shall work on the equipment, Whether cable is to be disconnected or not and reasons for isolation.

a) Shift personnel of Electrical Department shall de-energize process equipments like

motors for pump, compressor, blower and heaters after receiving Electrical Isolation & Energisation Clearance (EIEC) to de-energize from Control Room in ‘ELP’ Software. Separate De-energization request for same equipment shall be sent by Control Shift personnel for issuing permit to separate department. However number of De-energization request shall not exceed two.

b) Cable shall be removed from equipment side if mentioned in EIEC. c) Shift personnel shall record all details e.g. Danger tag no, name of equipment, date

& time of de-energizing, name of the person in a new danger tag and in De-Energization and Energization Register (DER). Yellow Danger tag shall be put on the feeder & counterfoil in Counterfoil tray of relevant area. In case it is found already de-energized, new tag shall be assigned. In case cable is disconnected, same must be recorded in DER.

d) Electrical/ Shift personnel shall confirm de-energisation to Control Room shift

coordinator through ‘ELP’ software by mentioning the danger tag no and Panel number. In case it is found to be de-energized, then new danger tag number is to be mentioned in the second EIEC.

e) Electrical Shift personnel shall energize process equipments, after receiving the all

EIEC for same equipment with Energisation request in electronic form and verifying that cable is connected in equipments side.

f) Energisation shall be done by racking in CB/ putting on switches/ putting fuses.

g) Danger tag shall be removed from feeder and tag counter foil from tray. The removed tag shall be retained in tag tray for a period of 1 month.

h) Energisation shall be confirmed to shift coordinator by recording details in ELP software.

i) Electrical Shift personnel shall record all details in DER.

9.1 Duties of Permit Issuer

The Operation department personnel will raise the permit in the prescribed format in the software system as required for de – energization of the particular equipment. On receipt of permit back from electrical section duly filled and acknowledged with a tag no, lock out the system and the tag number is mentioned on the respective work permit for execution of the job. Further the permit for energization of the electrical equipment is raised. On receipt of the permit back through the software and removal of tag and lock, the same is confirmed at site and equipment is taken on line as per requirement.

9.2 Duties of Electrical Section

On receipt of the permit, Electrical section personnel will do the needful and note the action taken on the permit and return the permit to Operations/Issuer dept. In addition to this the Electrical Maintenance personnel will put a tag number and lock it with required information on electrical panel and on equipment. On resumption of power supply, tag is removed from all respective places and the same is communicated to the Operations/Issuer dept.

GAIL(India)Ltd. NCR 10.0 EXPLANATORY NOTES FOR COLD WORK PERMIT

All the points of the cold work permit are explained in the “Explanatory notes for Hot work permit”. The Issuer and the receiver are to sign the permit. The permit shall be closed after job completion in the CMMS package by the issuer. The work request shall be closed by the receiver in the CMMS package by the receiver after the job completion.

11.0 EXPLANATORY NOTES FOR HOT WORK PERMIT The check – listed items in the HOT WORK PERMIT form is elaborated below to explain the

underlying concepts and highlight their significance.

1. Equipment work area inspected. Equipment or area where work is to be conducted should be inspected to ensure that it is safe to carry out the work and assess other safety requirements / stipulations.

2. Surrounding area checked, cleaned up, oil rags grass etc., removed. Unsafe conditions for performance of work may arise from surrounding area. It should be cleaned-up to remove flammable material such as oil, rags, grass etc.

3. Sewers manholes, CBD etc and hot surface nearby covered. Flammable gases may be released from nearby sewers. Hot un-insulated surfaces/pipelines may provide a source of ignition. Therefore, these are to be properly covered to prevent fires.

4. Considered hazard from other routine/operation and concerned persons alerted. Other activities (routine / non-routine) being carried out near-by which can create conditions unsafe for performance of the permit work, should be taken into consideration and the concerned persons should be alerted accordingly.

5. Equipment electrically isolated and tagged and its No.

Before issuing permit for mechanical / electrical work in the operating area, it should be ensured that electrical switches are locked-out and cautionary tags duly signed with date and time are attached. Wherever local locking arrangement is provided in the field, the same should be used. Refer format for electrical lockout / energizing as per the Annexure.

6. Fire water hose, portable extinguisher provided

Running water hose and portable fire extinguisher are required respectively to flush / dilute in case of release of any hazardous chemical or to quench sparks and to put out small fires immediately.

7. Fire water system checked for readiness (where ever available). In order to meet any contingency, it should be ensured that the fire water system including fire water pumps, storage, network etc. is checked and kept ready for immediate use.

8. Equipment blinded / dis-connected / closed / isolated / wedge opened.

Equipment / Vessel, on which the work permit is being issued, should be completely isolated from the rest of the plant with which it is connected during normal operation, in order to ensure that there is no change in the work environment with respect to presence of toxic / flammable gases, liquids, hazardous chemicals etc. in the course of the work.

Blinding is one of the most effective ways of isolation. Blinds should be installed as close to the vessel as possible. If lines cannot be blinded, they should be disconnected and the open ends should be made safe by installing pipe caps/plugs, blind flanges, mud packing.

9. Equipment properly drained / depressurized Equipment under pressure should be depressurized after isolation. This will be followed by draining / purging / water flushing etc. as the case may be. Equipment containing liquid hydrocarbons should be drained completely. There may be a possibility of overlooking liquid collected in pockets or inaccessible areas such as level gauges, small nozzles / bleeders on vessels, laterals in pipe work etc. All low point drains should be in unplugged condition.

10. Equipment properly steamed / purged / water flushed.

Purging of equipment (vessels, pipelines, compressors etc.) is done to make them free of flammable hydrocarbons and toxic gases. Steam / Inert Gas is used for gas freeing of vessels and pipes. Other means of purging is by displacement with water and final traces of gas removed by air eductor. All high point vents should be unplugged while purging. Purging may be done continuously or in batches to conserve purge medium. It should be done in a systematic manner to cover the entire equipment/plant and continued till the concentration of toxic / flammable gas is lowered to allowable level.

11. Gas test done and % reading ______ and found OK.

Gas test is checked by Explosive meter. No hot work shall be permitted unless the LEL meter reading is showing less than 5. The person who is checking the LEL must put his signature on permit.

12. Shield against sparks provided In order to protect against welding sparks which can provide ignition in operating areas, shields are to be provided. The shield material should be non-flammable and should be kept wet with water.

13. Proper ventilation and lighting provided

Proper ventilation and lighting are to be provided. Some types of work like welding may generate fumes. Facilities may be required for the speedy dispersal of these fumes.

14. Proper means of exit provided Proper means of exit is required in case of emergencies developed on account of the work or otherwise. Availability of an alternate route of escape should be considered.

