The Masonic Village at Elizabethtown was established in 1910 as a charitable tax-exempt institution and isowned and operated by the Right Worshipful Grand Lodge of Free and Accepted Masons of Pennsylvania.

Our caring communities and services assist individuals, families and children in realizing their potential and enjoying the highest possible quality of life through the traditions of Freemasonry.

Retirement Living is an independent living area that offers a variety of living accommodationswith accessibility to health care services, if needed. There are entrance fee and rental fee unitsavailable.

Personal Care is located in the Freemasons Building offering accommodations for 127 residents. This area provides some assistance with daily activities such as bathing, dressing, medications and transportation.

This lifestyle provides three meals a day with close proximity to the dining rooms. Medical services are easily accessible, and assistance is available 24 hours a day.

The Masonic Health Care Center cares for 453 residents requiring various levels of nursing care, including dementia, Transitional Care and Hospice Care. Bright, cheerful surroundings and a well qualified, caring staff help to promote a home-like atmosphere.

The Adult Daily Living Center provides day caring for individuals 18 and over with physical and/or cognitive challenges. This program offers a quality cost-effective alternative for families with adult relatives who would benefit from daily ongoing support services and socialization.

The Bleiler Caring Cottage responds to the needs of individuals with intellectual developmental disabilities. The cottage is home to 8 adults and provides support services and socialization helping those residents to enjoy a fulfilling lifestyle.

The Outreach Program provides information and direction to Masonic and non-Masonic families across the United States. Assistance is offered to individuals and families with varied needs. Referrals, education and home assistance are the tools used to guide and assist those in need.

Our Children’s Home was established in 1913 and was originally a single building unit. The Children’s Home was relocated in the early 1990’s to modern, attractive, well-equipped cottagesfor the children. Children are not required to have Masonic affiliation to receive serviceshowever, a Masonic Lodge sponsorship is needed to be considered for admission.

Services are provided to school age children who come from a variety of situations. The goals of the Children’s Home are to prepare children for independent living and create a homelike environment.

The Masonic Conference Center, formerly known as the Thomas Ranken Patton Masonic Institution for Boys, is also an integral part of the Masonic Village. This facility is the home of the Pennsylvania Youth Foundation, which offers programs for DeMolay, Rainbow Girls and Job’s Daughters. The complex includes an indoor swimming pool, enabling the youth groups andthe residents the opportunity for year round swimming.

CAREGIVERS/COMPANION REQUIREMENTS

In order to reduce risks to residents, effective October 1, 2007 guidelines are in place regarding the hiring of Home Health Agencies and private caregivers by residents. These guidelines pertain to services provided by a registered nurse, licensed nurse, nurse’s aide or a companion. A companion is defined as a person who assists with housework, cooking, letter writing, companionship, minor grooming, etc. These guidelines do not pertain to a relative who is providing assistance. It is applicable to any agreement between a resident and caregiver where there is a payment for services.

If you plan to hire a caregiver/companion, you should advise your social worker or the Director of Retirement Living. The agency/caregiver will be required to provide:

• Proof of current licensure if applicable• Proof of professional liability insurance• Proof of freedom from communicable diseases including a written report of a

tuberculin skin test within the past year• Criminal records report for the caregiver• Signed release of liability form (Releasing Masonic Village of liability)

In addition, you (resident) must provide a signed Waiver form releasing Masonic Village of any liability.

FIREARMS

In order to respect the interests of those residents who enjoy game hunting, residents may secure firearms in their living units while adhering to the following guidelines:

1. No handguns are permitted on the premises, in any living unit, or in any vehicle parked on the Masonic Village at Elizabethtown grounds.

2. Shotguns, rifles, bows, and competitive target pistols are permitted as long as these arms are locked in a cabinet in the resident’s unit at all times.

3. All ammunition and arrows must also be locked in a cabinet in the resident’s living unit at all times.

4. No firearms may be stored in the storage cubicles located in the lower level of the apartment buildings.

5. The aforementioned arms may not be loaded or discharged anywhere on the Masonic Village grounds except during stated hunting periods and in permissible areas.

6. Hunting on the Masonic Village grounds is restricted to specific areas, only upon approval of the Director of Environmental Services and Land Management. Hunting permits and maps are available in the Environmental Services and Land Management Director’s Office. Hunting is only permitted with bow and arrow. No firearms are allowed.

7. A list of firearms must be filed in the Administrative Office following occupancy of the resident’s living unit.

8. In order to maintain the safest environment possible, resident may be asked to remove from the Masonic Village grounds any firearms they possess if they violate any of the above stated guidelines or, if in the opinion of Administration, the resident is not handling these firearms in a safe manner which may endanger themselves or others.

GOLF CARTS

The use of golf carts is permitted on Masonic Village grounds but there are some guidelines that must be followed.

Golf carts must include the following safety features in order to be used on campus:

1. A governor, installed by the manufacturer, to be below the maximumspeed of 20 mph.

2. Rear view mirrors3. Turn indicator signals4. Back up beepers5. Headlights6. Brake lights7. Horn

Owners must also have the following:1. A valid driver’s license2. Proof of current liability insurance3. A signed Registration and Operator’s Agreement Form (Can be

obtained from the Administration Office)4. Vehicle number and parking sticker which will be assigned after

receipt of the Registration and Operator’s Agreement Form.

Golf carts must be operated on Masonic Village roadways. They are not to be operated on sidewalks, walkways or in the grass. Headlights and tail lightsshould be on at all times during operation.

MOVE-IN PROCESS

Upon arrival at the Masonic Village on the pre-arranged move-in date, all residents should contact the Marketing Representative. At this time, residents will meet with the Marketing Representative, receive two keys for their apartment or cottage, and settle all outstanding financial arrangements.

Residents should be prepared to present a check for the amount of the monthly fee for their unit prorated for the number of days remaining in the month; and for anyoptions specified on the Third Addendum of the Resident Agreement and anyremaining balance of the Entrance Fee if applicable. Under the rental plan, a deposit which the resident has previously paid for a full month's monthly fee will be applied to the first full month in which the resident occupies the apartment. This will assure that payment of the basic monthly fee is always made prior to the month for which it covers.

After completing all of the above, the resident may take occupancy of the living unit and proceed with moving their personal belongings into their unit. All moving vans, etc., should enter the Masonic Village grounds by way of Freemason Drive.

Great effort has been taken to present a landscaped site for your enjoyment; therefore, we request that you instruct all individuals assisting with your move-in to park all vehicles on paved areas only and not walk across lawns and flowerbeds. Your cooperation and attention to this request will assist staff in maintaining an attractively landscaped community.

Each resident will receive Resident Orientation. Residents will be greeted by Resident Welcome Volunteers who will provide you with a Quick Reference Directory and provide orientation to the Masonic Village at Elizabethtown.

INDOOR AND OUTDOOR FURNISHINGS

We encourage residents to tastefully furnish their balcony, terrace, or cottage porch. For the convenience of all residents and in order to maintain an aesthetically attractive Community for many years to come, we have established the following guidelines:

1. Planter boxes may be placed on the floor or railings of apartments andshould be secured to avoid any damage to units that may be located below.Residents living on the second or third floor should take extra care in avoiding over watering of outdoor plants and flower boxes, in order to avoid any inconvenience to their neighbors in the apartments below. Flower boxes may be installed but must be done by the Masonic Village Maintenance Staff at the resident’s expense.

2. Outdoor carpeting may be placed on balconies and patios; however, noadhesive, glue, or other type of fasteners may be used in order to allow for easy removal if a resident would decide to change living units. (The resident would be responsible for the cost of removing the carpet if an adhesive is used.)

3. Flags may be displayed from balconies, terraces, cottage porches, and decks;however, residents may not mount brackets into the brick. The mounting of any exterior brackets on or near any living unit must be approved by Administration and installed by the Masonic Village Maintenance Staff at the resident’s expense. No more than one flag should be placed outside each unit. Residents living in cottages may place a flagpole, no more than ten feet in height, in a landscaped bed at the front or rear of their cottage upon approval by Administration.

4. Outdoor decorations and garden borders should be limited to no more thantwo decorations per unit. Garden and flowerbed borders should be limited tothe area behind cottage units.

5. Residents living in the cottages may display holiday decorations throughoutthe holiday season; however, any decorations that are planned for placement on trees or shrubs should be reviewed by the Landscape Department to

assure there is no damage to the trees and plants. All holiday decorations should be removed no later than two weeks after the particular holiday.

6. In apartment buildings, residents may display decorations outside theirapartment entrance door, but not protruding into the corridor or hallway. All decorations in common areas such as lounge areas will be provided by Masonic Village staff. Residents may not place their personaldecorations in common areas which includes lobbies and lounges.

7. Artificial trees and wreaths are recommended indoor decoration during theHoliday Season. Please only use UL approved lights.

8. Administration will review all requests for screens, shades, or awnings,landscape lighting and invisible fencing at cottages. These additions would only be approved providing the residents of the aforementioned units agree to incur the cost for these additions. Also, the contractor to install these additions would be selected by Administration to assure quality. Requestsof this type should be submitted on a Maintenance Request Form.

9. When placing noise-making ornaments, such as wind chimes, considerationshould always be given to one's neighbors.

10. Clothing or any other items such as blankets, towels, or sheets may not behung from the balconies, terraces or porch areas in order to maintain the aesthetics of the Community.

11. Bird feeders may be placed on the walking trail or at other areas no closerthan ten feet from apartment buildings with prior approval byAdministration. One bird feeder may be placed in the landscaped beds around cottages as long as the placement of the bird feeder does not adversely affect the neighboring residents.

12. Barbecue and gas grills may not be used on the porches, balconies, terraces,common areas, or in the general vicinity of the apartment buildings to avoid excessive smoke entering neighboring units. Upon approval from Administration, electric grills may be permitted. Grills may be used on deck, patios and back yard areas of cottages.

13. Residents have access to the outdoor Recreation Area, which is located onEden View Road adjacent to the Woodworking Shop. An outdoor pavilion,gas grill, and games are available for use. To reserve the pavilion, pleasecontact the Event Coordinator at extension 33151. Reservations are taken on a first come basis.

14. The Veteran’s Grove, located off Freemason Drive, contains a picnic areathat is also available for use by residents for private family gatherings. Toreserve the Veteran’s Grove picnic area, please contact the Event Planning Office at extension 33151.

15. Residents are free to place political signs in the window, on the shelf outsidetheir apartment, their balcony, on the front or back porch of their cottage orin the landscape bed immediately off their porch or patio. All political signs must be removed immediately following the election.

PARKING

All residents are required to submit a parking application to the Retirement Living Administrative Office within seven days of occupying their living unit in order to obtain a parking decal for their automobile.

All parking, with the exception of the optional covered parking located on the lower level of the Buchanan Courtyard apartments, is available on a first come basis; therefore, there are no reserved outside parking spaces.

Residents who have selected optional covered parking located on the lower level of the Buchanan Courtyard apartments have been assigned a parking space which will be designated on the parking block. No resident may sublet his or her covered parking space to another resident.

Residents living in cottages are encouraged to park in their driveways. Cottages located at Truman, Franklin and Jackson Drives have parking spaces designated for off street parking. At no time should vehicles be parked on grassy areas.

Residents living in the Village Green Areas may use the parking lots across from the Berks Building, and at the rear of the McKee II building.

Parking regulations are for automobiles only. Parking of RV’s, boats and other pieces of equipment must be approved by the Director of Retirement Living.

PET GUIDELINESWith the approval of Administration, a resident may bring into the living accommodation up to two pets. The resident agrees that after a pet dies, the resident may only be able to replace a pet with the written approval of Administration to assure that the resident remains capable of caring for the pet in the living accommodation. If the pet becomes a nuisance to other residents, in the sole determination of Masonic Village Administration, the resident shall be required to remove the pet. The resident agrees that he shall properly care for the pet at all times. The resident agrees that if he enters Retirement Living with a pet, he will adhere to the guidelines set forth below. If a resident fails to comply with this provision in any way, the pet owner shall be required to remove the pet. The resident agrees that he shall assume financial and any other obligations for any damage caused by the pet.

