Welcome to theSilverline Directory of Community Resources

One of the most important services Franklin County Home Care Corporation (FCHCC) provides is access to information about issues, programs, and services for elders, caregivers, and persons with disabilities. This is the gateway that, many times, serves to introduce people in our community to the wide variety of resources and supports available to elders, persons with disabilities, families, and those helping to provide care.

Resource information is always changing. Experienced Resource Consultants are available to answer questions or assist with your inquiry through research and follow up as needed. An up-to-the-minute database is maintained electronically. In talking with you, we can give more specific information than is contained in the Directory and offer help to problem solve with you. Assistance with elder resource information across the United States is also available. Our Mission is to help elders and persons with disabilities remain in their homes safely, comfortably, and independently for as long as they are able and to support their caregivers as they try to bal-ance the competing needs of family, work, and self.

Franklin County Home Care Corporation is an Aging Services Access Point and Area Agency on Aging funded in part by the federal Older American’s Act, the state Executive Office of Elder Affairs, the United Way of Franklin County and other sources both public and private. FCHCC is operated by a local volunteer Board of Directors, a majority of whom are, by law, elders. The agency is an AA/EOE employer and does not discriminate in pro-gram admission, access, services, or employment practices.

The AARP Foundation Tax-Aide helps low-in-come to moderate-income taxpayers by assisting with tax services and ensuring they receive ap-plicable tax credits and deductions. The AARP Foundation Tax-Aide is available free to taxpay-ers with low- and moderate-income, with special attention to those 60 and older. The Benefits Counseling Program assists home-owners and renters age 60 and older or persons with disabilities to find resources to improve their financial security and help them stay in their homes for as long as they choose. FRTA contracts with the Department of Medical Assistance (DMA), Department of Developmen-tal Services (DDS), and Department of Public Health (DPH) for human service transportation services. They can arrange transportation by re-quest for elders and the disabled residents in their 40-town area. They use a network of public and private transportation providers to provide this service. In order to qualify for these services, you must be 60 or older or have a significant mobil-ity or functional impairment. Disabled custom-ers must have valid eligibility through the FRTA. They also provide services through their Med-Ride program.

Loan funds are available to improve or modern-ize homes or remove health and safety hazards. The 504 Grant program is available to very low-income homeowners age 62 or older. The grant funds are used to remove health and safety haz-ards or make the dwelling more accessible to household members with disabilities. To obtain a grant, the applicant must lack the ability to repay a Section 504 Loan.