15. Precautionary tags / boards provided

To prevent any unwarranted entry in the work area and also to caution other personnel taking actions, which may endanger people working on the permit job, precautionary tags/boards are to be provided like "No Entry" sign on roads.

16. Portable equipment / Hose nozzles properly grounded and cable is without any joint.

As a precaution against static electricity generation, portable equipment / hose nozzles e.g. nozzle of a shot / sand blasting gun, are to be grounded. Use of hydrocarbon lines for earthing should be avoided.

Department Depending on the criticality of the job, work permit issuer shall decide the type of standby

to be provided i.e. from which department, of which level, how many and also additional fire fighting support facilities.

18. Iron Sulphide removed / kept wet. Pyrophoric substances may be present in operating area / equipment handling

hydrocarbon. Iron sulphide scales is the most common pyrophoric substances encountered. These should be either removed to safe locations or kept wet all the time to prevent their auto-ignition.

19. Area cordoned off. In order to prevent the un-authorised entry of people and to avoid accidents during excavation jobs, work area is to be cordoned off.

20. Checked Flame / Spark arrestor on mobile equipment No vehicle / engine without flame / spark arrestor is permitted in operating areas.

21. Checked for oil / gas trapped behind lining in equipment Before undertaking hot jobs, a check should be done for oil / gas trapped behind lining in

the equipment. Many a times oil / gas trapped behind lining depicts itself in the form of swelling and can be confirmed by way of drilling holes.

22. Oxygen Deficiency test done & % reading _____ and found OK.

The oxygen percentage shall not be less than 19.5% for execution of any job in the confined space.

23. 4 – Tier system is followed.

The 4 – tier system implies a standby personnel from each of the department namely, Operations, Fire and Safety, Maintenance dept and Contractor are provided for the execution of the critical job.

12.0 Explanatory notes for Excavation /Vessel entry / Vessel box – up permit

The points that are other than the Hot work permit are explained.

1. Equipment / Area inspected

Equipment or area where work is to be conducted, should be inspected to ensure that it is safe to carry out the work and assess other safety requirements / stipulations. In case of vessel box-up permit, it should be ensured that the work is complete, all personnel are out, no maintenance gear is left behind and debris removed.

2. Gas / Oxygen deficiency test done and found OK Gas test includes measurement of : (a) Hydrocarbons by Explosimeter (b) Oxygen Deficiency by Oxygen Meter

Gas tests may be specified for vessel entry including open excavation where head of a man will be below ground level and when hot work is being carried out. The person carrying out gas test must wear proper protective gear. No hot work shall be permitted unless the Explosimeter reading is zero.

Vessel entry, where no hot work is to be carried out, may be permitted if combustible gases are up-to 5% of lower explosive limit (LEL). Entry with an air supplied mask may be permitted with LEL of upto 50%.

The oxygen level should be at least 19.5 % vol. and the concentration of toxic gases below the threshold limits.

3. Proper ventilation and lighting provided

Where natural ventilation is not available, fans/air eductors are provided. Some types of work like welding may generate fumes. Facilities may be required for the speedy dispersal of these fumes. Only approved reduced voltage extension lights (not exceeding 24 volts) are to be allowed for work inside vessels from consideration of personal safety.

4. Precautionary tags / boards provided

To prevent any unwarranted entry in the work area and also to caution other personnel taking actions which may endanger people working on the permit job, precautionary tags/boards are to be provided like "No Entry" sign on roads or "Caution-Men at Work Inside" on the manhole of a vessel.

5. Standby person provided for vessel entry

Whenever a person is entering a vessel or work is being carried out in confined space, it may be necessary to keep standby persons (minimum two) at the manhole or entry point holding the rope connected to the safety belt of the person inside. In case of any emergency inside or outside the vessel, the standby will be able to pull the person out.

6. Precaution against public traffic taken

In case hot work is to be carried out in close proximity of a public road, for example in the case of a trunk pipeline, it may be desirable to block-off/divert public traffic for the duration of the job as a precautionary measure.

7. Clearance obtained for excavation from Technical/ concerned departments

For any excavation work which may affect underground sewers / telephone lines / cables / pipelines etc., Technical Services Dept. and / or other concerned departments, should be consulted for obtaining the co-ordinates and the depth to which excavation can be resorted to, without damaging the existing facilities. Markers should be put around the area where excavation can be done and the depth can be indicated in the work permit.

8. Clearance obtained for road cutting from Technical / Fire / concerned departments.

Since road cutting can hamper the movement of the fire vehicles, initial clearance should

be obtained from Fire Department and final approval from the higher designated authorities. If the road is wide, preferably the road should be cut half at a time. Duration of road cut should be restricted as far as possible

9. Clearance obtained for dyke cutting

When the dyke is cut, any mishap in the tank farm can lead to a free flow of oil to

outside the bund. A high level authority should be designated for authorizing dyke cutting. Further, it should be ensured that dyke would be reconstructed in the shortest possible time. For example, if pipes have to be laid through the dykes, the pipes should be laid quickly and plugged / capped so that the dyke can be closed. Thereafter, balance of pipe fabrication can continue without any risk. Standby personnel should be available at the work site, who should possess a suitable size steel plate for blocking the opening in case of mishap/tank failure. If no work is undertaken in evening and night shifts, invariably the gap should be closed with steel plate and mud packing at the end of day's work.

GAIL(India)Ltd. NCR 13.0 EXPLANATORY NOTE FOR ELECTRICAL ISOLATION AND ENERGISATION CLEARANCE. As already explained, the Electrical Lock out Permit (ELP) shall be raised in the electronic form by the Issuer and shall be complied by the Electrical dept.

1. De enegisation request by the operation dept. The Issuer dept shall raise the de energisation request to the electrical dept. The issuer shall mention the permit numbers , the receiver departments , name of the equipment, date and also a special mention for the de energisation from the motor end. This request shall give the details of the permit number and receiver departments to the electrical dept. More than one permit and receiver dept may work on the equipment. How so ever the de energisation request shall not exceed more than two for the same equipment. 2. De energisation confirmation by electrical dept. The electrical dept shall de energise the respective equipment from the switchgear room panel, feeder and the tag number shall be mentioned. This shall give the proper communication to the operations dept and the peer group of the electrical dept. The electrical dept shall mention the name of the equipment , date and the time of de energisation and also the cable connection to the equipment, if removed. The entire procedure is done as per the Manual M4 of the ISO documentation of electrical dept. 3. Energisation request by the operation dept. This to be done as per the same procedure of de energisation. All the permits for the respective equipment shall be returned and request for energisation is raised by the Issuer dept. Also a special mention is made for the cable connection at the motor end, if removed earlier. 4. Energisation confirmation by the electrical dept. The energisation confirmation is given by the electrical dept after removing the danger tags and energizing the equipment. The cable connections are done as per the requirement and the equipment is test run in the presence of operations and maintenance dept for the final acceptance.