Guidelines:Dogs, cats, small birds, and fish are permissible pets in the community.

With the exception of Service Dogs, pets are not permitted in dining areas. Pets that are passing through common areas must be on a leash at all times.

When dogs or cats are moved through the building, they should be moved from the resident's apartment to the nearest outside exit.

Pets may not be exercised in the corridors of any building and pets living in apartment buildings must be walked in areas away from the building. If there is a question about where the pet can be walked, please contact Administration.

Sizes:For residents occupying apartments, dogs shall weigh no more than forty(40) pounds and stand no more than twenty-four (24) inches at the shoulder.

Licensure and Tags:

All dogs and cats must have a current Lancaster County license.

Every dog and cat must be registered with the Retirement Living Administration Office using the Pet Information Form which must be kept

current. Any changes should be reported to the Administrative Office immediately. In addition to the completion of the Pet Information form, the resident must provide written verification annually that the pet has been properly inoculated.

Dogs and cats are required to be "house broken". Pet owners shall be responsible for the immediate cleanup of feces after the exercise of their dog or cat. Both dog and cat owners must secure pet feces in sanitary bags and deposit it in designated trash receptacles. When disposing of cat litter, please make sure to use two bags before disposing of it. Cat litter may not be disposed of in the toilet.

Dogs and cats shall be on leashes of no more than eight (8) feet and be under the resident’s control at all times outside the confines of the pet owner's living unit.

Residents residing in cottages may, after receiving written approval from Administration have an invisible fence professionally installed in the rear yard area of the cottage within the limits approved by Administration.

The pet owner must supply Administration with the names of those persons who will be willing to assume immediate responsibility for the pet in case of an emergency. This information will be provided on the Masonic Village Retirement Living Pet Information Form. The alternative caretaker expenses are the responsibility of the pet owner.

If an alternative caretaker cannot be reached, the pet owner is responsible for making arrangements with a boarder. It is recommended that arrangements be worked out with a boarder before the need occurs. Masonic Village willnot be responsible for transporting pets to kennels.

*Please do not put food outside for stray cats that roam the campus. Those cats could be carrying diseases and are not de-clawed. Also, placing food outside draws other animals and rodents to populated areas.

PROLONGED VISITS AWAY FROM THE COMMUNITY

VACATION FORMResidents planning to be away from their residence for more than a day shouldnotify the Administration by submitting a Vacation Notice at least one week prior to the vacation if possible.

Extended periods of time away from Masonic Village-If you are going to be away from Masonic Village for an extended period of timewe recommend that you find someone to collect your in house mail for you. If you live in a cottage and depending upon the time of year that you are going to be away you may also want to consider having your cottage winterized. To have that done, you will also need to put in a work order to the Maintenance Department.

HOSPITAL STAYSIf you are admitted to a hospital and our Masonic Village staff is not involvedwith that admission, please notify someone in the Retirement Living Administrative Office at 14504. This is the only way we have of involving Social Services and the Pastoral Staff.

RECREATIONAL VEHICLES

Residents are permitted to park their Recreational Vehicles (RVs), trailers, or small fishing boats in the RV lot just past the Rooster’s Corner (WoodworkingWorkshop) located on Eden View Road.

The Retirement Living Administrative Office should be notified before the resident parks his/her RV, boat or trailer in the designated area. The parking spaces are numbered and a space will be assigned for parking. All vehicles must have a Masonic Village parking sticker which can be obtained at the Administrative Office located in the Clubhouse.

RVs may not be parked in driveways or any other parking lots other than designate RV lot.

REQUIRED MEDICAL INSURANCEMEDICAL EXPENSE REIMBURSEMENT

In accordance with the Resident Agreement, all residents shall be required to carry medical insurance including Medicare Part A, B and D, and a standardized Medicare supplemental insurance plan such as Plan C through Plan N or insurance subject to approval by the Masonic Village at Elizabethtown.

Upon completion of the application for residency at the Masonic Village, all residents should submit copies of insurance and photocopies of insurance cards to the Marketing Office. This information will be forwarded to the FinanceDepartment for review. After occupancy, residents should notify theAdministrative Office of any medical insurance changes.

Reimbursable medical services and prescription drugs provided by Norlanco physicians at the Masonic Health Care Center, or provided at Sycamore Square Family Medicine or Norlanco Clinic, will be billed by Norlanco or by Lancaster General Health. Norlanco is affiliated with Lancaster General Health. All questions concerning Norlanco billings and/or forms should be directed to the Norlanco Clinic or the phone number on the billing. All questions concerning Sycamore Square Family Medicine billing should be directed to that office

If a resident prefers to use the services of a physician or pharmacy outside the Masonic Village, the resident is responsible for completing and submitting all insurance forms for reimbursement.

RESIDENCE KEYS

In order to maintain the maximum security for the personal safety and well-being of each resident and his or her personal belongings, the following Key Procedure has been established. A key will be provided to the resident(s) occupying the living accommodation on the day they take occupancy and complete all financial arrangements.

The key for each apartment will open the entrance door into the unit, and the mailbox for each living unit in the apartment buildings. Residents occupying cottages on Tall Cedar Lane will be able to use their key to open the front door, kitchen door, and mailbox. Residents living in other cottages will have a key to the front door of their cottage. Residents living in cottages that have a storm doorshould not lock the storm door. Security staff will need to have access to the cottage in the event of an emergency.

If a resident is locked out of their apartment or cottage during the hours of 8 a.m. -4 p.m. Monday through Friday, a key may be obtained at the Retirement LivingAdministrative Office located in the Clubhouse or the Marketing Office located in the Freemason Cultural Center for residents who reside in the Village Green Area, Harding, Garfield and Polk Drives. If it is not convenient for you to come to the one of these offices, you may contact the Switchboard by dialing “0” and Security will be notified and will arrive as soon as they can to let you into your apartment or cottage. If you are locked out after 4 p.m. or anytime on the weekend, you will need to call the Switchboard for assistance.

The exterior doors of the apartment buildings will be locked from approximately 8:00 p.m. until 6:00 a.m. During the period that buildings in the Village Green area are locked they can be accessed by using a keypad. Residents will be able to gain access at all other apartment buildings by using an Access Card.

No staff other than Administration and Security shall have immediate access to keys to enter resident’s living units. As stated in section 11.3 of the Resident Agreement, Masonic Village maintains the right to enter a living accommodation to respond to a medical or fire emergency. Masonic Village employees may also enter a living accommodation of a resident is reported missing or has notresponded to calls. Unless otherwise directed by a signed waiver form,

housekeeping and maintenance staff may enter to perform scheduled or emergency duties.

Additional keys may be acquired for a resident’s unit by submitting a Maintenance Work Request to the Retirement Living Administrative Office.*

RESIDENCE UNIT TRANSFER(MOVING FROM ONE RESIDENCE TO ANOTHER)

In accordance with the Resident Agreement, if a resident wishes to move from his/her current living unit, the resident may do so by following the parameters outlined in the Change of Living Accommodations section which can be found in the Resident Agreement.

A refurbishment fee will be charged and will be determined by the current cost of labor and materials. The current refurbishment fee will be discussed with the resident at the time of the transfer request.

If a resident wishes to move to another apartment or cottage in Retirement Living, a letter requesting that move should be sent to the Director of Retirement Living. The Director of Retirement Living will forward the letter to Marketing requesting the individual(s) be put on the appropriate waiting list.

Please note if a resident is moving to Personal Care, The Health Care Center or off campus, there is no refurbishment charge. The refurbishment fee only pertains to internal moves.

SMOKING GUIDELINES

The Committee on Masonic Homes, at their meeting on July 14, 2006, approved a continuation of the Masonic Village wellness program by establishing the vision for the Masonic Villages to be smoke free communities.

The Masonic Villages have always focused on wellness as part of our communities’ culture. We have initiated multiple wellness programs over the last several years. We monitor trends and best practices and focus on input from our important constituents - our residents, families, volunteers, visitors, and employees. The Masonic Village smoke free vision was and is based on input from our constituents, changing thought in society, and our desire to provide leadership from a health and welfare perspective.

Effective August 7, 2006, all new residents from that date forward are covered by this new policy. Residents who were currently part of our campus, have contracted with us, or were scheduled for admission on or before August 7, 2006, who do smoke will be grandfathered under our current smoking policy. Those residents will be permitted to smoke in their particular living unit. However, no smoking is permitted in any other area within the buildings or Masonic Village property. Effective August 7, 2006, residents, families, volunteers, and those people doing business here are not allowed to smoke anywhere on Masonic Village property. This would pertain to cigarettes, cigars, pipes and electronic cigarettes.

Effective July 1, 2007, employees are not permitted to smoke anywhere on Masonic Village property.

SOLICITATION

Masonic Village has established a no solicitation policy. Outside businesses and vendors are not permitted to solicit to residents of Masonic Village. If a door to door salesman should come to your door selling something, you should inform that person that solicitation is not permitted on our grounds, and if he or she has any questions, he or she should be directed to the Retirement Living Administrative office.

As in any situation if you feel that the person appears suspicious or has an intention of causing harm, please contact the Switchboard who will notify Security.

If you receive a call or an e-mail asking you to verify some information, do not give out any information over the phone or on-line. Also be wary of anyone on the phone asking you to wire money to help them out. Scammers will try anything to convince you to send them money. Never give out your social security number,bank card number, etc.

Always ask questions, and don’t be afraid to say “No”. Protect yourself byknowing what to look for in a scam. If you ever have concerns or want advice onwhether or not something may be a scam, please feel free to contact the Administrative Office for assistance.

In order to assure that all residents receive equal service, regardless of their financial position, the Masonic Village at Elizabethtown seeks to safeguard itsresidents from the adverse impact of acceptance of gifts, gratuities, or tips.Employees are not permitted to accept or solicit monetary and material gifts, gratuities, or tips from residents, families, vendors, or the general public.

Employees will politely decline to accept any gifts of cash, gift certificates, or any items offered as an expression of appreciation to an employee. Please do not put an employee in jeopardy of job loss by giving them gifts or money

VEHICLE WASHING & MAINTENANCE

Residents wishing to wash their vehicles may do so at the Woodworking Shop located on Eden View Road. A hose is available on the south side of this building for your convenience. A vacuum is also available. Vehicles should not be washedat other areas near storm drains.

Cottage residents may wash cars in their driveway; however, soaps and detergents should not be washed down storm drains.

Resident Organizations

ART ASSOCIATION

The Art Studio (extension 33233) is located in the Cultural Center on the first floor, off the atrium. The Association meets weekly on Thursdays at 2:00 p.m. A key to the Studio is available to members so that they may work on art projects on their own time. The Association provides art desks, art supplies and some instruction. Members also work on projects for sale in the Art Gallery and at Autumn Day to help defray expenses. The Art Gallery is open Sunday afternoons.

BILLIARDS

A pool table is available for residents and guests on the main floor of Grand LodgeHall, in the room adjacent to the Solarium and in the James Buchanan Game Rooms.

BINGO

Bingo is held the first and third Saturday at 2:00 p.m. in the Keystone Room of the Clubhouse and the second and fourth Friday at 2:00 p.m. at Louis Eisenlohr.

BLOOD DONOR CLUB

Membership in the blood donor club is open to all residents of the Masonic Village. There is no cost to join, and once you are a member there will never be a charge for blood. The Club is run thru the Grand Lodge Blood Donor program, and they sponsor three blood drives on campus throughout the year.

BOCCE

Two bocce courts are located adjacent to the Brossman outdoor pool. Play usually begins in April and continues through September. Bocce for women is held Wednesdays at 9:00 a.m. and Mixed Bocce is held Mondays at 9:00 a.m.

