BACKGROUND

• Nowadays obesity is a serious worldwide

problem

• 50% of our patients have BMI over 27

• 1/3 of them are over 30

BACKGROUND

• Obesity has been considered a risk factor

of relapse in patients with surgical treatment for Stress Urinary Incontinence

OBJECTIVE

• To compare continence and complications rates between patients with different degrees of obesity and patients with normal weight

MATERIAL AND METHODS

• In our hospital all overweight incontinence patients first are invited to:

• 1.- Loose weight about 5% of BMI by:– Exercises program– Dietary restrictions

• 2.- Leave smoking• 3.- and other lifestyle interventions • Only when they have achieved the prior points and the sintoms

persist we proposed them an incontinence surgery

MATERIAL AND METHODS

Retrospective study that compares 459 obese patients (OW) vs. 87 patients with normal weight (NW) with SIU who were treated with Transobturator Suburetral Sling (TOT)

The obese patients were classified according to OMS criteria for obesity according to BMI as follow= NW: BMI = 18.5-24.9 OW 1: BMI 30-34.9, OW2: BMI 35 – 39.9 OW3: BMI > 40

BMI: Body mass index; SUI: Stress urinary incontinence.

MATERIAL AND METHODS

Rates of actuarial continence and of complications at 12 months for the different types of OW vs patients with normal BMI were compared

In the post operatorie the subjetive cure was evaluated by UDI -6.

Cured patient for SUI was defined as asymptomatic patient ( score 0 for questions 3 and 4 from UDI-6 )

All relapses were demonstrated by multichannel urodynamics

RESULTS

The age, antecedent of previous incontinence surgery and POP associated were not different beetwen patients with NW vs OW.

Parity was significantly higher in patients with OW The average follow-up for patients with OW and NW was of 9.9

and 9.5 months respectively

RESULTS

The rates of actuarial continence at 12 months follow up for the total group of OW patients vs NW patients were 84.3 vs 86.8; p=0.2

RESULTS

No statistically significant differences were found when the rates of actuarial continence at 12 months follow up in the different degrees of OW were compared to those of NW

There were no differences in the rates of intraoperative and postoperative complications between patients with NW and patients with OW

CONCLUSIONS

Our results suggest that TOT is as effective in patients with OW as in patients with NW, even in the subgroup of patients with OW stage 3, without a higher rate of intra and postoperative complications.

Introducción• Cada vez se ha tornado más relevante la calidad de

vida de las pacientes que son sometidas a cirugía de prolapso

• Éxito subjetivo– encuestas de calidad de vida – percepción de resultado quirúrgico.

• La encuesta PGI-I (Patient Global Impression of Improvement)– Consta de 7 categorías, las cuales traducen fracaso,

mejoría y éxito del resultado.

Materiales y Métodos• Estudio retrospectivo

• Hospital Padre Hurtado, Santiago, Chile– Años 2008 al 2011

• 812 pacientes• Pacientes sometidas a cirugías de prolapso • Evaluadas durante el primer año de postoperadas.

• Curadas sintomáticamente → mejor o mucho mejor.

• Se aplicaron el PGI-I y la encuesta de calidad de vida (pQoL)

Materiales y Métodos• PGI se modificó de las 7 categorías a 3 con el fin de definir

éxito subjetivo según las respuestas de las pacientes– mucho mejor y muchísimo → éxito– mejor → mejoría– Resto de las categorías → fracaso.

• Evaluación del resultado objetivo– Pop-Q

• Para establecer las correlaciones se utilizó el índice de Pearson

significativo con un p < 0,05

• 812 pacientes

• Edad promedio del grupo es de 59 años (36-78)

• Paridad promedio de 3 hijos (1-7)

La correlación obtenida al compara percepción simplificada postoperatoria (PSP) con PGI-I, pQoL y popQ se obtuvo alta correlación con significancia estadística

Conclusión• La evaluación del resultado subjetivo de las pacientes a

través de la percepción simplificada postoperatoria (PSP) en éxito, mejoría o falla demostró tener una alta correlación tanto con la evaluaciones de resultado subjetivo evaluado a través de las encuestas PGI-I y pQoL, así como con el resultado anatómico evaluado a través del POP-Q.

• Surge como una herramienta de evaluación del resultado quirúrgico que resulta bastante simple para aplicar a diario en el que hacer clínico

• La obesidad es una patología de alta prevalencia (mujeres)

• Considerada un factor de riesgo para falla del tratamiento quirúrgico de la incontinencia urinaria de esfuerzo (IOE)

• A lo largo del tiempo se ha demostrado que a corto plazo los resultados para pacientes con peso normal y obesas son comparables

• Comparar el resultado de la cirugía de tot en pacientes con peso normal y obesas a 7 años de seguimiento promedio

Materiales y métodos

• Estudio de cohorte restrospectivo que compara 294 pacientes obesas vs 65 pacientes con peso normal

• Todas portadoras de IO pura o mixta con predominio ioe, que fueron tratadas con TOT

• Las pacientes se clasificaron según los criterios de la OMS en:– Normopeso ( imc 18.5-24.9) – Obesidad tipo i ( imc 30-34.9)– Obesidad tipo ii ( imc 35-39.9 ) – Obesidad tipo iii (imc >40 )

Materiales y métodos

• Para evaluar el éxito de la cirugia se utilizo el score validado PGI-I

• Se consideraron como tratadas exitosamente todas aquellas pacientes que se encontraban mejor, mucho mejor o muchísimo mejor comparado con el preoperatorio, respecto de su sintomatologia de IO

• Se compararon variables demograficas entre los grupos y las tasas de curacion son expresadas como tasas de sobrevida actuariales utilizando el metodo de Kaplan Meier

• Se considero como significativo si p<0.005

• La tasa de sobrevida actuarial a 7 años de seguimiento fue similar entre las pacientes normopeso vs las pacientes obesas fue 67.2 %vs 67.6% log Rank: p=0.521.

Normopeso V/S Tipos Obesidad• Cuando se compararon las tasas de continencia a 7 años promedio entre

los distintos tipos de obesidad , estos fueron tambien comparables estadisticamente (Log Rank p=0.906) :

Conclusions

• Nuestro trabajo sugiere que el TOT a largo plazo es tan efectivo en mujeres con peso normal como en el grupo de mujeres obesas