The entire procedure of the ELP on the software package has improved the communication, documentation and overall the efficiency of the system of maintenance. The man hours lost is reduced with the help of ELP. The ELP software package is used only in the LPG Recovery plant.

14.0 EXEMPTED AREAS

The following areas are exempted where no work permit is required to carry out any job. No fabrication work is allowed beyond ______________________. All such jobs to be done in ___________________________ area or at place of Maintenance Shop , earmarked for this purpose.

As GAIL, Vijaipur has implemented CMMS i.e. Computerised Maintenance Management System, and work permits ( except Electrical isolation and Energization Clearance, which is again through an exclusive database, which operated between Control Rooms and Sub Stations) are issued through CMMS, which also helps in streamlining the maintenance activities including work permit system.

The salient features of CMMS are –

1. Simplifies the cumbersome manual processes involved in Equipment Maintenance, its schedule

2. Keeps the careful monitoring of Equipments 3. Keeps the maintenance records on line. 4. Improved availability, Reduced Downtime

The salient features of work permit system in CMMS are -

1. The Work permit system is under work control Module, which processes those jobs, which have been planned for a particular day (includes all types of jobs- Preventive, corrective , predictive etc) after being authorized by concerned department. The permits issued through CMMS for planned jobs and after job completion, job status could also be known based on which permit is closed.

2. New Work requests/orders and permits for any type of job can easily be generated here. 3. The formats with contents of Hot permit, Cold permit, Excavation/Vessel Entry Permit

are as per OISD-105. 4. Easy storage of all permits in Application Server that are issued on all days. 5. Easy monitoring of job status by Top Management at any particular time. 6. Flexibility in issuing permits with comparison to Manual system 7. For security reasons, only authorized persons can only enter into the package. 8. Easy retrieving data at any particular date and time. 9. Complete Information to Operations, Fire & Safety and all the departments including top

management for all jobs that are being carried out on any particular day. 10. At any particular day and time, a complete list of permits, whatsoever, can be generated

with Job Status. 11. Extension and Closure of the permits is strictly as per OISD-105. 12 Hot Permit, Excavation/Vessel Entry permits are to be closed on that day itself. 13. Fresh Permits will be issued on the next day. As these permits are computer generated, permits would be in black and white in colour only, against tri colours as specified in the standard. However for Identification of the permits in the field, permits have been stamped with colour as specified in OISD-105.However, it must be strictly ensured that relevant work permit must only be issued to the appropriate work.

Computerized Materials Management System (CMMS) package is a computerized software package. This package is basically a maintenance package in which Work Permit System is incorporated. The entire package is protected by different passwords for various departments.

The entire Vijaipur plant is divided into Plant, Area, Tag No. and Equipment No. under this package. All the plant equipments are brought within different maintenance schedules namely Modification, Corrective Repair, Predictive maintenance, Breakdown, Inspection, Miscellaneous, Opportunity maintenance and Construction. These equipments are maintained by various maintenance depts. namely Mechanical, Civil, Instrument, Electrical, Fire and Safety, Telecom, DVPL, HR, Medical etc.

GAIL(India)Ltd. NCR The Work Permit System is divided into various heads under the Work Control package of this CMMS system.

1. Work request: In order to perform a job within the plant, a work request is a must. All persons in the plant are given the rights to raise a work request to the concerned maintenance dept. Any job other than the preventive or predictive maintenance schedule shall involve a Work Permission letter for the clearance from respective departmental Heads. The permission letter is exempted for the Off – site locations.

2. Work Authorization: The work request shall be authorized by the concerned maintenance

dept. to plan for execution of the job for that day or later as per their manpower and schedule. The rights for work authorization are given only to the respective maintenance depts. All other depts. can view the status of the job if the work request No. is known.

3. Permit Issue: The work permit can be issued by the Issuer dept. i.e. Operations dept only.

The work request shall be authorized for the issue of any work permit. Only one permit can be issued for a work request. Permits are issued and extended only during the day light hours by the respective shift – in – charge of Operations dept. In order to perform any job beyond daylight hours or on Holidays, the permission letter shall be moved through respective dept. heads and the consent of OIC of plant in writing is must.

4. Permit Extension closure: The permit shall be closed every day by the issuer dept after

completion of the job for that day. A new permit shall be issued on the same work request for the next time of job execution. This is exempt for the cold work permit and Job card. The permit can be extended for a period of one week on the same work request. The software package can’t issue a new permit unless the previous permit for the same work request is closed.

5. Job Status: The status of the job is under a particular work request is known. Whether is

job is under request stage, authorized, Job in progress, job under execution, job completed is known. This is a view mode screen i.e all can access to see the status of the job but none can modify the status.

6. Initial Feedback: On completion of the job by the maintenance dept., the status of the job

with remarks and total man days, equipments, spares and man hours are entered as a feedback of the job. The work request is closed by the maintenance dept. on completion of the job. The rights for the same are given only to the respective maintenance dept. All others can only view the screen and can’t modify the status.

In case of failure of the CMMS package for any reason, the Work Permits are issued manually. Colour coding of Permits:

1. Create maintenance order or use the order created by PM planning. 2. Create WCA make entries like : Work Centre, Equipment and or Function Location, Object

list of Function Location and or Equipments etc. for which permit is required. These will get transferred to the Permit that is WCD. (Operation can review this list and change if required)

3. The specific instructions required to be passed on to operations by maintenance may be mentioned in the WCA long text field which is near right side of Short Text.

4. After receiving the physical permit maintenance user can start the job. 5. In case time renewal / closure is required by the user then same permit can be handed over to

the operation for needful. Permit Creator ( Operation Personnel ) :

1. Search for the permits requirement by transaction WCTL for the work centre and date etc. 2. The Operation person will identify the permit requirements by reading the REQ attached in the

WCA like REQ-CWP means for Cold Work Permit etc. 3. Operation or O&M group will be creating alone and in co ordination with F&S and Electrical

depts. ( Depending upon the permit type ). 4. Operations will go through the WCA details for long text, equipment details and object list and

modify the equipment and or object list in consultation with the requestor. 5. Operation will create the required WCD by entering the required details and can mention his

some of the comments or instruction in the WCD long texts. 6. After procedure execution by operations the permit after signing can be printed and handed

over to the maintenance person. 7. As received from maintenance person if permit is required to be extended then operations may

follow the procedure and also mention the “Additional Precaution” in the field “TEXT2” (at present) in the custom developed transaction YRIU002.