BOOK CLUB

This book discussion club is for those who love books and the adventure of reading. The book club meets the first Thursday of the month in the Grand Lodge Hall Recreation Room at 1:00 p.m.

BOWLING

Bowling is available off campus at Clearview Lanes located on Rt. 230 outside of Elizabethtown. Bus transportation is provided during league play. Fridays at 9:00 a.m.

CARD GAMES

Many different card games are played on campus at various times and in various locations.

Bridge Group – plays on Mondays from 6:45 p.m. in the Keystone Room. A total of 16 hands are played. The group uses rubber bridge scoring.

Duplicate – played from 1:00 p.m. to 4:00 p.m. on Tuesday in the Keystone Room of the Clubhouse. A bridge instructor is on hand to facilitate the games. Edward Scanlon, who is a nationally ranked player, teaches the most modern approaches to bidding and play, and also directs the weekly games under the York Boardroom's ACBL sanction. Each session includes a short lesson followed by a duplicate game. The fee to play each week is $5.00 per person (used to pay the instructor).

Pinochle – this group meets in the Keystone Room on Wednesday night at 6:45p.m. Cards and scorecards are provided and hands are changed after four games. Play is penny ante at 25 cents to play.

CERAMIC CLUB

The Ceramic Club (extension 33762) meets on Fridays from 8:30 a.m. to 11:30 a.m. in the ceramic studio in Smith North, lower level. Enter Smith from the parking lot, take the elevator down and turn right off the elevator and then left around to the

ceramic room. Ceramic activity includes the selection of greenware, painting and firing. The club sells their wares at Autumn Day and other events on campus. Proceeds of sales are used to purchase greenware, paints and glazes. Club members receive a key code and may use the studio whenever they wish.

CRAFT CLUB

The Craft Club meets in the Craft Room (extension 33387) on the lower level of the Clubhouse Wednesday mornings from 9:00 a.m. to 11:30 a.m. Crafters also work on their own in their own homes. Crafts range from knitting and crocheting to creating jewelry and everything in between. Proceeds from sale of craft items are donated to the Tree of Life.

COMPUTER “PLUS” CLUB

The Computer PLUS Club meets on the third Thursday from 1:30 p.m. to 3:00 p.m. in the Keystone Room located in the James Buchanan Drive Clubhouse. Various topics are discussed at each meeting regarding computers (Internet, software, and e-mail) as well as related technologies (e-books, I-Pad, Smart Phones and emerging technology). Guest speakers are also invited periodically to discuss pertinent topics. The club also has a six-station computer lab and six printers, Windows 7 and XP operating systems, all are connected to high speed Internet access. A wireless hotspot for laptops is also available in the lab for those needing access, with resident approval. Computer courses are given at a minimal charge on Tuesday and Thursday afternoons from 1:00 p.m. to 3:00 p.m. Open general lab sessions are available at this time when classes are not in session.

DANCES

Dances are held seasonally in the Keystone Room in the Clubhouse and are listed on the monthly Activities Calendar

GARDEN PLOTS

Individual garden plots are available in two sizes, either 25 x 50 feet or 25 x 25 feet. They are located off Commandery Road. Gardeners must sign up for space in the spring through the Activities office. Rototillers are provided but gardeners are expected to bring their own tools. A shed is provided for storage and water connection is also available. Contact the Activities Department for more information.

GENTLEMEN SINGERS

The GentleMen Singers is an ensemble group of men who perform at various events during the winter holiday season. They begin their rehearsals in the fall. Their purpose is to tell the story of the birth of Christ using all the Christmas Carols that everyone knows.

GIANT FOOD CARD VOLUNTEER PROGRAM

In cooperation with Giant Foods, resident volunteers sell discounted gift cards to employees for use at Giant. Sales take place at various convenient times and locations so that the employees can take advantage of this generous Giant program.

GOLF

Golf leagues are formed in early spring and run through fall. Usually there is a men’s league and a women’s league and a mixed league. Play takes place at the Par Line Golf Course on Rt. 230. Transportation is on your own. Contact the Activities Department for more information.

GREY LIONS OF E-TOWN (PENN STATE CLUB)

Club meets on the first Friday of the month at 2:00 p.m. in the Keystone Room of the Clubhouse. It is open to all Penn State alumni or anyone who has an interest in Penn

State. Programs consist of speakers involved in some way with Penn State, either as faculty or as alumni. Trips to State College are also part of the program as is the occasional tailgate party.

HAPPY HOUR

Happy Hour meets from 3:30 p.m. to 5:00 p.m. on the 4th Friday of the month in the Keystone Room at the Clubhouse. Purpose is to share fellowship and good cheer while enjoying snacks and adult beverages. There is usually piano entertainment and a sing-a-long. Program is funded through voluntary contributions to offset the cost and any excess funds are donated to the Tree of Life program.

HOLIDAY GREETING CARD PROJECT

This is a campus wide project that raises money for Masonic Charities and allows holiday greetings to be sent to other residents at the Masonic Village.

HANDBELL CHOIR

The Handbell Choir meets every Monday at 9:00 a.m. at Sell Chapel. This group performs at chapel services and other local churches during the holiday season. For more information contact Pastoral Care at extension 33137.

LIBRARIES

The libraries are run through a volunteer library committee. Contact the Activities Office to get the name and phone number of the head library volunteer. Libraries are located in the Grand Lodge Hall, the James Buchanan Clubhouse and the Health Care Center.

Mah Jong is being played at Sycamore South activity room on Monday evenings at 6:30 p.m., Grand Lodge Hall in the game room on Wednesdays at 12:45 p.m. and on Thursday evenings at 6:45 p.m. in the Smith North Recreation Room.

MEN’S CHORUS

A group of men brought together by their love of singing. They meet on Thursday at 7:00 p.m. in the Keystone Room. They perform at various times throughout the yearand provide the musical ministry for all Sell Chapel services on the 2nd Sunday of themonth.

MV & E RAILROAD CLUB

The members of the Railroad Club (extension 33253) meet on the first Tuesday of the month at 7:30 p.m. in the Model Railroad Building located on Freemason Drive,across from the Health Care Center. Membership is open to residents of the Villageand also the surrounding area. The club builds, operates and maintains model railroad layouts. It has “G, Standard, O, HO, N and Z” gauge layouts. The club has a one-timeinitiation fee and annual dues. The club holds regular “open houses” on the 1st

weekend of the month from September through April.

MOVIES

Movies are shown in both the Keystone Room in the Clubhouse and the Deike Auditorium in the Cultural Center. They are announced in the monthly Activities Calendar and can run the gamut from recently released to oldies but goodies. Movies usually begin at 7:00 p.m.

MVTV

MVtv is the Masonic Village closed circuit television station that carries announcements, Sell Chapel broadcasts, exercise programs and other programs that are of interest to the residents of the Village. The station is run by volunteers and is carried on the local cable network, channels 66 & 95.

QUILT CLUB

Meets on Mondays from 1:00-3:00 pm in the Sycamore North Activity Room

RETIREMENT LIVING RESIDENTS ASSOCIATION

An Executive Committee is made up of elected representatives of the various living components at the Masonic Village. They meet monthly prior to the Association meeting to discuss items of interest to the residents. Executive Committee meetingsare held at the Patton Campus at 2:00 p.m. on the Monday prior to an Associationmeeting. Monthly meetings of Association are held on the 1st Thursday of the monthat 9:30 a.m. in the Dieke Auditorium. All positions are elected and unpaid.

ROOSTER’S WOODSHOP

The woodshop is located on Eden View Road just down from the Farm Market. Sincewe are not a teaching facility there are no instructors. Some experience inwoodworking would be helpful, but lack of experience will not keep a resident from being allowed to join. However, members must be physically independent to join the Woodshop. To apply for membership, call the Woodshop at extension 33451 and ask to speak with the President or a Vice-President. An entrance fee is required, and annual dues are collected. In addition, the Administration Office requires that a Liability Release Form be signed. (revised February 3, 2014)

SHOW AND TELL

Show and Tell is a program that is historically held every other year in October in the Brossman Ballroom. It is an event that allows and encourages residents to share their hobbies or collections or talents with other residents. Hours are usually 10:00 a.m. to 3:00 p.m. Check the monthly Activities calendar for information.

SQUARE DANCING

Square Dancers meet every Wednesday evening at 7:00 p.m. in the White Ballroom on the main level of the Cultural Center. Everyone is welcome and the caller instructs as well as calls dances.

TENNIS

Tennis Courts are located on the Patton Campus on Bainbridge Road. All interested persons are encouraged to participate once the weather clears, usually in April. Contact the Activities Department for more information.

TRAVEL CLUB

The Club plans at least three travel events throughout the year. Past trips have been to Vermont, the Poconos, the Biltmore Estate in North Carolina and the Greenbriar Resort in West Virginia. All arrangements are made through the club, making traveling easy. For more information contact Roberta Ewing at extension 15173.

VILLAGE GREEN AREA RESIDENTS COUNCIL

The representatives to the Council are elected by the residents of the Village Green area of residential living. The Council meets monthly in the White Ballroom in the Cultural Center on the 4th Thursday of the month at 9:30 a.m. Representatives are elected for 2-year terms and are unpaid.

WOMEN’S CHORUS

Rehearsals are Monday afternoon at 1:30 p.m.in Smith North lower level unless they have sung at Sell Chapel the day before. Participation is open to all women who reside in the Village. There are no auditions and no requirement to read music. The purpose of the group is to have fun and to take their music seriously. They perform at Sell Chapel and at retirement homes or churches in the area.

FILE OF LIFEThe File of Life program is a countywide program whereby an Emergency MedicalInformation Sheet is completed and placed in a magnetic file pouch. The file pouch is then placed on the outside of the refrigerator. The file alerts emergency personnel that pertinent medical information is available to them, if necessary. Participating in this program can be very helpful to emergency personnel, paramedics, etc., when responding to the scene of an emergency.

The File of Life program is voluntary; however, participation is encouraged.

Materials for the File of Life program are distributed to each resident prior to occupancy.

Residents are encouraged to periodically update their medical information sheets so that emergency personnel have the most up-to-date information possible. New information sheets can be obtained from the Retirement Living Administrative Office.

HEALTH RELATED EMERGENCYIn the event an emergency situation arises which requires medical attention, theresident should activate the Emergency Call Bell System located in each bedroom and bathroom of a living unit. In addition, the resident should call 911 to alertoutside medical personnel. The Masonic Village at Elizabethtown Switchboard Operator will immediately call the resident’s apartment or cottage to verify that the Emergency Call Bell System was not inadvertently activated, and then proceed with the follow-up call to 911. Nursing staff will also be paged to respond to the scene. If you are concerned that you may not be able to get to a phone or your pull cord if you would fall or have a medical emergency, there is a personal security system that works in the confines of your apartment or cottage. It is called Cord Mate and you can find out more information by contacting your social worker.

If this procedure is properly followed, 911 will be called twice to ensure that emergency personnel are dispatched to the appropriate area. Security or other appropriate personnel will be summoned to unlock necessary doors for emergency personnel. residents in order to receive a quicker response by emergency personnel who are properly equipped to handle emergency situations.

We encourage each resident to enroll as a member of the Northwest Emergency Medical Services (NW EMS). Residents may contact the Retirement Living Administrative Office for information. (ext. 14504)

As a member of Northwest Emergency Medical Services, if you need emergency services, NW EMS will accept whatever Medicare and your supplemental insurance pays. If you do not have a NW EMS membership, you will be responsible for any balance remaining after the bill has been submitted to Medicare and a third party insurance.

It is important to know that non-emergency voluntary transports are not covered by the NW EMS membership and the amount charged will depend on the distance of the trip and the equipment needed.

