LOCATIONAL CLEARANCE FOR
BUSINESS
Properly filled-up Application
Form for Business/Mayor’s
Permit (provide a Photocopy)
Sketch of proposed Business
Location (in a separate sheet
or at the back of Application
Form)
Photocopy of Barangay
Business Clearance for the
current year
If the Property is not owned, submit any (that is applicable)
of the following:
a. Photocopy of Notarized and Valid Contract of Lease
(If sub-lessee, attach a photocopy of Property/Building Owner’s
authority to Sub-lessor)
b. Photocopy of Certification or Letter of Consent/Authorization from
Property/Building Owner to Applicant
* If renting, must include the monthly rental fee and the period of lease
* If allowed for free, it must be stated and the period of use of premises
c. Photocopy of Award Notice
d. Photocopy of Notarized Affidavit of Undertaking on Status of
Lot Occupancy
(if no proof of ownership or no clear status on property ownership
and when required)

Fee (upon assessment from the Zoning Officer)