Q.No.1 What is Safety Permit and to what extent does in ensure safety? Answer It is a written authorisation for a maintenance job to be done on a system or

on equipment after all practicable steps have been taken to ensure the safety of men, material and machines before the job is started and such safe conditions are maintained during the currency of the job. It is a check list and chances of missing any important requirement of job are avoided.

Q. No.2 Why is it necessary to issue a written permit? Answer A written permit doubly ensures that all the steps necessary to achieve safe

conditions of work were really taken and verified as having been taken before the job is started and/or that all precautions necessary will be taken during the currency of the job. In other words no reliance is placed on verbal instructions and communications as these are subject to human error. The written system ensures a positive and clear communication.

Q. No.2 (a) Does, mere taking a permit make the job safe? Answer No, mere taking a permit does not make the job safe, but it is safe to execute

the job under the conditions mentioned in issued SWP and taking prescribed precautions in correct sequence and true spirit.

Q. No.3 Why are three copies necessary? Answer In case of Hot work and Vessel entry/Vessel Box – up / Excavation permits,

Pink copy is given to permittee, Green copy to safety department and white copy is retained by the issuing authority.

In case of Cold work permit, “Yellow” copy is given to permittee, and White

copy is retained by issuing authority. The retention of white copy (Hot and Cold) ensure cross checking as well as

enables the issuing authority to keep a track of the permits issued and hazardous jobs in hand.

Copy to safety ensures that (A) an independent party whose sole job is to

propagate safety, knows about the hazardous jobs in progress or done (B) Enables him to cross check and make up deficiency, if any, in the precautions taken or to the taken.

The above system is followed only in the case of failure of software package, Computerised Materials Management System (CMMS). In CMMS , only one copy is issued to the permittee. This package is more efficient than the

manual system of issuing the permit. Everyone can access to this software know the status of the work permit and the current job status.This system is in the electronic form and accepted by OISD (Oil Industry Safety Directorate).

Q. No.3 (a) What are the various jobs that fall under Hot Work Permit? Answer The jobs requiring Hot work permits are:

i) Any work involving hazardous from toxic gas, corrosive fluids, steam, electric shocks and fire risk.

ii) Any work to be taken in a ‘Hazardous-Area’ involving use of open

flame apparatus/non-flame proof electrical equipment.

iii) Welding or burning or using of flame on vessel equipment etc. likely to contain explosive gas, liquid or poisonous chemicals.

iv) Work on electrical systems above 440 V.

v) Work on /or adjacent to railway tracks.

vi) Work on/or adjacent to gas holders or tanks containing flammable or

corrosive fluids.

vii) Entry into vessels, confined space, bunker containing crushed or loose materials, drains/sumps and excavations.

viii) Jobs like painting, insulation, white washing, NDT Testing, Inspection,

etc. located in the hazardous area.

ix) Jobs on fragile roof or at height above 3M, where proper platform is not available.

Q. No.3 (b) What are the various jobs that the fall under Cold Work permit? Answer The job requiring Cold work permits are:

i) Any cold work on mechanical equipments which does not require a man to enter in a confined space.

ii) Painting/insulation work up to 6 feet height and where there is non

chemical, mechanical or electrical hazard.

iii) Working on buildings and structures with proper approach and working platform.

iv) Working on electrical systems below 440 V.

v) Civil excavation and concreting jobs.

vi) For hot jobs in open area out side battery limit of plant where there is

no flammable liquid/gas, at the discretion of plant in charge.

GAIL(India)Ltd. NCR Q. No.4 (a) Who is authorized to issue a permit? Answer Only Control room shift – in – charge of the plant is authorized to issue a

permit. However, a list of authroised persons who can issue various types of permits is available with Control Rooms and with Fire & Safety Control Room.

Q. No.4 (b) Can a permit issuing authority issue a permit beyond his normal duty

hours? Can a permittee hold a permit beyond his normal duty hours? Answer No, if a permit is required to be valid beyond the normal duty hours of the

permit issuing authority, he should inform his successor, who should renew it for the period necessary during his duty hours with a special request from the permittee.

If the permittee also changes, he will inform his successor, who surrenders

the permit and get it revalidated in his name from the permit issuing authority of duty.

Moreover the permission to work beyond daylight shall be taken from the OIC

of the plant. Q. No.5 Why should not others issue a permit? Answer The permit issuing authorities have been specified as the Shift Incharge of

operations. It should be understood that the work operation used in this context means the personnel incharge of the production. The person incharge of the operation is naturally, by virtue of this position will be most conversant with the operation of equipment and machinery under his charge and is the only person who is adequately equipped and authorized to control the operations in such a way as to reduce a part or the entire system to safe conditions by adequate isolation, or by passing etc, which is essentially an operational feature. Obviously, therefore, such restrictions or control of the operations cannot be performed by any other than the operational man.

It is one thing to be aware of the methods of isolation, and achieving safe

conditions and it is entirely another thing to adjust the overall operation to achieve the safe conditions. Therefore, none other than the operation men are competent to issue a permit as the issuing of the permit is always likely to result in a system or equipment or a part of it not being available for operation.

Q. No.6 Who is to request for a permit? Answer The maintenance supervisor concerned should request for a permit from the

Shift Incharge Operation, If the Shift incharge operations finds any job being done without a Safety Permit, and if he considers such a job is necessary to be done under a Safety Permit, he may direct the maintenance men to stop the job and it should be complied with immediately.

Q. No.7 Is it necessary for the purpose of issue of Safety permit that the

maintenance supervisor should clearly details the type of maintenance he is planning?

Answer It is necessary that the permit issuing authority should clearly know the type

of maintenance being planned and the various alternatives also being

contemplated so that he may be able to assess clearly the extent to which the system/equipment or allied systems equipments should be reduced to safe conditions. Failure to mention the type of maintenance may defeat the Safety Permit procedure. If during the currency of the job any change in the maintenance programme is planned or proposed, the permit issuing authority should again be clearly told of the same so that the can re-assess the situation and allow the permit to be continued or issue a fresh permit or cancel the permit if necessary. To avoid misunderstanding the job should be clearly detailed in permission letter before issue of the permit.

Q. No.8 Can a Safety Permit be issued for any maintenance job to be done as a

prelude to the issue of a Safety permit? Answer In some cases certain operations as well as maintenance becomes

necessary as a prelude to the issue of a Safety Permit i.e. as a part of the process of reducing the system or equipment to safe conditions. During such situations maintenance personnel sometimes insist on a Safety Permit. In such cases, it may not be practicable to comply with or answer the question of the permit, but it should be possible to warn the men on the job, of the possible hazards and advise them to take necessary precautions by mentioning the same either on remarks column or on back of the permit.