URGENT HEALTH RELATED SITUATIONSIf a resident has an urgent condition needing medical attention you should activate the call bell located in their bathroom and bedroom or contact the Masonic Village at Elizabethtown Switchboard Operator (dial 0) and ask for a nurse. The nurse may resolve the situation over the phone or respond to the resident’s home to evaluate further the resident’s condition. In the event the nurse determines the resident’s situation needs further medical intervention, the necessary steps will be taken to provide the appropriate care for the resident as determined by the physician on call.

BANK – The Committee on Masonic Homes has selected Wells Fargo as the financial institution to provide availability of full service banking on site at theRetirement Living Clubhouse. This branch has been established primarily for thepurpose of providing the residents of the Masonic Village the convenience of full service banking on the grounds of the Masonic Village. This branch is not advertised to the general public as an Elizabethtown Branch of Wells Fargo Bank; however the general public is permitted to utilize the banking services of this branch as long as they adhere to all Masonic Village’s policies and do not interfere with the service intended for the residents of the Masonic Village. Employees of the Masonic Village may also use the branch bank during their non-working hours or during meal breaks with the approval of their immediate supervisors.

Wells Fargo Bank is located on the main floor of the Retirement Living Clubhouse. Hours are Monday through Thursday 9:30 to 3pm, Friday 9:30 to 4pm. The bank is closed Monday through Friday from 12 noon to 12:30. The telephonenumber for the bank is 367-6476.

Wells Fargo Bank provides the following banking services: checking accounts (various types) money market account, savings accounts, certificates of deposit, individual retirement accounts (IRA), banking by telephone, direct deposit, investments/discount brokerage, trust services, VISA/MASTERCARD, personal lending services, safe deposit boxes. Direct Banking Center available through toll free number 24 hours a day, 7 days a week.

PERSONAL SPENDING ACCOUNTS - An account is available for eachVillage Green Area resident to manage personal funds. Deposits and withdrawals can be made in the banking office (referred to as Mini-bank) located in Grand Lodge Hall on the first floor marked Clinic Room. Hours of operation are:Monday through Thursday, 11:00am to 12:00pm.

COPY AND FAX MACHINES – residents requiring the reproduction of materials of a personal nature are able to do so (for a small fee) through the use of either the copier in the Clubhouse Market located on the main floor of the Retirement Living Clubhouse or the copier in the Village Shoppe located on the main floor of the Grand Lodge Hall.

Fax transmission services are also available (for a small fee) to residents for personal use in the Retirement Living Administration Office. The fax number atfor the Retirement Living Administration Office is 361-5202.

The copy machine located in the Clubhouse Mail Room on the lower level of the Clubhouse is to reproduce information which is pertinent to the official administrative operations of the Masonic Village at Elizabethtown. This copy machine may also be used for the Resident’s Association or any subcommittees of the Resident’s Association. CRAFT BOUTIQUE – located on the lower level of the Retirement Living Clubhouse, the Craft Boutique sells items such as hand crafted jewelry, door hangings, knit items like afghans, children’s sweater and gloves and novelty gift items. The Craft Boutique is open all day, every day and purchase of items is on an honor system if someone is not in attendance. Extension 33387.

HAIR CARE CENTERS – located on the main floor of the Retirement Living Clubhouse and on the lower level of Grand Lodge Hall. This service area is available by appointment. All appointments should be made in advance bycalling the Hair Care Center at extension 14510 for the Clubhouse and extension 33258 for Grand Lodge Hall.

For your convenience, all services in the Hair Care Center are on a fee-for-service basis and are charged to the resident’s monthly bill. A listing of prices for all hair care services is available in the Hair Care Center and is also listed on the fee schedule in this book.

INFORMATION DESKS AND CORNER –Individual information desks are located in the Clubhouse, Cultural Center, Health Care Center and Grand Lodge Hall. They are staffed by resident volunteers and their purpose is to provide a resource for residents and visitors alike who may need assistance with locating facilities or activities on Campus. The Cultural Center information desk also provides tours of the campus. Phone extensions are: Clubhouse 14512, CulturalCenter 33519, Grand Lodge Hall 33205 and the Health Care Center 33634.An Information Corner which contains pamphlets and information packets to helpfamiliarize a resident with services available in the Elizabethtown and surroundingareas is located in the Clubhouse Keystone Room.

All information placed in the Information Corner is approved by the Administration and/or Activities Supervisor to ensure that we remain in compliance with the Masonic Village at Elizabethtown’s No Solicitation Policy.

LIBRARIES – Three main libraries, located in the main hallways of the Retirement Living Clubhouse (extension 33518); Grand Lodge Hall (extension 33592); and the Health Care Center. The libraries are open all the time and offer thousands of titles of non-fiction, fiction, reference and audio. No membership required to sign out books, audios or DVDs. Libraries are also available in other apartment buildings. Donations of new and gently used books and audio books are accepted.

LOST AND FOUND – Residents who have lost items should contact theRetirement Living Administrative Office. For any losses of significant valuecontact the Security Office at extension 33281.

Items found in any of the buildings or on site should be turned in to either the Retirement Living Administrative Office or the Security Office at extension 33281.

The Security Office is located on the lower level of the Grand Lodge Hall Building. Large sums of money or items of significant value will then be taken to the Retirement Living Administrative Office and Administration will be notified immediately.

The mailroom is staffed by volunteers to assist residents’ mailing needs(selling stamps, processing first class and priority mail packages). MASONIC LIFE CENTER – is a comprehensive wellness center providing fitness center and pool membership, fitness assessments, personal training,wellness coaching, wellness workshops, group fitness classes, wellness resources library and massage therapy by licensed and certified Personal Trainers are available to resident to assist you with your wellness goals and to help you achieve a healthy lifestyle.

The Masonic Life Center (MLC) is temporarily located in the White Ballroom at the Freemason Cultural Center. The Center operates Monday through Friday 6am to 7pm and Saturday 7am to 3pm. The MLC is a SilverSneakers® approved site and has received awards from the Internals Council on Active Aging, Wellness Councils of America and the NuStep Pinnacle Award for having one of the best Senior Wellness Centers in the nation.

MARKETSCLUBHOUSE MARKET – Located on the main floor of the Retirement

Living Clubhouse, the Clubhouse Market provides residents an opportunity to purchase staple goods, dairy products, greeting cards and snacks, sundries and gifts, for the convenience of the residents. FARM MARKET – Located on the western end of the campus on Eden View Road, the Farm Market strives to meet the fresh produce needs of the residents, employees, volunteers and families of the Masonic Village and the Masonic

Fraternity while also attracting the surrounding community with the highest quality farm-fresh produce and farm related items.

Within the 80 acre orchard, we grow 50 varieties of apples, 30 varieties of peaches, nectarines, cherries, apricots, plums and pears. During the season we purchase other fruits and vegetables from area farmers to resell in our market.

Apples are harvested from July through early November. Some of the varieties are Summer Rambo, Ginger Gold, Gala, Red & Golden Delicious, McIntosh, Honeycrisp, Empire, Stayman-Winesap, Granny Smith, Pink Lady and many more. We also offer Pick Your Own apples in September and October.

In spring we offer bedding plants, vegetable transplants and hanging baskets. Early spring will usher in fresh local asparagus and southern strawberries – June brings the local strawberry and homegrown cherry season. In the fall, in addition to apples, we sell thousands of Hardy Fall Mums, pumpkins, Indian corn, gourds, straw bales, corn stalks and more. During the Holiday season we make fruit baskets and gift boxes for shipment anywhere in the USA.

Along with the wonderful fruits and vegetables, the farm Market sells fruit pies, cookies and apple dumplings. We also have many gourmet preserves, jellies and fruit butter.

PUTTING GREEN – located adjacent to the parking lot of the outdoor Brossman Pool, the putting green is available for residents to practice their putting.

POOLSBROSSMAN OUTDOOR POOL – is operated for the enjoyment of the residents,employees, employee families, retirees, retire spouses, volunteers of the Masonic Village and the community. The pool is open from Memorial Day to Labor Day. The pool will close in the event of inclement weather and when the outside temperature falls below 70 degrees or at the Lifeguards discretion. Retirement Living residents will use their MVP card as entrance into the Brossman Pool.

Residents are permitted to take visitor(s) to the Brossman Pool. The resident will be required to obtain a day pass for their visitor(s) from the Masonic Life Center located on the Terrace level of the Freemasons Building. The Masonic Life Center is open from 6am to 7pm M-F and 7am to 3pm on Saturday. Day passes will not be available at the pool. Should a Retirement Living guest arrive after 3pm on Saturday or on Sunday, they may show their MVP card at the pool and their guest will be permitted to enter. All visitors must be accompanied by a resident at the pool. If the resident visitor(s) would like a season pass to the Brossman Pool they may purchase a 3 month pool membership at the MLC. All pool participants MUST sign in at the gate and show pool passes to the lifeguard or volunteer at the front desk upon entering the pool.

FLOHR POOL AND SPA – is located on the first floor of the Health Care Center and is operated for the enjoyment of residents. Open swim and Aqua Group Fitness classes are offered at various times. Schedules are posted at the pool and available at the Masonic Life Center. The Flohr pool is a warm water pool (90 degrees) and the Spa is 100 degrees.

VILLAGE BOUTIQUE – is a used clothing shop that provides gently used clothing for residents and employees and their families. It is located on the lower level of the Grand Lodge Hall and is open Monday through Friday 8:30 a.m. to 10 a.m.; Tuesday through Thursday 1 p.m. to 2:30 p.m.; Tuesday 3:00 p.m. to 4:30 p.m. Donations of clean clothing for resale in the Boutique are welcome. WALKING TRAILS and UNDERGROUND WALKWAYS – a system of walking trails is provided throughout the campus with trail indicators at various locations indicating the distances traveled. The buildings in the Village Green Area are all connected by underground walkways which allows for walking in inclement weather.

RESIDENT FINANCIAL ASSISTANCE

The Masonic Village considers each individual’s ability to pay for his or her On- Campus Services and may extend financial assistance to eligible individuals who are unable to pay for their On-Campus Services. The determination of a resident qualifying for assistance will be based on the individual’s ability to pay.

The need for financial assistance may be a sensitive and deeply personal issue for recipients. Confidentiality of information and preservation of individual dignity shall be maintained for all who seek charitable services.

Residents in need of financial assistance, in order to pay their monthly service fee, shall meet with the Director of Retirement Living to review the residents request and the circumstances leading to the resident's inability to meet their obligation to pay their monthly fee and other monthly expenses. It is recommended that a resident contact the Director of Retirement Living when their total assets are approximately $10,000. After meeting with the resident, the Director of Retirement Living shall submit a request to the Chief Operating Officer of Retirement Living for review and approval.

If the application is approved, financial support for the resident will begin. The resident will meet with the Director of Retirement Living regularly to review the resident's financial status.

TREE OF LIFE FUNDThe Committee on Masonic Homes made provision for a separate fund to beknown as The Tree of Life Fund, for those residents of Retirement Living whose funds have been depleted.

The leaves on the Masonic Tree of Life, as well as the stones at the base, are designed to be purchased and inscribed in honor or memory of an individual. They may also be purchased and inscribed as an expression of gratitude. An individualleaf costs $1,000.00, an acorn costs $5,000 and stones come in varying sizes ($10,000.00, $25,000.00 and above and $50,000.00 and above) and may be purchased through the Gift Planning Department of The Masonic Villages (extension 33566).

The Tree of Life Fund is used to subsidize payment of the resident's monthly service fee for those residents of Retirement Living whose funds have been depleted. Both principal and interest from this fund may be used for this purpose. Also, as per the Resident Agreement (section 7.2) residents receiving financial assistance will need to move to the smallest apartment available. The Director of Retirement Living will work closely with residents during the transfer process to ensure the protection of confidentiality.

