3Working inside Balancing tankConfined space hazards (If the work is to be conducted inside attenuation tank) such as presence of toxic and flammable gases OR lackof oxygen inside the deep attenuation tank. Those who work / go inside attenuation tank4416Ensure safe access into the Balancing tankEnsure enough lighting. 414Site Engineer, Gen.Foreman

Do gas test to detect presence of toxic / flammable gases or lackof oxygen inside the attenuation tank.Safety OfficerIf presence of any toxic / flammable gases are found, Engineering methods to eliminate the gases to be applied by Construction Engineers. - Try to evacuate the gases with exhaust fan; at the same time supply pressurized air into the base of the Balancing tankThis shall be continued till the toxic / flammable gases are fully evacuated.Site Engineer, Gen.Foreman

Likelihood / Severity / 1Remote / Minor / 2Unlikely / Major / 3Likely / Severe / 4Frequent / Fatality / Working inside Balancing tank -- CONTINUED Confined space hazards (If the work is to be conducted insideBalancing tank) such as presence of toxic and flammable gases OR lack of oxygen inside the deep balancing tank. Those who go / work inside attenuation tank4416Take gas test again. Do not allow to work till toxic / flammable gases is brought down to zero. 414Safety OfficerIf the oxygen percentage is below 20.9 % , provide air supply at normal pressure into the bottom of theBalancing tankand check whether the oxygen is building upto safe level of 20.9 %. Observe for half hour and If the oxygen content is remaining steady between 18.5 % and 20.9% , work shall be proceeded. Site Engineer, Gen.Foreman. Safety OfficerIf the oxygen % is not remaining steady in a range of 18.5 % - 20.9 %, the workers should use self contained breathing apparatus to work inside the Balancing tankIn such case, one worker should not work insideBalancing tankfor more than 20 minutes, if the Oxygen cylinder is designed to use for 30 minutes. Site Engineer, Gen.Foreman. Safety OfficerAir supplied breathing apparatus (attached with helmet) shall be used only if pure breathable oxygen from a cylinder or special non-lubricating type air compressor with regulating system can be provided. Air supplied from normal compressors contain harmful Carbon Monoxide due to lubricant in it's internal parts. Site Engineer, Gen.Foreman. Safety OfficerEnsure that the workers are trained to use the self contained breathing apparatus and briefed upon the hazards involved..