Q. No.9 Can more than one job be done under one permit? Answer No. There should be as many permits as there are jobs. A job may be

generally defined as any maintenance on one equipment. If the equipment are different, the jobs become separate and there should be as many permits as the equipments. If more than one maintenance job is to be taken on the same equipment at the same time, then one permit may be issued provide the permittee is same. If different permittees are there or different jobs have to be done at different times on the same equipment, separate permits are again required.

Q. No.10 Is it necessary to answer the questions in the permit form by Reqd or

Not Reqd.? Answer As the permit is meant to be a written communication to avoid human errors,

every endeavor should be made to answer the questions categorically by tick mark of the respective column of Reqd or Not Reqd. Leaving questions unanswered is bound to (a) create a doubt in the mind of the permittee, (b) may create a habit in the permit issuing authority which may ultimately lead to an avoidable human error and possibly an accident.

Q. No.11 Does the signatory himself fill up the permit and if so why is it

necessary? Answer A Safety Permit is essentially a means of clear and unambiguous

communication as well as document which ensures that all precautions practicable have been taken. In view of this, it will be necessary that the signatories of the permit have to make sure personally that the answers to the questionnaires are correct and clear. This can best be done only if the signatories themselves fill up the form. The introduction of another agency to fill up the form is bound to lead to inadequate vigilance and an assumption

that the agent has personally checked up the equipment which may not always be true.

Q. No.12 Does the signatory ensure the tags are attached or this should be done

by the permittee? Answer It is clear responsibility of the signatory of the issuer where necessary to

ensure blanking, isolation, fixing up of safety tags, cleaning, purging and ventilation and gas tests and advise the Safety and Fire Services. The tags have to be put in by the signatory himself and the tag numbers have to the mentioned. The permittee has absolutely no obligation or responsibility on this behalf, except in respect of the precautions that may be mentioned on the back of SWP. Permittee has not authority either to put, remove and tamper or alter the tags.

Q. No.13 Are tag numbers to be mentioned in the permit? Answer Yes, the tag numbers are to be mentioned in each permit. Q. No.14 Is the permit issuing authority bound to ensure the practice of

precautions mentioned in SWP? Answer The obligation to comply with the precautions in SWP is with the permittee.

This does not mean that the permit issuing authority can keep quiet if the permittee does not comply with the instructions. It is his duty to stop the work until the instructions in SWP are complied with for which he should be vigilant.

Q. No.15 Who is to arrange or keep at site the safety and fire fighting equipment

during the job? Answer This is generally the responsibility of the permit issuing authority. Permittee

will, however, render such assistance as possible in the larger interest of work and will not start the job if the necessary safety and fire fighting equipment is not provided at work site.

Q. No.16 Should the permit be made in the name of the supervisor or the actual

worker doing the job? Answer The permit should always be made in the name of a supervisor immediately

responsible for the execution of the maintenance job. It should not to be made in the name of a worker or non-supervisor or contractor. The supervisor is solely responsible for the execution of job and the deployment of men and material under his control. The supervisor shall be an employee of GAIL.

Q. No.17 Is the signature of permittee required? Answer Yes, is required on two occasions:

i) When the permit is handed over to the permittee to indicate that the permit has really been handed over.

ii) When the job is completed and the permit is to be surrendered.

Q. No.18 Who is the permittee and what are his responsibilities?

GAIL(India)Ltd. NCR Answer A permittee is a supervisor in charge of the particular maintenance on the

equipment or system requiring such maintenance. He is responsible to:

i) Ensure strict compliance with the instructions mentioned in SWP and on the back side of the permit.

ii) Procure and make available to his workmen the safety and fire

protection equipment etc. required for the job.

iii) If he is required to start/perform the job in the presence of any particular party or parties, he should arrange for the same and should not commence the job without the presence of such party or parties.

Q. No.19 Can one supervisor have more than one permit at the same time/ if more

than one permit is issued in the name of one individual, what are his responsibilities to ensure compliance with the permits?

Answer As mentioned earlier, permits should be issued in the name of the supervisor

only. However, the maintenance supervisor may be incharge of one or more sections of the same plant and in some cases one or more jobs at the same time and there is noting unusual in the supervisors having more than one permit at a time. It is his responsibility to:

i) deploy suitable men at his discretion on various jobs; ii) arrange for the necessary protective equipment;

iii) clearly explain to his men the precautions necessary on the particular

job;

iv) ensure that the men fully comply with the precautions and instructions mentioned in the safety Permit;

v) adequate supervision of such jobs to secure such compliance

vi) clearly instruct the workmen that the job has to the stopped as and

when required by the permit issuing authority or a safety representative; and

vii) take such additional precautions as are necessary, if so instructed by

the permit issuing authority or the safety man. Q. No.20 Can a permit be renewed? If so, what is the correct procedure of

renewal or extension? Answer Yes, permits can be renewed or revalidated. In case of renewals, the issuing authority and the permittee are both obliged

to inform their successors in shift about the permits in force and details of the precautions etc.

A fresh permit is necessary in case of Hot work/Vessel entry/excavation on

the following day if the same job is continuing.

GAIL(India)Ltd. NCR In case of Cold work permits, the same permit can be extended the following

days. All permits, whether surrendered or not, should be treated as having expired

by validity time. However, a permit cannot be renewed or revalidated beyond seven days and

a fresh permit is necessary. It should be distinctly understood that renewal or revalidation has to be done

(a) by the authority who is competent to issue a permit, (b) after ensuring all the necessary check as for a fresh permit and (c) the renewal is also recorded on the CMMS package of the permit issuing authority and in the register of SWP maintained by production.

Q. No.21 Can a reference be made in the permit to any other document or

commitment oral or written, without describing in details such commitment in the permit itself?

Answer Reference to such document or commitment is a short-circuiting of the safety

permit procedure and in no way binds the permittee. Since the permit is supposed to be a clear communication specifically relating to a particular job, it will not be correct to refer to I.O.M.’s or list of jobs sent as it is very likely that when the permit is surrendered, the importance of the reference to such document or commitment may not be properly appreciated and the equipment recharged or put in commission, probably with disastrous results. It is also likely that the permit issuing authority, by making a reference to an IOM may not even detail the job itself in the permit. However, only for the purpose of reference of the permit issuing authority, the numbers of Safety Work Permits issued on the same equipment or system may be mentioned on the current SWP issued for the same.