SERVICE FEE - INVOICE PAYMENTEach month residents receive a monthly invoice which includes the following: themonthly fee corresponding to the living unit which the resident occupies, basictelephone, and long-distance telephone charges, meal credits incurred during thepast month, and charges for any additional services which the resident has selected during the past month. The standard monthly invoice is always billed prior to the month it covers, but additional services (such as hair care services) would show upon the next month’s bill. However, in the Village Green Area the standard monthlyinvoice is bill for the current month.

The monthly invoice will be sent to the resident around the fifteenth day of each month and must be paid to the Masonic Village at Elizabethtown FinanceDepartment by the last day of the month. This will assure that payment of the basic monthly fee is always paid prior to the month for which it covers.

Payments may be dropped into any in-house drop box before or on the last day of the month to assure timely payment. The contents of the in-house drop boxes are picked up daily Monday through Friday.

Service Fee problems and questions should be referred to the Retirement Living Receivables Coordinator at extension 33325, or the Private Pay Coordinator/Billing Team Leader at extension 33321 both located in the Goodyear Building.

MEAL PLAN OPTIONS

Retirement Living residents have the opportunity to select one of three meal options. These options are described below.

StandardMeal Plan:

Includes one meal for the total number days of the month. Mealscan be used for any day of the month and for any meal of the day that is offered. The meals can be taken in the Restaurants at Grand Lodge Hall, Clubhouse or Eisenlohr, as well as Courtyard Café, Three Loaves Café or the Village Cafe. Meal credits will be given when residents are away from the Masonic Village at Elizabethtown for at least seven (7) consecutive days in a month or on the first day a resident is hospitalized or admitted to the Masonic Health Care Center.

20 MealPlan:

Includes 20 meals that can be used for any day of the month andfor any meal of the day that is offered. Each resident selecting thisplan would be provided a monthly meal credit in the amount of$54.75. Additional daily meal credits will be provided toresidents if they are away from the community for a consecutive number of days that would prevent them from eating 20 meals per month. For example, if a resident is away forconsecutive days in a 30 day month or 12 or more consecutivedays in a 31 day month. Daily meal credits will be provided on each day a resident is hospitalized or admitted to the Masonic Health Care Center if credits are available.

Opt Out: No meals are included. Each resident selecting this option wouldbe provided a monthly meal credit in the amount of $160.00. Meals in the dining rooms are available at a price of $7.75 per meal (with use of MVP card) for breakfast, lunch, dinner, orbrunch; or, $11.25 per meal for Holiday and Special ThemeDinners and Picnics.

A married couple may share their plans if both of them are on a meal plan, Standard or 20 Meal Plan. For example, if you both are on the 20 Meal Plan, you may use your 40 meals in any way you choose during the month or one person is on the standard plan and the other is on the 20 meal plan you may also share your total meals for the month. For married couples, if one person “opts out” and is not

on a meal plan, no meals can be shared. No meals will be carried over into the next month.

Residents wishing to change meal plans are able to do so by submitting their request on a Meal Plan Option Form to the Retirement Living Administrative Office at least thirty (30) days prior to the first day of the month in which they would like to change meal plans. This will allow the Finance Department the necessary time to make the proper adjustments to the resident’s monthly service fee invoice for the upcoming month. Only written requests submitted on the MealPlan Option Form will be processed for proper credit. Meal Plan Option Forms areavailable at the Retirement Living Administrative Office.

DINING SERVICESRetirement Living Residents have the opportunity to select one of three meal planoptions (See Meal Plan Options on Page H5) and dine in any of the six dining locations (See Dining Locations on Page H1).

The following is additional information that may enhance our service to you:1. Residents may eat their meal in any of the six dining locations and order by

selecting from the menu.2. For residents on a meal plan, any meals exceeding the allotted meals for the

month, a charge will be applied for the exceeded meal(s); as well as forHoliday and Special Theme Dinners. It will be added to resident’s monthly bill. For those who opt out of the meal plan, a charge will be applied to the resident’s monthly bill for lunch, dinner, or brunch or for Holiday and Special Theme Dinners. Please refer to the current Retirement Living Fee Schedule for up-to-date fees.

3. The Dining Services Program is designed to satisfy individual appetites. Theprogram will provide flexibility to order small, standard, or large portions of the entrée selection. Seconds will be available; however, all food served in one of the three restaurants or one of the three Cafés taken as one of the meals in the selected meal plan must be eaten in that area.

4. Residents may eat in the Courtyard Café, Three Loaves Café and VillageCafé, and pay cash, or use their meal plan meal. If the meal is purchased with cash, the unconsumed portion of food is permitted to be taken out. Please inform the server and a container will be provided.

5. Selected wine and beers are available for residents and their guests dining enjoyment in all the dining rooms and Three Loaves Cafe. See the wine menu at each of the dining rooms for prices. Residents may use their MVP card to charge their alcohol purchase or credit cards are accepted for guests to pay.

6. Cash will only be accepted in the Courtyard Café, Three Loaves Café andVillage Cafe. You may also use your MVP card in place of cash at the Cafés.

7. A Masonic Village Registered Dietitian is available, by appointment, to discuss resident’s special dietary needs or nutritional consults.

8. A week-at-a-glance menu is posted on all the communication bulletin boards located throughout various buildings. The weekly menu is also available via the Retirement Living telephone bulletin board by dialing extension 14528. Residents may also have his or her name put on an e-mail list by contacting the Dining Services Office at 14506 to receive weekly menus via e-mail on Fridays.

RESIDENT MASONIC VILLAGES PERSONALIZED CARDS(MVP) CARDS

Upon move in, resident will receive a MVP card with his or her picture on it. The MVP card will be used to track the meals that resident takes in any of the dining locations. The process for using the MVP card is simple. When dining in one of the dining locations, the resident’s MVP card will be collected during dessert course, at the end of your meal, or over the Café counter. The server will swipe the card at the server’s station and promptly return the card to the resident.

This process will not only accurately keep track of resident meals but will also help us to notice if a resident has missed meals. Security will be able to identify residents who are on the Standard Meal Plan and who have not shown up for their daily meal and provide assistance if necessary.

The MVP card can also be used instead of cash to pay for meals in the Village Cafe, Three Loaves Café and Courtyard Café that you do not want counted toward your meal plan. When using the MVP card in place of cash it does not affect or pertain to your meal plan in any way. All purchases made by residents with the

MVP card will be posted on his/her monthly statement. A replacement card can be issued by notifying the Retirement Living Administration Office.

GUEST SERVICES & RESERVATIONSWhen dining with guests in the Clubhouse, Eisenlohr Commons and the GrandLodge Hall Restaurants, we ask that you sign the guest book upon entering the restaurants.

To best serve the resident and their guests, reservations are recommended for tables of six in the following locations and recommended times. This will allow these tables to be reserved twice each meal period, thereby benefiting the greatest number of residents. Each resident or married couple may reserve a table of six no more than once per seven-day week.

This only pertains to residents on the Standard Meal Plan. It is very important that you call extension 10032 if you are not planning to eat a meal. At the end of each day, staff will be checking to see who did not attend a meal that day and then make a call to those residents who have signed a Dining Room Waiver Form asking to be checked on. If there is no response from the resident, Security will do a resident check to make sure there is not a need for emergency services. Please note, if a resident has submitted a Vacation Form, is in the hospital or the Health Care Center and will not be eating a meal that day, it is not necessary to provide notification to us.

HOUSEKEEPING SERVICES

Housekeeping services are provided two times a month. Definitions of the normal services provided are listed below. Please keep in mind housekeepers do not have control of personal items and do not move pictures, knickknacks, memorabilia, cosmetics, etc.

• The housekeeper will dust using a wool or micro fiber duster or vacuum. The housekeeper will dust door transom, louvered doors, exposed baseboards, window sills that are clear, refrigerator tops that are clear, cupboard fronts, vents/returns, heaters, around lights and other clearedsurfaces.

• Kitchen- Housekeeper will clean and dry the sink and clearedcountertops. They will spot clean exposed surfaces of appliances (stove, refrigerator, dishwasher). Cabinet fronts will be spot cleaned and floor will be damp mopped.

• Bathroom – Housekeeper will clean and dry sink, fixtures and vanitymirror. Cleared vanity countertop, shower and door will be cleaned. Thetoilet will be cleaned and sanitized. The Housekeeper will spot clean the exposed vanity front, exposed surfaces of washer and dryer and exposed baseboard. Floor will be damp mopped.

• Bedroom, Living area and Hallway – All open areas will be vacuumed.Small easily moved items such as ottoman and dining room chairs will be moved. Heavy furniture and decorative items will not be moved.

Within a reasonable period of time of the resident’s occupancy anniversary date, the Housekeeping Dept. will conduct an annual cleaning of each resident’s apartment. Annual cleaning will consist of the above services and the additional services listed below.

• Kitchen - Housekeeper will pull refrigerator forward to cleanunder/behind. Top of refrigerator must be clear.

• Bathroom – Housekeeper will wash vents and light fixtures in place. Washer and dryer will be tilted to clean underneath and behind. Stackedunits will be cleaned in place.

• Bedroom and Living Areas – Housekeeper will wash windows inside and out and inspect and wash window screens. Damaged screens will be repaired or replaced. High windows that do not tilt may be cleaned by an outside service. Windows not adjacent to living areas such as dormer

windows are not included. Window sheers will be washed and re-hung. Unusable sheers will be repaired or replaced. Vents and returns will be washed in place. Housekeeping staff will move and clean behind furniture not normally moved such as couches. Large items will not bemoved.

Additional housekeeping services will be available on a time and material basis or on a flat service fee, depending on the service which is provided. These fees can be found in Section F of this Handbook.

HOUSEKEEPING ADDITIONAL SERVICESExamples of additional services would be as follows:

Housekeeping cannot dispose of mattresses or other large furniture such as chairs and sofas. Residents will need to make other arrangements to dispose of these items.

If you have an Emergency Housekeeping situation that must be addressed immediately, call 14503 between 7:00 a.m. and 4:00 p.m. At all other times contact the Masonic Village Switchboard Operator by dialing '0' or 367-1121 and describe the location and the nature of the situation to the Operator. The operator will page the on-call Maintenance person to report the problem and secure immediate attention.

LANDSCAPING SERVICES

The landscape design has been carefully developed to provide an aesthetically pleasing and meticulously cared for Community setting.

Residents are welcome to provide suggestions for plantings, in writing to the Landscape Manager, c/o the Environmental Services and Land Management Department. All planting of trees, shrubs, and flowers will be done by the Landscape Department with the exception of the garden plots located at the rear of

the cottages. The Landscape Department will prepare the garden plot at a specified size and location; however, all future care and plantings in the garden plot are the responsibility of the resident.

In order to maintain the balance and beauty of the original landscape design, no resident, visitor, or any staff person, should install or remove any landscape materials without prior written authorization of the Landscape Manager.

Each fall, a form listing several available planting options for the spring is provided to those residents occupying cottages in order to make their selection. All annual planting will be done by the Landscape Department. If a resident residing in one of the living units would like to purchase an optional flower or piece of shrubbery which they would like to plant or have planted by our Landscape Staff, they may do so only after the Landscape Supervisor reviews the resident’s selection and approves the plant material and the location of installation. Any planting or design performed by the Masonic Village at Elizabethtown staff will be done on a time and material basis.

MAINTENANCE SERVICES

Maintenance of all living areas and common spaces is the responsibility of the Masonic Village at Elizabethtown (Masonic Village) Maintenance Personnel. In the event any resident encounters a situation which requires the Maintenance Personnel to make a repair, a Maintenance Work Request should be filled out and placed in any Retirement Living drop box, unless the situation is deemed to be an emergency. Maintenance Work Request Forms are located in the trash room or laundry room in each Retirement Living apartment building, at the desk located outside the Administrative office, or at the Retirement Living Administrative Office.

