Dear Rising Voices Applicant:
We are very excited that you are interested in participating in the Rising Voices program. This program is offered by the Alan Madden Foundation, a non-profit organization that is dedicated to helping youth in the foster care system. Through our partnership with Guitar Center, you will receive a musical instrument of your choosing from the attached list of approved instruments, and you will also receive free weekly lessons from the professional instructors at your local participating Guitar Center. We are very proud of the Rising Voices program because of the many benefits of learning to play a musical instrument. Studies clearly show that learning to play an instrument improves your math and reading skills, teaches you responsibility and perseverance, improves your listening skills and coordination, and gives you the opportunity to have fun and make new friends. To be eligible to participate in the Rising Voices program, you must either be currently placed in foster care, have previously been in foster care for at least six (6) months, or be a biological child of the foster family of a foster child that is also participating in the Rising Voices program. You will be required to provide proof of your eligibility before being accepted in the Rising Voices program. Once we verify your eligibility, we will notify you of your acceptance and you will then need to schedule an orientation meeting at your local participating Guitar Center to receive your instrument and schedule your lessons. In order to make the most of your experiences in the Rising Voices program, we recommend that you attend all of your scheduled lessons, be prepared for your lessons by practicing your instrument as directed by your instructor, take good care of your instrument, participate in your music recitals, and above all, have fun and make new friends. If you successfully complete six (6) months in the Rising Voices program, by attending your scheduled lessons and recital(s), you will be permitted to keep your instrument. CONDITIONS
COMMITMENT. Students are expected to:
✓ Practice your instrument as directed by your instructor.
✓ Have no more than three (3) unexcused absences.
✓ Attend class with a desire to learn.
✓ Maintain your loaned musical instrument in good condition.
✓ Participate in recitals as arranged by the Alan Madden Foundation and/or Guitar Center.
✓ Attend school regularly and be a student in good standing.
STUDENT REGISTRATION
ATTENDANCE. Due to the limited funding available for this program, good attendance is expected. If a student is unable to make
a scheduled lesson, they must contact their instructor at least twenty-four hours in advance so that there will be no charge for the
missed lesson and so it can be rescheduled, if possible. Failure to properly notify their instructor in advance is considered an unexcused
absence. If a student accumulates three (3) unexcused absences, their participation in the Rising Voices program may be discontinued.
DISCONTINUATION. The Alan Madden Foundation, the Rising Voices program and/or Guitar Center reserves the right to
discontinue a student’s participation in this program due to poor attendance, lack of interest, disruptive behavior, lack of funding or
for any other reason. A student whose participation has been discontinued may be required to return their instrument and may not
re-apply for the program for a period of up to twelve (12) months.
PHOTOGRAPHIC AND AUDIO/VIDEO RELEASE. (1) I, individually and on behalf of Minor Participant(s) do hereby grant the
released Alan Madden Foundation Inc., Rising Voices, and Guitar Center, Inc., the irrevocable right and permission to photograph or
otherwise record (Photograph) me or minor participant for all purposes, including advertising and promotional purposes related to
the Rising Voices program in any manner, in any and all media now or herein after known in perpetuity throughout the world, without
restriction as to alteration. I waive any right to inspect or approve the use of the Photographs and acknowledge and agree that the
rights granted by this Photo Release are without compensation of any kind. (2) I acknowledge and agree that I have no right, title or
interest in the Photographs and agree that such Photographs and the copyright therein are the exclusive property of the Released
Parties. I agree to release and discharge the Released Parties from any claims, actions, damages, demands, costs, expenses including
attorney’s fees or lawsuits of any kind by reason of the sale, distribution or use of such Photographs.
NOTICE OF NON-DISCRIMINATION. The Rising Voices programs and services reflect the cultural diversity of our community. We do
not discriminate on the basis of religion, race, color, ethnicity, sex, gender identity / expression, national origin, sexual orientation,
age, disability, veteran status, or other status protected by applicable law.
DAMAGE TO INSTRUMENTS. You are expected to maintain your loaned musical instrument in good condition. Your loaned instrument
will not be replaced if you fail to immediately notify the Alan Madden Foundation or your instructor of any damage.