Q. No.22 Can a direction be given in the permit to the permittee to that a party or

parties have to be informed? Answer It is the responsibility of the permit issuing authority to inform all such parties

that the considers necessary to be informed which includes operating personnel, concerned maintenance depts, fire and safety services, etc. however, nothing should be mentioned in the safety permit that particular party or parties have to be informed. If considered necessary, an entry can be made of the Names and Designations of the parties so informed. In other works permittee is not obliged to inform anybody once the permit is issued.

Q. No.23 Can a direction be given in the permit to the permittee that a party or

parties have to the present during the maintenance job? Answer Yes, if considered expedient, the permit issuing authority may clearly mention

that the job should be started/performed in the presence of a particular party or parties. For example, in the presence of Fire personnel, Safety Supervisor, Supervisor (Prod.) etc. When such a direction is given in the permit, it is the duty of the permittee to ensure that the same is complied with.

Q. No.24 Can the permit be cancelled or withdrawn or the job stopped during the

period for which the permit is given?

time during the period for which the permit is valid at the discretion of the issuing authority or the Safety Personnel. This is so, as the continuance of safe conditions on the equipment or system cannot always be assumed throughout the period for which the permit is valid and therefore great vigilance is necessary on the part of the operation and safety personnel.

Yes, it is necessary to surrender the permit at the end of the day or

completion of the job, which ever is earlier, so that permit issuing authority knows about the final status of the job and he can take further necessary action.

Q. No.25 Why is it necessary to surrender the permit after a maintenance job is

over? Answer After a maintenance job is over the permittee has to fill up permit and hand

over the permit to the permit issuing authority as a token that the job entrusted to him through the permit is complete. Permittee should also arrange that all tools, tackles, Safety and Fire Fighting equipments are removed from site and the area surroundings the work is kept clean to avoid accidents.

Q. No.26 What is the procedure for testing the equipment or system as a prelude to handing it over to the process personnel?

Answer During the currency of the Safety permit, only maintenance job can be done.

If testing of the equipment or system is required the authority who issues the permit should be informed and the safety permit surrendered. A separate temporary permit to test the equipment should be obtained. Such a temporary permit is necessary because the conditions on the equipment or the system during the test would have changed and a fresh assessment is necessary to ensure safe conditions during the test.

If after the test, it is necessary to conduct further maintenance, a fresh permit

has again to be obtained. If, however, after the test, the equipment was found ready to be handed over

for operations, the temporary permit should be surrendered. There is no separate form for the temporary safety permit. It is the same as the Safety Work Permit form.

Q. No.27 What should the permit issuing authority do after the permit is

surrendered? Answer He will arrange for the removal of Safety tags, mentioned in the permit, also

the corresponding stubs are to be removed from the safety tag box/board. As more than one permit is likely to be current on the equipment or system, the surrendering of one permit may not clear the equipment/system. It is for this reason that the tag numbers mentioned in the permit should be tallied with the stubs in the tag box and cancelled. If some more tags or permits are current on the system, the equipment/system can be energized or commissioned only after the other permit are also surrendered, and the responsibility to ensure this, is entirely on the Shift In charge/Operations.

When the equipment or system is ready for energising or commissioning, the

Shift In charge operation will request the electrical maintenance, where

necessary, to make electricity available by sending him the concerned permits.

Before making electricity available, the Shift In charge electrical maintenance

will remove the tags put on the equipment or the system by electrical maintenance, tally with the stubs in the safety tag box, cancel the tags and stubs and fill up energisation / de-energisation permit , sign and send the permit back to the Shift In charge operations. The equipment is now ready for energizing or commissioning.

Q, No.28 What should be done, if during the occurrence of a job on Safety Work

Permit, directions are issued by Senior Supervisors of Officers? Answer a) If the directions are to stop the job, or take additional precautions, as a

measure of safety, they should be obeyed immediately. b) If the compliance of the directions involves any changes in the

maintenance planned, proposed or in progress or if it, in any way involves another equipment/system or the energizing or commissioning of the equipment/system under maintenance, such direction should be obeyed only after appraising the permit issuing authority, who will reassess the situation and amend, cancel or issue a fresh permit, irrespective of the source from which it comes, be it even from OIC of plant.

Q. No.29 Why is it necessary to keep a record in a register, of all the permits

issued? Answer Some times number of permits books are used simultaneously to avoid loss

of time when a permit is sent for electrical isolation. In such cases and otherwise also, such a register is useful for a consolidated record of issue of permits. But, in our case, CMMS is taking care of such noting. However, during plant shutdowns or whenever CMMS is not operational, it is advised to note in a register for consolidated record of all issued permits. Moreover the hard copies of the permittee is received and filed for a period of three years as per the OISD guidelines.

Q. No.30 What is understood by interlinking or interlocking of permits? Answer As already stated (Q. No.9) that whenever number of jobs by different

permittees are to be carried out on particular equipment, separate permits have to be issued. Such permits should be interlinked with each other. This interlinking/interlocking has to be mentioned on the permits and against each permit entered in the register. This will avoid the energizing of equipment before all permits are cancelled / surrendered.

Q. No.31 It is often alleged that the Safety Permit procedure is elaborate, involves

lot of time and clerical work to responsible supervisors and sometimes leads to friction between engineers/supervisors of different departments. Is there any truth in this?

Answer All the above allegations apparently appear to be true as long as mishaps do

not occur. But danger is always ‘around the corner’ in factories like ours. If the system, in spite of the above allegations brings about a certain degree of

safety to men, machines and materials, and prevents even a single mishap, it is worth observing strictly.

It is only the experienced and mature supervisors who can be made

responsible for the practice of this system and it is perhaps their most important function, however, time or labour consuming it may be. The strict compliance of the system by the engineers/supervisors is also perhaps one of the best means of educating the non-supervisory personnel who are most likely to be involved in the execution of the job and, therefore, in the mishaps that may occur.

If the system is viewed in proper perspective and with the respect that is due

to it, there is no place for any friction between engineers/supervisors. A system like this is indispensable in a complex processing industry like ours,

as long as human agency is indispensable in its running.

Gravel Pit Death Scene (1990)

The operator of this bulldozer died of burns a few hours after he hit a 12-inch natural gas pipeline near Simms, MT. He was working in a gravel pit as part of a highway construction project. The line had been marked, but apparently the marks were obliterated at the point where it was struck.

Burning Bulldozer (1985)

This bulldozer was clearing mesquite from pasture land with a root plow when it hit an 8-inch natural gas liquids pipeline, which was 22 inches deep. The impact cut the top off a section of the line. The bulldozer operator escaped before the gas ignited which happened about three minutes after impact. The bulldozer engine, which had not been turned off, caused the ignition. The farmer had not called for a locate before starting to clear the land to convert it to cropland.