In the event that a situation is deemed an emergency which must be addressed immediately, the Maintenance Office at extension 33105 should be contacted between the hours of 7:00 a.m. and 4:00 p.m. (weekdays). At all other times, the Masonic Village Switchboard Operator should be contacted by dialing “0” or 367-1121 and the location, nature of the problem, and the resident reporting the incident should be reported to the attending operator. The operator will page the on-call Maintenance Personnel to report the problem area for immediate attention. Twenty-four hour emergency maintenance service isavailable. Below is the maintenance staff schedule.

Maintenance repairs included under the monthly fee, according to the Resident Agreement, consist of those repairs which are due to normal conditions of wear and tear within the resident’s living unit. Major appliances, including range, refrigerator, and optional appliances installed by the Masonic Village at the resident’s expense, are also covered under the Masonic Village Maintenance Agreement.

Other maintenance requests, such as installing additional shelving, addition of phone jack, etc., are available to residents for a fee. See the fee schedule.

The only exception to this procedure is for those services which are outlined in theTwo -Hour Picture and Light Installation.

SNOW REMOVAL - ROADS, DRIVEWAYS, PARKING LOTS IMPORTANT INFORMATION FOR SNOW & ICE OCCURRENCES

The Masonic Village Maintenance and Environmental Services & Land Management crews undertake snow and ice control operations whenever road conditions call for these services. The first priority is to maintain reasonable safe and passable (not necessary bare) roadways and parking lot surfaces for, first and foremost, emergency vehicle followed closely by staff and residents.

If at all possible please refrain from driving while snow and ice control operations are in progress. The Masonic Village bus transportation is available for your use at scheduled times if road conditions permit. The fewer vehicles there are on the roadways, the easier it is to plow and treat the roads. Consequently, the following procedure will be used in regard to snow removal.

Emergency access routes will be cleared to provide medical emergency assistance. Main access routes will be cleared next to provide access to parking areas and driveways. (Note: some parking spaces in the parking lots will be done at this time to expedite the clearing of that particular lot.) Once all main access routes are open, unoccupied parking spaces will be cleaned.

For residents who park in a parking lot, depending on the severity of the snow and time of day, parking spaces will periodically be cleaned as spaces are opened or a notice will be put on Masonic Village TV for the residents to move their vehicles to facilitate the complete clearing of a parking lot.

Cottages will be cleared of snow beginning with the sidewalks and concrete pads in front of the garages for emergency access. Driveways will be cleared after all roadways are cleared for emergency access. This plan is reviewed yearly to assure its effectiveness in providing a safe and accessible Community.

SNOW REMOVAL - AUTOMOBILESIn order to assist residents in removing snow from their automobiles in anorganized manner, the following procedure should be followed:

1. Residents wishing to have children from the Masonic Village Children’s Home assist them in cleaning snow from their automobile should contact theHarrison Cottage at extension 33621. If there is no answer, leave your nameand telephone extension in order to receive a return phone call.

2. Sometime later in the day, the resident should verify that their car was cleaned off. The resident should place a minimal donation of $5.00 in an envelope marked “Children’s Home Snow Removal”, write your name on the envelope along with the child’s name that cleaned your car, and place it in the drop box.

3. The Children’s Home Director will see that the children receive their envelopes accordingly.

TRASH REMOVAL - APARTMENTS1. Place regular household trash in the gray receptacles provided in the Trash

Room nearest your apartment.2. Utilize the recyclable bin in the Trash Room for recyclable items, such as

tin, glass, and plastic drink containers.a. NOTE: All items should be rinsed thoroughly before being placed

in recycle bins.b. DO NOT PUT OIL CANS, TIN FOIL, STYROFOAM, OR

AEROSOL CANS INTO RECYCLING BINS.3. Newspapers are to be placed in the recyclable bin. Phone books can be

placed on the floor in the trash room. Shoe boxes, and cereal boxes should be put in the trash.

4. Pet litter must be placed in a plastic bag, closed, and put in the gray receptacle with regular household trash. Pet litter should never be flushed down the commode.

5. Please keep trash container lids closed at all times.

Please wait for the Housekeeping Staff to return recyclable carts back to the Trash Room, do not place recycled items on the Trash Room floor.

SPECIAL PRECAUTIONS1. Residents who use syringes to administer insulin or any other medication

should dispose of the syringes by placing them in a coffee can or container, place the lid on the container, seal the lid with heavy tape to prevent spillage, and place the container in the Trash Room on the floor next to the regular trash containers. Housekeeping will dispose of these containers safely as infectious waste.

2. Separate battery containers for batteries; such as flashlight, hearing aid, etc.,are provided in each trash room.

3. The trash compactor at the Clubhouse Dock is for the Masonic Village Staff use. For your safety, please DO NOT use the compactor on the Loading Dock area for trash disposal.

4. Compact fluorescent (CFL’s) or regular fluorescent (tube) lamps should beput in a sealed plastic bag (such as a zip lock sandwich bag) and given to a member of the Housekeeping staff to be placed in a proper recycling container. Compact fluorescent lamps (CFL) or regular fluorescent tubes for disposal should not be put in the trash, recycling or battery containers. All

types and sizes of incandescent light bulbs should be placed in the regular trash. They are not recyclable.

5. When disposing of used batteries in the red battery recycling buckets, it isrequested that batteries with side by side terminals (such as nine voltbatteries used in smoke detectors) be taped with electrical tape to preventany possibility of overheating caused by an electrical short. In lieu of tape,side by side terminal batteries can be placed, individually, in one of thesmall plastic bags provided in the trash rooms. If the lamp fits and the bagcan be closed, these bags may also be used to enclose compact fluorescent lamps (CFL’s) to be given to the Housekeeping Staff for proper recycling. Please do not put items other than batteries in the red battery recycling buckets.

TRASH REMOVAL - COTTAGES1. Regular household trash should be placed in sealed garbage bags or trash

containers and set at the end of your cottage driveway prior to 5:00 a.m. on Tuesday mornings.

2. Recyclable items, such as glass, aluminum, metal cans, and plastic beveragecontainers, should be placed in the green recycle bins provided to you andset along with household trash prior to 5:00 a.m. Tuesday mornings. Newspapers should be put into paper grocery bags or tied up with twine and should not be put out if it is raining. Phone books should be placed in the regular trash.

3. Individual bags are provided to each cottage for disposal of batteries, suchas flashlight, hearing aid, etc.

Holiday Schedule for CottagesFor holidays celebrated on a Monday,( Memorial Day and Labor Day), trashpick-up will be on a Wednesday. When Independence Day, Christmas and New Year’s Day fall on a Monday or Tuesday, the trash pick-up will also be Wednesday.

PICTURE AND LIGHT INSTALLATION - TWO-HOUR, NO COST ASSISTANCEAll residents are provided two hours of Maintenance Personnel’s time upon takingoccupancy of their living unit to provide assistance in hanging pictures and like items. The above stated tasks are the only tasks which are provided to the resident

at no cost during the two-hour period. Residents have 30 day from their move in date to use this assistance.

The Masonic Village at Elizabethtown provides up to fifty-pound picture hooks for pictures hung during this period, at not cost to the resident. If additional assistance is required in hanging pictures, light fixtures or shelving beyond the two-hour period, it can be provided to the resident on a fee for service basis.

We fully appreciate that this is an extremely busy time period for all involved in moving. Therefore, this two-hour period of assistance is provided as a courtesy andconvenience for all residents at the time of occupying their living unit. These two hours of assistance to the resident are not for other services requested by a resident.

WINDOW TREATMENTS - APARTMENTS & COTTAGESAll living units are furnished with sheer curtains at the time each resident takesoccupancy of their unit. Residents may hang additional window treatments if they wish; however, the removal of all sheers must be approved by Administration to assure that any replacement of the sheers maintains an aesthetically pleasing view from the exterior of the building. All window treatments, including mini-blinds, vertical blinds, or shades of any type must be off-white to blend with the sheers which were originally provided.

Any sheers which are removed should be returned to the Housekeeping Supervisor for proper storage. Residents should make arrangements with an outside vendor toinstall or remove any and all window treatments, with the exception of the original installing of the sheers provided by the Masonic Village at Elizabethtown (Masonic Village). The Masonic Village staff will not install or remove window treatments which are preferred by the resident.

COTTAGE WINTERIZINGIf you are traveling over the winter and will be away for at least two weeks, we encourage you to take advantage of the option to have our Maintenance staff perform the following tasks at your cottage.

• Close off the main water supply valve• Place ice maker in off position• Turn off water heater (and softener if applicable)• Set cottage heat/cool thermostat to 65 degrees Fahrenheit

If you would like to have this service provided at your cottage for prolonged (two weeks or longer) visits away from the Masonic Village, please complete a work order and bring to the Administrative Office or put in any Retirement Living drop box. Please include in your work order the expected date of your return. With that information, our Maintenance staff can make sure everything is operating for your return.Other suggestions you may want to consider for prolonged visits away include thefollowing:

Stop mail and newspaper deliveryTurn off answering machineDisconnect electrical plugs from outlets for TV sets and computersRemove perishable food from refrigerator

Again, all of the suggestions as well as the option to have your cottage winterized during prolonged visits away from Masonic Village are optional.

DRY CLEANINGHighlander Cleaners makes available a 15% discount on dry cleaning andlaundering for residents of the Masonic Village at Elizabethtown. To receive the discount, clothes may be taken to the Clubhouse Market on the main level of the Clubhouse or Village Shoppe on the main level of Grand Lodge Hall. Please follow the procedure below.

ü Ask the Store Volunteer for a Highlander Dry Cleaning Ticket and complete the ticket, listing work to be completed. Only work listed on the ticket will be completed.

ü Residents will place their items to be dry cleaned or washed in theappropriate dry cleaning bag provided by Highlander for processing.

ü All dry cleaning and laundry must be received at the Clubhouse Market by 4:00 p.m. on Monday and picked up by Highlander Cleaners on Tuesday.

ü Items placed in the Village Shoppe by Thursday will be picked up by Highlander Cleaners on Friday morning.

ü Residents can pick up their cleaned and laundered items in theClubhouse Market on Fridays and the Village Shoppe on Fridays.Residents pay the Store volunteer for their dry cleaning services at the time of pick up.

Residents taking their clothes directly to Highlander Cleaners independently will not receive the Masonic Village discount.

Dry cleaning coupons are not redeemable for dry cleaning services though the Masonic Village dry cleaning procedure. Coupons may only be used at the Highlander Cleaners establishments in Elizabethtown and Mount Joy.

Shoe repair is also available through Highlander Cleaners as are minor clothing repairs.

EVENT PLANNING DEPARTMENTThe Masonic Village at Elizabethtown has an Event Planner who is responsible forcoordinating events and programs that happen on Campus. If you need to plan an event in areas such as the Picnic Area located near the Rooster’s Corner, the Lodge Dining Room or the Veteran’s Grove, you should contact the planner (Rich Whitman) at #33151.

MAIL DELIVERYAll US mail for residents is delivered to your US Postal Box by the ElizabethtownPost Office not later than 5:00 p.m., Monday through Saturday.

For residents in the apartments, in-house memos, notices, etc will be distributed in your in-house mailboxes located adjacent to the mailboxes on Monday, Wednesday & Friday. In-house mail to cottages is delivered on those days by volunteers living on your street.

Residents living on James Buchanan Drive apartments and Tall Cedar Lane cottages will receive a parcel pick-up slip for US Mail delivered parcels and United Parcel Service. Upon receiving a pick-up slip in your in-house mailbox, your parcel will be located on the parcel shelf beneath your in-house mailbox; you can then drop the pick-up slip in the drop box. If the parcel is too large for the parcel shelf, Housekeeping will deliver the package to your apartment door.

