REWARDS ON YOUR ROAD TO RECOVERY

Good health includes the whole person, body and mind. Since getting and staying healthy can be a challenge, CareSource will reward you for taking steps to improve

your health. If you are a HIP Member who is treated for Substance Use Disorder through attending Intensive

Outpatient Programs (IOP), you can earn rewards!

1. Receive a $10 reward for each in-network IOP session up to $100.*2. Track your rewards in the free CareSource mobile app or on your MyCareSource account.

If you do not have a MyCareSource® account, visit My.CareSource.com to register for one.Once your account is set up, you can earn $10 for attending an approved in-network IOP session.

3. Use your rewards to shop for food, shopping or technology. You can also save rewards sincethey add up over time.

How to earn your $300. $100 can be earned for attending IOP Meetings (in-network)* and is part of the $300 total rewards that can be earned across all incentive programs. All of these rewards are earned when you take preventive care steps. These offerings will be shown to you in your MyHealth Journey. * Please note • $10.00 per IOP session attended that can total up to $100.00 per calendar year. Rewards based on claims are recorded once

claims are submitted by providers and processed by CareSource. It can sometimes take up to four weeks to show rewards forthese activities in your account.