GENERAL PURCHASE CONDITIONS 1. Consignee: C&P Incharge, GAIL (India) Limited, NCR GAS(O&M), Bharat Scouts & Guides Bldg. 16, Ring

Road, IP Estate, New Delhi-110002. Any expenditure and/or demurrage incurred in respect of a wrong delivery shall be recovered from supplier.

2. Quotation duly sealed & superscribed with enquiry no. should reach GAIL office on or before 1400 hrs

on the due date of opening along with samples, if required, failing which offer will not be considered. Quotations received late are liable to be rejected. Quotations will be opened on the due date at 1500 hrs.

3. The rates should be quoted on FOT dispatch point basis (i.e. including taxes, duties, P&F etc) and

freight paid upto NCR, New Delhi. Accordingly, the date of LR/RR etc. shall be considered as date of delivery. Applicable rates of excise duty, central sales tax/local sales tax, packing & forwarding and freight charges should be clearly specified. In absence of same it shall be presumed that quoted prices are inclusive of same. Price quoted subject to escalation shall not be accepted. Conditional discount shall not be considered for evaluation. Rates of applicable sales taxes with and without form ‘C’ should be mentioned in offer.

4. Sales Tax and Central Sales Tax Registration No. must be mentioned in the quotation. GAIL willl not

issue any concessional Form- C / D. 5. Rate should be quoted according to unit of measurement mentioned in NIT and no alternative unit will

be considered. The rate shall remain firm & fixed till complete execution of order. 6. Validity of quotation should not be less then 4 months from the opening date. Quotations with less

validity are likely to be ignored. 7. Offers subject to prior sale will not be considered. 8. Required quantities at the time of placement of order can be changed ± 25% of the quantities specified

in enquiry. 9. Liquidated Damages: In case of delay in delivery beyond contractual delivery period, liquidated

damages @ 1/2% of total order value per week of delay or part thereof subject to maximum of 5% of the total order value shall be leviable.

10. Normally, no price negotiation will be held. Change in price or substance of offer after opening of bids

will not be permissible & shall lead to rejection of bid. 11. i) Quotation should accompany Earnest Money Deposit (EMD) for value indicated in Notice Inviting

Tender in the form of (a) bank draft payable at NOIDA. (b) Bid Bond valid for a period of two months beyond bid validity. Quotations without – EMD/Bid Bond are likely to be ignored.However,no earnest money will be necessary :-

a) For purchases upto Rs 1.00 lac. b) For purchases from Public Sector Undertaking. c) For purchases from firms registered with NSIC. d) For purchases from firms registered with GAIL/DGS&D provided they are registered upto

monetary limits and for the items they intend to quote against the tender.

ii) EMD shall be forfeited, if the bidder :-

a) Withdraws bid/Changes quoted rates/modifies terms and conditions of the bid without consent of GAIL within validity period including extension thereof or

b) Fails to accept order or c) Fails to submit the Contract Cum Equipment Performance Bank Guarantee (CPBG)/

Security Deposit pursuant to placement of order. iii) E.M.D of unsuccessful bidders will be released on finalisation of order.

iv) E.M.D of successful bidder shall be adjusted towards Security Deposit (S.D.) or shall be released on submission of CPBG/S.D.

v) In case the order value is above Rs. one lakh, security deposit (SD) will be submitted

by the successful bidder either in the form of Demand Draft or Contract Cum Equipment Performance Bank Guarantee (CPBG) as per format prescribed by GAIL for 10% of the total order value. The CPBG should be valid for 3 moths beyond guarantee/warranty period and is to be submitted within 15 days from the date of receipt of order.

vi) The Bid Bond/CPBG shall be from Nationalised Bank or Indian Scheduled Bank having

net worth of more than Rs. 100 Cr.

12. Necessary testing facility to be provided free of charge to inspecting officer at vendor’s premises. 13. When stores are rejected, the same will be intimated to the vendor with the details of such rejected

stores as well as the reasons for their rejection. The material will be lying at the consignee's premises at the risk and cost of the supplier. The supplier will also be called upon either to remove the materials or to give instructions as to their disposal within 14 days and in the case of dangerous/infested and perishable materials within 48 hrs. failing which the consignee will either return the materials to the supplier on freight to pay or otherwise dispose them off at the supplier's risk and cost. The consignee will also intimate the concerned paying authority, quantity of the material so rejected so as to recover the freight charges from the supplier. The purchaser shall also be entitled to recover handling and stores charges for the period during which the rejected stores are not removed @ 5% of the value of materials for each month or part of a month till the rejected stores are finally disposed off.

14. Warranty:

The supplier shall warrant that everything to be furnished hereunder shall be free from all defects and faults in material, workmanship and manufacture and shall be of the highest grade and consistent with the established and generally accepted standards of the materials of the type offered and in full conformity with the specifications, drawings or samples, if any, and shall if operable, operate properly. This warranty shall survive inspection of payment for and acceptance of the goods but shall expire twelve months from the date of operation or 24 months from the date of despatch, whichever is earlier.

15. If an order is placed on a bidder, in preference to the lowest acceptable offer, in consideration of

shorter delivery period the supplier will be liable to pay to GAIL the difference between the ordered rate and the rate quoted by the lowest acceptable bidder in case he fails to complete the supply within the specified delivery period. This is without prejudice to other rights under terms of order.

16. Payment: 100% Payment will be released thru E-payment within 15 days of receipt and acceptance of

material/installation at site/stores. In case of payment through bank, all charges shall be borne by vendor. All payment shall be release through E-payment only, hence bidder are advised to open their account with “SBI, HDFC OR ICICI” bank immediately if not opened and inform GAIL, so that bill payments etc. can be credited directly to their respective account. In case of payment through D.D., D.D charges shall be to vendor’s account.

17. Invoice: In case where documents are not routed through Bank, original plus one copy of invoice may

be directly sent to Incharge (F&A) GAIL, Noida immediately after dispatch with copies to purchaser and the consignee mentioned in purchase order. Invoice must bear the purchase order no. with date and should also indicate the dispatch particulars. It may be noted that the documents will be retired only if the dispatches are made as per the terms of the purchase order.

18. Repeat Order: 50% of ordered quantity may be reordered on the same rates, terms & conditions

during the delivery period including any extension thereof or within six months from the date of original supply order whichever is later.

19 AIL reserves the right to accept/reject any or all tenders without assigning any reason whatsoever. 20. Packing & Marking : While dispatching ordered stores, it will be the responsibility of the supplier to

properly pack the consignment so as to enable its delivery at destination free from loss, damage or pilferage. Each packing must contain a list of stores packed therein. Each packing/bundle must be prominently marked with order no. and packing no. & consignee name & address.