The Grand Lodge Hall Mailroom, located on the main floor of Grand Lodge Hall, will accept Federal Express (FedEx) packages for mailing. The hours for accepting packages are from 10:00 a.m. to 11:45 a.m. and 1 to 1:30 p.m., Monday through Friday.

Stamps are available for sale at the Grand Lodge Hall Post Office, the Courtyard Market on the main level of the Clubhouse and the Masonic Health Care Center Gift Shop.

MASONIC VILLAGE TV (MVtv)The Retirement Living Residents Association funded the establishment of a volunteer-run television channel. It is dedicated to information for the residents of Masonic Village at Elizabethtown. Daily campus activities, off campus trips for shopping and entertainment, emergency notices and many other items of interest

PHARMACY ACCESS & MEDICATION DELIVERYThe Masonic Village at Elizabethtown has a Pharmacy located at the Masonic Health Care Center on the main floor. Residents have access to the Pharmacy 6 days a week; Monday & Friday 8am to 5pm; Tues., Wed., Thurs. 8am to 6pm and Saturday 2pm to 4pm. Requests for medications may only be obtained when accompanied by a written prescription.

The Masonic Village participates with prescription drug companies that will allow us to bill resident’s drugs on-line. If a resident has a plan we can bill on-line, co- payment charges will appear on current monthly statements. More information on which Plans we participate with can be obtained from the staff in the Pharmacy.The Pharmacy extension is 33554.

If your insurance company must be billed manually for prescription drugs, the Finance Department can submit a bill on a monthly basis. Prescription drug plans generally have a deductible and a co-payment. Residents will be responsible for any balance not covered by this insurance. If a resident has Medical Assistance Pharmacy or PACE, a claim will be filed by the finance Department with the payer for the remaining balance.If you are eligible for PACE/PACENET, some insurance companies will require you to change your coverage to exclude prescription drugs for the period that you are on PACE/PACENET. PACE and PACENET are Pennsylvania's prescription assistance programs for older adults, offering low-cost prescription medication to qualified residents, age 65 and older. For information, call 1-800-225-7223.Mail-order prescription plans will only be utilized at the Masonic Village under certain circumstances which are described in the Resident Information Packet received by each resident during their health appraisal at the Masonic Health Care Center. If you are not a resident in the Health Care Center and are able to order mail-order medication on your own, you may be able to utilize mail-order pharmacy benefits.Refills of medications can be done by fax. You will need to provide the appropriate refill drug request form from your outside pharmacy such as Medco, CVS, Weis, etc. The form can be sent, hand delivered or faxed direct from the outside pharmacy or the resident. The fax number for the Pharmacy is 361-3908.

Residents are responsible for picking up their own medications from the Pharmacy. Medications will only be delivered if the resident is physical unable to ambulate from the bus drop off point at the Masonic Health Care Center to the Pharmacy orif the resident is being treated for an illness which the physician or consultant requests the resident not to leave their living unit. Prescription delivery is once a day, Monday through Friday beginning at 3:00 p.m. in the Library of the Retirement Living Clubhouse, in the lobby of Smith Apartments, Eisenlohr Apartments, Village Apartments and Brown Apartments.

Retirement Living residents who reside in the Village Green Area and are not able to pick up medications at the Pharmacy may drop off the pill bottle prior to 7:45 a.m. in the pill drop box which is located inside the front door in the Grand Lodge Hall. Residents can then pick up their medication between 3:45-4:45 p.m. on the same day in the all-purpose room.

Residents should notify the Pharmacy staff if they are physically unable to pick up their prescription. Delivery to individual apartments or cottages will be done in extreme situation under the direction of the physician.

The Pennsylvania State Board of Pharmacy Regulation requires the Pharmacist to provide prescription consultations; therefore, whenever there is a new prescription, a Registered Pharmacist will offer counseling. Residents have the right to refuse counseling.

ELEVATOR EMERGENCY - APARTMENT BUILDINGSEach elevator is equipped with an emergency phone located in a box on the frontwall under the floor selection numbers.

In the event of an emergency, a resident should open the box, pick up the receiver, and inform the Switchboard Operator as to which elevator they are in and the nature of the emergency. The phone is automatically connected to the Masonic Village at Elizabethtown’s switchboard; therefore it is not necessary to dial “0” for the operator.

Emergency personnel will then be summoned by the Switchboard Operator to address the situation.

EMERGENCY PREPAREDNESSThe Masonic Village has a plan in place to address the following types ofemergencies:

• Severe Weather including tornados• Chemical Spill (on site and off site)• Bomb Threat• Biological and Chemical Terrorism• Radiological Threat

All employees have access to these plans through the Emergency Preparedness Manual.

FIRE SAFETYFIRE ALARMS AND EXTINGUISHERS - APARTMENT BUILDINGSMasonic Village at Elizabethtown Switchboard Operator must be advised of allalarm test and drills to prevent a false alarm being sent to the base station.

Each Friday at noon, the Masonic Village siren is tested for a ten-secondperiod.

The apartment buildings are equipped with smoke and heat detectors, along with a sprinkler system. The smoke detectors, heat detectors, and sprinkler systems are connected to the Masonic Village Switchboard to provide 24 hour monitoring of all alarms. Cottages are equipped with smoke detectors and are also connected to the Masonic Village Switchboard.

Fire extinguishers are located on each floor of the apartment buildings and under the kitchen sink where applicable in many of the Retirement Living units. Employees of the Masonic Village receive regular fire safety training.

Fire extinguishers are serviced annually by Masonic Village staff. You will be notified and given instructions when it is time to have your extinguisher serviced.

FIRE HAZARDS:Do not store any items in your oven as they may ignite when the oven is turned on.Please clean your dryer lint trap on a regular basis in order to remove a potential fire hazard.

FIRE DRILLS, REPORTING AND EVACUATIONIf a resident discovers a fire in their living unit or any other building on theMasonic Village at Elizabethtown (Masonic Village) grounds, they should go to the nearest fire alarm box located in the hallway of each apartment building and pull the lever and/or dial 200 if you are able. (In order for residents living in apartments to best prepare themselves for evacuation of the buildings in the event of an emergency, residents should familiarize themselves with the exits and fire alarm pull boxes within the apartment building in which they reside.) After pulling the lever, the resident should immediately go to the nearest exit and leave the building by way of a doorway or stairwell only. (Do not use elevators)

If the alarm sounds, all residents should evacuate their building• Don’t wait for someone to tell you to evacuate, leave when the alarm sounds.• Do not attempt to fight the fire or conduct a rescue of residents. You should

pull the lever, dial 200, evacuate, and let staff/ fire responders take over.• Close your unit door behind you but do not lock it.• If you encounter heavy smoke or flames, use an alternate exit.

• If you must exit through an area with smoke, crawl on your hands and kneeskeeping your head one to two feet from the floor.

• For residents living in apartments, if you cannot exit using the stairwayunassisted, you should move to the nearest fire exit stairway and wait forassistance.

• Once you are outside, do not go back in the building.• Keep away from the building and stay with the group so fire responders know

you are out of the building.• If you are unable to exit your apartment or cottage, place towels or clothing

around the door to keep smoke out. Dial 200 to notify the operator or 911 and tell the person you are unable to exit. Stay by the window to signal the fire department with a flashlight or white cloth.

If an alarm is sounded, staff is responsible for verifying the alarm and will silence the alarm if there is no fire. It should be noted that the alarm will continue to ring until staff verifies that there is not a fire. If the alarm is a false alarm, staff will sound the “All Clear” signal (5 second pulse alarm) for residents in the James Buchanan Apartments or Tall Cedar Cottages or silence the alarm in other Retirement Living areas and notify the residents that it is safe to return to their living units. If there is an actual fire, the alarm would continue to sound and staff and the local fire company would be on-site to assist in evacuation. ELEVATORSMUST NEVER BE USED DURING A FIRE IN ORDER TO PREVENT THEPOSSIBILITY OF AN INDIVIDUAL BEING TRAPPED IN THEELEVATOR.

LIABILITY INSURANCE REQUIREMENTSResidents are reminded that they responsible for any damage caused to MasonicVillage property or to visitors as a result of the resident's negligence. Residents are required to secure a liability insurance policy that covers the resident's personal belongings, and a minimum of $300,000.00 in liability insurance.

SAFETY GUIDELINES FOR THE USE OF ASSISTIVE DEVICES Motorized scooters/wheelchairs, manually-operated wheelchairs, walkers, quad canes, canes, and other assistive devices may be used by residents in all areas of Retirement Living.

In order to maintain the health, safety, and comfort of our residents, we request that a resident wishing to utilize a motorized device obtain a physician order for Occupational Therapy and receive a therapy assessment (call #33338). This assessment will assist the resident in determining the proper type of device and resident's ability to operate the motorized device safely without endangering himself or other residents.

In order to maintain open and safe paths of travel, residents are asked to follow the following guidelines.

1. We ask that anyone possessing a scooter or electric wheelchaircomplete a registration form. Registration forms to register yourscooter or electric wheelchair may be obtained from the Social Services’ Offices in the Clubhouse, the Retirement Living Administrative Office or The Grand Lodge Hall. After completing the registration form and submitting it to the Occupational Therapy Office located in the Health Care Center, the resident will receive a sticker listing his or her Motorized Assisted Device number. The resident should attach the sticker to his or her electric scooter or wheelchair.

2. Do not park motorized scooters, wheelchairs, manually operatedwheelchairs, or walkers in common areas, hallways, or commonwalkways.

3. Residents wishing to park such assistive devices may use the coatrooms conveniently located adjacent to the Clubhouse, Eisenlohr, and Café dining areas. Also, residents living in the Village Green Area may park their assistive device in the Scooter Parking Area outside the Dining Room in the Grand Lodge Hall or in designated areas located throughout the tunnels.

4. Assistance devices may also be parked in the designated area outsidethe Hair Care Center in the Clubhouse. However, we ask if you are parking your scooter or wheelchair for an extended period of time, you park in the coat room located across from the Dining Room on the first floor. Residents using the Hair Care Center need to have easy access to their wheelchair or scooter.

5. When medically necessary, residents are permitted to remain seated intheir electric/manual wheelchair while in the Dining Rooms. If not medically necessary, residents who can walk should park their electric scooter or electric wheelchair in the coat room on the first floor in the Clubhouse and in the Scooter Parking Area in the Grand Lodge Hall. Residents can then proceed into the dining room with a walker or cane if needed. After a resident has been seated in the Dining Room, dining staff will offer to place walkers in the coat rooms at the Eisenlohr and Clubhouse Dining Rooms and in the designated walker area inside/outside the Dining Room in Grand Lodge Hall.

6. Residents who have any concerns about this procedure are encouragedto discuss their concerns with the Director of Retirement Living.

7. Residents may lose their right to operate a scooter if their safety orthat of the community is at risk. Residents may also lose their scooterprivileges if they incur multiple safety violations.

8. Management personnel will discuss accidents and safety violationsrelated to electric scooters or wheelchairs and determine if any actionsneed to be taken to ensure the safety and rights of residents, visitors and employees at Masonic Village.

9. When residents enter Retirement Living, they are required to purchase$300,000 of liability insurance. Residents are encouraged to contact their insurance company to verify that their current coverage protects them while operating a scooter or electric wheelchair.

10. Residents will be responsible for the cost of repair for any damagecaused by their electric wheelchairs or scooters.

SECURITY SERVICEThe Masonic Village at Elizabethtown has always placed the security, safety, andwell-being of residents as a primary concern in developing and maintaining services and care for our residents. In order to develop a secure and safe environment, the Masonic Village maintains a Security Staff who monitor and patrol all buildings and areas of the Masonic Village grounds. Also, the Chief

Executive Officer or an Administrator is on-call 24 hours a day, seven days a week, to address any safety or security situations which require administrative action.

