If your health assessment score falls between 80–100, you’re doing great! Keep up the great work. Be sure to review the health assessment report to identify any areas that could use some attention and make a plan.

MODERATE RISK–score of 60-79

If your health assessment score falls between 60–79 you’re doing well in some areas and may have opportunity for improvement in others. Take this time to review your health assessment report and make a plan to improve one or more risk areas that were identified. Each risk area will include links to provide information and guidance on changes you can make. You may also want to print out your health assessment report to share with your medical provider at your next appointment.

HIGH RISK–score of 0-59

If you scored between 0–59, you are in the high-risk category based on some or all of your health behaviors. It is time to make an appointment with your medical provider. Call to set up an appointment to talk about how to reduce your risk factors and adopt some healthier behaviors. Take this opportunity to print out your health assessment report and bring a copy with you to your appointment and make a plan with your medical provider.