21. Despatch documents : An advice note indicating the order no. shall accompany each consignment. A duplicate advice note along with Railway or Lorry Receipt must be sent to the consignee by Regd. Post/Courier in advance before arrival of stores at destination. Any demurrage / wharfage etc. incurred on account of late/ none receipt of dispatch document /wrong dispatch of consignment will be recovered from supplier. All RRs/LRs must be in the name of consignee as stated in the purchase order and only where specifically agreed the same can be consigned to “Self”. Required test certificates must be sent alongwith dispatch documents

22. Unless permitted by GAIL, for order value upto Rs.50,000/- supplier shall dispatch all the ordered items

in single lot/consignment and for order value exceeding Rs. 50,000/- supplier can dispatch the ordered items in maximum two lots/consignments.

23. Failure and termination :

Time and date of delivery shall be the essence of the contract. If the supplier, fails to deliver the stores or any installment thereof within the period fixed for such delivery in the schedule or at any time repudiates the contract before the expiry of such period, the purchaser may without prejudice to any other right or remedy available to the purchaser, recover damages for breach of the contract as follows:

i) Recover from the contractor, as agreed, liquidated damages and not by way of penalty a sum

equivalent to 1/2% (half) of the total order value per week for such delay or part thereof (this is an agreed, genuine pre-estimate of loss or damage duly agreed by the parties without there being any proof of actual loss or damage caused by such delay) where delivery thereof is accepted after expiry of the aforesaid period. It may be noted that such recovery of liquidated damages may be upto 5% of the total order value.

ii) Purchase or authorise the purchase elsewhere on the account and at the risk of the supplier, of

the stores not so delivered, or others of a similar description (where stores exactly complying with the particulars are not in the opinion of the purchaser which shall be final, readily procurable) by serving prior notice to the supplier without cancelling the contract in respect of the installment not yet due for delivery; or

iii) Cancel the contract or a portion thereof by serving prior notice to the supplier and if so

desired, purchase or authorise the purchase of the stores, not so delivered or others of a similar description (where stores exactly complying with particulars are not, in the opinion of the purchaser which shall be final, readily procurable) at the risk and cost of the supplier. If the supplier had defaulted in the performance of the original contract, the purchaser shall have the right to ignore his tender for risk purchase even though the lowest. Where the contract is terminated at the risk and cost of the supplier under the provisions of this clause, it shall be in the discretion of the purchaser to exercise his discretion to collect or not, the security deposit from the supplier, on whom the contract is placed at the risk and expense of the defaulted supplier.

iv) Where action is taken under sub-clause (ii) or sub clause (iii) above the supplier shall be liable

for any loss which the purchaser may sustain on that account, provided the purchase or if there is an agreement to purchase, such agreement is made, in case of such failure to deliver the stores, within 6 months from the date of such failure and in case of repudiation of the con-tract within six months from the date of cancellation of contract. The supplier shall not be entitled to any gain on such purchase and the manner and method of such purchase shall be at the entire discretion of the purchaser. It shall be necessary for the purchaser to serve a notice of such purchase on the supplier.

v) It may be noted that clause (i) above provides for recovery of liquidated damages at the rate

of 1% (one percent) of the total order value per week (or part thereof) of delay upto a ceilling of 10% of total order value. Liquidated damages for delay in supplies thus occurred will be recovered by the paying authorities of the purchaser specified in the supply order from the bill for payment of the cost of the material submitted by the supplier or his foreign principals in accordance with term of the supply order or otherwise.

vi) Notwithstanding anything stated above, equipment and materials will be deemed to have been

delivered only when all its components, parts are also delivered. If certain components are not delivered in time, the equipment and material will be considered as delayed until such time as all the missing parts are also delivered.

24. General Conditions : The bidders are requested to note the following conditions which among others, shall form part of the purchase order arising out of this tender :

i) FORCE MAJEURE : The delivery period indicated in the purchase order should be strictly

adhered to and normally no extension in the delivery would be granted. However if at any time during the currency of the contract the performance in whole or in part by either party, of any obligation under the contract shall be prevented or delayed by way of any war, hostilities (whether war is declared or not), acts of public enemy, civil commotion, sabotage, fire, floods, explosion, epidemics, strikes, lock-outs or acts of GOD, provided notice of any such happening is given by either party to other in respect of such non performance or delay in performance & deliveries under the contract shall be resumed as soon as practicable after the event has come to an end/ceased to exist, provided that if performance in whole or part of the contract is prevented or delayed for a period exceeding 60 days either party may, at its option, terminate the contract.

ii) SUBLETTING AND ASSIGNMENT : The supplier shall not, save with the previous consent in

writing of the purchaser, sub-let, transfer or assign the contract or any part thereof or interest therein or benefit or advantage thereof in any manner whatsoever provided nevertheless that any such consent shall not relieve the supplier from any obligation, duty or responsibility under the contract.

iii) ARBITRATION : All disputes or difference whatsoever that shall at any time arise between

the parties relating to execution of this Contract/Purchase order shall be referred to the Sole Arbitrator appointed by the General Manager of Gas Authority of India Ltd. whose award shall be final and binding on both the parties.

iv) The contract shall deem to have been entered at GAIL, New Delhi and all suits in respect of

this contract shall be under the jurisdiction of the court in New Delhi.

time or from time to time the exercise of any of the powers and rights conferred on you under the contract with the said _____________________________________ and to enforce or to forbear from endorsing any powers or rights or by reason of time being given to the said __________________________ which under law relating to the sureties would but for provision have the effect of releasing us.

from us in manner aforesaid will not be affected or suspended by reason of the fact that any dispute or disputes have been raised by the said M/s.___________________________ and/or that any dispute or disputes are pending before any officer, tribunal or court.

4. The guarantee herein contained shall not be determined or affected by the liquidation or winding up

dissolution or changes of constitution or insolvency of the said but shall in all respects and for all purposes be binding and operative until payment of all money due to you in respect of such liabilities is paid.

5. This guarantee shall be irrevocable and shall remain valid upto______________ If any further extension of

this guarantee is required, the same shall be extended to such required period on receiving instruction from M/s.______________________ on whose behalf this guarantee is issued.

6. The Bank Guarantee’s payment of an amount is payable on demand and in any case within 48 hours of

the presentation of the letter of invocation of Bank Guarantee. Should the banker fail to release payment on demand, a penal interest of 18% per annum shall become payable immediately and any dispute arsing out of or in relation to the said Bank Guarantee shall be subject to the jurisdiction of Delhi Courts.

7. We have power to issue this guarantee in your favour under Memorandum and Articles of Association and