In the event a resident observes any unauthorized individual or suspicious situation, they should contact the Masonic Village Switchboard Operator by dialing “0” or 367-1121 and give their name, report the time, location, and description of the incident and any individual associated with the situation. The Switchboard Operator will notify the Security Personnel on duty and provide them with the necessary information to address the situation appropriately.

Masonic Village has an alarm notification system which is designed to alarm staff should a resident’s pull cord, fire pull station, smoke detector or sprinkler system be activated. In the case that one of the systems is activated, appropriate staff will respond.

SEVERE WEATHER PREPAREDNESSMasonic Village monitors the National Weather Service during severe weatherevents and we try to alert employees and residents of any tornado warnings that require sheltering actions. A campus siren will sound three 30-second signals to indicate a tornado warning. Secondary notifications are also usually made through the phone lines and on Masonic TV (channel 95). Should the National Weather Service issue a tornado warning for the vicinity of the Masonic Village at Elizabethtown (Masonic Village), Retirement Living residents should seek shelter according to the following guidelines:

- If you cannot get to a location below ground level, take refuge in interiorrooms on the lowest floor level.

- Avoid rooms with large windows.- Use basement, tunnels or rooms below ground level if available

Shelter location examples would include:

- Bathrooms- Closets- Interior building hallways away from windows- Basements- Tunnels

SPIRTITUAL SERVICES

The Masonic Village at Elizabethtown has always placed the spiritual needs of the residents as a priority in the development of quality of life and wellness. In order to meet the spiritual needs of residents of the Masonic Health Care Center, Personal Care, and Retirement Living areas, we have a very active Pastoral CareStaff offering diversified programming and services. A Pastor is on-call sevendays a week, 24 hours a day to provide spiritual comfort and strength as needed and requested by all residents of the Masonic Village. In case of emergency Pastors are available to all residents for their spiritual care.

The Masonic Village has a thriving church on campus. The Congregation of Sell Chapel was formed in 2004 and has over 380 members. It is located in the Village Green Area on the campus. Services at Sell Chapel are held every Sunday morning at 8:15 a.m. and 10:00 a.m. and all residents are welcome. Communion is offered monthly, as well as the celebration of religious Holidays throughout the year. Shuttle transportation is provided on Sunday mornings to Sell Chapel. The dates and times are listed on the monthly Activities Calendar. Church services are broadcasted on our MVtv as well as other services for those residents who are not able to attend. Our other chapel, Carpenter Chapel is located on the Patton Campus.

Masonic Health Care Center/Freemasons Building - Church services are held every Wednesday and Sunday at 2:00 p.m. in the Masonic Health Care Center.

For any Pastoral need, please call the operator to page them. The Pastoral Care Office is located in the Masonic Health Care Center. The office telephone number is 33106.

Other Pastoral Services are offered to maintain the spiritual wellness for all residents. Included in those services:

• Support groups for widows called “Special Singles” and a monthly supportgroup for widowers

• Bible Study Groups – weekly in Grand Lodge Hall and Keystone Room• Men’s Bible Study – weekly in Sell Chapel• Monthly Study Groups for men and women• Jewish services and a study group• Roman Catholic masses

SELL CHAPEL - PARKING LOT SHUTTLE SERVICEDuring Sunday services a Shuttle Bus Service runs for those residents parking atthe Freemasons Cultural Center beginning at 7:45 a.m. and running until 8:15 a.m. When the service is over the Shuttle Bus will drop residents off at the designated areas and begin to take residents over to Sell Chapel for the second service. At the conclusion of the second service the Shuttle Bus will return the last group of residents to the designated areas.

Residents are asked to park in either the Child Day Care parking lot or the twoparking areas directly across from there. You may stay in your car until theShuttle Bus gets there. Just wave your hand and the bus driver will pick you up. The bus driver will drop you off and pick you up at your car if you are in the above-designated areas

The professional staff consists of individuals who are educated at a bachelors or masters level in college and attend annual continuing education seminars to enhance their skills and level of knowledge.

Telephone Services – Frequently Asked QuestionsQ – How do I call someone who lives or works on campus?A - All you need to do is dial the 5 digit extension. If you are calling someone who has a cell phone, you will need to dial the complete number. Be aware that some cell phone numbers might be a long distance call.

Q – How do I make a local call?A – You will need to dial 9 first and then enter the 7 digit telephone number.

Q – Can I make long distance calls?A – Yes – To make a call within the area code simply dial 9 and then the 7 digit telephone number. To place a long distance call outside the area code dial a 9 then 1 followed by the 7 digit telephone number.

Q – Do I need to dial a 9 before calling 911?A – No- Simply dial 911 in case of an emergency.

Q – When do I get a bill for phone service?A – Fixed telephone charges will be billed on your monthly service fee invoice. Long distance charges will be included on the monthly service invoice. If you have long distance charges, you will also receive a log of your monthly long distance calls. Please note, that is not a bill!

Q – If a family member or friend is trying to get a hold of me while I am on the phone, will the Switchboard be able to interrupt my call?A – No – You should let your family and friends know if there is an emergency and your line is busy they should call the Switchboard who will have a security officer come and notify you of the emergent call.

Q – What Internet services are available?A – Masonic Village does not provide individual Internet services. Dial up services are available but if you decide to use a dial up service, check with the Telecommunications Department to make sure the number you are dialing is not long distance.

Q – What other options for Internet service are available?A – Windstream offers Digital Subscriber Line (DSL). This high speed Internet service runs through your phone line. Windstream can be contacted at 1-866-648-2567. Comcast offers high speed Internet service and can be contacted at 1-855-638-2855.

Q – Will my Internet service fee be on my monthly bill?A – No – You will be billed directly by your Internet Service Provider.

Q – Who should I call if I have problems with my Internet service?A – You will need to contact your Internet Service Provider. Masonic Village is not responsible for issues related to Internet service.

TRANSPORTATION - NON-MEDICAL TRIPSResidents are provided the opportunity to utilize the Masonic Village atElizabethtown (Masonic Village) transportation system, consisting of several buses, and automobiles. Transportation is provided appointments at the Masonic Health Care Center, scheduled trips and outings, religious services at Sell Chapel, entertainment at Deike Auditorium, and other programs scheduled through the Retirement Living Activities Department. Any trips off the Masonic Village grounds which require the use of chartered buses are at the expense of those residents participating in the trip.

Transportation on the Masonic Village grounds takes place in the form of a Shuttle bus which runs between the Retirement Living Areas, Masonic Health Care Center and Sycamore Square Family Medicine. This schedule is posted on the bulletin board of each apartment building.

Residents may invite visitors and family to travel on local bus trips upon availability at the time the bus is ready to depart.

Residents may request non-medical trips to Hershey, Lancaster, Harrisburg, and York (30 miles or less from the Masonic Village) by calling extension 14528 and selecting #2 on the menu. Cost for trips can be found in Section F – Fee Schedules. Several residents may share the cost and the minimum would be one-half hour increments. These requests must be made at least two weeks prior to the requested date of transportation. The charges for these services will be placed on your monthly statement from the Masonic Village.

Transportation may also be arranged to the Harrisburg International Airport. Thisservice may be arranged by calling extension 14528 and selecting #2. Cost fortrips can be found in Section F – Fee Schedules. These requests must be made at least two weeks prior to the requested date of transportation. The charges for these services will be placed on your monthly statement from the Masonic Village.

If for any reason you would need to cancel from a trip on a weeknight or weekend, please call the Operator (0) to let them know, and the bus driver will then be

notified. To cancel a trip during the day, within the hours of 8:00 a.m. and 4:00 p.m. please dial 33205 and leave a message if no one is available.

TRANSPORTATION - OUTSIDE MEDICAL APPOINTMENTSIn order to meet the transportation needs of residents who are scheduled for outsidemedical appointments by a Masonic Village physician or consultant, the following procedure should be followed if a resident is unable to drive himself or herself:

The resident should contact the Dispatcher at extension 33031. Transportation arranged through the Dispatcher for outside medical appointments will be on a fee for service basis. Transportation services are available from 8:00 a.m. to 2:30 p.m. for residents scheduling their own outside medical appointments and a two-weeknotice to the dispatcher is required.

SHUTTLE BUSFor the convenience of the residents at the Masonic Village at Elizabethtown, theavailability of a shuttle bus is provided for on and off campus trips. The On- Campus and Off-Campus Shuttle Schedules are available in the Shuttle Schedule of this handbook beginning on page P3. Please review the schedule and become familiar with the times available for your area.

Daily On Campus ShuttleThe residents residing in apartment buildings will be picked up at the vestibule oftheir building or gazebo depending on the location; residents residing in cottages can be picked up at their driveway. The available pick up times for are listed initalics on the schedule. In order for residents living in cottages to schedule apick up time, a call will need to be placed to 808-1178, at least one (1) hour before the requested time of pick up. When boarding the shuttle to return back to your cottage, inform the driver and he/she can drop you off at your driveway.

Elizabethtown ShuttleA shuttle to Elizabethtown will be offered at 8:30 a.m. on Monday, Wednesday,and Friday and at 1:15 p.m. on Tuesday and Thursday. Residents can sign up forthis shuttle by calling extension 33205.

OVERNIGHT VISITOR NOTIFICATION

The Masonic Village at Elizabethtown staff encourages all residents to continue their interaction and involvement with family and friends after entering the facility. In order to facilitate this need, residents may invite family and friends to stay overnight up to eight consecutive days in their living unit with the understanding that the resident is present in the living unit at the time of such a stay.

However, family and friends of a resident may stay overnight in a resident's living unit if the resident is currently occupying a room at the Masonic Health Care Center or an outside hospital. In the event that a situation may arise in which we would need to evacuate any area of the Masonic Village, the Masonic Village’s primary concern is for the safety of all residents and visitors. Therefore, for the safety of all residents and their visitors, the Retirement Living Administrative Office should be notified of any overnight visitors. This will facilitate the Masonic Village’s efforts to see that all residents and visitors are safely evacuated, if necessary.

A Telephone Emergency Reference Card is attached to the telephone in each unit for procedures involving health related emergencies and urgent health relatedsituations. An Administrator is on call 24 hours a day to address issues which require administrative attention. A Social Worker is also available 7 days a week; between 8:00 a.m. and 4:00 p.m. Housekeeping and Maintenance are on call evenings and weekends for emergency issues.

VISITING & ACCESS BY RELATIVES & FRIENDS

Visiting hours for residents are generally considered to be unlimited for friends and relatives visiting residents who live in Retirement Living.

It shall be the responsibility of the individual resident to monitor effectively the visits of relatives and desired visitors to insure that one's neighbors are not inconvenienced by loud noise, bad manners, or inappropriate behavior. Failure of any visitor to maintain good manners and behavior will result in that visitor being asked to leave the premises and the grounds of the Masonic Village at Elizabethtown.

Access to each retirement living apartment building is available by following the procedure listed below:

James Buchanan Apartments- The front door to the Clubhouse will be openfrom 5:00 am until 8:00 pm each day. All other doors accessing James Buchanan apartments will be locked 24 hours a day. Access is available for residents through the use of their personal access card with the card reader that controls each exterior door of those apartments.

Sycamore South, Sycamore North, Village, Brown, Smith North, Smith South,Charles and Louis Eisenlohr Apartments - The doors to the vestibules will beopen between the hours of 5:00 a.m. and 8:00 p.m. Access is available forresidents at other times through the use of their personal access card with the cardreader that controls each exterior door.

Village Green Area Apartments- Access is available for residents and visitors through all exterior doors between the hours of 5:00 a.m. and 8:00 p.m. Access is available for residents at other times through the use of a key pad.

Volunteer service at the Masonic Village provides opportunities to make new friends, learn new skills, develop new interests and contribute meaningful and significant service to benefit other residents. Prospective volunteers, through a personal interview, will share their time availability, interests, abilities and skills. Every effort is then made to assign volunteers to an area which meets the interests of the volunteer and the needs of the Masonic Village.
